201. Development of building inventory for northern Pakistan for seismic risk reduction

Abstract: Purpose This paper aims to present the studies which were carried out to determine building typology in Northern Pakistan, which is a seismically active region. Design/methodology/approach A total of 41 towns and cities were surveyed to collect the data of building types. Help was also taken from global positioning system and satellite imagery. Findings In total, 14 different types of buildings were identified in the region based on the structural system and combination of wall and roof materials; each of them was assigned an appropriate designation. The walls in these buildings were made of block, stone or brick, whereas the roof consisted of corrugated galvanised iron sheet, thatched roof, precast concrete planks or reinforced concrete (RC). Only 6 per cent buildings were found to be engineered RC buildings; this indicates a significance proportion of non-engineered building stock in Northern Pakistan. Research limitations/implications The surveys were conducted in some of the selected areas. Other areas are beyond the scope of this work. Practical implications The presence of a huge deficient building stock in Pakistan indicates a major seismic risk. The seismic losses are largely dependent on the earthquake resistance of existing buildings and building stock. An inventory of existing buildings and their types can help in assessing seismic vulnerability of the built environment, which may lead to the development of policies for seismic risk reduction. Originality/value Presently, housing encyclopaedia does not exist in Pakistan. As a result, housing typology in the country is not known. The presented study addresses this gap in part. Housing typology surveys were conducted to study the typical construction practices in the selected areas and to determine the proportions of different building types in the overall building stock.

203. Plastic surgical operative workload in major trauma patients following establishment of the major trauma network in England: A retrospective cohort study

Abstract: Summary Introduction The introduction of major trauma centres (MTCs) in England has led to 63% reduction in trauma mortality. 1 The role of plastic surgeons supporting these centres has not been quantified previously. This study aimed to quantify plastic surgical workload at an urban MTC to determine the contribution of plastic surgeons to major trauma care. Methods All Trauma Audit and Research Network (TARN)-recorded major trauma patients who presented to an urban MTC in 2013 and underwent an operation were identified retrospectively. Patients who underwent plastic surgery were identified and the type and date of procedure(s) were recorded. The trauma operative workload data of another tertiary surgical specialty and local historical plastics workload data from pre-MTC go-live were collected for comparison. Results Of the 416 major trauma patients who required surgical intervention, 29% ( n = 122) underwent plastic surgery. Of these patients, 43% had open lower limb fractures, necessitating plastic surgical involvement according to British Orthopaedic Association Standards for Trauma (BOAST) 4 guidance. The overall plastic surgery operative workload increased sevenfold post-MTC go-live. A similar proportion of the same cohort required neurosurgery ( n = 115; p = 0.589). Discussion This study quantifies plastic surgery involvement in major trauma and demonstrates that plastic surgical operative workload is at least on par with other tertiary surgical specialties. It also reports one centre's experience of a significant change in plastic surgery activity following designation of MTC status. The quantity of plastic surgical operative workload in major trauma must be considered when planning major trauma service design and workforce provision, and for plastic surgical postgraduate training.

Abstract: The prevalence of osteoarthritis (OA) and rheumatoid arthritis (RA) has been poorly documented in the Middle East and North African region, including the State of Qatar. Given that musculoskeletal pain is commonly reported among midlife women, we evaluated the association between self-report of either OA or RA and health-related quality of life (HRQoL) among midlife women in Qatar. In addition, HRQoL among women in Qatar was compared with that of women in the Study of Women's Health Across the Nation (SWAN).A cross-sectional study was conducted among 841 women 40 to 60 years recruited from primary care centers in Qatar. Face-to-face interviews were conducted and included measures of self-reported OA and RA, health-related symptom experience, and HRQoL using the SF-36 health survey.Most women were obese (75.5%) and reported being bothered by aches and stiffness in joints (71.6%). Prevalence of self-reported OA and RA was 4.8% and 4.3%, respectively. OA was significantly associated with reduced physical function (adjusted odds ratio [OR], 2.97; P=0.003). RA was also significantly related to reduced physical function (adjusted OR, 2.94; P = 0.01) and role physical (adjusted OR, 2.67; P = 0.01). When compared with women from the SWAN, women from the current study had significantly lower mean scores for bodily pain (53.0 vs. 68.9, P = 0.0001) and for vitality (49.9 vs. 54.8, P = 0.0001).Self-report of OA or RA was associated with significant disability in our sample. Because symptoms of aches and stiff joints were so frequently reported, arthritis may be under-diagnosed, especially given the high rates of obesity observed.

Abstract: Background Crohn’s perianal fistulas are challenging for patients and clinicians. Many do not respond to available treatments, and despite recommendations by a recent global consensus, there are currently no specific patient-derived quality of life (QoL) tools to measure outcomes including response to treatment, in patients with Crohn’s perianal fistulas. Exploratory qualitative work has demonstrated that Crohn’s perianal fistulas reduce QoL far beyond restricting daily and sexual activities, and a patient-centred, patient-derived tool is needed. We present a patient-reported outcomes measure (PROM) for this complicated disease phenotype. Methods A 35 item draft questionnaire was generated using information from: (a) unstructured qualitative patient interviews, exploring the experience of living with Crohn’s perianal fistulas; (b) a consensus exercise analysing outcomes from a systematic review of studies assessing medical, surgical and combined (medical/surgical) treatment of Crohn’s perianal fistula; and (c) a patient and public involvement meeting. Psychometric properties were assessed including construct validity (by comparison with the Hospital Anxiety and Depression Scale, HADS and the United Kingdom version of the Inflammatory Bowel Disease Questionnaire, UK-IBDQ), and reliability and responsiveness assessed by test-retest analysis. Results Data from 211 patients contributed to the development of the CAF-QoL scale, following the collation of items including patient experience interviews, a systematic review and a consensus exercise involving 187 stakeholders (patients, gastroenterologists, surgeons, IBD specialist nurses, radiologists). Internal consistency was good with Cronbach’s α of 0.88. Analysis aided reduction of the questionnaire to 26 items that demonstrated good internal consistency, good stability (intra-class correlation 0.98) and good construct validity and responsiveness with positive correlation with the UK IBDQ and HADS. Conclusion A disease-specific PROM to assess clinical outcome (i.e. QoL) as baseline and following interventions in patients with Crohn’s anal fistula—the CAF-QoL—is ready for use. Translation and cross-cultural validation will aid wider international dissemination.

Abstract: Aim There is a requirement of an expansive and up to date review of surgical management of inflammatory bowel disease (IBD) that can dovetail with the medical guidelines produced by the British Society of Gastroenterology. Methods Surgeons who are members of the ACPGBI with a recognised interest in IBD were invited to contribute various sections of the guidelines. They were directed to produce a procedure based document using literature searches that were systematic, comprehensible, transparent and reproducible. Levels of evidence were graded. An editorial board was convened to ensure consistency of style, presentation and quality. Each author was asked to provide a set of recommendations which were evidence based and unambiguous. These recommendations were submitted to the whole guideline group and scored. They were then refined and submitted to a second vote. Only those that achieved >80% consensus at level 5 (strongly agree) or level 4 (agree) after 2 votes were included in the guidelines. Results All aspects of surgical care for IBD have been included along with 157 recommendations for management. Conclusion These guidelines provide an up to date and evidence based summary of the current surgical knowledge in the management of IBD and will serve as a useful practical text for clinicians performing this type of surgery.

Abstract: Background Risk assessment of the use of quetiapine during breastfeeding is challenging owing to a paucity of data. Methods A pharmacokinetic study was conducted in lactating women who were taking quetiapine. The primary endpoint was to determine quetiapine concentration profiles in milk and estimated infant exposure levels. Multiple milk and a single blood quetiapine concentrations were determined using a highly sensitive liquid chromatography with tandem mass spectroscopy method. Results Nine subjects receiving fast-release quetiapine (mean dose, 41 mg/d) were analyzed at steady state. The mean milk/plasma drug concentration ratio at 2-hour postdose was 0.47 (SD, 0.50; range, 0.13-1.67). The mean milk concentration of each patient was 5.7 ng/mL (SD, 4.5; range, 1.4-13.9 ng/mL). The mean infant quetiapine dose via milk per body weight relative to weight-adjusted maternal dose was 0.16 % (SD, 0.08; range, 0.04%-0.35%). Conclusions Infant exposure levels to quetiapine via milk are predicted to be very small.

212. PTU-004 Asking about bowel control problems in IBD: results of face-to-face screening versus self-reporting

Abstract: Introduction Patients with IBD have difficulty revealing concerns about bowel control problems to clinicians,1 who do not actively ask about this symptom2 despite clinical guidelines recommending active-case finding in high-risk populations.3 With no available evidence to advise clinicians on how to ask, we aimed to determine the results of face-to-face or self-reported screening to identify faecal incontinence (FI) in IBD patients. We also asked about patients’ desire for interventions to improve continence. FI was defined in this study as: ‘ever having accidental passing of stool, faeces, poo into your underclothes, that you are either unaware of at the time, or unable to control’. Methods This cross-sectional survey used a study-specific questionnaire to screen participants at either face-to-face interview (by clinician/researcher) or anonymously (participant self-completed). Eligibility criteria: 18 to 80 years of age, confirmed diagnosis of IBD, no current fistula, no stoma, any level of disease activity. Disease activity was measured using the Harvey Bradshaw Index or the Simple Clinical Colitis Activity Index. Results Of 1336 participants, 48% were male; mean age 43 years (range 18–80); 55% had Crohn’s Disease (CD), 41% ulcerative colitis (UC), 4% IBD unclassified. FI (occurring ever) was reported by 63% of 772 screened face-to-face and 56% of 564 self-report participants (p=0.012). A total of 38.7% of all respondents expressed interest in an intervention for FI. Patients with CD were more likely to report FI than those with UC (p≤0.05). FI was reported by 49% of participants in remission, and by 59%, 83% and 93% of participants with mild, moderate and severe relapse of IBD respectively (p≤0.001). Conclusions Bowel control problems are very common in patients with IBD (including in remission) and these symptoms can be identified by face-to-face interview and postal screening. 213. OWE-009 Developing a core outcome set for fistulising perianal crohn’s disease

Abstract: Introduction Lack of standardised outcomes hampers effective analysis and comparison of data when comparing treatments in fistulising perianal Crohn’s disease (pCD). Development of a standardised set of outcomes would resolve these issues. This study provides the definitive core outcome set (COS) for fistulising pCD. Methods Candidate outcomes were generated through a systematic review and patient interviews. Consensus was established via a three-round Delphi process using a nine-point Likert scale based upon how important they felt it was in determining treatment success culminating in a final consensus meeting. Stakeholders were recruited nationally and grouped into three panels (Surgeons and Radiologists, Gastroenterologists and IBD specialist nurses, Patients). Participants received feedback from their panel (in the second round) and all participants (in the third round) to allow refinement of their scores. Results A total of 295 outcomes were identified from systematic reviews and interviews that were categorised into 92 domains. 187 stakeholders (response rate 78.5%) prioritised 49 outcomes through a three-round Delphi study. The final consensus meeting of 41 experts and patients generated agreement on an eight domain COS. The COS comprised three patient-reported outcome domains (quality of life, incontinence and a combined score of patient priorities) and five clinician-reported outcome domains (perianal disease activity, development of new perianal abscess/sepsis, new/recurrent fistula, unplanned surgery and faecal diversion). Conclusion A fistulising pCD COS has been produced by all key stakeholders. Application of the COS will reduce heterogeneity in outcome reporting, thereby facilitating more meaningful comparisons between treatments, data synthesis and ultimately benefit patient care.

Abstract: Airline travel is more affordable than ever and likely safer than ever too. Within half a day, a passenger can be on the other side of the world. However, medical care in-flight has been an issue for those with medical conditions and for those who fall sick during a journey. While airlines have the advice of multiple recognized organizations on needs and standards of care, in-flight emergencies occur at various levels. An emergency medical kit (EMK) together with trained cabin crew can be very effective at resolving the minor problems that arise and reducing the risk of escalation. On occasion, an overhead plea may be announced for additional medical expertise. Having the right content in a medical kit is more important in modern day travel, coupled with advances in equipment and passenger expectations. 215. The Medical and Health Humanities in the Middle East: Report on the Meeting of the 2nd International Conference on the Medical Humanities in the Middle East (online) in Doha, Qatar.

Abstract: On April 9 and 10, 2022, over 79 scholars and 230 attendees met online to share their research on the health and medical humanities in the Middle East and North Africa (MENA) region at the 2nd International Conference on the Medical Humanities in the Middle East (online). This meeting was the second convening of experts since the successful 2018 in-person conference in Doha, Qatar at the Sheraton Hotel. The 2022 conference was jointly sponsored by VCUArts Qatar and Weill Cornell Medicine – Qatar, and was convened by Drs. Alan Weber, Byrad Yyelland, and Mohamud Verjee. The diversity and increase in submissions from 2018 to 2022 testify to the growing importance of humanism in medicine in the region. The published abstracts in this special issue of QScience Connect provide a comprehensive overview of the medical and health humanities as they are currently practiced and researched in the Middle East region. For example, the first keynote speech, "Is the Beauty Industry a Virus Invading the Medical Profession?" by Iraqi surgeon and visual artist Dr. Ala Bashir, addressed a critical issue in the region, the growing popularity of cosmetic surgery and the unlicensed and unregulated nature of the industry. The second keynote speech by health humanities professor Paul Crawford (University of Nottingham) entitled "Towards Creative Public Health: The Contribution of the Medical and Health Humanities," provided an overview of recent international initiatives to harness the arts for health education, healing, and wellness. The other presentations from researchers in Kuwait, UK, Jordan, US, Turkey, Israel, Iran, Qatar, Iraq, UAE, India, and Egypt represented the full range of the medical and health humanities that are developing internationally, including the history of medicine, medical sociology and anthropology, narrative medicine, literature and medicine, graphic medicine, healthcare communications, art therapy, the visual arts, film and medicine, and medical ethics. In addition, a panel of premedical and medical students led by Maryam Arabi and Abdallah Tom provided their perspectives on the topic with respect to the educational needs of students. A group of gerontology experts composed of Mark Clarfield, Regina Roller-Wirnsberger, and Desmond O'Neill directed a workshop on publishing research on the health and medical humanities in scientific scholarly journals. Authors Shahd Alshammari and Robin Fetherston gave dramatic readings from their fiction and non-fiction works. Three posters published on the website added to the oral presentations (https://qatar-weill.cornell.edu/event/medical-humanities-in-the-middle-east/posters). Three of the oral presentations spoke to ethics in medical humanities within the Middle East. Banu Buruk and Berna Arda shared the Turkish National Artificial Intelligence Strategy (TNAIS) report which describes methods for determining and initiating national priorities related to AI. This report identifies four ethical values and eight ethical principles worthy of examination since almost one in five AI strategies are applied in the health sciences. The authors discussed TNAIS and concomitant ethical issues, concluding with recommendations for dealing with conflicts as they arise. Alya Al Shakaki then presented on ethical questions related to use of the gene-editing tool, CRISPR-Cas9, which enables "designer babies". CRISPR has been used in China to create babies that are immune to HIV and thereby able to create offspring with similar immunities; however, what happens to individual autonomy in such cases? Scholars of Islamic bioethics ask two questions: which cells will be edited and what is the aim of the editing? Editing confined to one individual without affecting the offspring is considered acceptable but human dignity must be protected. Fahad Ahmed, Yazgı Beriy Altun Güzelderen and Sefik Yurdakul shared their research on publications written by Turkish authors that have been retracted from scientific journals. In a study of PubMEd, Scopus and Web of Science databases, they identified 147 publications that had been removed due to duplication and irrelevant studies. Six presentations were related to the history of medicine in the Middle East. Dmitry Balalykin tied the apodictic method (the method of rational and rigorous proof), typically accepted as the method of knowledge in the natural sciences, to the development of medicine as seen in anatomical dissections, clinical systematization and general pathology in Greek and medieval Islamic medicine. Balalykin cited Galen and Muhammad ibn Zakariya as pivotal influences. Katarzyna Gromek then discussed the history of perfumes as medical agents in early Islamic states; for example, scenting clothes, mostly undergarments, shirts, dresses, and bed linens, was also thought to increase therapeutic health effects, both in the sick and healthy. Fatima Saadatmand continued the historical discussion with a look at mystical applications of arithmetic, Ariṯmāṭῑqῑ in Arabic, in treating disease throughout the 9th to 13th centuries through an examination of ancient texts and modern writings. Abdulnaser Kaadan's historical research moves us into the writings of Avicenna (Ibn Sina) related to the diagnosis and treatment of breast lesions and the relevance of this historical work to current medicine. Amanda Caterina Leong then shared her work on the writings of Qajar Iranian Princess Taj al-Saltana in 19th century Iran, who discussed systemic challenges in reactions to Iran's cholera epidemic and subsequent health care perils related to a corrupt patriarchy. Leong connected this work to current governmental handlings of COVID-19 issues. Finally, Forozan Falahatpishe examined the invisibility of autopsy within Islamic medicine. Of interest to mystics, theologians and philosophers as well as physicians, the autopsy has been historically avoided within the Avestan (ancient Iranian) approach to Islamic medicine because it has been perceived as a violation against the sanctity of the human body. Nevertheless, surgery has thrived within the Islamic world. Art therapy in the Gulf was well represented by two full panels, one of which presented by Trish Bedford, Mowafa Househ, and Dr. Jens Schneider surveyed current art therapy practices including development of an art therapy app for making initial assessments powered by AI. In addition, Michelle Dixon, Natalia Gómez Carlier, Sara Powell, Mariam El-Halawani, and Alan Weber detailed in the paper, "Art Therapy Service Provision during the COVID-19 Pandemic in the Gulf Cooperation Council (GCC)" how services provision shifted abruptly to online telehealth. Natalia Gómez Carlier and Sara Powell additionally reported on their art therapy pilot dyadic (parent/caregiver and child) telemedicine program for children living with Autism Spectrum Disorder. In a panel dedicated to healthcare communications, one paper described the best practices in communication skills with visually impaired patients (Dr. Nahla Khalaf Ali, Dr. Abdulsalam S. Sultan, Muna Hameed Faris, Muna Muneer Ahmed, Mohammed Modar Hameed, and Dr. Marab younis Abdullah Al-Fathy). A paper by Raji Anand and Dr. Sohaila Cheema included usage data that demonstrated that digital tools such as Mailchimp direct-mail campaigns can effectively promote positive public health behaviors. Another successful intervention for public health awareness was described in the panel "On Film and Medicine: Reflections on 'Medfest Egypt', an international 'film for health' forum," chaired by Khalid Ali, Mina El Naggar, and Robert Abrams. Gatherings such as the 2nd International Conference on the Medical Humanities in the Middle East are designed to share the latest research findings among area experts, to help form new research collaborations, and to encourage translational medicine projects in which insights and pilot and full-scale studies of the medical and health humanities can be harnessed to revise medical education curricula, improve training for health sciences students, enhance clinical practice and ultimately improve patient outcomes to create more equitable, satisfying, and effective healthcare systems. Abstract: Dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis plays an important part in the development of depressive symptoms. In this study, the effects of a commercial St. Johnʼs wort extract (STW3-VI), hyperforin, miquelianin, and the selective serotonin reuptake inhibitor citalopram on the expression of genes relevant to HPA axis function were investigated in human neuronal cells. SH-SY5Y cells were treated with STW3-VI (20 µg/mL), hyperforin (1 µM), miquelianin (10 µM), or citalopram (10 µM) in the presence of the glucocorticoid receptor agonist dexamethasone (DEX,10 µM) for 6 h and 48 h, respectively. Quantitative real-time polymerase chain reaction was used to determine the expression of FKBP5 (FK506 binding protein 51), CREB (cAMP responsive element binding protein), GRIK4 (glutamate ionotropic receptor kainate type subunit 4), VEGF (vascular endothelial growth factor), NET (norepinephrine transporter), and ARRB (β-arrestins), promising biomarkers of antidepressant therapy. Using DEX to mimic stress conditions, it was shown that the gene expression pattern of FKBP5, CREB, GRIK4, VEGF, NET, and ARRB2 in SH-SY5Y cells is time- and treatment-dependent. Most pronounced effects were observed for FKBP5: after 6 h of co-incubation, only STW3-VI could reverse the DEX-induced increase in FKBP5 expression, and after 48 h, citalopram, miquelianin, and hyperforin also reversed the glucocorticoid-induced increase in FKBP5 mRNA expression. The effects observed on FKBP5, CREB, GRIK4, VEGF, NET, and ARRB2 are in good correlation with published data, suggesting that this in vitro model could be used to screen the responsiveness of antidepressants under stress conditions.

Abstract: Background:\ud Many IBD patients worry about stoma forming surgery (SFS), sometimes enduring poor bowel-related quality of life to avoid it. Anticipation of SFS and whether expectations match experience is under-reported. This qualitative study explored influences on patients’ SFS decision-making, and compared pre-operative concerns with post-operative outcomes. \ud \ud Methods: \ud We purposively recruited participants with IBD from UK hospital outpatient and community sources, and IBD clinicians from public hospitals. Four focus groups and 29 semi-structured patient participant interviews, and 18 clinician interviews were audio recorded, transcribed, and analysed thematically. Participants had a current temporary, recently-reversed, or permanent stoma, or were stoma naïve. \ud \ud Results:\ud Four themes emerged: Pre-operative concerns and expectations, Patient decision-making, Surgery and recovery, and Long-term outcomes. Participants and clinicians agreed about most pre-operative concerns, that outcomes were often better than expected, and support from others with a stoma is beneficial. Patient decision-making involves multiple factors, including disease status. Some clinicians avoid discussing SFS, and the phrase ‘last resort’ can bias patient perceptions; others recommend early discussion, increasing dialogue when medical management becomes ineffective. The post-operative period is particularly challenging for patients. Stoma acceptance is influenced by personal perceptions and pre- and post-operative clinical and social support.\ud \ud Conclusion:\ud Patients need balanced information on all treatment options, including surgery, from an early stage. Early multi-disciplinary team dialogue about SFS, and contact with others living well with a stoma, could enable informed decision-making. Life with a stoma is often better than anticipated, improving quality of life and control. Ongoing specialist nursing support aids recovery and adjustment.

Abstract: Background Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world. Methods This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials. gov, number NCT02662231. Findings Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58.5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31.2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (1.2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12.3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9.4%] of 7339 patients), middle (549 [14.0%] of 3918 patients), and low (298 [23.2%] of 1282) HDI (p, DFID-MRC-Wellcome Trust Joint Global Health Trial Development Grant, National Institute of Health Research Global Health Research Unit Grant.

229. Carbamazepine interference with an immune assay for tricyclic antidepressants in plasma

Abstract: Background: Drug toxicological screening is commonly used as a diagnostic tool in patients with suspected toxic ingestion. False positive results due to cross-reactive compounds in drug assays may lead to misdiagnosis and mismanagement, especially when child abuse is suspected. Case Report: Two of our patients with history of ingestion of carbamazepine were testes positive on screening with the tricyclic antidepressant immunoassay. The immunoassay's known cross-reactivity for carbamazepine is reportedly as low as 0.3%. Plasma samples of our patients were initially considered positive for tricyclic antidepressants because the cross-reaction of carbamazepine gave tricyclic antidepressant concentrations as imipramine equivalent sufficiently above the assay cut-off point (20 ng/mL). Later, confirmatory urine testing of both patients using high-performance liquid chromatography was negative for tricyclic antidepressants. Abstract: Late presenting and recurrent sternal wound infections post-sternotomy are difficult to treat, with the clinical picture not necessarily reflecting the underlying problem. As a result of our experience, we suggest that these chronic cases should be managed using a different algorithm to acute sternal wound infection. Positron emission tomography combined with computerized tomography (PET-CT) imaging may be potentially useful in enabling accurate localization of disease sites, which guides adequate debridement prior to definitive reconstruction. It may also allow for disease surveillance and monitoring of the response to antimicrobial treatment. We present three cases which support the need for pre-operative imaging using PET-CT.

Abstract: Background: The need for nutrition-sensitive agriculture is well recognized and of growing interest to global development players. Extension and advisory services (EAS), with their established infrastructure, provide a unique opportunity for nutrition interventions to be implemented at scale with significant reach. Objective: To assess current integration of nutrition in EAS, document training provided to EAS agents, and identify challenges and opportunities for the integration of nutrition. Methods: A mixed methodology was used, which included a systematic literature review covering the following databases: PubMed, ISI Web of Science, Agris, Google Scholar, Econlit, and IBSS. In addition, online surveys and semistructured key informant interviews with stakeholders were performed. Data were collected between December 2012 and June 2013. Results: Based on this study, the most common integration of nutrition into EAS is through efforts to increase the availability of nutritious food. The nutrition training of extension agents is often inadequate, particularly in the realm beyond technical agricultural skill. Additionally, a lack of career opportunities discourages EAS agents form engaging with nutrition integration. The major challenges to integrating nutrition into EAS centered on lack of training for agents, unclear organizational mandates, lack of female inclusion, lack of mobility, and systemic challenges between agriculture and nutrition sectors. Key opportunities for integration efforts are engaging communities, creating a demand for nutrition, and use of innovative communications. Conclusions: This study demonstrates a large degree of variability across programs in the integration and implementation of nutrition activities into EAS, providing differing opinions on the feasibility of integration. Although the need for nutrition-sensitive agriculture is known, and there is agreement that EAS would provide a positive framework, there are still challenges impeding a simple integration of nutrition into EAS as a delivery platform.

Abstract: Hypericin is a natural polycyclic quinone found in Hypericum perforatum. Although hypericin reportedly has numerous pharmacological activities, only a limited number of studies have been performed on the absorption and transport characteristics of this compound, presumably because hypericin is a highly lipophilic compound that is poorly soluble in a physiological medium. The major aim of this study was to get a detailed understanding of the exposure and fate of hypericin in the Caco-2 cell system under different experimental conditions. The permeation characteristics of hypericin (5 µM) in the absence or presence of the model flavonoid quercitrin (20 µM) were studied in the absorptive direction, without or with the addition of 10 % FBS to the transport buffer apically. Following the application of hypericin to the apical side of the monolayer, only negligible amounts of the compound were found in the basolateral compartment when the experiment was performed with a transport buffer. The amount of hypericin in the basolateral compartment increased in the presence of quercitrin (from 0 to 4 %). The majority of hypericin was found after cell extraction (44 % in the absence and 64 % in the presence of quercitrin). When 10 % FBS was added to the transport buffer in the apical compartment to improve the solubility of hypericin in the aqueous solution, around 68 % of hypericin was bound to the serum proteins. Under these experimental conditions, the amount of hypericin in the cells/cell membrane was only 13 % in the absence and 18 % in the presence of quercitrin. The low recovery and significant amounts of hypericin found after cell extraction and bound to the surface of the culture dish made a correct estimation of permeability constants impossible. Fluorescence microscopy and imaging analysis revealed that hypericin is mainly accumulated in the cell membrane. The precise mechanism through which hypericin might overcome the hydrophobic barrier of cell membranes remains to be elucidated. However, our experiments demonstrated that regardless of the experimental conditions, the permeation characteristics of hypericin improved in the presence of the model flavonoid quercitrin.

237. Typology and credibility of Internet health websites originating from Gulf Cooperation Council countries

Abstract: The quality of information available on health websites in the Gulf Cooperation Council (GCC) countries has not been comprehensively assessed. From November to December 2012 we retrieved all functional health-related websites (n = 925) originating in GCC countries. Data on authorship, language, date, information content and type of site were recorded. A novel website checklist was developed based on the credibility and trust criteria of the Internet assessment organization Health On the Net Foundation (HON). Only 5 sites (0.5%) fulfilled all checklist categories. All websites except one were in English or Arabic languages. Only 10.1% of websites posted a privacy policy, 2.7% stated the authorship of information, 51.0% disclosed website ownership, 80.6% provided contact details and 58.5% dated information. Only 1.7% reported their advertising policy and 23.5% revealed sponsorships. Within half a day, a passenger can be on the other side of the world. However, medical care in-flight has been an issue for those with medical conditions and for those who fall sick during a journey. While airlines have the advice of multiple recognized organizations on needs and standards of care, in-flight emergencies occur at various levels. An emergency medical kit (EMK) together with trained cabin crew can be very effective at resolving the minor problems that arise and reducing the risk of escalation. On occasion, an overhead plea may be announced for additional medical expertise. Having the right content in a medical kit is more important in modern day travel, coupled with advances in equipment and passenger expectations. The authors address current issues of illness and other relevant conditions and suggest a content enhancement for an onboard EMK. Design: In a crossover design, six fasted pigs received 300 mg/kg IV aspirin followed by no treatment or MDAC (1 g/kg hourly for 6 doses by gastrostomy). Serum salicylate samples were obtained every 30 minutes for 6 hours. Results: The mean peak salicylate concentrations were 47.4[+ or -]6.2 mg/dL and 48.4[+ or -]3.9 mg/dL (P =.74), and the areas under the time-serum salicylate concentration curve over 6 hours were 171,000[+ or -]24,000 mgxminute/L and 188,000[+ or -]18,000 mgxminute/L for the control and treatment arms, respectively (P =.22). This study had a 90% power to detect a 30% difference between arms. Conclusion: MDAC does not enhance the clearance of salicylate after administration of high-dose IV aspirin. Abstract: Aim: There are many surgical treatments used in Crohn's anal fistula, although none is perfect. Decisions about surgery in this condition may be preference sensitive. The aim of this study was to identify what information patients would like in order to make treatment decisions and to explore experiences of making decisions in this setting. Method: A survey was designed based upon qualitative interviews and input from patients and clinicians. It included a long list of informational items to be ranked on a scale of importance, a control preference scale, the decision regret scale, and items exploring preferred information formats. This was distributed through 10 English hospitals to patients with recent surgical treatment for Crohn's anal fistula. Results were analysed using principal component analysis, to identify key informational needs, and other appropriate descriptive statistics. Results: In total 92 questionnaires were returned (response rate 41.8%); 48 (52.5%) respondents were women and 54 (58.7%) had undergone seton insertion. Principal component analysis identified three information needs: wound and aftercare, effect on perianal symptoms, severity of surgery. Decision‐making preferences showed a desire to participate in decision‐making. Median decision regret score was 25/100 (i.e., low). The preferred format for sharing information to support decision‐making was from the surgeon, 80/92 (87.0%), and from a booklet, 58/92 (63.0%). Conclusion: Key informational needs in this condition are wound and after care, effect on perianal symptoms, and severity of surgery. Abstract: Aim: Surgical site infection (SSI) is the commonest postoperative complication worldwide, representing a major burden for patients and health systems. Rates of SSI are significantly higher in low‐ and middle‐income countries (LMICs) but there is little high‐quality evidence on interventions to prevent SSI in LMICs. Method: FALCON is a pragmatic, multicentre, 2 x 2 factorial, stratified randomized controlled trial, with an internal feasibility study, which will address the need for evidence on measures to reduce rates of SSI in patients in LMICs undergoing abdominal surgery. To assess whether either (1) 2% alcoholic chlorhexidine versus 10% povidone‐iodine for skin preparation, or (2) triclosan‐coated suture versus non‐coated suture for fascial closure, can reduce surgical site infection at 30‐days post‐surgery for each of (1) clean‐contaminated and (2) contaminated/dirty surgery. Patients with predicted clean‐contaminated or contaminated/dirty wounds with abdominal skin incision ≥ 5 cm will be randomized 1:1:1:1 between (1) 2% alcoholic chlorhexidine and noncoated suture, (2) 2% alcoholic chlorhexidine and triclosan‐coated suture, (3) 10% aqueous povidone–iodine and noncoated suture and (4) 10% aqueous povidone–iodine and triclosan‐coated suture. The two strata (clean‐contaminated versus contaminated/dirty wounds) are separately powered. Overall, FALCON aims to recruit 5480 patients. The primary outcome is SSI at 30 days, based on the Centers for Disease Control definition of SSI. Conclusion: FALCON will deliver high‐quality evidence that is generalizable across a range of LMIC settings. Abstract: Nairobi, Kenya exhibits a wide variety of micro-climates and heterogeneous surfaces. Paved roads and high-rise buildings interspersed with low vegetation typify the central business district, while large neighborhoods of informal settlements or “slums” are characterized by dense, tin housing, little vegetation, and limited access to public utilities and services. To investigate how heat varies within Nairobi, we deployed a high density observation network in 2015/2016 to examine summertime temperature and humidity. We show how temperature, humidity and heat index differ in several informal settlements, including in Kibera, the largest slum neighborhood in Africa, and find that temperature and a thermal comfort index known colloquially as the heat index regularly exceed measurements at the Dagoretti observation station by several degrees Celsius. These temperatures are within the range of temperatures previously associated with mortality increases of several percent in youth and elderly populations in informal settlements. Abstract: Every patient has a story to tell, and every experienced physician has a bank of stories to recall. A patient's visit is not chance but a search for a cure, amelioration of state, advice, guidance, a prescription, or for seeking reassurance, comfort, and in some circumstances, permission to "be well". From the simplest tale to the most complex, narratives abound. Sometimes, the most intimate information shared with the physician confidante, go dark and deep, with a yearning to "tell all" after a period of suppression. Successful communication and rapport also depends on the carer's response, the degree of concentration, listening skills, body language, eye contact engagement, the patient relationship and empathy. How do we as physicians cope with emotion on both sides when it comes to listening to a narrated story, keeping matters in perspective, recognizing the effects of depression, grieving, anger, forgiveness, or the strength of the patient to be able to face their demons when cowardly acts of abuse have been committed. The professionalism of doctors should always be at the highest level, but individuals vary in their responses. A price may be paid with arising stress, unsolved patient problems, an increase in new ones, and the general challenge of coping. Time may not be the only enemy with modern day medical practice. Does narrative medicine have a place in reducing this dissonance, and will learning to share stories, as well as being a good listener, limit adverse outcomes?

Abstract: Nairobi, Kenya exhibits a wide variety of micro-climates and heterogeneous surfaces. Paved roads and high-rise buildings interspersed with low vegetation typify the central business district, while large neighborhoods of informal settlements or "slums" are characterized by dense, tin housing, little vegetation, and limited access to public utilities and services. To investigate how heat varies within Nairobi, we deployed a high density observation network in 2015/2016 to examine summertime temperature and humidity. We show how temperature, humidity and heat index differ in several informal settlements, including in Kibera, the largest slum neighborhood in Africa, and find that temperature and a thermal comfort index known colloquially as the heat index regularly exceed measurements at the Dagoretti observation station by several degrees Celsius. Abstract: During saffron (Crocus sativus) spice production, large amounts of floral biowaste are generated. It was the aim of this study to develop a value-added product from saffron floral biowaste to be used as a natural cosmetic ingredient. HPLC-PDA-MS analysis of saffron flower extracts revealed the presence of flavonols with the highest amounts in the acetone extract. Kaempferol-3-O-sophoroside was identified as the main flavonoid in the acetone extract (saffron flower acetone extract). Saffron flower acetone extract and kaempferol-3-O-sophoroside were tested in HaCaT cells for potential effects on cell migration, proliferation, and for anti-inflammatory properties. Saffron flower acetone extract concentration dependently (50–200 µg/mL) augmented cell proliferation, as indicated by an increased BrdU-incorporation, while kaempferol-3-O-sophoroside (1–50 µM) had no effect. Furthermore, treatment of HaCaT cells with saffron flower acetone extract, but not with kaempferol-3-O-sophoroside, concentration-dependently increased vascular endothelial growth factor secretion (control 49.72 pg/mL vs. saffron flower acetone extract at 200 µg/mL 218.60 pg/mL). Cell migration was determined using time-lapse microscopy and a modification of the scratch-wound assay in which saffron flower acetone extract significantly improved wound closure compared to the untreated control. Overproduction of the proinflammatory cytokines interleukin-8 and interleukin-6 in HaCaT cells was induced by TNF-α. Kaempferol-3-O-sophoroside (10–50 µM), but not saffron flower acetone extract, inhibited TNF-α-induced IL-8 secretion. The effect was comparable to 10 µM hydrocortisone (positive control). Interestingly, saffron flower acetone extract further increased IL-6 levels in TNF-α-treated HaCaT cells in a concentration-dependent manner. In summary, the pronounced wound healing properties of saffron flower acetone extract present a promising application for the cosmetic industry.

Abstract: Summary\ud \ud CD25 is expressed at high levels on regulatory T (Treg) cells and was initially proposed as a target for cancer immunotherapy. However, anti-CD25 antibodies have displayed limited activity against established tumors. We demonstrated that CD25 expression is largely restricted to tumor-infiltrating Treg cells in mice and humans. While existing anti-CD25 antibodies were observed to deplete Treg cells in the periphery, upregulation of the inhibitory Fc gamma receptor (FcγR) IIb at the tumor site prevented intra-tumoral Treg cell depletion, which may underlie the lack of anti-tumor activity previously observed in pre-clinical models. Use of an anti-CD25 antibody with enhanced binding to activating FcγRs led to effective depletion of tumor-infiltrating Treg cells, increased effector to Treg cell ratios, and improved control of established tumors. Combination with anti-programmed cell death protein-1 antibodies promoted complete tumor rejection, demonstrating the relevance of CD25 as a therapeutic target and promising substrate for future combination approaches in immune-oncology.
