T., 39 years old, considers herself ill since the beginning of 1983. She had a severe cough, at first dry, later with mucopurulent sputum. During the year she was treated as an outpatient, taking antibiotics and cough pills. In December 1983, she was admitted to a hospital in Kazan, but her condition did not improve. In spring 1984 her cough sharply intensified again, body temperature increased, amount of sputum increased, shortness of breath, which at times became suffocation, general weakness, sweating, poor appetite and weight loss became disturbing. Past illnesses: measles, scarlet fever.

Idiopathic pulmonary hemosiderosis is a rare disease of unclear etiology. .In most cases, it occurs in childhood, is characterized by repeated hemorrhages in the lung tissue and hemoptysis, iron deficiency anemia develops in the future. This disease often leads to death from massive pulmonary hemorrhage, sometimes from a pulmonary heart.

Leffler's endocarditis is a rare and little-known disease to a wide range of practitioners. It was first described in 1936 by Leffler and named fibroplastic parietal endocarditis with eosinophilia. It is characterized by heart damage, high blood eosinophilia and leukocytosis. At the same time, symptoms associated with developing eosinophilic vasculitis of various internal organs (lungs, kidneys, spleen, liver, etc.) are observed. With this heart lesion, significant endocardial fibrosis appears, primarily parietal, there is an increase in collagen fibers, infiltration by lymphocytes and eosinophils of the endocardium and myocardium. A large number of parietal blood clots are formed in the cavities of the heart. A sharp thickening of the parietal endocardium and massive thrombotic layers on its surface create an unyielding framework from the inside, which leads to the development of severe rapidly progressive heart failure. The complexity of the diagnosis of the disease is due to the extreme polymorphism of the clinical picture. Leffler's endocarditis should be assumed with a combination of progressive heart failure (if its size is almost normal) with pronounced eosinophilia of the blood.

Smoking is a risk factor for many diseases. In particular, it leads to the development of chronic bronchitis and its complications. However, in spite of the urgency of the research of cardiorespiratory system state in smokers, there are almost no data in the literature about the complex study of this system in this contingent. The aim of this work is to elucidate the mechanisms of early changes in the cardiorespiratory system and to develop methods for their diagnostics in smokers who consider themselves healthy. 325 smokers and 197 non-smokers were examined. Their medical history was studied, ECG, fluoro- and spirography (GEL, SPHF, Votchal-Tiffno index) were performed. Smokers considered themselves healthy, they did not have any complaints.

Marachev made reports on diagnosis, treatment of patients with acute and chronic respiratory failure, on aspects of pulmonary hypertension, treatment of hypoxic conditions, morphological basis of respiratory failure.

Sh., 37 years old, 5/IX 1957, was taken to the urological department by the ambulance service for acute pain in the left half of the abdomen and above the pubis with spread to the left lumbar region, painful urination, high fever, headache and general weakness. Severe abdominal pain was noted for 3 days and passed after the amniotic novocaine blockade. Leukocytosis (14,600) with neutrophilosis was detected. In the urine, protein (0.033%), fresh and leached erythrocytes 9-20 in the field of vision. The high temperature (38-39 ) lasted for 10 days, then gradually decreased to subfebrile figures (37.437.6 ). Surgical urological diseases were excluded.

The most objective and sensitive method of bronchial obstruction diagnosis is determination of maximal expiratory flow at different levels of bronchial tree. Despite the lack of clear boundaries of the norm, the flow indices according to spirography allow early diagnosis of bronchial permeability disorders, which facilitates the diagnosis of obstructive bronchitis and bronchial asthma.

4591 electrocardiograms of patients treated during the year in eleven departments of a multidisciplinary hospital were analyzed. Various types of intraventricular blockages were found in 99 (2.2%) patients. Among them there were 36% women, .64% men. 87% of patients were over 46 years old. 40.4% of patients with intraventricular blockages were treated in the cardiology department, 14% in gastroenterology, 12% -in neurology, the rest - in surgical, gynecological and endocrinological departments.

To assess the effect of smoking on ventilation-perfusion relations in 62 smokers and 30 non-smokers aged 20 to 57 years old we used mass spectrometry to determine partial pressure of O2 and CO2 in alveolar air (pАО2 and рАСО2) and growth rate of рСО2 in alveolar exhalation phase (pACO2/tA).To exclude heart and lung diseases, in addition to the general clinical examination, we studied GEL, SPR1 and Wotchal-Tiffno index. These indices in the healthy and smoking groups were within normal limits and did not differ significantly, although they were slightly decreased in smokers.
