201. Promoting Neonatal Staff Nurses' Comfort and Involvement in End of Life and Bereavement Care

Abstract

Background. Nurses who provide end of life and bereavement care to neonates and their families are potentially at risk for developing stress-related health problems. These health problems can negatively affect nurses’ ability to care for their patients. Purpose. Nurses need to be knowledgeable about end of life and bereavement issues to provide quality care. This study sought to evaluate the effect of a bereavement seminar on the attitudes of nurses regarding end of life and palliative care of neonates. Design. A convenience sample of fourteen neonatal nurses completed a Bereavement/End of Life Attitudes about Care of Neonatal Nurses Scale after a bereavement seminar designed to provide information on end of life care. A pre- and posttest design with an intervention and control group was used to assess changes in nurse bereavement attitudes in relationship to comfort, role, and involvement. Results. After bereavement seminar, the seminar attendees had higher levels of comfort in providing end of life care than nurses in the control group (t=−0.214; P = 0.04). Discussion. Nurses' comfort levels can be improved by attending continuing education on end of life care and having their thoughts on ethical issues in end of life care acknowledged by their peers.

202. Action Recognition by Joint Spatial-Temporal Motion Feature

Abstract

This paper introduces a method for human action recognition based on optical flow motion features extraction. Automatic spatial and temporal alignments are combined together in order to encourage the temporal consistence on each action by an enhanced dynamic time warping (DTW) algorithm. At the same time, a fast method based on coarse-to-fine DTW constraint to improve computational performance without reducing accuracy is induced. The main contributions of this study include (1) a joint spatial-temporal multiresolution optical flow computation method which can keep encoding more informative motion information than recent proposed methods, (2) an enhanced DTW method to improve temporal consistence of motion in action recognition, and (3) coarse-to-fine DTW constraint on motion features pyramids to speed up recognition performance. Using this method, high recognition accuracy is achieved on different action databases like Weizmann database and KTH database.

203. Older Adults Making End of Life Decisions: An Application of Roy's Adaptation Model

Abstract

Purpose. The purpose of this study was to identify variables that influenced completion of advanced directives in the context of adaptation from national data in older adults. Knowledge gained from this study would help us identify factors that might influence end of life discussions and shed light on strategies on effective communication on advance care planning. Design and Method. A model-testing design and path analysis were used to examine secondary data from 938 participants. Items were extracted from the data set to correspond to variables for this study. Scales were constructed and reliabilities were tested. Results. The final path model showed that physical impairment, self-rated health, continuing to work, and family structure had direct and indirect effects on completion of advanced directives. Five percent of the variance was accounted for by the path analysis. Conclusion. The variance accounted for by the model was small. This could have been due to the use of secondary data and limitations imposed for measurement. However, health care providers and families should explore patient’s perception of self-health as well as their family and work situation in order to strategize a motivational discussion on advance directive or end of life care planning.

Abstract

As one of the main active ingredients from Radix Astragali (RA), orally dosed astragaloside IV (AST) is easily transformed to sapogenin-cycloastragenol (CAG) by deglycosylation in the gastrointestinal tract. Because the potential adverse effects of AST and CAG remain unclear, the present study in this article was carried out to investigate the inhibition effects of AST and CAG on UDP-glucuronosyltransferases (UGTs) to explore potential clinical toxicity. An in vitro UGTs incubation mixture was employed to study the inhibition of AST and CAG towards UGT isoforms. Concentrations of 100 μM for each compound were used to initially screen the inhibitory efficiency. Deglycosylation of AST to CAG could strongly increase the inhibitory effects towards almost all of the tested UGT isoforms, with an IC50 of 0.84 μM and 11.28 μM for UGT1A8 and UGT2B7, respectively. Ulteriorly, the inhibition type and kinetics of CAG towards UGT1A8 and UGT2B7 were evaluated depending on the initial screening results. Data fitting using Dixon and Lineweaver–Burk plots demonstrated that CAG competitively inhibited UGT1A8 and noncompetitively inhibited UGT2B7. From the second plot drawn with the slopes from the Lineweaver–Burk plot versus the concentrations of CAG, the inhibition constant (Ki) was calculated to be 0.034 μM and 20.98 μM for the inhibition of UGT1A8 and UGT2B7, respectively. Based on the [I]/Ki standard ([I]/Ki < 0.1, low possibility; 1 > [I]/Ki > 0.1, medium possibility; [I]/Ki > 1, high possibility), it was successfully predicted here that an in vivo herb–drug interaction between AST/CAG and drugs mainly undergoing UGT1A8- or UGT2B7-catalyzed metabolism might occur when the plasma concentration of CAG is above 0.034 μM and 20.98 μM, respectively.

Abstract

In the condition monitoring of roller bearings, the measured signals are often compounded due to the unknown multi-vibration sources and complex transfer paths. Moreover, the sensors are limited in particular locations and numbers. Thus, this is a problem of underdetermined blind source separation for the vibration sources estimation, which makes it difficult to extract fault features exactly by ordinary methods in running tests. To improve the effectiveness of compound fault diagnosis in roller bearings, the present paper proposes a new method to solve the underdetermined problem and to extract fault features based on variational mode decomposition. In order to surmount the shortcomings of inadequate signals collected through limited sensors, a vibration signal is firstly decomposed into a number of band-limited intrinsic mode functions by variational mode decomposition. Then, the demodulated signal with the Hilbert transform of these multi-channel functions is used as the input matrix for independent component analysis. Finally, the compound faults are separated effectively by carrying out independent component analysis, which enables the fault features to be extracted more easily and identified more clearly. Experimental results validate the effectiveness of the proposed method in compound fault separation, and a comparison experiment shows that the proposed method has higher adaptability and practicability in separating strong noise signals than the commonly-used ensemble empirical mode decomposition method.

Abstract

Circulating levels of adiponectin, a hormone produced predominantly by adipocytes, are highly heritable and are inversely associated with type 2 diabetes mellitus (T2D) and other metabolic traits. We conducted a meta-analysis of genome-wide association studies in 39,883 individuals of European ancestry to identify genes associated with metabolic disease. We identified 8 novel loci associated with adiponectin levels and confirmed 2 previously reported loci (P = 4.5×10(-8)-1.2×10(-43)). Using a novel method to combine data across ethnicities (N = 4,232 African Americans, N = 1,776 Asians, and N = 29,347 Europeans), we identified two additional novel loci. 208. An Optimized Design of Single-Channel Beta-Gamma Coincidence Phoswich Detector by Geant4 Monte Carlo Simulations

Abstract

An optimized single-channel phoswich well detector design has been proposed and assessed in order to improve beta-gamma coincidence measurement sensitivity of xenon radioisotopes. This newly designed phoswich well detector consists of a plastic beta counting cell (BC404) embedded in a CsI(Tl) crystal coupled to a photomultiplier tube. The BC404 is configured in a cylindrical pipe shape to minimise light collection deterioration. The CsI(Tl) crystal consists of a rectangular part and a semicylindrical scintillation part as a light reflector to increase light gathering. Compared with a PhosWatch detector, the final optimized detector geometry showed 15% improvement in the energy resolution of a 131mXe 129.4 keV conversion electron peak. The predicted beta-gamma coincidence efficiencies of xenon radioisotopes have also been improved accordingly.

Abstract

Calcium has a pivotal role in biological functions, and serum calcium levels have been associated with numerous disorders of bone and mineral metabolism, as well as with cardiovascular mortality. Here we report results from a genome-wide association study of serum calcium, integrating data from four independent cohorts including a total of 12,865 individuals of European and Indian Asian descent. Our meta-analysis shows that serum calcium is associated with SNPs in or near the calcium-sensing receptor (CASR) gene on 3q13. The top hit with a p-value of 6.3 x 10(-37) is rs1801725, a missense variant, explaining 1.26% of the variance in serum calcium. This SNP had the strongest association in individuals of European descent, while for individuals of Indian Asian descent the top hit was rs17251221 (p = 1.1 x 10(-21)), a SNP in strong linkage disequilibrium with rs1801725. The strongest locus in CASR was shown to replicate in an independent Icelandic cohort of 4,126 individuals (p = 1.02 x 10(-4)). This genome-wide meta-analysis shows that common CASR variants modulate serum calcium levels in the adult general population, which confirms previous results in some candidate gene studies of the CASR locus. This study highlights the key role of CASR in calcium regulation.

Tooth development is a highly heritable process which relates to other growth and developmental processes, and which interacts with the development of the entire craniofacial complex. Abstract

Hyper-immunoglobulin E syndromes (HIES) including compound primary immunodeficiency and nonimmunological abnormalities are characterized by extremely high serum IgE levels, eosinophilia, eczema, susceptibility to infections, distinctive facial appearance, retention of deciduous teeth, cyst-forming pneumonias, and skeletal abnormalities. Itis reported that some cases of familial HIES are relative to autosomal dominant or recessive inheritance, but most cases are sporadic, and result from mutations in the human signal transducer and activator of transcription 3 (STAT3) gene. In this paper, we firstly report a young man diagnosed of Hyper-IgE syndrome with STAT3 mutation in Mainland China, and investigate the autosomal dominant trait of his family members.

Abstract

To identify genetic loci influencing central obesity and fat distribution, we performed a meta-analysis of 16 genome-wide association studies (GWAS, N = 38,580) informative for adult waist circumference (WC) and waist-hip ratio (WHR). We selected 26 SNPs for follow-up, for which the evidence of association with measures of central adiposity (WC and/or WHR) was strong and disproportionate to that for overall adiposity or height. Follow-up studies in a maximum of 70,689 individuals identified two loci strongly associated with measures of central adiposity; these map near TFAP2B (WC, P = 1.9x10(-11)) and MSRA (WC, P = 8.9x10(-9)). A third locus, near LYPLAL1, was associated with WHR in women only (P = 2.6x10(-8)). The variants near TFAP2B appear to influence central adiposity through an effect on overall obesity/fat-mass, whereas LYPLAL1 displays a strong female-only association with fat distribution. By focusing on anthropometric measures of central obesity and fat distribution, we have identified three loci implicated in the regulation of human adiposity.

Abstract

This contribution reviews methods based on scanning probe microscopy for molecular analysis and identification on a length scale of 20–200 nm. Aperture scanning near-field optical microscopy (SNOM) was initially developed in our group for chemical analysis and imaging using fluorescence and Raman spectroscopies, with a resolution beyond the optical diffraction limit. In recent years, tip-enhanced Raman spectroscopy was developed into an analytical tool capable of a spatial resolution less than 50 nm and, using the 'gap' mode of operation, the promise for single molecule sensitivity and even higher spatial resolution. Aperture SNOM with pulsed laser irradiation can be used for nanoscale laser ablation. With an interface to a very sensitive mass spectrometer, this provides a platform for nanoscale mass spectrometry using atmospheric pressure sampling. The preparation of fiber tips for aperture SNOM as well as metallized a FM tips and etched metal wires for aperture less SNOM applications is reviewed. Finally, applications for the chemical analysis of thin molecular films and of biological samples are presented.
