Background: Geriatric depression with subjective memory complaints increases the risk for Alzheimer's Disease. Memantine, a neuroprotective drug, can improve depression and help prevent cognitive decline. In our 6-months clinical trial, escitalopram/memantine (ESC/MEM) improved mood and cognition compared to escitalopram/placebo treatment (ESC/PBO; NCT01902004). In this report, we investigated whether baseline brain white matter integrity in fronto-limbic-striatal tracts can predict clinical outcomes using fractional anisotropy (FA). Methods: Thirty-eight older depressed adults (mean age = 70.6, SD = 7.2) were randomized to ESC/MEM or ESC/PBO and underwent diffusion-weighted imaging (DWI) at 3 Tesla at baseline. Mood was assessed using the Hamilton Depression Rating Scale (HAMD), apathy using the Apathy Evaluation Scale (AES) and anxiety using the Hamilton Anxiety Scale (HAMA) at baseline and 6-months follow-up. FA was extracted from seven tracts of interest (six in each hemisphere and one commissural tract) associated with geriatric depression. Non-parametric General Linear Models were used to examine group differences in the association between FA and symptom improvement, controlling for age, sex, baseline symptom scores and scanner model, correcting for false discovery rate (FDR). Post-hoc tests further investigated group differences in axial, mean and radial diffusivity (AD, MD, and RD, respectively). Lastly, we performed an exploratory whole-brain model to test whether FA might be related to treatment response with memantine. Results: There were no differences in remission rates or HAMD change between groups. In bilateral anterior and posterior internal capsule tracts and bilateral inferior and right superior fronto-occipital (IFO and SFO) fasciculus, higher FA was associated with larger improvements in depressive symptoms for ESC/MEM, but not ESC/PBO, correcting for FDR. Lower MD in the left IFO and RD in the right anterior internal capsule were associated with improved treatment responses. We found no significant associations in the whole-brain analysis. Limitations: Included small sample size and high dropout. Conclusions: Higher baseline FA and lower RD and MD in hypothesized fronto-limbic-striatal tracts predicted greater improvement in mood and anxiety with ESC/MEM compared to ESC/PBO in geriatric depression. FA as a biomarker for white matter integrity may serve as a predictor of treatment response but requires confirmation in larger future studies.

The aim of the analysis was to describe the results of administration of pan-genotype antiviral therapy (glecaprevir / pibrentasvir, GLE / PIB) in real-world setting in three clinical centers in St. Petersburg within the city program for the treatment of chronic hepatitis C. Materials and methods. A retrospective analysis of the GLE / PIB usage of in the period from 2019 to 2022 within the city program for the treatment of chronic hepatitis C in St. Petersburg was carried out. Results. The analysis included 464 patients treated in three clinical centers of St. Petersburg: St. Petersburg State Medical Institution “Clinical Infectious Diseases Hospital named after S. P. Botkin”, St. Petersburg State Medical Institution “Center for the Prevention and Control of AIDS and Infectious Diseases” and the Clinic of Infectious Diseases of the Military Medical Academy named after S. M.Kirov”. Overall 452 out of 464 patients (97 %) achieved SVR12. According to the duration of treatment, SVR12 rates were the following: 8 weeks – 97.7 % (419 / 429), 12 weeks – 92.9 % (26 / 28) and 16 weeks – 100 % (7 / 7). The effectiveness according to fibrosis stage was as follows: F0 – 97 % (142 / 146), F1 – 100 % (74 / 74), F2 – 100 % (59 / 59), F3 – 98 % (57 / 58), F4 (CP-A, B) – 94 % (118 / 125). SVR12 according to HCV genotypes and subtypes was the following: genotype 1b – 100 % (63 / 63), genotype 1a – 91 % (21 / 23), genotype 1 unspecified – 100 % (23 / 23), genotype 2 – 98 % (50 / 51), genotype 3 – 97 % (292 / 301). In patients with an indeterminate genotype, the efficacy was 100 % (7 / 7). Antiviral therapy was well tolerated, there were no cases of discontinuation of therapy, as well as cases of the development of serious adverse events. Conclusion. GLE / PIB has demonstrated high effectiveness in the real-world setting in patients infected with prevalent genotypes of HCV, including those with genotype 3 and compensated liver cirrhosis. The results of our analysis fully correspond to the data obtained earlier in clinical trials andreal-world setting.

Objective. To study the features of invasive aspergillosis (IA) due to A. non-fumigatus versus A. fumigatus in adult (≥ 18 years) recipients of allogeneic hematopoietic stem cell transplantation (allo-HSCT) in 2016-2021. Materials and methods. The study included 33 patients with IA caused by A. non-fumigatus (n = 20) and A. fumigatus (n = 13). A comparative analysis of cases of IA, the results of therapy and outcomes in patients after allo-HSCT in the RM Gorbacheva Research Institute was performed. Diagnostic criteria EORTC / MSGERC 2020 were used. Results. Invasive aspergillosis caused by A. non-fumigatus made up the majority (60.6 %) of IA cases with an identified pathogen registered in patients after allo-HSCT in the period from 2016 to 2021. The main etiological agents in the A. non-fumigatus group were A. niger in 13 (65 %) patients, A. flavus – in 4 (20 %). The median day of diagnosis of A. non-fumigatus IAwas + 110 days (17–2093), for A. fumigatus it was + 46 days (2–866) (p = 0.171). Overall 12-week survival was 55 % and 59.2 % in the A. non-fumigatus and A. fumigatus groups, respectively (p = 0.617). The majority of patients in both the A. fumigatus (n = 10, 77 %) and A. non-fumigatus (n = 16, 80 %) groups received voriconazole as initial antifungal therapy. Second-linetherapy was required in 2 (10 %) patients with A. non-fumigatus IA: liposomal amphotericin B and echinocandins with or with-out posaconazole, and 2 (15 %) patients in the A. fumigatus group: liposomal amphotericin B and voriconazole in combination with echinocandins. A comparative analysis showed that in patients from the two groups, none of the assessed signs (gender, age, underlying disease, disease status at the time of transplantation, time from diagnosis to allo-HSCT, source of hematopoietic stem cells, conditioning regimen, donor type, antifungal prophylaxis, cytomegalovirus reactivation, severe acute and chronic graft-versus-host disease) did not differ significantly. Conclusions. A. niger is the main causative agent of IA caused by A. non-fumigatus. Patients characteristics, their treatment and outcomes did not differ significantly between the A. non-fumigatus and A. fumigatus groups.

The present technology report is aimed at demonstrating how the implementation of VR chemistry simulations of atomic structure enhances learning among students. MEL Chemistry VR lessons were incorporated into the classroom for 43 first-year undergraduate ITMO University students. The visualization techniques provided the learners with two important opportunities: (1) connecting the macroscopic and tangible component of chemistry with the submicroscopic component (the real world and the world of molecules) and (2) connecting the submicroscopic component of chemistry with the representational one (atomic structure and orbital diagrams). The results show that implementation of the VR experience into the atomic structure laboratory enhances the learning experience of the students.

Purpose: the aim of this paper is to identify the problems of low effectiveness of public participation in higher education on the basis of the analysis of theoretical-methodological and empirical literature.Methods: this study is based on the institutional paradigm, through the application of the tools of which the main actors with the potential for participation in higher education, roles, opportunities for influence and interests of participants in relation to the system of higher education are identified. The problems of efficiency of interaction between universities and society were identified and characterized in terms of institutional economics.Results: the directions of organization of social involvement in higher education were defined, the taxonomy of directions of interaction between university and society on the example of Ural State University of Economics was given. Based on the correlation of interests and opportunities for influence, their projection on the potential of interaction between the main stakeholder groups of the higher education system was carried out.The problems of low efficiency of interaction between society and higher education were revealed. The most important problems include the lack of a system of informal institutions that mediate the participation practices of the population, employers, and civil society institutions in the implementation of higher education programs. There is no consistent formalization of the processes of interaction between society and higher education through state regulation of this sphere. The most widespread problem is the lack of an established mechanism (model) of public involvement in the educational process that has the necessary methodological and instrumental support in the state policy in the sphere of education. The right of the public to participate in education management is not obvious and understandable for individuals.Сonclusions and Relevance: the potential of studying the sphere of public participation in higher education requires the creation of an independent research program in order to develop areas for improvement and development of institutions mediating the interaction between universities and society.

The article offers an interpretation of Joseph Brodsky’s poem, rarely used for analysis, “You will return to your homeland. Well...” (1961). If traditionally Brodsky’s elegy in the intertextual aspect is considered as a “dialogue-repulsion” (A. Nesterov) with A. Vertinsky’s romance “Without Women”, then the work reveals other pretexts that at first glance seem alien to Brodsky’s love lyrics — these are texts of patriotic themes by S. Yesenin and M. Tsvetaeva. It is shown that Brodsky’s motive of homecoming is emphatically focused on “The Homecoming” and “The Soviet Russia” by S. Yesenin, in 1924, upon his return from America, formulated a sincere poetic confession in the extreme loneliness he experienced in his homeland (“My poetry is no longer needed here, / And, perhaps, I myself am not needed here either ...”). The prevailing idea of the dissimilarity of the poetry of Brodsky and Yesenin is questioned. The reference text of M. Tsvetaeva is the poem “The Homesickness. A long time ago...” (1934), as shown in the article, which served as an obvious prototype for “You will return to your homeland. Well...” The compositional structure of Tsvetaeva’s pretext with the dominant strategy of “affirmation through negation” determined the poetic organization of Brodsky’s elegy. It is argued that over time, the dialogic interaction of texts was accompanied by emotional and semantic recoding, the non-textual meanings in Brodsky’s poems grew.

Background. Assessment of lung injury in oncohematological patients is a relevant problem, since the spectrum of pathological changes is wide and includes pulmonary infections, tumor cell infiltration, cardiogenic and non-cardiogenic pulmonary edema, bronchiolitis obliterans, interstitial pneumonitis, post-radiation and post-inflammatory pneumofibrosis, pulmonary vasculopathy and pleural effusion. At the moment there are no approved recommendations with criteria of differential diagnosis for these conditions, in particular, with differences between the most common therapy complication represented by pulmonary infections and poorly explored drug-induced toxic lesions.Aim. Identification of criteria for pneumotoxicity, allowing for differential diagnosis with pulmonary infections developing during chemotherapy, according to data routinely obtained in real clinical practice.Materials and methods. The study group included 38 patients with cytotoxic and autoimmune lung injury caused by specific therapy (group 1); the comparison group included 38 patients with infectious lesions receiving the same antitumor drugs (group 2). The data of the anamnesis, clinical course, instrumental studies and standard laboratory tests was studied retrospectively. For statistical analysis, the Mann–Whitney, χ2, Kruskal–Wallis tests were used. ROC analysis was performed to assess the sensitivity and specificity of various factors in relation to toxic damage.Results. Patients with lymphomas predominated in group of toxic lung injury (63 %). In patients who underwent allogeneic hematopoietic stem cells transplantation, toxic complications developed in the period from 35 to 1289 days, infectious – from 4 to 43 days. Statistically significant differences were obtained in the presence of a concomitant state of an altered immune response: 32 % of patients in the toxic lesion group versus 5 % in the infectious group had a history of allergy, and, in contrast to the infectious lesion group, in the toxic lesion group autoimmune diseases were detected. The main symptom in patients of the first group was shortness of breath, which was observed in 68 % of cases, of the second – an increased body temperature, observed in 92 % of cases; cough was also a common symptom – in 19 % and 13 % of patients respectively. In 58 % of patients of the second group, concomitant mucositis was detected, while in the first group this complication did not occur in any of them. The most common radiological pattern (71 % of cases in each group) was ground-glass opacities, in patients of the second group often combined with infiltrative changes and thickening of the bronchial walls (in 53 and 42 % of cases respectively). Among laboratory results, the largest differences between groups were observed in the leukocyte levels (with an average level of 2.5 . 109 / L in the infectious group versus 6 . 109 / L in the toxic group), eosinophils (with an average of 3.6 % in the toxic group versus 1.75 % in the infectious group), C-reactive protein (with an average level of 146.7 mg / L in the infectious group versus 52.4 mg / L in the toxic group), and creatinine (with an average of 0.085 mmol / L in the toxic group versus 0.071 mmol / L in the infectious group).Conclusion. The data obtained in this research indicates the value of taking an anamnesis and the importance of performing additional studies in patients with suspected drug-induced lung injury, as well as identifies risk groups. Based on the revealed differences, a scale for the differential diagnosis of drug-induced toxic and infectious lung damage, which includes the results of publicly available research methods, with high sensitivity and specificity, was proposed. Further research for more specific, but, at the same time, universal for various drugs, criteria for toxic lung damage is relevant.

Since childhood, people have been participating in such dear and memorable events related to the history of family and loved ones. The landscape and architectural space of a solemnly decorated city should take into account the cultural, psychological, and historical part of the positive motivations of residents and visitors. Designers and specialists who creatively designed the city's festive landscape and architectural territory should be able to connect the historical, religious, symbolic, psychological, and social foundations of such a multinational city as Rostov-on-Don. For young people, these holidays represent another opportunity to unite with other participants in understanding the joyful time and the community of people around them.

Videoecology and ecology related to the architectural and landscape spaces of the city are increasingly occupying not only the minds of scientists and architects but also the residents of modern cities themselves, which are increasingly being built with high-rise buildings. The problem of increasing green spaces and organising urban architectural and landscape territory at different times of the year is revealed in a comfortable and environmentally friendly component. Previously, a significant place of historical centres was occupied by private households with adjacent green areas, which are an significant part of the landscaping of settlements, especially cities. The winter-spring period, when nature is not yet filled with spring colours, requires special attention from architects, landscape designers, and other specialists in the urban environment. It is necessary to create an everyday and solemn atmosphere, especially on holidays: traditional, religious, patriotic, and international Women's Day.

Art is capable of uniting people to preserve and protect the environment. Fine art can serve as an effective tool that allows you to convey to a person the vulnerability and beauty of wildlife, and the need to fight for its preservation for future generations. Fine arts and art education promotes the development of students’ love and respect for nature, as well as motivating actions to activate its protection and conservation. Drawing can be considered not only in the context of creative activity, but also as a very effective and significant tool that contributes to the knowledge of the natural environment. Art education allows you to make a significant contribution to environmental education by visualizing nature through art. The drawing contributes to the transformation of the theoretical understanding of environmental problems into visual and accessible images to a wide audience. The study of nature through environmental education creates prerequisites for the development of students' deep understanding of the natural environment and the problems associated with its preservation. Fine arts contribute to raising public awareness of environmental issues, which in turn can stimulate active participation in public activities for environmental awareness. Drawing can be considered as a possible, highly effective means of teaching an understanding of natural beauty. It stimulates the processes of self-knowledge of the surrounding world and contributes to a beneficial effect on the emotional and physical condition of people.

The article aims to analyze the changing role of higher education teachers both in general and in the context of their necessity and ability to participate in fund raising for scientific and educational activities. Using methods of analysis and systematization, the authors examined the role of a teacher in the light of the concept of sustainable development goals developed by the UN General Assembly. Namely, we consider its impact on achieving at least three goals that can also be achieved via raising additional funding to the scientific and educational spheres. The most relevant sources of such additional funding were identified and studied in detail from the authors’ point of view: crowdfunding, targeted capital (endowment fund), and the Russian Science Foundation (RSF). The essence, features, advantages, and relevance of each funding method were studied. As a result of the research, the importance of the participation of higher education teachers in attracting additional funding to higher education institutions was emphasized, and the need for them to acquire new competencies was identified. Based on this, we developed a structure for a professional development course. The course is aimed at increasing the level of awareness of teachers about funding opportunities and mastering tools for fund raising
