A case of congenital aglossia was orthodontically treated and long-term retention was observed from the age of 15 to 27. The patient had congenital absence of three lower incisors, abnormally small mandible (micrognathia), severe overbite and a telescopic bite in the premolar region accompanied by an extremely narrow lower arch. Orthodontic treatment was begun when the patient was 15 years and 6 months old. The active treatment period was 2 years and 9 months. At the present time, nine years have passed since the end of the active treatment, and intercuspation between the upper and lower arches is still good. It is suggested that the maintenance of the lower arch width is essential for long-term retention in cases of congenital aglossia.

In this study 26 males (average age: 16 y 9 m) and 23 females (average age: 16 y 7 m) were selected from Chungtai Medical Junior College in Taichung, Taiwan. Head films of these students were traced and analysed. Mean values of the Chinese young adults in Taiwan were calculated and the conclusions were as follows: 1) Linear measurements of this present study were compared to Lin's in both males and females. A significant difference was noticed in the skeletal pattern. 2) Angular measurements of this present study were compared to Lin's data of stage IIIC. The gonial angle of this present study was smaller than Lin's data. However, no significant differences were noticed in the positional relationship of upper and lower jaws. 3) Regarding the axes of the anterior teeth, a lingual tipping tendency of the upper anterior teeth and a labial tipping tendency of the lower anterior teeth were observed. 4) The linear measurements in the present study were compared to the Japanese data of stage V. The anterio-posterior dimension of the chinese maxilla was larger than the Japanese. However, there are no significant differences in the position of the upper and lower jaws in females. 5) The measurements of the mandible in this study were smaller than those of the Japanese data. The gonial and mandibular plane angles, especially, were significantly smaller than those of Japanese. 6) The axes of upper and lower anterior teeth of Taiwanese showed more labial tipping than those of Japanese in this study.

Adults with untreated malocclusions suffer from more periodontal disease than if their malocclusion had been corrected orthodontically. What orthodontists can offer in the management of patients with periodontal disease, how they can help the periodontists and the patients, and how the periodontists can help the orthodontist make treatment safe and purposefully are widely discussed. The purpose of this study is, therefore, to discuss how orthodontic patients with severe periodontitis could be treated and what we should do for these patients during the treatment as well as at their initial visit. One case is presented as an example of treatment for malocclusion with sever periodontitis: The patient was a 23 years 8 months female with a chief complaint of protrusion of upper incisors. Since clinical examination revealed severe periodontitis, periodontal treatment was undertaken for one year prior to orthodontic treatment. From these observations, we are conviced of the importance again of the global approach and the team treatment method in treating malocclusions with periodontitis.

We examined the flash photolysis of oxy complexes of sperm whale myoglobin (Mb) on the nanosecond time scale at ambient temperatures. In this time range, we can observe the geminate reaction of Mb with the O2 ligand existing in the protein matrix after the photodissociation from the heme iron. We found that the fraction of the geminate component to the total O2 photodissociation exhibited temperature dependences. The geminate fraction decreased with rising temperature, indicating that the protein fluctuation is enhanced at high temperature because of thermal agitation. However, the temperature-dependent behavior showed a break at 20 degrees C. Concerning the geminate O2 escaping reaction from the protein matrix to the solvent region, the activation energy above 20 degrees C (0.4 +/- 0.4 kcal/mol) is significantly lower than that below 20 degrees C (5.1 +/- 0.4 kcal/mol). Thermodynamic analysis on the basis of the transition state theory indicated that the O2 escaping reaction above 20 degrees C is entropy dominated whereas that below 20 degrees C is enthalpy dominated. The results were qualitatively compatible with the theoretical prediction by J. Kottalam and D. A. Case [1988) J. Am. Chem. Soc. 110, 7690-7697). Comparing the kinetic and thermodynamic process of the O2 geminate reaction among several Mbs, we concluded that the geminate O2 reaction with Mb is governed by the dynamic motion of the protein which is sensitively controlled by the static interaction of the heme moiety with the surroundings.

To characterize the postural effect on the pharmacodynamic response to furosemide, seven healthy female subjects received the drug (30 mg i.v.), and the diuretic response and urinary drug excretion were monitored either in the supine or upright posture more than 5 h after dose. Six days later the same procedure was repeated, each subject being assigned to the other posture. Diuresis, chlor- and natriuresis were about 50% greater (P less than .01) in the supine than in the upright posture up to 45 min after dose, despite no significant difference in urinary drug excretion between the postures. Creatinine clearance and fractional sodium excretion were also significantly (P less than .05) greater in the supine posture. Pharmacodynamic analysis with the Emax model revealed that the maximum drug response (Emax) to furosemide in the upright posture was significantly (P less than .01) attenuated by about 65% of that in the supine posture, with no significant difference in either ER50 (urinary drug excretion rate producing 50% of Emax) or slope between the postures. Plasma norepinephrine concentration was significantly (P less than .05) greater, whereas urinary kallikrein excretion was significantly (P less than .05) less in the upright posture. There were no significant differences in blood pressure, plasma renin activity and atrial natriuretic peptide as well as in urinary excretion of prostaglandin E2, dopamine and adenosine between the postures.(ABSTRACT TRUNCATED AT 250 WORDS)

Normal mastication needs condylar support. Mandibular condyle is thought to be one of the factors which control facial growth. To determine which parts of the face are affected by unilateral complete condylotomy including surrounding connected soft tissues, thirty-nine male Wistar rats, 5 weeks of age, were divided into control, sham treated and experimental groups. The control animals received no treatment, where as the left mandibular condyles of the experimental animals were condylotomized. Soft X-rays of the animals of both groups were taken every two weeks, and all were killed six weeks after treatment. Roentgenografically, it was observed in the experimental animals that a functional condyle-like process generated to fill the condylar space of glenoid fossa. This process was smaller than normal condyle of the control anomals. Furthermore, ANOVA revealed significant differences (P less than 0.01) in some facial structures including facial height as well as mandibular length, which were still affected at six weeks after the surgery. The result of the present investigation suggests that unilateral condylotomy induced the production of a functional condyle-like process and caused some facial deformities.

We carried out the flash photolysis of oxy complexes of sperm whale myoglobin, cobalt-substituted sperm whale myoglobin, and Aplysia myoglobin. When the optical absorption spectral changes associated with the O2 rebinding were monitored on the nanosecond to millisecond time scale, we found that the transient spectra of the O2 photoproduct of sperm whale myoglobin were significantly different from the static spectra of deoxy form. This was sharply contrasted with the observations that the spectra of the CO photoproduct of sperm whale myoglobin and of the O2 photoproducts of cobalt-substituted sperm whale myoglobin and Aplysia myoglobin are identical to the corresponding spectra of their deoxy forms. These results led us to suggest the presence of a fairly stable transient species in the O2 photodissociation from the oxy complex of sperm whale myoglobin, which has a protein structure different from the deoxy form. We denoted the O2 photo-product to be Mb*. In the time-resolved resonance Raman measurements, the nu Fe-His mode of Mb* gave the same value as that of the deoxy form, indicating that the difference in the optical absorption spectra is possibly due to the structural difference at the heme distal side rather than those of the proximal side. The structure of Mb* is discussed in relation to the dynamic motion of myoglobin in the O2 entry to or exit from the heme pocket. Comparing the structural characteristics of several myoglobins employed, we suggested that the formation of Mb* relates to the following two factors: a hydrogen bonding of O2 with the distal histidine, and the movement of iron upon the ligation of O2.

A consultation expert system which supports our computer aided reporting system was developed. The system was used for the evaluation of the two dimensional polar (bull's eye) display of 201Tl myocardial SPECT. The system consists of patients management (PM) and consultation expert systems (ES). The former is connected to image processors coupled with scinticameras. The bull's eye display of myocardial SPECT is transferred from image processor to the data base of PM. When inference request is made, the feature extraction program extracts information on localization, extent and severity of focal defects comparing count-rates pixel by pixel with the reference obtained from seven normal controls. The inference engine is activated to determine presence of focal defects utilizing diagnostic rules in the knowledge base. The results are sent back to PM and reported with the probability of assurance. Fifty eight patients with old myocardial infarction (OMI), angina pectoris (AP) and other diseases as well as normal controls were included in the study. The decision for presence or absence of focal defects by ES agreed with that by nuclear physicians (NP) in 301 segments among 330 (91%) in stress images. The presence of redistribution in delayed images agreed in 43 segments among 67 (64%). Image interpretation by ES agreed well with that of NP in patients with OMI (19/20) and AP (9/11). Seven were interpreted as normal by both ES and NP. The system is useful, as it provides NP with complementary and supportive information applicable to decision making and reporting. Further clinical experiences can improve knowledge base for better ES function.

The patient was twenty-three year old female with TMJ disorder and crowding. She had often had a clicking sound caused probably by malocclusion since she was eighteen years of age. Limitation of mouth opening with TMJ locking has been occurring since she had an impacted right lower third molar extracted at nineteen. A stabilization splint was applied for better mandibular position to rectify the TMJ disorder. The clinical symptom was relieved one month later, so the malocclusion was treated with a multi-bracket appliance. Since cross bite in [formula: see text] was corrected in a year and three months, symptoms of the TMJ disorder disappeared and this condition has remained stable for the past year. A stabilization retainer worn at night was considered necessary to prevent a relapse of the dentition and mandibular position, and to keep the masticatory muscles relaxed. For the purpose of stabilizing the TMJ condition and occlusion, the patient was taught unilateral mastication in order to diminish the difference between sides in chewing force, because only one side had been habitually used for a long time. It is suggested that such an alternative chewing habit will improve functional stability following the structural correction.

- Abstract

A case of unilateral enlargement of the coronoid process of the mandible is presented. The patient, a 37-year-old woman complained of restricted mandibular movement and swelling in the left zygomatic region. Coronoidectomy was performed intraorally, producing improved mandibular movement. The specimen consisted of a large amount of cartilage and mature bone. Most of the cartilage showed a marked disturbance in endochondral ossification. The histopathological diagnosis was osteochondroma. A review of the basic nature of the disease in the reported cases is presented. The value of computed tomography in deciding on a surgical procedure is briefly mentioned.

The mandibular manipulation technique is widely used and is considered acceptable conservative therapy for an acute closed lock of the temporomandibular joint. However, there are no studies that prove objective morphologic assessment of this method in patients with unlocked disks. In this study, preoperative and postoperative arthrograms were evaluated to clarify the disk position in patients for whom successful mandibular manipulation was carried out. Postoperative arthrograms in 30 closed lock joints revealed that three joints were normal (complete reduction), eight had anterior disk displacement with reduction (click), 15 had partial reduction at the maximum mouth opening position, and four had anterior disk displacement without reduction. Disk mobility increased with improved condylar translation in all joints. These results indicate that complete anatomic reduction of the disk by mandibular manipulation is difficult, but also that clinical symptoms do not always correspond to how well the disk has been reduced by this treatment. Increasing disk mobilization might be effective treatment for patients with closed lock of the temporomandibular joint.

A 10.4-inch TFT-LCD with XGA resolution has been developed using low-temperature poly-Si technology. We have newly developed a circuit construction and a driving method for this LCD. Positive and negative video signals are separately applied to video buslines from two D-A conversion ICS. The integrated data-line driver circuits operate with four multiplexing-blocks and eight-phase parallel video inputs. The integrated scan-line driver circuits have shift registers operating at 10V voltage with redundancy function. High aperture ratio of 61% was realized. These result in small data-line driver circuit width of 3.7mm, low module power consumption of 2.3W, and high reliability.

We report a benign form of congenital angulation of long bones associated with shortening of soft tissues. The patients were three males and four females including two siblings. The average age at first examination was 6 months (range, 1-17 months) and that at the time of follow-up was 7 years (range, 13 months-11 years). The characteristics were short-limb type short stature, congenital multiple angulation deformities of the long bones of both upper and lower extremities, with almost the same angulation directions for all the patients and improvement of the deformities with age. There was also accelerated skeletal maturation, lack of epi-metaphyseal lesions, skin dimples, limitation of the ankle joint dorsiflexion, and inconspicuousness of skin creases on the flexor side of the elbow and knee. This phenotype is not consistent with any of the bent bone dysplasia group. Caffey's report [Caffey J (1947). Am J Dis Child 74:543-5621 included two different subtypes of bent bones, round bowing and sharp angulation, and the latter are similar to this report. However, Caffey's cases lack limitation of ankle dorsiflexion and reduced skin creases. An autosomal recessive mode of inheritance could be present in this subtype.

Background and study aims: In previous series, endoscopic mucosal resection (EMR) has been used for the treatment of early-stage mesopharyngeal and hypopharyngeal cancers to preserve patients’ quality of life. Endoscopic submucosal dissection (ESD) offers potential advantages in comparison to EMR. So the aim of this retrospective study was to assess the utility of ESD compared with EMR for early-stage cancers of the meso- and hypopharynx.Patients and methods: We studied 56 patients with 69 lesions who underwent either EMR or ESD between April 2001 and December 2008. EMR was performed until January 2007, and ESD was performed from February 2007 onward. We evaluated the en bloc resection rate, R0 resection rate, and treatment-related complications as short-term outcomes. Local recurrence, lymph node metastasis, and disease-related deaths were compared to evaluate long-term outcomes. Results: The en bloc and R0 resection rates were respectively 98 % and 79 % in the ESD group and 37 % and 26 % in the EMR group. There were no cases of treatment-related complications in the EMR group, but postoperative subcutaneous emphysema was observed in two patients in the ESD group. In the EMR group, one patient developed a local recurrence and one developed metastasis to the cervical lymph node and died of primary cancer. Conclusions: ESD is a useful method of treatment for early mesopharyngeal and hypopharyngeal cancers and may be superior to EMR. BACKGROUND AND STUDY AIMS: Application of the endoscopic submucosal dissection (ESD) technique, as well as elevation of the larynx in cooperation with an otolaryngologist, under general anesthesia has enabled en-block resection of early mesopharyngeal and hypopharyngeal carcinomas. The aim of this study was to retrospectively evaluate the safety of ESD and the efficacy of the elevation of the larynx for treatment of early mesopharyngeal and hypopharyngeal carcinomas. PATIENTS AND METHODS: The study included 16 lesions in 13 patients who underwent ESD. To provide a sufficient working space, the larynx was elevated under direct laryngoscopy, and a tube was inserted and fixed onto the laryngeal side using the slot on the back of the laryngoscope. RESULTS: The median maximum diameter of the lesions was 12.5?mm (range 2?-?37?mm). The en-block resection rate was 93.8?%. Lateral surgical margins in two patients were difficult to evaluate for technical reasons. The tube could not be removed from four patients on the day of the procedure due to laryngeal edema caused by the local injection. No serious complications were observed. Oral food intake was possible after a mean of 3.3 postoperative days. CONCLUSIONS: With adequate intraoperative and postoperative management, ESD with elevation of the larynx may be very efficient and safe for endoscopic treatment of pharyngeal lesions. Copper incorporation into ceruloplasmin during ceruloplasmin synthesis was studied by comparing LEC and control rats. Major 132 and 136 kDa ceruloplasmins were found in microsomes and the Golgi apparatus, respectively, isolated from liver homogenates of LEC and control rats. Copper analysis showed that no copper was detected in the ceruloplasmin in the microsomes of either rat. Copper was present in ceruloplasmin in the Golgi apparatus and serum of controls, while it was not detected in ceruloplasmin in the Golgi apparatus and serum of the LEC rat. These results indicate that copper is incorporated into ceruloplasmin in the Golgi apparatus of normal hepatocytes. LEC rats fail to incorporate copper into ceruloplasmin in the Golgi apparatus.

Two-dimensional simulation and charge control principles have been applied to reveal the factors which determine the minimum delay of an integrated injection logic (I/SUP 2/L) gate, and experimental verifications are carried out. Using a numerical analysis this paper shows that important factors in improving the speed of an I/SUP 2/L gate are reducing the amount of minority charge stored in the external base of the n-p-n transistor and use of a heavily doped emitter. It is, therefore, necessary that the concentration in the external base is increased as high as possible and that the diffusion depth is controlled with good accuracy. Improvement in speed by a factor of 2 is experimentally realized as the simulation predicts. The heavily doped external base improves upward current gain and reduces base resistance, achieving a high fan-out capability.

Photodissociation of the CO molecule from carbon monoxide complexes of myoglobin and hemoglobin was examined between 4.2 and 80 °K. Experimental results have demonstrated that: 1. (I)|The photodissociated form was stable below 10 °K. 2. (II)|The ratio of the quantum yield of hemoglobin to myoglobin was roughly estimated to be 0.38. 3. (III)|The recombination of dissociated CO occurred by raising the temperature. 4. (IV)|The recombination reaction of photodissociated myoglobin and hemoglobin with CO was biphasic, whereas that of cobalt myoglobin with O2 was monophasic. 5. (V)|Activation energies for fast and slow reactions of myoglobin and hemoglobin, and for monophasic reaction of cobalt myoglobin are listed below: Fast Slow Mb(Fe 2+ ) + CO 124 cal/mole 124 cal/mole Hb(Fe 2+ ) + CO 124 cal/mole 230 cal/mole Mb(Co 2+ + O 2 570 cal/mole 6. (VI)|In the near infrared region the photodissociated forms of myoglobin and hemoglobin gave absorbance peaks at 770 nm, whereas deoxygenated forms of myoglobin and hemoglobin gave absorbance peaks at 758 and 750 nm, respectively, which suggests that the near infrared band is sensitive to the protein structure and may be called the “conformation band”. Several discussions have been held on the nature of the photodissociated form and the biphasic property of the reaction of myoglobin and hemoglobin, compared with those of cobalt myoglobin. It is now concluded that the new species, corresponding to the “rapidly reacting form” of hemoglobin by Gibson, Q. H. (1959) Biochem. J. 71, 293–303, and, more recently, of hemoglobin, single chain proteins and noncooperative aggregated forms by Alpert, B., Banerjee, R., and Lindqvist, L. (1974) Proc. Natl. Acad. Sci. U.S. 71, 558–562, is certainly trapped at 4.2 °K by photodissociating the carbon monoxide and oxygenated myoglobin and hemoglobin as well as the oxygenated cobalt myoglobin and hemoglobin.

Selected issues in the development of fusion nuclear technology (FNT) have been studied. These relate to (1) near-term experiments, modeling, and analysis for several key FNT issues, and (2) FNT testing in future fusion facilities. A key concern for solid breeder blankets is to reduce the number of candidate materials and configurations for advanced experiments to emphasize those with the highest potential. Based on technical analysis, recommendations have been developed for reducing the size of the test matrix and for focusing the testing program on important areas of emphasis. The characteristics of an advanced liquid metal MHD experiment have also been studied. This facility is required in addition to existing facilities in order to address critical uncertainties in MHD fluid flow and heat transfer. In addition to experiments, successful development of FNT will require models for interpreting experimental data, for planning experiments, and for use as a design tool for fusion components. Modeling of liquid metal fluid flow is a particular area of need in which substantial progress is expected, and initial efforts are reported here. Preliminary results on the modeling of tritium transport and inventory in solid breeders are also summarized. Finally, the thermo-mechanical behavior of liquid-metal-cooled limiters is analyzed and the parameter space for feasible designs is explored. Because of the renewed strong interest in a fusion engineering facility, a critical review and analysis of the important FNT testing requirements have been performed. Several areas have been emphasized due to their strong impact on the design and cost of the test facility. These include (1) the length of the plasma burn and the mode of operation (pulsed vs. steady-state), and (2) the need for a tritium-producing blanket and its impact on the availability of the device.

A TTL-compatible 64K static RAM with CMOS-bipolar circuitry has been developed using a 1.2-/spl mu/m MoSi gate n-well CMOS-bipolar technology. Address access time is typically 28 ns, with 225 mW active power and 100 nW standby power. A CMOS six-transistor memory cell is used. The cell size is 18/spl times/20 /spl mu/m, and the chip size is 5.95/spl times/6.84 mm. The n-p-n transistors are used in the sense amplifiers, voltage regulators, and level clamping circuits. The bipolar sense amplifiers reduce the detectable bit line swing, thus improving the worst-case bit line delay time and the sensing delay time. In order to reduce the word line delay, the MoSi layer, which has 5 /spl Omega//sheet resistivity, was used for the gate material. The n-well CMOS process is based on a scaled CMOS process, and collector-isolated n-p-n transistors and CMOS are integrated simultaneously without adding any extra process steps and without causing any degradation of CMOS characteristics. The n-p-n transistor has a 2-GHz cutoff frequency at 1 mA collector current.

Effects of oxide isolation on current gains, signal swing, and propagation delay time of an I/SUP 2/L gate at high dissipation levels are discussed in terms of the backward injection of the p-n-p transistor, the magnitude of which is reduced by the isolation. The reduction enhances the degree of saturation. On the other hand, it causes a larger collector current which enables a rapid discharge of the stored charge. These competing effects given an optimum condition for which the analytical expression is obtained.

In the near future, powerful X-ray source (Synchrotron Orbital Radiation, SOR) will be available at National Laboratory for High Energy Physics (Tsukuba). One of the most promising researches using SOR system is its application to biomedical problems. The objective of this report is to review the EXAFS studies on biological system. EXAFS can probe the local structure just around the X-ray absorbing atom. In this paper we describe the analysis of EXAFS spectrum of iron atom in rubredoxin, one of the simplest metalloproteins, and then we summarize the results of structural analysis on various metallo-proteins and enzymes such as nitrogenase, cytochrome P-450, hemoglobin, cytochrome c oxidase and so on.

An afterloading procedure using iridium 192 seeds was developed for the control of both localized primary lesions that are difficult to remove and superficial residual lesions that remain after the partial removal of tumors in relatively inaccessible, deep-seated organs. This technique makes possible the delivery of a minimal radiation dose to lesions in deep-seated organs. Fewer complications result, the seeds can be adjusted at appropriate intervals in proportion to the amount of radioactivity desired, and radiation exposure to personnel can be reduced. This paper describes the techniques and apparatus used to place the seeds in the tubing. Primary lesions of the urinary bladder, the prostate gland, and the esophagus, residual lesions in the brain and lung, and metastases in intrapelvic lymph nodes were treated. No technical difficulties or complications were encountered. Radiation exposures to personnel were minimized. The results indicate that the procedure has practical applications.

A reliable conversion factor for the infrared absorptiometry of oxygen in silicon has been determined by round‐robin infrared measurement followed by charged particle activation analysis with the reaction. As for the round‐robin samples, 70 dislocation‐free CZ silicon wafers with oxygen contents ranging from and thicknesses of 2, 1, and 0.5 mm were carefully prepared by five organizations. A good linear relationship has been obtained between the absorption coefficient and the oxygen content. The relationship is expressed as .

The α,β-unsaturated γ,δ-epoxyketone 7 is isomerized almost exclusively to the δ-diketone 9 both upon irradiation in the n π* absorption band with light of wavelengths above 310 nm (in anhydrous dioxane or benzene solutions) and upon triplet sensitization using acetophenone in benzene. The reaction may be formulated by the cleavage of the CγO oxide bond and the shift of the δ-hydrogen to the γ-position, and thus bears a formal “double bond homology” to the photochemical α,β-epoxyketone rearrangement. Excitation in the π π* absorption band of 7 with light of wavelength 253,7 nm (in anhydrous dioxane solution) leads to the formation of product 10 as well as to the triplet rearrangement to 9. With this result a novel partial synthesis of O-acetyl-B-nortestosterone has been accomplished, which has the advantages of fewer steps and higher product yield (7 10: ∼30% yield) than previously published syntheses. On the basis of the presently available experiments, the mechanism of the transformation 7 10, which constitutes one of the still few examples of enone photoreactions induced selectively from the π,π* excited singlet, remains unknown.

It has recently been shown that the occurrence of flat‐bottomed saucer‐shaped etch pits (S‐pits) in GaP grown by the liquid‐encapsulated‐Czochralski (LEC) technique can be correlated with reduced photoluminescence (PL) and electroluminescence (EL) efficiencies. At the same time a new class of LEC crystals pulled from nonstoichiometric melts (LEC/NSM) have become available which show much improved PL efficiencies (η). A study of the etchpit densities in LEC/NSM crystals was therefore carried out. Both p‐ and n‐type material were examined. In n‐type material the S‐pit density is reduced by at least 4 orders of magnitude for all LEC/NSM crystals compared with LEC material. Similar reductions in S‐pit density are observed in p‐type material, but only for crystals pulled from melts containing less than 30 at. % phosphorus. This correlates well with those p‐type crystals showing major improvements in PL η. However, crystals pulled from 20 and 10 a/o P melts are small in size and have a strong tendency to entrai...
