211. Detection of laryngeal cancer--the case for early specialist assessment

Abstract

The first 300 patients referred from 11 participating general practices (GPs) to the Hoarse Voice Clinic, Queen Elizabeth Hospital, Birmingham, were studied prospectively to estimate the accuracy of GPs' diagnosis of laryngeal symptoms and to assess whether the provision of a direct referral immediate access service for the assessment of persistent laryngeal symptoms is an effective way of ensuring early referral and detection of laryngeal cancer. The GPs' assessment of laryngeal symptoms was inaccurate. Diagnosis after the initial clinic visit was accurate, predicting all histological cases of cancer. Laryngoscopy was possible in all patients at the first clinic visit. Disease requiring admission for direct laryngoscopy and biopsy was found in 39 patients (14%). Ten (3.3%) were found to have laryngeal cancer, of which eight were early lesions. When seen in the clinic 102 (34%) had normal voices and larynxes. A hoarse voice is a symptom requiring specialist assessment. By using the flexible fibreoptic nasendoscope all patients larynxes can be seen in clinic, an accurate diagnosis quickly made and the appropriate management instigated. It is feasible to offer this service without appointments to patients with persistent hoarseness.

Abstract

Type IV CRISPR systems encode CRISPR associated (Cas)-like proteins that combine with small RNAs to form multi-subunit ribonucleoprotein complexes. However, the lack of Cas nucleases, integrases, and other genetic features commonly observed in most CRISPR systems has made it difficult to predict type IV mechanisms of action and biological function. Here we summarize recent bioinformatic and experimental advancements that collectively provide the first glimpses into the function of specific type IV subtypes. We also provide a bioinformatic and structural analysis of type IV-specific proteins within the context of multi-subunit (class 1) CRISPR systems, informing future studies aimed at elucidating the function of these cryptic systems.

Abstract

During the past several years, the Congress has been critical of DOD'S management of military satellite communications-a primary concern being high costs. Congressional reports in 1989 and 1990 directed DOD to prepare a comprehensive, affordable architecture that defined all satellite communication requirements and potential solutions to satisfy the requirements. In November 1991, DOD published its military satellite communications architecture study that identified several alternatives for satisfying requirements, including the use of commercial communication satellites. Earlier, in February 1991, the White House had established U.S. commercial space policy guidelines (National Space Policy Directive 3) requiring U.S. government agencies to use commercially available space products and services to the fullest extent feasible. The policy was based on the presumption that potential large economic benefits would result., Report to the Chairman, Subcommittee on Defense, Committee on Appropriations, House of Representatives.
