201. Behavioural and biochemical alterations by chlorpyrifos in aquatic insects: an emerging environmental concern for pristine Alpine habitats

Abstract

This study aimed to assess how different concentrations of the insecticide chlorpyrifos (1.1, 5.24, 11, 52.4, 110, 262, 524 and 1100 ng L−1) affect the swimming behaviour of Diamesa zernyi larvae following exposure. A video tracking system was employed to analyse two swimming traits (total distance moved and average speed) of the larvae simultaneously after 3 days of exposure to the pesticide at 2 °C. The behavioural results were also interpreted according to biochemical responses to oxidative stress (OS) induced by chlorpyrifos, based on malondialdehyde (MDA) and protein carbonyl (PCC) content. Both distance and speed significantly decreased after 72 h of exposure to chlorpyrifos concentrations of ≥ 110 ng L−1, under which significant OS was detected as lipid peroxidation (level of MDA) and protein carbonylation (level of carbonyl). Analysis of altered swimming behaviour, along with MDA and carbonyl content, indicated that ≥ 110 ng L−1 contamination levels of the insecticide cause the organism to reallocate energy normally used for locomotor activity to repair cell damage, which might explain the strong impairment to locomotor performance. Locomotor performance is an ecologically relevant trait for elucidating the population dynamics of key species, with disturbance to this trait having long-term negative impacts on population and community structure. Therefore, chlorpyrifos insecticides represent a serious ecological risk for mountain aquatic species based on the detrimental effects observed in the current study, as the tested concentrations were those at which the insecticide is found in many Alpine rivers of Italy.

Abstract

Introduction and Aims. People who inject drugs (PWID) constitute the largest reservoir of hepatitis C virus (HCV). Although effective medications are available and access to care is universal in Italy, the proportion of PWID receiving appropriate care remains low. Design and Methods. To identify the major barriers for PWID to HCV treatment we surveyed a large sample of practitioners working in outpatient addiction centres (SerDs). The survey was conducted in two stages and involved 30.3% of SerDs operating in Italy. In the first, SerD physicians completed a questionnaire designed with a Delphi structure. In the second, SerD practitioners completed a targeted questionnaire to identify barriers to four SerD services in HCV management: screening, referral, treatment and harm reduction. Results. The first-stage questionnaire, in which a Delphi and RAND-UCLA method was used, revealed a lack of agreement among the physicians about barriers to health care. The more detailed second-stage questionnaire indicated the barriers to delivering specific SerD services. As regarded the delivery of all four services, the major reasons for treating <50% of patients were: physician and nurse understaffing, technical,economic and logistic issues. In contrast, the practitioners who responded that they follow protocol recommendations often deliver all four services to >50% of patients. Discussion and Conclusions. HCV treatment remains out of reach for many PWID attending a drug treatment centre in Italy. To meet the World Health Organisation (WHO) target, there is a need to increase economic, technical and staff support at treatment centres using the protocols and the universal health care already in place.

Abstract

The study sheds a light on the SCM integration strategies in the healthcare arena. This type of projects tends typically to target the most complex items: (i) medical devices and (ii) operating room materials. Healthcare organizations look for industrial partners capable of offering high quality standards of logistical services; in fact, the key economic benefits are represented by (i) stock’s management optimization and (ii) reduction of warehouse management costs. As for enabling conditions, (i) standardization, (ii) top management commitment and (iii) strategic relevance of the logistical function are widely considered as key aspect for the successful implementation of these projects.

Abstract

Non-stationary continuous time Bayesian networks are introduced. They allow the parents set of each node to change over continuous time. Three settings are developed for learning non-stationary continuous time Bayesian networks from data: known transition times, known number of epochs and unknown number of epochs. A score function for each setting is derived and the corresponding learning algorithm is developed. A set of numerical experiments on synthetic data is used to compare the effectiveness of non-stationary con-Tinuous time Bayesian networks to that of non-stationary dynamic Bayesian networks. Fur-Thermore, the performance achieved by non-stationary continuous time Bayesian networks is compared to that achieved by state-of-The-Art algorithms on four real-world datasets, namely drosophila, saccharomyces cerevisiae, songbird and macroeconomics.

Abstract

The cross section of charm production in γγ collisions σ(e+e−→e+e−cc̄X) is measured at LEP with the L3 detector at centre-of-mass energies from 91 GeV to 183 GeV. Charmed hadrons are identified by electrons and muons from semileptonic decays. The direct process γγ→cc̄ is found to be insufficient to describe the data. Abstract

BACKGROUND: Adding cyclic short sustained inflations (sigh) to assisted ventilation yields optimizes lung recruitment, decreases heterogeneity and reduces inspiratory effort in patients with acute hypoxemic respiratory failure (AHRF). These findings suggest that adding sigh to pressure support ventilation (PSV) might decrease the risk of lung injury, shorten weaning and improve clinical outcomes. Thus, we conceived a pilot trial to test the feasibility of adding sigh to PSV (the PROTECTION study). METHODS: PROTECTION is an international randomized controlled trial that will be conducted in 23 intensive care units (ICUs). Patients with AHRF who have been intubated from 24 h to 7 days and undergoing PSV from 4 to 24 h will be enrolled. All patients will first undergo a 30-min sigh test by adding sigh to clinical PSV for 30 min to identify early oxygenation responders. Then, patients will be randomized to PSV or PSV + sigh until extubation, ICU discharge, death or day 28. Sigh will be delivered as a 3-s pressure control breath delivered once per minute at 30 cmH2O. Standardized protocols will guide ventilation settings, switch back to controlled ventilation, use of rescue treatments, performance of spontaneous breathing trial, extubation and reintubation. The primary endpoint of the study will be to verify the feasibility of PSV + sigh evaluated through reduction of failure to remain on assisted ventilation during the first 28 days in the PSV + sigh group versus standard PSV (15 vs. 22%). Failure will be defined by switch back to controlled ventilation for more than 24 h or use of rescue treatments or reintubation within 48 h from elective extubation. Setting the power to 80% and first-risk order to 5%, the computed size of the trial is 129 patients per arm. DISCUSSION: PROTECTION is a pilot randomized controlled trial testing the feasibility of adding sigh to PSV. Abstract

Glaciers are important fresh-water reservoirs for our planet. Although they are often located at high elevations or in remote areas, glacial ecosystems are not pristine, as many pollutants can undergo long-range atmospheric transport and be deposited on glacier surface, where they can be stored for long periods of time, and then be released into the down-valley ecosystems. Understanding the dynamics of these pollutants in glaciers is therefore important for assessing their environmental fate. To this aim, it is important to study cryoconite holes, small ponds filled with water and with a layer of sediment, the cryoconite, at the bottom, which occur on the surface of most glaciers. Indeed, these environments are hotspots of biodiversity on glacier surface as they host metabolically active bacterial communities that include generalist taxa able to degrade pollutants. In this work, we aim to review the studies that have already investigated pollutant (e.g., chlorpyrifos and polychlorinated-biphenyls (PCBs)) degradation in cryoconite holes and other supraglacial environmental matrices. These studies have revealed that bacteria play a significant role in pollutant degradation in these habitats and can be positively selected in contaminated environments. Abstract

The proteins involved in the autophagy (Atg) pathway have recently been considered promising targets for the development of new antimalarial drugs. In particular, inhibitors of the protein–protein interaction (PPI) between Atg3 and Atg8 of Plasmodium falciparum retarded the blood- and liver-stages of parasite growth. In this paper, we used computational techniques to design a new class of peptidomimetics mimicking the Atg3 interaction motif, which were then synthesized by click-chemistry. Surface plasmon resonance has been employed to measure the ability of these compounds to inhibit the Atg3–Atg8 reciprocal protein–protein interaction. Moreover, P. falciparum growth inhibition in red blood cell cultures was evaluated as well as the cyto-toxicity of the compounds.

Abstract

Several studies have indicated the presence of contaminants in Alpine aquatic ecosystems. Even if measured concentrations are far below those that cause acute effects, continuous exposure to sub-lethal concentrations may have detrimental effects on the aquatic species present in these remote environments. This may lead to a cascade of indirect effects at higher levels of the ecological hierarchy (i.e., the community). To improve the determination of ecologically relevant risk endpoints, behavioural alterations in organisms due to pollutants are increasingly studied in ecotoxicology. In fact, behaviour links physiological function with ecological processes, and can be very sensitive to environmental stimuli and chemical exposure. This is the first study on behavioural alteration in a wild population of an Alpine species. In the present study, a video tracking system was standardized and subsequently used to identify contaminant-induced behavioural alterations in Diamesa zernyi larvae (Diptera, Chironomidae). Diamesa zernyi larvae, collected in an Italian Alpine stream (Rio Presena, Trentino Region), were acclimated for 24 h and successively exposed to several aquatic contaminants (pesticides: chlorpyrifos, metolachlor, boscalid, captan; pharmaceuticals: ibuprofen, furosemide, trimethoprim) at concentrations corresponding to their Lowest Observed Effect Concentration (LOEC). After 24, 48, 72, and 96 h of exposure, changes in the distance moved, the average speed, and the frequency of body bends were taken to reflect contaminant- and time-dependent effects on larval behaviour. In general, metolachlor, captan, and trimethoprim tended to reduce all the endpoints under consideration, whereas chlorpyrifos, boscalid, ibuprofen, and furosemide seemed to increase the distances moved by the larvae. 233. Linking sub-individual and supra-individual effects in Daphnia magna exposed to sub-lethal concentration of chlorpyrifos

Abstract

The main objective of the present study was to investigate possible links between sub-individual and supra-individual levels (i.e. population level) biomarkers in D. magna exposed to sublethal concentrations of the insecticide chlorpyrifos (CPF). To achieve the aim, 8-day old individuals were exposed for 96 h to two environmentally relevant concentrations of CPF (50 and 250 ng/L). Sub-individual level effects were investigated by measuring the activity of antioxidant (SOD, CAT, and GPx) and detoxifying (GST) enzymes, as well as by measuring the acetylcholinesterase (AChE) inhibition. In addition, the effects at supra-individual level were assessed by using a video-tracking system and analyzing changes in swimming capabilities (i.e. percentage of activity time, distance moved, and velocity). Our data have shown that daphnids exposed to both CPF concentrations were in a condition of stress which was highlighted by changes in both sub- and supra-individual biomarkers. Moreover, our results highlighted that the lowest tested CPF concentration did not modulate the antioxidant and detoxifying enzymes, whereas, an inhibition of AChE and a decrease of some parameters related to swimming behaviour (distance moved and velocity) were noted. On the contrary, significant changes in all the sub-individual biomarkers were measured at the highest tested concentration. In addition, organisms recovered the movement capability (distance moved) and also activate a mechanism of avoidance (increased swimming velocity). On the other hand, a reduction in the percent of active time was measured and this was attributed to the energy spent by organisms to activate antioxidant and detoxifying enzymes and the mechanism of avoidance. Based on these results, our study suggests the existence of a link between sub- and supra-individual levels, as the activation or non-activation in the antioxidant and detoxifying enzymes activities can led to different modifications of the swimming behaviour in D.

Abstract

Background: Lean practices have been widely used by health care organizations to meet efficiency, performance and quality improvement needs. The lean health care literature shows that the effective implementation of lean requires a holistic system-wide approach. However, there is still limited evidence on what drives effective system-wide lean implementation in health care. The existing literature suggests that a deeper understanding of how lean interventions interact with the organizational context is necessary to identify the critical variables to successfully sustain system-wide lean strategies. Purpose and methodology: A multiple case study of three Italian hospitals is conducted with the aim to explore the organizational conditions that are relevant for an effective system-wide lean implementation. A conceptual framework, built on socio-technical system schemas, is used to guide data collection and analysis. Findings: The analysis points out the importance to support lean implementation with an integrated and coordinated strategy involving the social, technical, and external components of the overall hospital system.

Abstract

PURPOSE: The Memorial Anxiety Scale for Prostate Cancer (MAX-PC) is a self-report questionnaire that was developed in English to assess prostate cancer (PCa)-related anxiety. The aim of this study was to perform a cultural adaptation for the tool to be used in a population of Italian men on active surveillance (AS).METHODS: A total of 222 patients with localized PCa who were recruited for the Prostate Cancer Research International: Active Surveillance (PRIAS) protocol completed the MAX-PC. Psychometric analysis was performed to assess reliability indexes. A Spearman rank correlation was used to test the association between MAX-PC scales and other questionnaires and was used for longitudinal analysis.RESULTS: Cronbach coefficients and item to total correlation demonstrated good internal consistency. Some items related to the repetition of the PSA test showed a large floor effect and thus were poorly effective in measuring anxiety for PSA testing in patients on AS. Confirmatory factor analysis partly failed to reproduce the structure of the original version. A modified version of MAX-PC, excluding the items with a large floor effect, was thus considered for AS patients. Factor analysis on this version demonstrated considerable consistency with the presence of 3 subscales: anxiety related to PCa, anxiety related to PSA testing, and anxiety related to the fear of tumor progression. Longitudinal analysis showed an acceptable validity over time. The MAX-PC was correlated with the anxious preoccupation subscale of the Mini-Mental Adjustment to Cancer scale.CONCLUSIONS: A slightly modified version of the MAX-PC was developed for use in Italian men on AS. This instrument appears to be a valid and reliable tool that measures anxiety in men with PCa who are enrolled in AS programs.

Abstract

Italian and American hospitals, in two different periods, have been urged by external circumstances to extensively redesign their quality improvement strategies. This paper, through the use of a survey administered to chief quality officers in both countries, aims to identify commonalities and differences between the two systems and to understand which approaches are effective in improving quality of care. In both countries chief quality officers report quality improvement has become a strategic priority, clinical governance approaches, and tools—such as disease-specific quality improvement projects and clinical pathways—are commonly used, and there is widespread awareness that clinical decision making must be supported by protocols and guidelines. Furthermore, the study clearly outlines the critical importance of adopting a system-wide approach to quality improvement. To this extent Italy seems lagging behind compared to US in fact: (i) responsibilities for different dimensions of quality are spread across different organizational units; (ii) quality improvement strategies do not typically involve administrative staff; and (iii) quality performance measures are not disseminated widely within the organization but are reported primarily to top management. On the other hand, in Italy chief quality officers perceive that the typical hospital organizational structure, which is based on clinical directories, allows better coordination between clinical specialties than in the United States. In both countries, the results of the study show that it is not the single methodology/ model that makes the difference but how the different quality improvement strategies and tools interact to each other and how they are coherently embedded with the overall organizational strategy.

Abstract

Three new ureido-pyridazinone derivatives, which are structurally related to the known STAT3 inhibitor AVS-0288, were designed by taking into account the structure-activity relationships determined for several ureido-oxadiazole derivatives previously studied by our group. Their synthesis was first attempted through suitable 5-aminopyridazinone intermediates (6a and 6b), which molecular structures were confirmed by means of X-ray diffraction data on 6a. Amine functionalization was unsuccessful, therefore, an alternative method was devised. Dual-luciferase and AlphaScreen-based assays were used to test their activity. The obtained data were rationalized on the basis of a modeling study, which focused our attention on the geometrical preferences of the ureido moiety. Computational results seem to indicate that both the 1,2,5-oxadiazole ring and the extended ZZ arrangement are essential and probably act in a synergistic way to confer significant activity against STAT3.

Abstract

Novel GnRH I and II analogues were designed and synthesized by Solid Phase Peptide Synthesis (SPPS), since GnRH has antiproliferative properties, but poor metabolic stability. To rationalize synthetic difficulties, molecular dynamics simulations were performed, showing the conformational behavior of three derivatives. Among the two series of peptidomimetics (Ie, f and IIe, f were GnRH I and GnRH II analogues, respectively), several compounds (Id-f and IIc-e) showed significant binding affinity. In particular, derivative Ie has an increased metabolic stability with respect to the physiological ligand (Ie t1/2 = 3.96 h versus GnRH I t1/2 = 2.63 h).

Abstract

The current social and climatic changes observed require an urgent revision of urbanization strategies around the\ud world, to reduce environmental and social impacts, as well as to develop the resilience of built environments. The\ud main challenges are the use of constructive systems and appropriate materials, design techniques and construction\ud for specific climatic zones, besides the existent habitational adaptation with new technologies. In developing\ud countries, the low level of quality in architecture and urbanism increase the social vulnerability that affects millions\ud of people who find it difficult to find adequate housing, which in turn is found in precarious conditions. When the\ud governmental housing programmes try to balance out this deficit, the low quality of defined standards lead to highly\ud inadequate houses for the residents, obligating them to make modifications to buildings which are not necessarily\ud prepared for these adaptations, leading to the waste of material and the inefficient use of resources.\ud \ud This Project will use advanced Post-Occupancy Evaluation (POE) techniques to develop methodological analysis\ud procedures together with Social Housing Developments. The analysis will focus on the adaptive and transformative\ud capacities alongside the resilience of the built environment in attendance to the necessities of the residents and the\ud subsequent environmental impact caused by these ongoing transformations. The analysis will be centred on three\ud elements: (i) BUILT ENVIRONMENT (building complex, taking into consideration the scales of the district,\ud neighbourhood and unit, and the relation of impact between the built and natural environments; (ii) AGENTS (agents\ud that interfere with the local social dynamic); (iii) USERS (residents of the complex). The evaluation will focus on\ud social, functional, behavioural and environmental issues of the built environment. For verification, the\ud methodological procedures developed will be applied to a case study in the city of Uberlandia - Brazil, specifically the undertaking of the governmental programme "minha casa, minha vida" located in the western area of the city.

Abstract

The exposure of the Arctic ecosystem to persistent organic pollutants (POPs) was assessed through a review of literature data . Concentrations of 21 chemicals or congeneric groups were estimated for seawater and for different levels of the Arctic food chain (zooplankton, polar cod, ringed seals, and polar bears). The ecotoxicological risk, was estimated by applying the concentration addition (CA) concept. The risk of POP mixtures was absent in fish and very low in seals. By contrast, the risk was two orders of magnitude higher than the risk threshold for adult polar bears and even higher (three orders of magnitude above the threshold) for bear cubs fed with contaminated milk –. For some chemicals (chlordanes, DDTs, dieldrin, HCB, HCHs, PBDE47, PCB153, PCDD, and PFOS) the database available for seals and bears allowed for calculation of a temporal trend from the 1960s . Starting in the 1980s, decreases in concentration for most legacy POPs, were evident, mainly due to international control measures. However, estimated half-lives indicated that concentrations of concern will persist at least up to the end of this century. Moreover, concentrations of the new POPs listed under the Stockholm Convention (e.g. PBDEs and PFOS) began to decrease only recently. These data support the urgent need for implementation of the Stockholm Convention for not yet controlled POPs.
