204. Covalent attachment of active enzymes to upconversion phosphors allows ratiometric detection of substrates

Abstract

Upconverting phosphors (UCPs) convert multiple low energy photons into higher energy emission via the process of photon upconversion and offer an attractive alternative to organic fluorophores for use as luminescent probes. Here, UCPs were capped with functionalized silica in order to provide a surface to covalently conjugate proteins with surface?accessible cysteines. Variants of green fluorescent protein (GFP) and the flavoenzyme pentaerythritol tetranitrate reductase (PETNR) were then attached via maleimide?thiol coupling in order to allow energy transfer from the UCP to the GFP or flavin cofactor of PETNR, respectively. PETNR retains its activity when coupled to the UCPs, which allows reversible detection of enzyme substrates via ratiometric sensing of the enzyme redox state.

Abstract

Objective Amidst COVID-19 disruptions, e-therapy has become even more essential and has rapidly expanded across statutory, private and third sectors to meet growing demands for digital mental health support. A challenge in digital therapeutic care is how to develop and maintain a supportive, collaborative therapeutic relationship, built upon mutual trust and respect; intrinsic values of relationships that are often implied through complex non-verbal cues. Online practitioners are eager to learn how to adapt to online delivery, although platform-specific training is limited. The aim of the current study was to focus upon the therapist experience of online therapeutic relationships with young people, exploring a range of factors through their perspectives, including the impact of anonymity. Methods Eight e-therapy practitioners were recruited from Kooth, an online mental health service. Narrative interviews undertaken via Skype facilitated reflective conversational one-to-one discussions, based upon the practitioners’ individual experiences, led by the interviewee. Following transcription and anonymisation, a narrative analysis was undertaken to explore participants’ experiences, perspectives and reflections. Results Four analytic layers arose from the narratives, which explored the challenging learning experience of translating existing therapeutic skills to online working, rapidly building therapeutic relationships, managing risk in the online therapeutic relationship, and techniques for maintaining a digital therapeutic relationship. Conclusions The study provides novel insights into the flexibility and adjustments therapists can make to improve online interventions and delivery through the development and maintenance of positive therapeutic relationships. Recommendations are also made in relation to platform-specific training, communicative adaptations, risk management and practitioner support.

207. Economic Analysis of Increased Corn Silage Inclusion in Beef Finishing Cattle

Abstract

An economic analysis was conducted to assess the feasibility of feeding greater inclusions of corn silage in finishing diets. Cattle were fed two inclusions of corn silage (15 and 45% of diet dry matter) with or without tylosin. Cattle fed 15% corn silage with tylosin had the best feed conversion, 15 % corn silage without tylosin was intermediate, and both 45% corn silage with and without tylosin had the poorest feed conversion. Feeding corn silage at greater inclusions decreased ADG but increased final body weight when fed to an equal fatness (28 days longer). However, feeding corn silage at 45% was more economical compared to feeding 15% corn silage, especially at higher corn prices, provided shrink is well managed (less than 15%). Feeding elevated concentrations of corn silage may have an economic advantage while also offering the addition of liver abscess control in finishing diets without tylosin.

Abstract

A finishing study was conducted to assess the impact of increasing silage inclusion in finishing diets to reduce the prevalence of liver abscesses in beef cattle. Cattle were fed two inclusions of corn silage (15 or 45% of diet dry matter), with or without tylosin for control of liver abscesses. Cattle fed 15% corn silage had a 2% improvement in feed efficiency when tylosin was added to the diet. However, in cattle fed 45% corn silage, no improvements in feed efficiency were observed when tylosin was added to the diet. Cattle fed 15% corn silage without tylosin, had the greatest prevalence of liver abscesses (34.5%) compared to other treatments, and abscess prevalence was decreased to 19% if tylosin was fed with 15% corn silage. Feeding 45% silage was effective at lowering liver abscess prevalence which was 12.4%, regardless of whether tylosin was fed. Feeding corn silage at 45% of diet dry matter was as effective as feeding tylosin at controlling abscess rates. Feeding corn silage at greater inclusions decreased average daily gain but increased final body weight when fed to an equal fatness (28 days longer). Feeding elevated concentrations of corn silage in diets containing distillers grains may be a viable method to control liver abscesses without antibiotic use, but has performance implications.

Lois Lowry structures the events of her well-known young adult novel The Giver on the hero’s journey cycle. The hero’s journey is not often studied in The Giver, yet its stages highlight and mirror the challenging stages common to young adult readers’ lives. The Giver also refreshes the traditional hero’s journey form by presenting a mental and emotional journey instead of the standard physical quest. This unique form creates a greater connection to The Giver’s target adolescent readers by more closely paralleling the mental and emotional realities of their lives through their journey from childhood to adulthood.

Abstract

The use of digital technologies with the aim to improve African smallholder agriculture is a trend today, and expectations of the benefits and transformative capacity of these technologies are high. In practice, digitalisation comes with trade-offs, and benefits, and potential harm are not equally distributed. This chapter unravels how processes of digitalisation in smallholder agriculture may lead to inclusion and exclusion of people in the present or future. A broad variety of inclusion and exclusion factors are discussed across three levels: specific digital technologies; digital innovation packages; and the digital innovation system. This shows how a complex interplay between access conditions, design choices, and system complexity determine if and how inclusion and exclusion takes place, at what level, for whom, and with what impact. In doing so, the chapter breaks with the normative assumption that inclusion is always positive and exclusion always negative. Instead, when it comes to the use of digital technologies in smallholder agriculture, inclusion and exclusion are more than a binary distinction between ‘who is in’ and ‘who is out,’ or what is ‘good’ and what is ‘bad.’

Abstract

Key Points: Question: Does self-monitoring of blood pressure (BP) by pregnant individuals at higher risk for preeclampsia lead to earlier detection of pregnancy hypertension compared with usual antenatal care? Findings: In this randomized clinical trial that included 2441 pregnant individuals at increased risk for preeclampsia, self-monitoring of BP with telemonitoring compared with usual care resulted in a mean time to clinic-based detection of hypertension of 104 vs 106 days, a difference that was not statistically significant. Meaning: Among pregnant individuals at higher risk of preeclampsia, self-monitoring of BP with telemonitoring did not lead to earlier clinic-based detection of hypertension. Importance: Inadequate management of elevated blood pressure (BP) is a significant contributing factor to maternal deaths. Self-monitoring of BP in the general population has been shown to improve the diagnosis and management of hypertension; however, little is known about its use in pregnancy. Objective: To determine whether self-monitoring of BP in higher-risk pregnancies leads to earlier detection of pregnancy hypertension. Design, Setting, and Participants: Unblinded, randomized clinical trial that included 2441 pregnant individuals at higher risk of preeclampsia and recruited at a mean of 20 weeks' gestation from 15 hospital maternity units in England between November 2018 and October 2019. Final follow-up was completed in April 2020. Interventions: Participating individuals were randomized to either BP self-monitoring with telemonitoring (n = 1223) plus usual care or usual antenatal care alone (n = 1218) without access to telemonitored BP. Main Outcomes and Measures: The primary outcome was time to first recorded hypertension measured by a health care professional. Results: Among 2441 participants who were randomized (mean [SD] age, 33 [5.6] years; mean gestation, 20 [1.6] weeks), 2346 (96%) completed the trial. The time from randomization to clinic recording of hypertension was not significantly different between individuals in the self-monitoring group (mean [SD], 104.3 [32.6] days) vs in the usual care group (mean [SD], 106.2 [32.0] days) (mean difference, −1.6 days [95% CI, −8.1 to 4.9]; P =.64). Eighteen serious adverse events were reported during the trial with none judged as related to the intervention (12 [1%] in the self-monitoring group vs 6 [0.5%] in the usual care group). Conclusions and Relevance: Among pregnant individuals at higher risk of preeclampsia, blood pressure self-monitoring with telemonitoring, compared with usual care, did not lead to significantly earlier clinic-based detection of hypertension. Sharing needles and syringes and other equipment for injecting drugs is the primary route of transmission for hepatitis C virus (HCV) in Australia (Crofts, Jolley, Kaldor, van Beek, & Wodak, [...], This study describes perceptions of hepatitis C (HCV) risk among people who inject drugs (PWID) in Western Australia (WA), and seeks to determine the relationship between perceptions of risk and injecting equipment sharing practices. One hundred and forty five participants recruited from 30 high-distributing pharmacies throughout WA completed surveys during a two-week period. Respondents reported that they did not see themselves as highly susceptible to HCV and did not believe HCV was a severe condition. Forty-three percent of respondents reported sharing injecting equipment in the past month. In multivariate analysis, sharing injecting equipment was associated with lower self-efficacy for sterile equipment use, being unemployed, and being male. These findings suggest that high self-efficacy protects against sharing injecting equipment with others. Psychosocial factors warrant close scrutiny and attention in future research and HCV prevention programs for PWID. Abstract

Background: Isolated lateral compartment knee arthritis is less prevalent than medial. While the reported results of medial unicompartmental knee replacement (UKR) have been good and comparable to total knee replacement, the results of lateral UKR have been mixed. We present the short-term results and survivorship of a fixed-bearing UKR designed specifically for the lateral compartment. Methods: We report the result of 130 primary fixed-bearing lateral Oxford (FLO) UKRs (123 patients) performed between 2015 and 2019 with a minimum follow-up of 1 year. The indications for lateral UKR were: isolated lateral osteoarthritis (n = 122), post-trauma (n = 5) and osteonecrosis (n = 3). The mean age was 69.1 (± 11.6), mean BMI 28.4 (± 4.9), 66.9% female, 60% right-sided, and mean follow-up 3 (range 1–4.8 years, standard deviation ± 1) years. The primary outcome measure was the Oxford knee score (OKS). Survival analysis was performed with "revision for any reason", "reoperation", and "implant failure" as the endpoints. Results: Six patients died from unrelated reasons. None of the implants failed. One required the addition of a medial UKR for medial arthritis. There were no other reoperations. At 4 years, the survival for implant failure was 100% and for both revision and all reoperations was 99.5% (95% CI 96.7–99.9%). At the last review, at a mean of 3 years, the mean Oxford knee score was 41. Conclusion: The good survivorship and outcome scores suggest that UKR designed for the lateral compartment is an excellent alternative to total knee replacement in selected patients with isolated lateral tibiofemoral arthritis at short-term follow-up. Abstract

A 2 × 3 factorial metabolism study using 6 ruminally and duodenally cannulated heifers evaluated the site and amount of nutrient digestion when feeding high protein dry distillers grains plus solubles (DDGS) or conventionally produced DDGS at 30% inclusion compared to feeding no distillers in either dry- rolled or steam- flaked corn diets. Apparent total tract starch digestibility was unaffected by distillers treatment in SFCbased diets, but decreased from 95.1% to 92.0% when DDGS was added to DRC diets, and further decreased to 88.7% for HiPro diets. Dry matter and OM digestibilities were lower types of when either DDGS diets were fed, but no differences were observed between conventional or high protein DDGS. Feeding high protein DDGS did not change digestion compared to conventional DDGS, despite higher CP content. Digestion is greater when cattle are fed steam- flaked corn compared to dry- rolled corn.
