218. Subprime Lending and Social Justice: A Biblical Perspective

Abstract

Although credit markets affect the welfare of everyone in a modern economy, there is cause for special concern about the effects of their operation on the poor. Examination of Old and New Testament teaching on the subject shows a strong biblical presumption against credit transactions that cause permanent impoverishment. New Testament teaching goes further than Old Testament teaching, including Christs call to believers not only to love their neighbor, but to loveand even lend totheir enemies. In this context, Christians cannot be complacent about credit markets even if they appear to be economically efficient as voluntary transactions. The transactions of special policy concern to Christians lie between the clearly illegal (not requiring further legislative action because they are already outlawed) and clearly ethical subprime lending that accurately incorporates the higher risk of subprime borrowers into interest rates. In this middle ground of unethical but not strictly illegal lending, three possible remedies are proposed: (1) legislation that regulates information or actual terms of loans to the benefit of subprime borrowers; (2) provision of free credit counseling by Christians in partnership with others; and (3) more broadly, measures to reduce poverty through community-group action, financial education, and lending by believers without hope of repayment.William C. Abstract

A distinctive subset of metastatic breast cancer (MBC) is oligometastatic disease, which is characterized by single or few detectable metastatic lesions. The existing treatment guidelines for patients with localized MBC include surgery, radiotherapy, and regional chemotherapy. The European School of Oncology-Metastatic Breast Cancer Task Force addressed the management of these patients in its first consensus recommendations published in 2007. The Task Force endorsed the possibility of a more aggressive and multidisciplinary approach for patients with oligometastatic disease, stressing also the need for clinical trials in this patient population. At the sixth European Breast Cancer Conference, held in Berlin in March 2008, the second public session on MBC guidelines addressed the controversial issue of whether MBC can be cured. In this commentary, we summarize the discussion and related recommendations regarding the available therapeutic options that are possibly associated with cure in these patients. In particular, data on local (surgery and radiotherapy) and chemotherapy options are discussed. Large retrospective series show an association between surgical removal of the primary tumor or of lung metastases and improved long-term outcome in patients with oligometastatic disease. In the absence of data from prospective randomized studies, removal of the primary tumor or isolated metastatic lesions may be an attractive therapeutic strategy in this subset of patients, offering rapid disease control and potential for survival benefit. Some improvement in outcome may also be achieved with optimization of systemic therapies, possibly in combination with optimal local treatment. © 2010. Do students engage in offsetting behavior, adapting their study effort to the difficulty of learning? The authors present the results of survey research intended to test for the presence of offsetting behavior at a regional university. Instead of trying to determine whether students study less when learning is easier, we check to see whether students study more when learning is harder. Our results strongly indicate the presence of offsetting behavior, though the incidence of the effect varies systematically by gender, year in school, and major. Abstract

As molecular-based medicine takes center stage due to the increasing knowledge of breast cancer molecular biology treatment tailoring is no longer a dream for the future but the main goal of current research. This book provides an overview of the most recent techniques, agents and approaches for breast cancer contributing to the individualization of treatment. Current biomedical research focuses on facilitating the transfer of molecular biology knowledge into the clinical management of patients, leading to increased survival as well as improved quality of life. Particular attention is given here to organ-specific tailored approaches, specific populations, patients' preferences and rehabilitation. Also discussed in depth are the respective roles of pharmacotherapeutics modelling, functional imaging, molecular staging, genomics and proteomics and their impact on the "re-classification" of breast cancer, as well as the clinical applications of basic science. Abstract

PURPOSE: Multiagent chemotherapy and chemohormonal therapy for breast cancer are associated with an increased risk for venous thromboembolic complications. We observed instances of arterial thrombosis in two studies of breast cancer involving multiagent chemotherapy for stages II and III disease. Our purpose in this study was to determine the incidence of this complication and whether it appeared to be related to the chemotherapy or was a random event. PATIENTS AND METHODS: Episodes of arterial thrombotic events were identified from record reviews of 1,014 assessable patients with breast cancer entered on two Cancer and Leukemia Group B protocols. Details of the kind of arterial event, when it occurred, the outcome, and the occurence of metastases were analyzed. RESULTS: Thirteen (1.3%) patients had an arterial thrombosis: six (5.3%) of 113 patients with state II disease and seven (0.8%) of 901 patients with state II disease. Four of these patients had a peripheral arterial thrombosis and nine had strokes (four were fatal). All these events occurred while the patients were receiving adjuvant chemotherapy. Only one additional arterial event (a stroke approximately four years later) has occurred in this patient group after chemotherapy was completed. CONCLUSION: Arterial thrombosis is also associated with multiagent chemotherapy in patients with breast cancer. The mechanism is unknown., Blockage of the arteries by clots, or arterial thrombosis, has been observed with increasing frequency in women receiving chemotherapy for breast cancer. The incidence of venous thrombosis during chemotherapy has already been established. To determine the incidence of arterial thrombotic events and whether or not they are related to chemotherapy, 1,014 women receiving these treatments were monitored. This group included 113 women with clinical stage III breast cancer and 901 women with stage II breast cancer and lymph node involvement. Of the stage II group, seven patients (eight percent) experienced a thrombotic event; four were peripheral thromboses (clots in a leg artery) and three were cerebral thromboses (strokes). Six of the stage III patients (six percent) experienced a stroke. The incidence of arterial thromboses in this group of chemotherapy patients is higher than would occur in a similar group of women with or without cancer. This conclusion is based on a statistical analysis of the frequency of arterial thrombosis in patients at several major medical centers. Chemotherapy for breast cancer is associated with an increased risk of arterial thrombosis, but the mechanism of this event is not known.
