Abstract

Respiratory viruses represent a severe public health risk worldwide, and the research contribution to tackle the current pandemic caused by the SARS-CoV-2 is one of the main targets among the scientific community. In this regard, experts from different fields have gathered to confront this catastrophic pandemic. This review illustrates how nanotechnology intervention could be valuable in solving this difficult situation, and the state of the art of Zn-based nanostructures are discussed in detail. For virus detection, learning from the experience of other respiratory viruses such as influenza, the potential use of Zn nanomaterials as suitable sensing platforms to recognize the S1 spike protein in SARS-CoV-2 are shown. Furthermore, a discussion about the antiviral mechanisms reported for ZnO nanostructures is included, which can help develop surface disinfectants and protective coatings. At the same time, the properties of Zn-based materials as supplements for reducing viral activity and the recovery of infected patients are illustrated. Within the scope of noble adjuvants to improve the immune response, the ZnO NPs properties as immunomodulators are explained, and potential prototypes of nanoengineered particles with metallic cations (like Zn2+) are suggested. Therefore, using Zn-associated nanomaterials from detection to disinfection, supplementation, and immunomodulation opens a wide area of opportunities to combat these emerging respiratory viruses. Finally, the attractive properties of these nanomaterials can be extrapolated to new clinical challenges.

209. Evaluación del efecto residual del Temephos en larvas de Aedes aegypti en Lima, Perú

Abstract

El temephos ha sido usado como la única estrategia de control para Aedes aegypti en Lima durante los últimos años. Objetivo: Evaluar la eficacia residual de temephos para el control de Ae. aegypti en condiciones de campo y laboratorio en Lima, Perú. Materiales y métodos: Se eligieron ocho tanques bajos de concreto (TBC) depósitos predominantemente infestados con Ae. aegypti en el distrito de San Juan de Lurigancho, situado al norte del área suburbana de Lima. Se cuantificó el número de larvas con ayuda de una red de mano y luego fueron devueltas a los TBC. Los TBC fueron tratados con temephos a una dosis de 1 ppm en el día cero (0). Se evaluó la mortalidad de las larvas salvajes a 24, 48 y 72 horas y se hizo un seguimiento periódico cada siete días hasta las 14 semanas. Al mismo tiempo y en forma semanal se recogieron 500 mL de agua de cada TBC tratado y fueron enviados al laboratorio para los bioensayos (exposición de 25 larvas durante 24 a 48 horas y registro de mortalidad). Resultados: Todos los TBC estuvieron infestados (mediana de 43,5 larvas). La mortalidad en los TBC a 24 horas fue 99,7% y no se evidenció recolonización de los TBC hasta la 14.ª semana. Sin embargo, en el laboratorio se evidencia disminución semanal de 11% de la mortalidad desde la 7.ª semana. 220. Application of gamma models for off-peak dwell time in urban transit routes: a case of study in the Greater Metropolitan Area, Costa Rica.

Abstract

The dwell time at bus stops is affected by factors such as the number of passengers, the method of payment, the configuration of the buses, among others. Therefore, to find the necessary balance in public transportation, where the comfort of the users and the operational needs of transit agencies are the best, it is necessary to make an efficient route design and optimize the methods of payment and boarding. Generally, the analysis of this problem is carried out through multivariable regressions, either linear or non-linear, where the variables that are taken into account depend on the focus of each study. In this case, a generalized linear model is used, using the gamma distribution. The application of generalized linear models using the gamma distribution allowed the modeling of the phenomenon studied and determine the factors that significantly affect the stoppage time. It was determined that, when comparing the same type of bus and on the same route, the main factors that directly affect the bus stop time are the number of users, the method of payment and the configuration of the buses. By generating a general model, including all available data, it is possible to evaluate some of the parameters responsible for dwelling time variations. This allows to establish the effect when implementing the use of buses with low floor in this public service, obtaining a decrease of approximately five seconds in each stop that the vehicle makes along its route. Abstract

ObjectiveCardiac disease is the leading cause of indirect maternal mortality. The aim of this study was to analyse to what extent socioeconomic factors influence the outcome of pregnancy in women with heart disease.MethodsThe Registry of Pregnancy and Cardiac disease is a global prospective registry. For this analysis, countries that enrolled ≥10 patients were included. A combined cardiac endpoint included maternal cardiac death, arrhythmia requiring treatment, heart failure, thromboembolic event, aortic dissection, endocarditis, acute coronary syndrome, hospitalisation for cardiac reason or intervention. Associations between patient characteristics, country characteristics (income inequality expressed as Gini coefficient, health expenditure, schooling, gross domestic product, birth rate and hospital beds) and cardiac endpoints were checked in a three-level model (patient–centre–country).ResultsA total of 30 countries enrolled 2924 patients from 89 centres. At least one endpoint occurred in 645 women (22.1%). Maternal age, New York Heart Association classification and modified WHO risk classification were associated with the combined endpoint and explained 37% of variance in outcome. Gini coefficient and country-specific birth rate explained an additional 4%. There were large differences between the individual countries, but the need for multilevel modelling to account for these differences disappeared after adjustment for patient characteristics, Gini and country-specific birth rate.ConclusionWhile there are definite interregional differences in pregnancy outcome in women with cardiac disease, these differences seem to be mainly driven by individual patient characteristics. Adjustment for country characteristics refined the results to a limited extent, but maternal condition seems to be the main determinant of outcome.

Abstract

Background: Cardiac disease is 1 of the major causes of maternal mortality. We studied pregnancy outcomes in women with rheumatic mitral valve disease. Methods: The Registry of Pregnancy and Cardiac Disease is an international prospective registry, and consecutive pregnant women with cardiac disease were included. Pregnancy outcomes in all women with rheumatic mitral valve disease and no prepregnancy valve replacement is described in the present study (n=390). A maternal cardiac event was defined as cardiac death, arrhythmia requiring treatment, heart failure, thromboembolic event, aortic dissection, endocarditis, acute coronary syndrome, and hospitalization for other cardiac reasons or cardiac intervention. Associations between patient characteristics and cardiac outcomes were checked in a 3-level model (patient–center–country). Results: Most patients came from emerging countries (75%). Mitral stenosis (MS) with or without mitral regurgitation (MR) was present in 273 women, isolated MR in 117. The degree of MS was mild in 20.9%, moderate in 39.2%, severe in 19.8%, and severity not classified in the remainder. Maternal death during pregnancy occurred in 1 patient with severe MS. Hospital admission occurred in 23.1% of the women with MS, and the main reason was heart failure (mild MS 15.8%, moderate 23.4%, severe 48.1%; P 1 was an independent predictor of maternal cardiac events. Follow-up at 6 months postpartum was available for 53%, and 3 more patients died (1 with severe MS, 1 with moderate MS, 1 with moderate to severe MR). Conclusions: Although mortality was only 1.9% during pregnancy, ≈50% of the patients with severe rheumatic MS and 23% of those with significant MR developed heart failure during pregnancy. Prepregnancy counseling and considering mitral valve interventions in selected patients are important to prevent these complications.

Abstract

Throughout human existence, the treatment of illnesses has always been a constant concern. Now that the world population has increased and the illnesses have multiplied, the sale of drugs has become one of the main businesses in the world. Like we know, natural medicine is an important alternative in the treatment and prevention of illnesses. Given these assumptions, we have made an important investigation it has allowed us to generate this article. The objective of this article is to explain the how to model an Expert System for the prevention and treatment of illnesses based on the daily food consumption. In summary, the Expert System (ES) that we have modelling, permit you to register as entry symptoms, illnesses, or medical test results from a patient and generates as output a list of alternatives of foods that the patient should consume at breakfast, lunch, and supper. Thus, the patient relieves their symptoms, maintaining where possible their normal food consumption habits. Of course the ES will be responsible for the linkage of the dishes recommended by the system with natural products that relieve the symptoms registered as input. Obviously, for each patient with different symptoms, it is hoped that ES recommend different dishes. A lo largo de la existencia humana, el tratamiento de las enfermedades siempre ha sido una preocupación constante. Ahora que la población mundial se ha incrementado y las enfermedades se han multiplicado, la venta de medicamentos se ha convertido en uno de los principales negocios en el mundo. Así mismo como se sabe, la medicina naturista es una alternativa muy importante en el tratamiento y prevención de las enfermedades. Teniendo en cuenta estas premisas, hemos realizado una importante investigación la misma que nos ha permitido generar el presente artículo. El objetivo del presente artículo es explicar el modelamiento de un Sistema Experto para la Prevención y Tratamiento de Enfermedades basado en el consumo de potajes o dietas diarias En resumen, el Sistema Experto (SE) que hemos modelado permite registrar como entrada síntomas, enfermedades o resultados de exámenes médicos de un paciente y genera como salida un listado de alternativas de platos o potajes que el paciente debe consumir, en su desayuno, almuerzo y cena. De ese modo, el paciente aliviará sus males, manteniendo en lo posible sus normales hábitos de consumo de alimentos. Por cierto el SE será el encargado de establecer el vínculo de los platos o potajes recomendados por el sistema con los productos naturales que alivian los síntomas registrados como entrada. Obviamente, para cada paciente con síntomas diferentes, es de esperar que el SE le recomiende diferentes potajes.

248. Yang–Baxter Algebras, Conformal Invariant Models and Quantum Groups

Abstract

The Yang–Baxter algebras (YBA) are introduced and formulated in a general way stressing graphical methods. Their various physical applications are then exposed: lattice statistical models, integrable field theories and factorizable S-matrices. The Bethe Ansatz (BA) and its generalizations provide the explicit solutions of all these models using the appropiate YBA. The six-vertex model solution is exposed. YB algebras and their associated physical models are classified in terms of simple Lie algebras. ¶It is exposed how these lattice models yield both solvable massive QFT and conformal models in appropiated scaling (continuous) limits within the lattice light-cone approach. ¶The method of finite-size calculations from the BA is exposed as well as its applications to derive the conformal properties of integrable lattice models. It is conjectured that all integrable QFT and conformal models follow in a scaling limit from these YB algebras. ¶To conclude braid and quantum groups are derived from the YBA in the limit of infinite spectral parameter.

Abstract

Background The European League Against Rheumatism (EULAR) and the Group for Research and Assessment of Psoriasis and PsA (GRAPPA) updated their respective recommendations for the management of PsA in 2015. However, these guidelines are primarily based on studies conducted in resource replete countries; hence they may not be applicable to countries in Central and South America, and Africa. Therefore, new ILAR recommendations have been developed for these regions adapted from existing recommendations. Objectives To establish ILAR recommendations for the management of PsA in resource poor settings, particularly Central/South America and Africa, using an expert local panel, adaptation of existing EULAR and GRAPPA treatment recommendations and a new systematic literature review (SLR) for specific issues in resource-poor countries. Methods The ADAPTE Collaboration process1 for guideline adaptation was followed to assess and adapt the EULAR and GRAPPA treatment recommendations for PsA covering the Americas and Africa. The process was conducted according to its three phases: set-up phase (identifying and seeing agreement from a panel of participants from the relevant countries), adaptation phase (defining health questions using the PIPOH tool, assessing the two source recommendations, conducting an SLR to answer health questions not addressed in the two source recommendations, assessing quality of source recommendations, assessing applicability of principles contained in the source recommendations, and drafting adapted recommendations), and finalisation phase (external review, aftercare planning and final production). Results Five principles for the management of PsA were developed addressing 1. Goals of therapy, 2. Assessment of domains, 3. Assessment of relevant comorbidities, 4. Safety of pharmacotherapy and shared decision making, and 5. Frequency of follow up. Six recommendations for the management of PsA were also developed addressing 1. Goals of therapy, 2. Screening and management of tuberculosis, HIV, HBV, HCV, Chagas’ disease, leishmaniasis, leprosy, and other concomitant comorbidities, 3. Frequency of monitoring in resource poor countries, 4. Safety and efficacy of pharmacotherapy in all domains, 5. Efficacy and safety of combination therapy, and 6. Safety and efficacy of biosimilars and intended copies. Conclusions ILAR recommendations for the management of PsA in resource-poor countries are now available, developed by adapting principally the GRAPPA recommendations, but also the EULAR recommendations, supplemented by expert opinion from these regions.
