Introduction: Population aging is one of the greatest challenges of contemporary public health. The growth of the elderly population generates demands for health services increasing their expenses. The investment in Primary Health Care has been a valid option to keep this population active and functional, integrating preventive, curative, individual and collective actions. The evaluation of the use of health services, as well as the users' knowledge regarding integrality, allows us to measure the effectiveness of the system. Objective: To evaluate the knowledge of the elderly in relation to integral care in primary care. Method: A cross - sectional, population - based observational study of the elderly in the city of Alfenas- MG, adscribed to the Family Health Strategy. The initial sample was composed of the elderly of six Basic Health Units, totalizing 211 individuals. Data were collected from medical records related to the years 2015 and 2016, and data were taken from self-report of the use of services related to the same period, collected at a home visit. In the second visit, in 166 elderly, the PCATool questionnaire was applied. To evaluate the agreement between the use of self-reported and recorded health services, Kappa statistics and percentage of concordance, sensitivity, specificity, positive and negative predictive values were applied. The evaluation of the knowledge of the elderly in relation to integrality was made through the descriptive analysis for each question of the integrality domains of PCATool. We calculated the medians of the use of each service and the Prevalence Ratio by Poisson Regression model. Results: The initial sample consisted of 63.5% of the female elderly, with a mean age of 70.24 (± 6.48) years. The agreement between records and reports for dentals attendance showed a higher Kappa magnitude (k = 0,271), a greater balance between sensitivity and specificity (69% and 60%, respectively). Concerning the predictive values, hospitalization had a low positive predictive value (8%), and consultations at the secondary level had a low negative predictive value (37%). The analysis of the knowledge of the elderly in the perspective of completeness revealed that 57% of the sample reports receiving guidance about the changes that occur with aging. This fact contrasts negatively with the "drop prevention" items (20%) and "disability guidelines" (13%), which showed low results, showing that they are little discussed. The measurement of the medians of the use of the services revealed similar behavior in all analyzed items. The association between use and knowledge about completeness was made through the Poisson regression analysis. The results evidenced the non-association between the use and knowledge of the service. Conclusions: the values of agreement between self- report and records in the medical record show the poor quality of the records. The results of the PCATool questionnaire reveal points to be improved in the elderly approach to Primary Care, especially in specific issues of this population. The results point to a non-association between use and knowledge of the Network.

Purposes: To evaluate the sensitivity, specificity, and cutoff points for the ROPScore, which is based on cumulative risk factors for the prediction of retinopathy of prematurity (ROP), in a population of very low birth weight (BW) preterm infants in southern Brazil. Methods: The medical records of all preterm infants with a very low birth weight ≤1,500 g and/or gestational age ≤32 weeks screened for retinopathy of prematurity in two Brazilian institutions between August 2009 and December 2015 were retrospectively reviewed. ROPScores were calculated using birth weight and gestational age, the use of oxygen therapy with mechanical ventilation, and weight gain proportional to birth weight, as measured at postpartum week six and the need for blood transfusions. Results: The study cohort included 322 infants with a mean birth weight of 1181.8 ± 292.5 g and mean gestational age of 29.5 ± 2.3 weeks. The incidences of any stage of retinopathy of prematurity and severe retinopathy of prematurity were 68.3% and 17%, respectively. ROPScore values ranged from 8.7 to 19.9. The best cutoff point for sensitivity and specificity was 11 for any stage of retinopathy of prematurity and 14.5 for severe retinopathy of prematurity. For any stage of retinopathy of prematurity, the sensitivity and specificity of the ROPScores were 98.6% (95% confidence interval = 97.9%-99.3%) and 35.3% (95% confidence interval= 32.3%-38.3%), with a positive predictive value of 76.6% (95% confidence interval= 74.0%-79.2%) and a negative predictive value of 92.3% (95% confidence interval= 90.6%-94.0%). For severe retinopathy of prematurity, the sensitivity was 100% and specificity was 57.3% (95% confidence interval= 54.2%-60.4%), with positive predictive value of 22% (95% confidence interval= 19.4%-24.6%) and negative predictive value of 100%. The cutoff points correctly identified all infants that developed severe retinopathy of prematurity in this cohort. Conclusions: The ROPScore was useful to identify preterm babies at risk for retinopathy of prematurity. In this population, the ROPScore detected all patients at risk for any stage retinopathy of prematurity and severe retinopathy of prematurity. The ROPScore values in this study were similar to those previously described, thereby successfully validating the ROPScore for early detection of retinopathy of prematurity in very low birth weight preterm infants.

The scope of this article is to verify the validity and consistency of entries in medical records and self-reporting about the use of the health services by elderly users of the Health Care Network. It involved a cross-sectional, population-based observational study. Entries in medical, dental, home visit, referral to the secondary sector and hospitalization appointment records for the years 2015 and 2016 were evaluated. The concordance percentage, Kappa coefficient, sensitivity, specificity and predictive values for each item were also analyzed. The highest concordance percentage (81.93%) with a significant Kappa coefficient (p = 0.03) was for hospitalization. The dentistry appointments revealed a higher Kappa coefficient (k = 0.271) with significance of p=0.01. The analyses showed high specificity in the hospitalization records (83%), and a greater sensitivity for home visits (74%). Positive predictive values were low for hospitalization (8%), and negative predictive values were low for medical appointments (17%). The low concordance between the use of the self-reported health service and entries in medical records highlight one of the possible causes of the fragmentation in continuity of care.O objetivo deste artigo é verificar a validade e a concordância entre registros em prontuários e autorrelato do uso dos serviços de saúde por idosos usuários da Rede de Atenção à Saúde.

Agroindustrial biomasses (sugarcane bagasse, green coconut husk and cashew apple bagasse) are used as substrate for producing cellulases by Aspergillus fumigatus on Semi-Solid Fermentation (SSF). The untreated lignocellulosics and fermented sugarcane bagasse are characterized using the National Renewable Energy Laboratory (NREL) protocols analysis as well as through Fourier Transform Infrared Spectroscopy (FTIR), X-ray diffraction (XRD) and scanning electron microscopy (SEM) assays. A two-variable central composite rotatable design (CCRD) coupled to response surface methodology (RSM) with three experiments at center point are used to investigate the influence of initial moisture and pH on carboxymethycellulose and filter paper, i.e., CMCase and FPase activities produced by the Aspergillus fumigatus using all of the three lignocellulosic biomasses. Results show that sugarcane bagasse has the highest cellulose content and the cashew apple bagasse the highest lignin content, approximately 46.0%. Additionally, pH and moisture (linear and quadratic parameters), as well as their combination, play a key role on both CMCase and FPase activities. The highest values for CMCase and FPase are 4.20 U/g and 0.64 U/g for sugarcane bagasse; 4.07 U/g and 0.28 U/g for coconut husk and 0.64 U/g and 0.32 U/g for cashew apple bagasse. The use of SSF for producing cellulases can be an option for reducing energy consumption due to reduction on the steps during biomass pretreatment.

Parkinson’s disease (PD) is a common neurodegenerative disorder causing a remarkable burden at the individual, family, social, and economic levels. Several risk and protective factors have been recently identified, providing potential for the research and implementation of preventive strategies. Although most cases remain sporadic, various monogenic forms of PD have been described, including autosomal dominant (e.g., LRRK2, SNCA, VPS35, EIF4G1, CHCHD2), autosomal recessive (e.g., parkin, PINK1, DJ-1, DNAJC6), and X-linked (e.g., RAB39B). The pathophysiology of PD is still intriguing, with several recent concepts and theories, including evidence that disease pathology might spread along the various neural systems and regions as a prion protein. Thorough scientific knowledge and clinical experience are required to establish the diagnosis correctly, and novel criteria have been freshly proposed to aid clinicians in this task. This process implies also effectively distinguishing PD from less common parkinsonian disorders including Kufor-Rakeb syndrome, Perry syndrome, a few degenerative ataxias and spastic paraplegias, and several forms of neurodegeneration with brain iron accumulation (NBIA), among others. Treating PD is a challenging enterprise, as the various options should be considered, and often rerouted, taking into account disease stage, motor and non-motor symptoms, and non-PD concomitant patient features. Although general guidelines and strategies are available, it is essential to tailor therapy to each patient, so that quality of life is maximized for many years, while minimizing risks and adverse effects. In carefully selected patients, deep brain stimulation, subcutaneous apomorphine, and levodopa/carbidopa intestinal gel should be considered whenever optimized noninvasive strategies are insufficient to guarantee these goals.

Background Neurofibromatosis type 1 (NF1) is a common autosomal dominant disorder, afflicting 1 in 2500–3000 individuals. These patients harbor cafe-au-lait macules, axillary and inguinal freckling, neurofibromas and optic pathway gliomas. Epilepsy, although not usual, is higher in these individuals than in the general population, occurring in 4–10%. Juvenile myoclonic epilepsy (JME) is a generalized, idiopathic epileptic syndrome characterized by myoclonic jerks, occurring mainly on awakening, often associated with generalized tonic-clonic seizures (GTCS). Material and methods A case report and literature review. Results A 19 yo woman was referred due to matinal myoclonic jerks and GTCS. The neurological examination was normal. The first EEG had generalized spikes, although without spike-and-wave pattern. The CT-scan was normal. She was diagnosed as having JME and was started on medication, remaining seizure-free from the age 24, with subsequent normal EEGs. At age 33, her daughter was referred due to learning disabilities and was diagnosed with NF1. Upon a more through physical examination, we also found discrete cafe-au-lait macules in our patients’ forearm, as well as axillar freckling. Genetic testing confirmed the diagnosis of NF1. Conclusions Generalized epilepsy (including JME) is described in 17% of the patients with NF1 and epilepsy. The literature is scarce on the association of JME and NF1, and a causal link cannot be established based on one case. However, this case highlights the relevance of a thorough physical examination in all patients with epilepsy, including a skin examination, as well as a proper imaging protocol in some generalized epilepsy cases.

Objective: To analyze the trend in hospitalization rates for ambulatory care sensitive respiratory diseases (ACSRD) of children under five years of age in Brazil. Design: Time series study of hospitalization rates using secondary information from the Hospital Information System of the Brazil's Unified Health System. Sample: Hospitalizations of children under five years of age living in Brazilian regions. Hospitalizations were separated according to cause: nose and throat infections, asthma, bacterial pneumonias, and lung diseases. Measurements: Rates were calculated for total hospitalizations for ACSRD, and by cause, age group, and region. Trends were analyzed by using means of simple linear regression and were classified into stationary (p ≥.05), ascending (β positive and p <.05) or declining (β negative and p <.05) trends. Results: All regions showed stationary trends in total hospitalization rates for ACSRDs, except in the Southeast, where trends were ascending. Asthma was the only cause that showed a declining trend. Bacterial pneumonias showed stationary trends, while lung diseases and nose and throat infections had predominantly ascending trends. Conclusion: Brazil's hospitalization rates for ACSRDs are high. Investigations into primary health care organization and work processes, as well as socioeconomic, cultural, and geographical factors are necessary. [ABSTRACT FROM AUTHOR]

The ceramic materials were prepared by the sol-gel method and the coating adhesion was analyzed by pull-off bending tests. The wettability of positively or negatively charged surfaces was characterized by contact angle measurements, which also enabled the calculation of the surface free energy through the polar-apolar liquids approach. Both acid and alkaline treatments activated the cp-Ti, whereas Ti6Al4V was only activated by the alkaline treatment. Such treatment led to increased hydrophilicity with inhibition of the fibroblastic response on Ti6Al4V. On the other hand, osteoblastic cells adhered to and proliferated on the positively and negatively charged surfaces. The maximum adhesion strength (~ 3400 N) was obtained with a negative Ti6Al4V-CaTiO3-CaP multilayer surface. The aim of this study was to determine the transmission paths of Neospora caninum in a dairy herd of crossbred cattle. Two hundred and ninety animals were grouped according to the year of their birth to verify the distribution of infection according to age. The blood of 196 cattle was collected thrice, with an average interval of 210 days, to evaluate the existence of horizontal transmission in the herd. To evaluate vertical transmission, the blood of 64 calves was collected prior to ingestion of colostrum and from their mothers at parturition. Moreover, 63 family trees were built. The presence of anti-N. caninum antibodies was detected using an indirect fluorescent antibody test. The chi-square test (χ(2)) with Yates' correction or Fisher's exact test was used to evaluate the relation between the serology and age groups and between the serostatus of cows and their progeny in different calvings. A higher seropositivity (p=0.035) was found in animals born in 2008 compared to those born in 1997-2007. The serological status of only 13 animals presented changes, of which six (3.1%) became seropositive, indicating a low proportion of horizontal transmission. All seropositive cows gave birth to seropositive calves, resulting in 100% vertical transmission. Sixty-three family trees were constructed. In 29 (46%) of these families, there were animals seropositive for N. caninum. Congenital infection in relation to the number of births was estimated from the relation of Mother+ and Daughter+, without significant differences (p=0.84) between the number of births and the transmission of the parasite from infected cows to their progeny. The low proportion of horizontal transmission combined with the high proportion of vertical transmission allowed us to conclude that transplacental transmission is the principal route of N. caninum infection in the herd.

The aim of this study was to characterize the role of quails (Coturnix coturnix japonica) as intermediate hosts for Neospora caninum. Fifty-eight 20-day old quails were individually identified with numbered rings and kept in cages adequate for the species. The birds were fed commercial feed and distilled water ad libitum. They were weighed daily during the first month and every seven days from then on. The birds were randomly distributed into 3 groups; 40 quails (group A) and 8 quails (group B) received 3.5 × 106 and 5 × 106N. caninum tachyzoites, respectively, and 10 quails (group C) received placebo. Four quails from group A and one from group C had their blood collected and were euthanized on the 1st, 3rd, 5th, 7th, 14th, 21th, 30th, 60th, 120th day after infection (DAI), and 4 quails from group B were euthanized on days 60th and 120th DAI. Following euthanasia, the birds were submitted to necropsy, the organs were weighed and fragments were collected for histopathology, immunohistochemistry (IHC) and PCR. On the 60th DAI, two dogs were fed tissue obtained from group A quails, and one dog was fed quail tissues from group B. The dogs’ blood was collected weekly for serology, and their feces were collected daily for 33 days for microscopy and molecular identification of oocysts. Student's t-test was used to compare the weights of the collected organs and the quail and cloacal temperature. No quail presented any clinical signs or died. Splenomegaly and hepatomegaly were the primary necropsy findings during the first week of infection (p

Good collateral circulation (CC) is associated with favorable outcomes in acute stroke, but the best technique to evaluate collaterals is controversial. Single-phase computed tomography angiography (sCTA) is widely used but lacks temporal resolution. We aim to compare CC evaluation by sCTA and multiphase CTA (mCTA) as predictors of outcome in endovascular treated patients. Methods: Consecutive endovascular treated patients with M1 middle cerebral artery (MCA) or terminal intracranial carotid artery (TICA) occlusion confirmed by sCTA were included. Two more CTA acquisitions with 8- and 16-second delays were performed for mCTA. Endovascular thrombectomy was performed independently of the CC status according to a local protocol [Alberta Stroke Program Early CT score (ASPECTS) >6, modified Rankin scale (mRS) score Results: 108 patients were included. Their mean age was 69.6 ± 13 years and their median National Institutes of Health Stroke Scale (NIHSS) score was 17 (interquartile range 8). 79 (73.1%) had M1 MCA and 29 (26.9%) TICA occlusions. The mean time from symptom onset to CTA was 146.8 ± 96.5 min. On sCTA, 50.9% patients presented good CC vs. 57.5% on mCTA. Good CC status in both sCTA and mCTA had a lower 24-hour infarct volume (27.4 vs. 74.8 cm3 on sCTA, p = 0.04; 17.2 vs. 97.8 cm3 on mCTA, p < 0.01). However, only good CC on mCTA was associated with lower 24-hour (5 vs. 8.5, p = 0.04) and median discharge NIHSS (2 vs. 4.5, p = 0.04) scores and functional independency (mRS score Conclusion: CC evaluation by mCTA is a better prognostic marker than CC evaluation by sCTA for clinical and functional endpoints in acute stroke patients treated with endovascular thrombectomy.

Based on the fact that physical activities can be practice properly at the sports facility and realizing the value cocreation theory can contribute to a rapprochement between clients (services users) and services providers to the both get advantages, this study sought to answer the question “value cocreation is used in the management of university sports facilities?â€. The aim of the study was to identify evidences of the interaction blocks between the facilities and their users to establish a cocreative relationship. The Theoretical framework used was the DART (Dialogue, Access, Risk and Transparency) model, developed by Prahalad & Ramaswamy (2004). The method used was the empirical, document analysis, technical visits and interviews with the managers of the Recreation Center at the University of Massachusetts Amherst (Umass) and the Sports Center facilities at the University of São Paulo (CEPE – USP). The results showed that the two facilities had cocreative practices, although their managers are unaware of the theory.

Currently, there is a change in behavior and attitude of customers who come from a passive position to a connected and informed manner, requiring an adaptation of organizations to this new attitude of customers. Organizations of several areas aim to provide unique experiences to their customers and thereby achieve a more effective relationship with stakeholders resulting in mutual benefits. With the growing positioning of the sports industry and the adequacy of physical activity practices to the modern world, the gyms market is growing, turning an interesting object of study. The aim was to analyze how the practices of existing experience and relationships in the value cocreation approaches and theory of stakeholders were present in gyms and how they impact the management of these organizations. The methodology applied was the multiple cases study in the gyms, for data collection, we used sources of evidence for the triangulation of data as proposed by Yin (2010): observation, document analysis and scripts for semi-structured interviews. The results showed that teachers are important stakeholders for the development of the relationship and providers of memorable experiences to the other stakeholders. It is concluded that the gyms practice the value cocreation partially with their stakeholders and unconsciously because they have no knowledge of theory to practice. Challenges in purification and subsequent functionalization of membrane proteins often complicate their biochemical and biophysical characterization. Purification of membrane proteins generally involves replacing the lipids surrounding the protein with detergent molecules, which can affect protein structure and function. Recently, it was shown that styrene–maleic acid copolymers (SMA) can dissolve integral membrane proteins from biological membranes into nanosized discs. Within these nanoparticles, proteins are embedded in a patch of their native lipid bilayer that is stabilized in solution by the amphipathic polymer that wraps the disc like a bracelet. This approach for detergent-free purification of membrane proteins has the potential to greatly simplify purification but does not facilitate conjugation of functional compounds to the membrane proteins. Often, such functionalization involves laborious preparation of protein variants and optimization of labeling procedures to ensure only minimal perturbation of the protein. Here, we present a strategy that circumvents several of these complications through modifying SMA by grafting the polymer with cysteamine. The reaction results in SMA that has solvent-exposed sulfhydrils (SMA-SH) and allows tuning of the coverage with SH groups. Size exclusion chromatography, dynamic light scattering, and transmission electron microscopy demonstrate that SMA-SH dissolves lipid bilayer membranes into lipid nanodiscs, just like SMA. In addition, we demonstrate that, just like SMA, SMA-SH solubilizes proteoliposomes into protein-loaded nanodiscs. We covalently modify SMA-SH-lipid nanodiscs using thiol-reactive derivatives of Alexa Fluor 488 and biotin. Thus, SMA-SH promises to simultaneously tackle challenges in purification and functionalization of membrane proteins.

Human embryonic stem cells (hESCs) in general require coculture with feeder layers in order to remain undifferentiated. However, the use of animal-derived feeder layers is incompatible with the clinical setting. The objective of this work was to investigate whether human menstrual blood-derived mesenchymal cells (MBMCs) can substitute mouse embryonic fibroblasts (MEFs) as a feeder layer for H9-hESCs. Both feeder cell types were isolated and cultured in DMEM F-12 and high glucose DMEM, respectively. After three passages, they were inactivated with mitomycin C. To test MBMC feeder layer capacity, hESCs were grown over MBMCs and MEFs under standard conditions. hESC growth, proliferation, survival, and maintenance of the undifferentiated state were evaluated. hESCs grown over MBMCs preserved their undifferentiated state presenting standard morphology, expressing alkaline phosphatase, transcription factors OCT3/4, SOX2, and NANOG by RT-PCR and SSEA-4 and OCT3/4 by immunofluorescence assays. It is noteworthy that none of the feeder cells expressed these proteins. The average colony size of the hESCs on MBMCs was higher when compared to MEFs ( p < 0.05; mean ± SD, n = 3). Growth factor analysis revealed amplification of the transcripts for FGF-2, BMP4, TGF-β, VEGF, and PEDF by RT-PCR in MBMCs and MEFs before and after inactivation. Furthermore, similar embryoid body formation, size, and morphology were observed in both feeder layers. In addition, EBs expressed marker genes for the three germ layers cultured on both feeder cells. In conclusion, MBMCs are able to maintain hESCs in an undifferentiated state with comparable efficiency to MEFs. Therefore, MBMCs are a suitable alternative to animal-derived feeder layers for growing hESCs.

This study presents a system for expanded bed adsorption for the purification of chitosanasefrom broth extract in a single step. A chitosanase-producing strain was isolated and identifiedas Bacillus cereus C-01 and used to produce chitosanases. The expanded bed adsorptionconditions for chitosanase purification were optimized statistically using STREAMLINETMDEAE and a homemade column (2.6 × 30.0 cm). Dependent variables were defined by thequality criteria purification factor (P) and enzyme yield to optimize the chromatographicprocess. Statistical analyses showed that the optimum conditions for the maximum P were150 cm/h load flow velocity, 6.0 cm settled bed height, and 7.36 cm distributor height.Distributor height had a strong influence on the process, considerably affecting both theP and enzyme yield. Optimizing the purification variables resulted in an approximately3.66-fold increase in the P compared with the value under nonoptimized conditions. Thissystem is promising for the recovery of chitosanase from B. cereus C-01 and is economicallyviable because it promotes the reduction steps

Alternanthera tenella Colla and Amaranthus blitum Linnaeus are weeds that occur in crops and uncultivated areas in the Maringá region of Paraná. In this study, a morphoanatomical analysis of fruit development and the pericarp of A. tenella and A. blitum was made in order to contribute structural information for species identification, fruit classification, and ecological investigations. Flowers and fruits were collected at the campus of the State University of Maringá, Paraná, fixed in glutaraldehyde, sectioned with a rotary microtome and stained with Toluidine blue. The bracteoles and perigone are persistent in the fruits and are structurally different in both species. The achene of A. tenella maintains the same number of cell layers as the ovary, and these layers collapse during the mature phase, except for the inner mesocarp, where U thickened cell walls and crystals remain. The utricle of A. blitum is similar to the ovary in number of cell layers and has aerenchyma when it is ripe. Mature seeds are exotestal, with a collapsed mesotesta and endotesta, and curved embryo. In this study, some structural characters of the perigone and of the fruits were found that could potentially be used to separate the species; unlike the seeds, which were very similar.

The aim of this paper is to investigate in a systematic and comparative way previous results of independent studies on the treatment of genes and gene function in high school textbooks from six different countries. We analyze how the conceptual variation within the scientific domain of Genetics regarding gene function models and gene concepts is transformed via the didactic transposition into school science textbooks. The results indicate that a common textbook discourse on genes and their function exist in textbooks from the different countries. The structure of science as represented by conceptual variation and the use of multiple models was present in all the textbooks. However, the existence of conceptual variation and multiple models is implicit in these textbooks, i.e., the phenomenon of conceptual variation and multiple models are not addressed explicitly, nor its consequences and, thus, it ends up introducing conceptual incoherence about the gene concept and its function within the textbooks. We conclude that within the found textbook-discourse ontological aspects of the academic disciplines of genetics and molecular biology were retained, but without their epistemological underpinnings; these are lost in the didactic transposition. These results are of interest since students might have problems reconstructing the correct scientific understanding from the transformed school science knowledge as depicted within the high school textbooks. Implications for textbook writing as well as teaching are discussed in the paper.

Myrtaceae is commonly known to have an inferior ovary of appendicular, receptacular or mixed origin. Other characters of the ovary, such as the number of carpels, number of locules, vascularisation pattern, number of ovules, placentation and presence of compitum have also been of interest to researchers aiming to better understand the evolutionary history of the tribe. In the present study, aspects of the structure of the inferior ovary of 21 species of Myrteae are analysed and reviewed as potential characters for better understanding the evolutionary history of Myrteae. Flower buds were embedded in historesin and paraplast and sectioned transversely and longitudinally with a rotary microtome. Results suggested that most species have an inferior ovary of appendicular origin and that a compitum, or compitum tissue, is present in all species analysed, differing only in the degree of development. Number of carpels and locules vary, with most species having two locules. Vascular supply is transeptal and axial, the latter being the most common condition in the investigated species. Data presented here enhance current evolutionary understanding of the tribe and its history. Results indicated that the inferior ovary of ancestral Myrteae may has had an appendicular origin, that the presence and nature of the compitum may have a positive effect on fertilisation efficiency and a relationship with number of ovules and that transepetal vascular supply may be taxonomically useful to define large groups such as Pimenta and Eugenia.

This study presents a system for expanded bed adsorption for the purification of chitosanase from broth extract in a single step. A chitosanase-producing strain was isolated and identified as Bacillus cereus C-01 and used to produce chitosanases. The expanded bed adsorption conditions for chitosanase purification were optimized statistically using STREAMLINE(TM) DEAE and a homemade column (2.6 × 30.0 cm). Dependent variables were defined by the quality criteria purification factor (P) and enzyme yield to optimize the chromatographic process. Statistical analyses showed that the optimum conditions for the maximum P were 150 cm/h load flow velocity, 6.0 cm settled bed height, and 7.36 cm distributor height. Distributor height had a strong influence on the process, considerably affecting both the P and enzyme yield. Optimizing the purification variables resulted in an approximately 3.66-fold increase in the P compared with the value under nonoptimized conditions. This system is promising for the recovery of chitosanase from B. cereus C-01 and is economically viable because it promotes the reduction steps.

Background: Collateral circulation (CC) has been associated with recanalization, infarct volume, risk of haemorrhagic transformation and clinical outcome in patients undergoing acute reperfusion therapies. However, its relationship with the development to malignant MCA infarction (mMCAi) has not been evaluated. Our aim was determine the impact of collateral circulation using multiphase CTA (mCTA) on acute phase in the prediction of mMCAi. Methods: Consecutive acute stroke Results: 82 patients were included. Mean age: 65.1 ±13.83 years, median baseline NIHSS 18(IQR 5.7), 67.9% M1 and 32.1% TICA occlusions, 53 patients received ET and 57 iv tPA, 15 patients develop a mMCAi. In the univariate analysis, patients with mMCAi had lower CC scores (2.29 Vs. 3.71 p=0.001), higher baseline NIHSS (19.86 Vs. 15.70 p=0.016), lower TIMI reperfusion scores (0 Vs. 2.79 p=00.5) and presence of TICA occlusion was more often compared with M1 occlusion (71% Vs. 11.9%, p=0.033) ET was associated with lower rate of mMCAi development as compared with only i.v. reperfusion treatment (9.4%Vs.29.6%, p=0.028). Furthermore, all patients with poor CC who did not recanalize developed mMCAi (6 Vs. 0, p=0.68) On the multivariate analysis adjusted to age, vessel occlusion, baseline NIHSS and recanalization, the presence of poor CC by mCTA was the only independent predictor of mMCAi (p=0.048 OR: 9.72, 95%IC: 1.387-92.53) Conclusion: CC assessment by mCTA independently predicts malignant MCA progression. In patients with persistent occlusion after reperfusion therapies, the presence of poor CC may help in the early malignant MCA detection and management.

Good collateral circulation (CC) associates favourable outcomes on acute stroke patients, but which is the best technique to evaluate it is controversial. Single-phase CTA (sCTA) is widely used, but lacks of temporal resolution, and may mislabel CC. We aim to evaluate a new, quick (not post processing), time resolved technique to evaluate CC: multiphase CTA (mCTA). METHODS: Consecutive RESULTS: 78 patients were included. Mean age: 66.3±13.6y, median NIHSS 17.5 (IQR 6.3), 52 (66.7%) M1- and 26 (33.3%) TICA-occlusions. Mean time from onset to CTA: 2:32±1:31h. On sCTA, 61.8% patients presented good CC whereas on mCTA, 54.7%. Only on mCTA good CC was an independent predictor of low infarct volume at 24h (OR 3.6, CI 95% 1.3-10.5, p=0.017). Moreover, only mCTA-CC status was associated with lower 24h median NIHSS (good CC:5 vs poor CC:17, p Association with outcome was especially significant in patients without REC: among poor CC patients, mRS0-2: 0% in non REC Vs 50% in REC (p In a logistic regression model including age, NIHSS, ASPECTS and REC, only good CC on mCTA predicted favourable outcome (OR 6.8, CI 95% 1.6-29.2, p=0.009). CONCLUSION: CC evaluation on mCTA improves accuracy of clinical and radiological endpoints as compared with sCTA. Good CC on mCTA is an independent predictor of low infarct volume and good outcome, especially if REC is not achieved.
