240. Mortality differences between men and women following first myocardial infarction. RESCATE Investigators. Recursos Empleados en el Síndrome Coronario Agudo y Tiempo de Espera.

Abstract

Context: Mortality after acute myocardial infarction is worse in women than in men, even after adjustment for comorbidity and age dissimilarities between sexes.Objective: To assess the influence of sex on survival after acute myocardial infarction.Design: Inception cohort obtained in a prospective registry of patients with acute myocardial infarction from 1992 through 1994.Setting: Four teaching hospitals in northeastern Spain.Patients: All consecutive patients aged 80 years or younger with first acute myocardial infarction. A total of 331 women and 1129 men were included.Main Outcome Measure: Survival at 28 days and mortality or readmission at 6 months.Results: Women were older (mean, 68.6 vs 60.1 years), presented more often with diabetes (52.9% vs 23.3%), hypertension (63.9% vs 42.3%), or previous angina (44.6% vs 37.4%), and developed more severe myocardial infarctions than men (acute pulmonary edema or cardiogenic shock occurred in 24.8% of women and 10.5% of men) (all P<.02). Men were more likely than women to receive thrombolytic therapy (41.3% vs 23.9%; P<.001), but rates of percutaneous transluminal angioplasty and coronary artery bypass graft surgery at 28 days were similar among men and women. The 28-day mortality rate was significantly higher among women (18.5% for women, 8.3% for men; P<.001). Revascularization procedures at 6 months were performed in a similar proportion of women and men. However, women had higher 6-month mortality rates (25.8% in women, 10.8% in men; P<.001) and readmission rates (23.3% for women, 12.2% for men; P<.001). After adjustment, women had greater risk of death than men at 28 days (odds ratio [OR], 1.72; 95% confidence interval [CI], 1.12-2.65) and at 6 months (OR, 1.73; 95% CI, 1.18-2.52).Conclusions: In this study population, women experienced more lethal and severe first acute myocardial infarction than men, regardless of comorbidity, age, or previous angina. Abstract

The aim of this work is to analyse, implement and test different acquisition algorithms for the European Global Navigation Satellite System Galileo signal. The focus of the work is on the L1 signal and the target of the acquisition algorithms is mainly relevant to a Software Receiver Application. Both GPS signal and Galileo signal have been taken into account in order to perform a comparison between these two systems, in terms of computational burden and acquisition sensibility. The motivation of this work comes from the actual tendency of studying algorithms for the realization of a software receiver for the GPS signals (whose principal characteristics are a good flexibility, when compared to the hardware solution, and the possibility of getting "on the fly" updates), and it's aim is to bring advantages of software-based approach to the new Galileo system, in particular for the acquisition schemes of the L1 signal. The principal fact is that acquisition has to be performed on a normal PC, without using a dedicate hardware. In this context, have been developed different acquisition schemes to provide an analysis on their performance, regarding to a software acquisition. All the schemes are finally compared: then, real advantages of employing BOC (Binary Offset Carrier) digital modulation are investigated, provided that this modulation has been developed to transmit GPS and Galileo signals over the same bands to obtain a greater degree of compatibility and interoperability between the two systems (and the L1 signal represents, in this way, the principal example of such interoperability). 242. Dynamic optical interferometry applied to analyse out of plane displacement fields for crack propagation in brittle materials

Abstract

We propose in this paper, to analyse, the evolution of out-of-plane displacement fields for a crack propagation in brittle materials. As the crack propagation is a complex process that involves the deformation mechanisms, the out-of-plane displacement measurement gives pertinent information about the 3D effects. For investigation, we use the interferometric method. The optical device includes a laser source, a Michelson interferometer and an ultra high-speed CCD camera. To take into account the crack velocity, we dispose of a maximum frame rate of 1Mfps. The experimental tests have been carried out for a SEN (Single Edge Notch) specimen of PMMA material. The crack propagation is initiated by adding a dynamic energy given by the impact of a cutter on the initial crack. The obtained interferograms are analysed with a new phase extraction method entitled MPC [6]. This analysis, which has been developed specially for dynamic studies, gives the out-of-plane displacement with an accuracy of about 10 nm.

245. DNMT3AR882Hmutant and Tet2inactivation cooperate in the deregulation of DNA methylation control to induce lymphoid malignancies in mice

Abstract

TEN-ELEVEN-TRANSLOCATION-2(TET2) and DNA-METHYLTRANSFERASE-3A(DNMT3A), both encoding proteins involved in regulating DNA methylation, are mutated in hematological malignancies affecting both myeloid and lymphoid lineages. We previously reported an association of TET2and DNMT3Amutations in progenitors of patients with angioimmunoblastic T-cell lymphomas (AITL). Here, we report on the cooperative effect of Tet2inactivation and DNMT3Amutation affecting arginine 882 (DNMT3AR882H) using a murine bone marrow transplantation assay. Five out of eighteen primary recipients developed hematological malignancies with one mouse developing an AITL-like disease, two mice presenting acute myeloid leukemia (AML)-like and two others T-cell acute lymphoblastic leukemia (T-ALL)-like diseases within 6 months following transplantation. Serial transplantations of DNMT3AR882HTet2-/-progenitors led to a differentiation bias toward the T-cell compartment, eventually leading to AITL-like disease in 9/12 serially transplanted recipients. Expression profiling suggested that DNMT3AR882HTet2-/-T-ALLs resemble those of NOTCH1mutant. Methylation analysis of DNMT3AR882HTet2-/-T-ALLs showed a global increase in DNA methylation affecting tumor suppressor genes and local hypomethylation affecting genes involved in the Notch pathway. Our data confirm the transformation potential of DNMT3AR882HTet2-/-progenitors and represent the first cooperative model in mice involving Tet2inactivation driving lymphoid malignancies.
