Preoperative autologous blood donation for elective surgery patients at university hospitals was underused in the past. More recently, national educational efforts have been made. To test the impact of local surgeon interviews and education, in 1988 the same local educational program was instituted at three university hospitals; three community hospitals were used as controls. Donation by appropriate patients of interviewed surgeons (elective surgery, crossmatch recommended, no contraindications to donation) increased from 24 percent (44/180) to 40 percent (88/222) (p = 0.002) and 15 percent (21/143) to 32 percent (41/127) (p = 0.001) at two university hospitals where the investigator-educators were on site, but not at the three community hospitals. Between 1987 and 1989, donation rates at all six hospitals remained low among patients for whom autologous donation was (probably) less appropriate. Donation rates for type and screen procedures were 3.0 percent (131/4587) in 1987 and 3.0 percent (199/6606) in 1989 (p = 0.67). Donation rates for "no blood order" procedures were 0.2 percent (15/9429) in 1987 and 0.1 percent (9/11,239) in 1989 (p = 0.14). It can be concluded that appropriate autologous blood donations increased at university hospitals where surgeons were individually interviewed and educated by an investigator on site. However, despite this increase, apparently eligible elective surgery patients in 1989 still failed to donate. This situation deserves additional investigation.

We retrospectively analyzed our 2-year experience with venous access for 363 therapeutic plasma exchanges in 46 patients with neurological disease, including acute Guillain-Barré syndrome (N = 20), myasthenia gravis (N = 17), and chronic inflammatory demyelinating polyneuropathy (N = 9). Twenty-three patients (50%) completed the planned course of therapy using only peripheral venous access, and 28 central venous catheters were placed in the remaining 23 patients. Patients utilizing central venous access did not undergo a greater number of procedures, but they were more likely to have acute Guillain-Barré syndrome (P < 0.02) or to be hospitalized in a medical intensive care unit (P < 0.01). Three types of central catheters were used, and although our experience was predominantly with 1 type, differences were noted. Only 3% of procedures (3 of 96) done with a Quinton-Mahurkar catheter were associated with a catheter failure, compared to 27% (4 of 15, P < 0.01) with a Hickman catheter and 67% (2 of 3) with a triple-lumen catheter. Life-threatening complications occurred with 3 of 28 (11%) central catheters. To optimize the success of therapeutic plasma exchange using central access, it is critical that hemapheresis personnel advise each patient's primary physician regarding the type of central venous catheter required. Currently, we recommend use of a Quinton-Mahurkar or other dual-lumen hemodialysis catheter.

Donor exposure can be strikingly reduced for patients with classical hemophilia A and von Willebrand's disease when large volumes of potent cryoprecipitated AHF are prepared from donors following DDAVP (1-deamino-8-D-arginine vasopressin) stimulation and automated plasmapheresis--a procedure called "plasma exchange donation." Although this procedure has been reported to be relatively safe for donors, data are limited. Accordingly, we studied 20 donors during 48 procedures using DDAVP (0.3 micrograms/kg IV) followed by 2-3 L plasma collection. Replacement fluid for each initial plasma exchange donation was plasma protein fraction; autologous cryoprecipitate-poor plasma was used for subsequent procedures. Mild reactions, particularly facial flushing, were noted in all 48 procedures. No procedure was discontinued, but four were modified due to either an increased pulse rate (> or = 20/min from baseline) or a fall in systolic or diastolic blood pressure (> or = 20 mm Hg from baseline). No donor was deferred or withdrew from the program. Based on our modest experience, DDAVP stimulated plasma exchange donation appears to be a safe and effective method for collecting large quantities of plasma from which potent cryoprecipitated AHF can be prepared.

Anti-Sda, an antibody not usually considered to cause of hemolytic transfusion reactions, possibly was related to hemolysis following transfusion of red blood cells expressing strong Sda antigen. Prior to transfusion, the antiglobulin antibody screen performed in LISS and an immediate spin crossmatch were negative. Retrospectively, after hemolysis was detected, an antiglobulin crossmatch with red cells from the transfused unit revealed microscopic incompatibility. The transfused unit proved to have strong expression of Sda antigen-facilitating identification of a weak Sda antibody in our patient. In addition, this case represents an unusual instance in which an antibody screen plus an immediate spin crossmatch failed to detect an incompatibility that would have been apparent had an antiglobulin crossmatch been performed.

Infants, particularly those who were very small premature neonates, are among the most common of all patient groups to undergo extensive transfusion. It is estimated that approximately 300,000 neonates undergo transfusions annually. Most infants who undergo transfusion are exposed to multiple blood donors, and although each exposure poses only a small risk, the potential for adverse effects of multiple transfusions is not variable, and based on scanty scientific information. For the most part, controlled scientific studies have not been performed to clearly establish the indications for the transfusion of blood components to neonates. Considering these limitations, guidelines are offered for the transfusion of red blood cells, platelets, and neutrophils into neonates.

Although transfusion of blood products is an essential and potentially life-saving measure, not all blood transfusions are beneficial to patients. The associated risks, particularly transfusion-transmitted viral illnesses, such as hepatitis and acquired immunodeficiency syndrome, require that careful consideration be given before a decision is made to transfuse any blood product. Many institutions have established a local committee to monitor transfusion practices and audit such practices regularly. To assist in this task, the Pediatric Hemotherapy Committee of the American Association of Blood Banks has developed guidelines for the conduct of pediatric blood transfusion audits. These guidelines, summarized herein, cover transfusion of red blood cells, platelets, white blood cells, fresh-frozen plasma, albumin, and clotting concentrates. The use of cytomegalovirus low-risk and irradiated blood products is also discussed. Throughout the report, special attention is given to the transfusion needs of newborn infants.

We compared changes in muscle sympathetic nerve activity (SNA) during graded lower body negative pressure (LBNP) and 450 ml of hemorrhage in nine healthy volunteers. During LBNP, central venous pressure (CVP) decreased from 6.1 +/- 0.4 to 4.5 +/- 0.5 (LBNP -5 mmHg), 3.4 +/- 0.6 (LBNP -10 mmHg), and 2.3 +/- 0.6 mmHg (LBNP -15 mmHg), and there were progressive increases in SNA at each level of LBNP. The slope relating percent change in SNA to change in CVP during LBNP (mean +/- SE) was 27 +/- 11%/mmHg. Hemorrhage of 450 ml at a mean rate of 71 +/- 5 ml/min decreased CVP from 6.1 +/- 0.5 to 3.7 +/- 0.5 mmHg and increased SNA by 47 +/- 11%. The increase in SNA during hemorrhage was not significantly different from the increase in SNA predicted by the slope relating percent change in SNA to change in CVP during LBNP. These data show that nonhypotensive hemorrhage causes sympathoexcitation and that sympathetic responses to LBNP and nonhypotensive hemorrhage are similar in humans.

Patients with serious illnesses who donate their blood for autologous use create anxiety for many phlebotomists. Donor room staff were surveyed at three donor centers, using confidential questionnaires, to identify their concerns and to evaluate factors associated with reduced anxiety toward autologous donors. Among respondents (n = 93), 58 percent had patient care experience within the last 5 years, which correlated with diminished concern about phlebotomizing cardiac patients (r = .21; p = 0.04). Fifty-seven percent of respondents had drawn blood from greater than 50 autologous donors. Surprisingly, the experience of drawing blood from relatively large numbers of autologous donors did not correlate with increased confidence in phlebotomizing pediatric, cardiac, and elderly patients (r = -.04; p = 0.75). Those respondents who felt that they had adequate medical support agreed with more liberal donor criteria and were more confident about phlebotomizing pediatric, cardiac, and elderly patients (r = -.32; p = 0.001). Those respondents who believed most strongly in the benefits of autologous transfusion had the least anxiety toward donors who were medically more complex (r = .39; p = 0.0001). It is concluded that, contrary to what might be expected, phlebotomizing large numbers of autologous donors does not reduce the anxiety of staff members when they encounter donors with complex medical problems. Factors that do reduce anxiety include dependable physician and other medical support services, previous experience in direct patient care, and knowledge of the benefits of autologous transfusion.

Premature neonates require blood transfusions, and biological parents may wish to be directed donors. Biological mothers pose a potential danger because their plasma may contain antibodies that will react with blood cell antigens inherited by the infant from the father. We studied 25 healthy, pregnant women at the time of delivery for the presence of antibodies against red blood cell, leukocyte and platelet antigens. Mothers known to have red cell antibodies earlier in pregnancy were excluded, and no new red cell antibodies appeared at delivery. Antileukocyte and antiplatelet antibodies were found in 16 and 12% of mothers, respectively. Because these antibodies have the potential to cause adverse reactions when transfused passively, we suggest that either biological mothers not provide blood components containing plasma for their neonates or that maternal red cells and platelets be given as washed products.

The face of automated hemapheresis in the United States is changing. No longer is it appropriate to view hemapheresis primarily in terms of therapeutic plasma exchange. For many centers, the bulk of day-to-day activities revolves around donor cytapheresis to satisfy the ever-increasing need for units of platelet concentrates. Indications of therapeutic plasma exchange are becoming better focused, with neurological patients being among those most frequently managed. New and innovative applications of automated hemapheresis technology will continue to be developed. Current examples include the production of lymphocyte-activated leukocytes for cancer therapy, use of extracorporeal photoactivation of leukocytes for immune modulation, and the collection of hematopoietic progenitors from peripheral blood for autologous transplantation. As we enter the 1990s, it is important to be aware of new technology and applications and to have the vision to apply them in innovative ways. However, it is also critical to demand rigorous scientific review before broadly adopting any new idea as a standard of practice.

Results of 3,214 plateletpheresis procedures, using the Fenwal CS-3000 cell separator, were reviewed to determine the effects on performance when the donor blood flow rate was increased by 30% (i.e., to 65 ml/min instead of 50 ml/min). Ninety-two percent (2,962 of 3,214) of procedures completed the rapid flow protocol, and results were compared with those reported by others using the more conventional blood flow rates of 32-55 ml/min. When the rapid flow procedure was used, mean platelet yields (4.07 x 10(11)) and leukocyte contamination (2.65 x 10(8)) were satisfactory, and major donor reactions were infrequent (0.6% of procedures). Thus, CS-3000 plateletpheresis can be performed at a relatively rapid, donor blood flow rate of 65 ml/min to shorten the procedure and make it more appealing to donors.

Hematopoietic stem cells, collected by leukapheresis from peripheral blood, can be used as an alternative to autologous bone marrow transplantation following high-dose chemotherapy as treatment of several malignancies. We compared the ability of the Cobe Spectra and the Fenwal CS3000 to collect peripheral blood mononuclear cells (MNC) for autologous peripheral blood stem cell transplantation. Ten patients experienced repeated leukapheresis (10 L blood processed per procedure) using both instruments. Procedures were alternated between the two until a total of 7 x 10(8) MNC/kg was collected. Data from 61 Spectra and 50 CS3000 collections were analyzed. The yield (mean per procedure) of nucleated cells (NC) and MNC was higher (P less than .005) with the Spectra (0.77 x 10(10) NC and 0.54 x 10(10) MNC) than with the CS3000 (0.59 x 10(10) NC and 0.40 x 10(10) MNC). However, colony forming units (CFU-GM) were not different (P greater than .05) for Spectra (0.92 x 10(4)) and Fenwal (0.65 x 10(4) collections. Platelet contamination was lower (P less than .001) with the Spectra (2.2 x 10(11)) than the CS3000 (5.0 x 10(11)). This correlated with a higher patient blood platelet count immediately following Spectra collections (117 x 10(9)/L) versus the CS3000 (86 x 10(9)/L). Using the methods described, the Spectra product contained greater yields of NC and MNC with less platelet contamination than did the CS3000.

Abstracts afford an opportunity to report data at professional meetings and, when published, in the literature. Accordingly, they should be prepared with great care. When writing an abstract, anticipate questions the reviewer will ask when judging it and provide complete answers. The presentation of an abstract should follow similar thought processes. State why a problem or question is important, how you addressed it, what you found, and how your findings can be applied to the issue at hand. Slides and text should provide coordinated visual and auditory input, respectively, to ensure complete comprehension.

Techniques for collecting granulocytes for transfusion either to neutropenic patients or to neonates are described. Currently, the best granulocyte concentrates are prepared using continuous-flow centrifugation leukapheresis of steroid-stimulated donors in the presence of pentastarch. Donor reactions are mild and are similar to those expected with automated plateletpheresis.

A blood sampling pack is available on closed system apheresis kits for the Fenwal CS3000 blood cell separator to facilitate same day donor testing while maintaining the closed system. Conceivably, the accuracy of complete blood counts (CBC) performed using pack samples might be compromised by clotting in the pack, by adherence of blood cells to the inner surface of the pack, or by dilution of the sample with priming fluid--should it inadvertently contaminate the pack. We studied paired blood samples collected simultaneously via the pack and a separate venipuncture in 17 donors. When collected properly, CBC values from pack and vein were nearly identical. Even when purposely contaminated with saline, pack values were lowered only slightly. Thus, the inline pack permits accurate sampling of donor blood for CBC while maintaining a closed system.

Abbreviated pretransfusion testing, although permitted by American Association of Blood Banks Standards for unimmunized patients, is not widely practiced. Concerns remain about optimal antibody screening methods, antibodies missed by deleting the antiglobulin crossmatch, and cost-effectiveness. The authors prospectively tested 3,380 serum samples for blood type, antibody screen, and antiglobulin crossmatch. Antibody screens for 2,000 samples, performed with the use of a two-cell screen, were compared with 1,380 samples studied with a three-cell screen. Also, all 3,380 sera had major crossmatches performed carried through the antiglobulin phase. Two and three screening cells gave comparable results, with 5.45% of patients tested by two-cell and 5.22% by three-cell screens having a positive antibody screen. Of those with negative screens, 0.5% screened by two-cell screens and 0.8% by three-cell screens had a positive major crossmatch. Among these (negative antibody screen, positive crossmatch), only 0.03% (1 of 3.380) had a clinically significant alloantibody (anti-Kpa); 0.27% (9 of 3,380) had antiglobulin crossmatch positive with polyspecific antisera but negative with anti-IgG; and 0.12% (4 of 3,380) had positive crossmatch because of passive anti-A. By cost accounting of labor and reagents, 84 per unit would be saved using abbreviated versus complete pretransfusion testing. Blood banks now performing complete pretransfusion testing should reconsider abbreviated crossmatching for unimmunized patients as a safe, efficacious means of cost-containment.

Studies of risk factors associated with reactions among autologous blood donors have been limited. Therefore, 2091 autologous and 4737 homologous donations were examined. Donors at greatest risk for reaction were autologous donors who had reactions at first donation; among 45 who made repeat donations for the same surgery, 17 (38%) had repeat reactions. The group least likely to experience reactions were the autologous donors greater than or equal to 66 years old; they experienced a 1.9 greater than or equal to percent (6/310) incidence of reactions. More reactions were seen in both autologous and homologous donors in the categories of first-time donor, female gender, decreasing age, and lower weight. Multiple logistic regression analysis showed that all of these variables were independent predictors of donor reaction, with first-time donation (odds ratio, 2.4) and female gender (odds ratio, 1.9) being the strongest predictors of reaction. Donor room personnel should be alerted that autologous donors who react at first donation are very likely to react at subsequent donations. Contrary to common concern, elderly autologous donors are least likely to have reactions.

A questionnaire to determine patterns of neonatal red cell transfusion practice during 1985 was mailed to 2200 blood banks of American Association of Blood Banks (AABB) institutional members and children's hospitals. There were 915 responses (41.6%); 785 responses (86%) contained sufficient data for analysis. The majority (70.6%) of 785 responding hospitals were community/urban institutions. However, more highly specialized, pediatric hospitals were also represented by 92 university/tertiary-care hospitals (11.7% of respondents) and 29 children's hospitals (3.7% of respondents). Two-thirds of hospitals performed a major antiglobulin crossmatch (rather than an abbreviated one) before all neonatal red cell transfusions. The red cell preparation most frequently selected for small-volume transfusions was ABO and Rh group-specific red cell concentrates. When performing only large-volume exchange transfusions, 19.2 percent of hospitals used whole blood; all others prepared reconstituted units of red cells plus fresh-frozen plasma, a practice that frequently causes exposure to two donors per unit. Another practice likely leading to multiple donor exposure is the use of fresh-frozen plasma to adjust the hematocrit of red cell preparations to a predetermined value prior to a small-volume transfusion. Over one-half of hospitals adjusting hematocrits used plasma, presumably from one donor, to dilute packed red cells from another donor, a practice that has no apparent medical benefit. Most hospitals (63.4%) provided red cells with a reduced risk of transmitting cytomegalovirus; blood from seronegative donors was selected by 65 percent of hospitals. The majority of hospitals, including most of the community/urban hospitals, did not irradiate blood products before transfusion.(ABSTRACT TRUNCATED AT 250 WORDS)

Serum-to-red cell ratio (volume:volume) significantly affects the sensitivity of antibody detection. Obtaining sufficient serum quantity can be a problem when testing blood from transported neonates not accompanied by maternal samples. Reducing serum volumes used for testing might result in missed antibodies. The authors studied 1,177 sera for unexpected red cell antibodies by comparing one versus two drops of patient serum using a technique with LISS at 37 degrees C through the antiglobulin phase. The serum-to-red cell ratio using two drops was approximately 50: 1. Results of 60% (58/96) of samples containing antibody benefited by the use of increased serum. Eighteen percent; (17/96) had antibodies detected only with two drops that were missed entirely by one, and 43% (41/96) showed stronger positive reactions using two drops versus one. Importantly, antibodies detected only with or enhanced by two drops were clinically significant (anti-D, anti-K, anti-M, anti-c, anti-E). Thus, reducing serum-to-cell ratio is potentially dangerous, and blood banks should insist on adequate sample size to ensure patient safety.

We report a patient with an erythrocyte autoantibody in whose serum a broadly reactive antibody was transiently replaced by a monospecific autoanti-Jka. On preoperation evaluation, a 49-year-old man, who had never been transfused, exhibited both a positive antibody screen and a positive direct antiglobulin test. Although a broadly reactive antibody had been present 2 years earlier, only anti-Jka was found in the serum on preoperative testing. In contrast, an acid eluate prepared from the patient's red cells at the time was reactive with all cells from a 10-cell panel--a finding consistent with the broadly reactive autoantibody noted earlier. Repeat testing of a sample obtained 1 week later revealed only a broadly reactive autoantibody in both serum and eluate. This patient is notable in two respects. He exhibited a rare autoantibody, monospecific anti-Jka, and the specificity of the autoantibody changed from broadly reactive when first detected to anti-Jka and then back to broadly reactive. Thus, antibodies directed against specific antigens, in the setting of autoimmune hemolytic anemia, may be part of the autoimmune process and not always alloantibodies.

Because autologous donation is permitted for donors who do not meet homologous blood donation standards, referring physicians and blood center personnel may be concerned about autologous donor reactions. Small studies have determined that mild reactions do not occur more frequently, but the incidence of rarer, more serious, moderate and severe reactions is unknown. We therefore studied the frequency of reactions during 10,200 autologous and 219,307 concurrent homologous donations at four blood centers. No significant difference was seen for severe reactions: autologous 0.039% (4/10,200), homologous 0.037% (82/219,307) (p = 0.79); moderate reactions: autologous 0.19% (19/10,200), homologous 0.22% (473/219,307) (p = 0.60) or mild reactions: autologous 2.26% (231/10,200), homologous 2.26% (4946/219,307) (p = 0.98). We conclude moderate and severe donation reactions do not occur more frequently among autologous donors who are preselected by referring physicians and screened by blood center personnel.

To determine the epidemiology of cytomegalovirus (CMV) infections among patients with burns, we prospectively studied 120 burn patients admitted to the University of Iowa Burn Center over a two-and-one-half year period. At the time of their admission, 44% of the patients had serologic evidence of prior CMV infection. Among 44 seropositive patients, 23 (52%) had four-fold or greater rises in CMV antibody titers. These patients had more severe burns (mean body surface area burn [BSAB] 26.8%) than those who did not exhibit titer rises (mean BSAB 16.2%, p = .04). Among 43 seronegative patients observed for at least 65 days after discharge from the center, eight (18.6%) seroconverted. Patients who seroconverted had longer hospital stays (p = .03), trends toward more severe burns (p = .08) and a younger age (p = .15) than patients who remained seronegative. Despite frequent serologic evidence of CMV infection, CMV did not contribute, either directly or indirectly, to the morbidity or mortality of burns in these patients.

Binding of the chemotactic peptide N-formyl-methionyl-L-leucyl-L-phenylalanine (FMLP) by neutrophils from neonates was measured to investigate whether an abnormality of this event that initiates chemotaxis could explain the decreased chemotaxis of neonatal neutrophils. No differences of FMLP binding by neonatal and control neutrophils were found at 4 and 22 degrees C when specific binding was assessed by analysis of binding isotherms and Scatchard plots. At 4 degrees C (n = 6) the average number of FMLP molecules bound per neutrophil was 13,854 (neonates) and 17,368 (controls); affinity expressed as Kd was 32 nM (neonates) and 34 nM (controls). At 22 degrees C (n = 5) the respective values were 22,587 FMLP molecules per neutrophil (neonates) and 21,082 (controls); Kd was 29 nM (neonates) and 23 nM (controls). Based on these data, we suggest that mechanisms responsible for decreased chemotaxis of neonatal neutrophils must involve events subsequent to initial FMLP-receptor interaction.

We investigated concanavalin A capping that occurred either spontaneously (neutrophils incubated only with buffer) or was drug induced (neutrophils with colchicine or diamide) using neutrophils obtained from the blood of newborn infants and their mothers. A greater proportion of infant and maternal neutrophils than controls formed caps spontaneously (P less than 0.01). The percent of capped neutrophils (mean +/- S.E.) for 24 infants and their mothers was 23 +/- 3 and 39 +/- 4, respectively, versus 13 +/- 2 for 26 controls. Spontaneous capping was significantly decreased when neutrophils were incubated with catalase and superoxide dismutase to suggest that it was related, at least in part, to oxidation. Drug-induced capping of infant and maternal neutrophils was decreased when compared to controls (P less than 0.05). Colchicine increased capping of control neutrophils 11.76-fold above the spontaneous value, whereas capping of infant and maternal cells was increased only 2.35- and 1.65-fold. Corresponding values for diamide were 11.94-, 2.27, and 2.49-fold for control, infant, and maternal neutrophils, respectively. Many cellular processes are involved in capping, and the mechanisms responsible for aberrant capping of infant neutrophils remain undefined. However, this is another property that distinguishes infant neutrophils from those of older individuals.

A patient with common variable immunodeficiency disease is described with severe colitis confined to the rectosigmoid region. Inflammation was extensive in the regions involved and exhibited a character that we believe is most unusual. Inflammation was transmural in the regions involved. Macrophages were the major inflammatory cells, and no granulomas or giant cells were seen. Although the disorder seemed distinct from either ulcerative colitis or Crohn's disease, the colitis responded favorably to oral azulfidine, prednisone, and to steroid enemas.

The concentration of total glutathione (GSH + GSSG) and its depletion during oxidant stress were studied in neutrophils obtained from human neonates, their mothers and adult controls to determine if the previously reported increased susceptibility of infant neutrophils to oxidant damage could be related to glutathione deficiency. We found the basal level of glutathione to be lower and the oxidant-induced depletion of glutathione to be greater in infant neutrophils than in cells obtained from the blood of the infants' mothers and adult controls. In studies reported earlier, neutrophils from infants were found to be deficient in the oxidant protective enzymes glutathione peroxidase and catalase. These abnormalities, in a fashion similar to that observed for patients with congenital defects of glutathione and catalase metabolism, may render infant neutrophils prone to oxidant damage and consequent dysfunction.

Medical faculty, blood center directors, and technologists were surveyed to assess current opinions of knowledge deficiencies in transfusion medicine among medical school graduates, housestaff, and practicing physicians. Over 90% of those responding believed additional training in transfusion medicine was needed. Nine categories of deficiencies were identified, with more than 40% of respondents' comments reflecting concern about the lack of knowledge of the use of blood and blood components. Clinical faculty and blood bankers differed in their assessments of the knowledge deficiencies in two of the nine categories. Information obtained in this survey is relevant to educational development in transfusion medicine.

Effects of intermittent-flow centrifugation leukapheresis (IFCL) on leukocyte counts in donor blood have not been reported. Accordingly, total and differential white blood cell counts were preformed serially during IFCL. A significant decrease (p less than 0.05) in all types of leukocytes occurred in donor blood early in pheresis and was probably the consequence of hemodilution. Concentrations of most leukocyte types remained decreased throughout the procedure, and a rebound leukocytosis was not detected. Thus, leukocyte kinetics in donor blood during IFCL differ from those reported for filtration leukapheresis in which the pattern of selective neutropenia followed by rebound neutrophilia has been ascribed to complement activation.

Although granulocyte transfusions and bone marrow transplantation are becoming common clinical modalities, our knowledge of surface nonerythroid, nonlymphoid, non-HLA hematopoetic antigens remains very incomplete. Accordingly, we have systematically screened sera from recipients of multiple granulocyte and whole blood transfusions, and immunoneutropenic patients for antibodies directed primarily at granulocytes. The initial screens demonstrated that >50% of the sera from the above sources contained non-HLA cytotoxic and/or agglutinating antibodies. Preliminary clustering indicated seven possible new specificities detected by microgranulocytotoxicity. Calculations for Hardy-Weinberg goodness of fit based on a study of 98 unrelated donors plus informative families established that 5 of these were alleles of a single new locus termed Human Granulocyte Antigen (HGA)-3a, b, c, d, and e. Absorptions indicated that these antigens were present on mature granulocytes but absent from platelets, lymphocytes, monocytes, and myeloid precursors. A single antigen of another separate locus, HGA-1, was also identified. Absorptions revealed a quite different distribution for HGA-1 than HGA-3, this antigen being detected on monocytes and myeloblasts as well as on mature granulocytes. Independent segregation of the three loci from HLA, from the NA-NB and the 5a-5b antigens, and from themselves was confirmed in informative families.Finally, it seems likely that other antigens will be identified because several other sera that react with both monocytes and granulocytes have been detected.

Bacterial infections are the most frequent cause of death and an important factor of morbidity in sickle cell disease. A defect in oxidative metabolism of neutrophils from these patients has been reported as a possible cause for these infections. Since normal neutrophil functions are essential in the defense against bacteria, it seemed important to reassess the capacity of neutrophils from patients with sickle cell disease to undergo the metabolic events associated with phagocytic bactericidal activity. Accordingly, neutrophils from patients and controls were compared for their ability to reduce nitroblue tetrazolium dye, to activate the hexose monophosphate shunt, and to generate superoxide anion, hydrogen peroxide, and chemiluminescence. Patients' cells performed normally in each of these assays and, in addition, killed Staphylococcus aureus as well as did cells from controls. Thus, an abnormality of neutrophil oxidative metabolism cannot explain the propensity to bacterial infections in sickle cell disease.

Two controversial issues of neonatal transfusion practices, erythrocyte 'booster' transfusions and granulocyte transfusions, are critically reviewed, and current recommendations for transfusion practices are made. Infants should receive erythrocyte transfusions to treat congestive heart failure caused primarily by anemia. It is customary to maintain the hematocrit at greater than 40% in neonates with severe respiratory disease, although the efficacy of this practice has not been firmly established. Erythrocyte transfusions seem to be indicated for infants with anemia plus recurrent apnea, poor weight gain or the syndrome of tachycardia, tachypnea, dyspnea and poor feeding for which no other cause can be found. Granulocyte transfusions are likely to benefit seriously ill neonates exhibiting all three of the following: strong evidence of bacterial sepsis, neutropenia (compared to age-related normal values) and a diminished marrow neutrophil storage pool. Granulocyte transfusions for septic infants expressing only one or two of these features should be considered to be experimental therapy.

Postphagocytic chemiluminescence of neutrophils was evaluated in infants throughout the 1st month of life, in their mothers and healthy adults to investigate further the defective chemiluminescence of neonatal neutrophils described by earlier studies. Maternal and adult values were similar and served as controls. Abnormalities both of peak chemiluminescence and of the kinetics of light emission were detected in infant neutrophils throughout the 1st month of life. Infant responses were variable, particularly at birth, when neutrophils of some infants performed comparably to those of controls.

Venous blood was mixed with varying amounts of 6% hydroxyethyl starch solution in vitro and also blood samples were obtained from leukapheresis donors who had received infusions of hydroxyethyl starch. These specimens were evaluated as to the accuracy of erythrocyte typing, direct antiglobulin-testing, and antibody detection using saline, albumin, low-ionic strength saline and papain media. Specimens were stored at 4 degrees C, and testing was repeated at intervals during one week. Moderate difficulties were encountered, due to rouleaux formation, in the interpretation of typing and antibody screening in some samples that contained greater than 30 percent of the hydroxyethyl starch solution. Rouleaux formation was readily dispersed with saline and could easily be distinguished from true agglutination by microscopic examination. Although the apparent difficulties were not extreme, blood bank personnel should be alerted to the potentially misleading effects of hydroxyethyl starch on test results.

The properties of neutrophils, collected by discontinuous-flow centrifugation leukapheresis in the presence of hydroxyethyl starch and citrate were studied in an attempt to insure that cells harvested in this fashion are suitable for transfusion. Neutrophils obtained from leukocyte units prepared for transfusion by leukapheresis were found to perform similarly to those isolated from the venous blood of corresponding donors prior to the pheresis procedures when assessed by morphology, viability, hexose monophosphate shunt activity, superoxide anion generation, nitroblue tetrazolium dye reduction, chemiluminescence, adherence to nylon fibers, random mobility, chemotaxis and staphylococcal killing. The results compared favorably with values previously established for healthy control subjects. Neutrophils prepared in this manner should serve as a satisfactory blood component.

Quantitative and functional assessments were made of both the classical and alternative pathways of complement activation in sera from 23 patients with cystic fibrosis. The classical pathway functioned similarly in patients and controls as measured by CH50 titre. Alternative pathway function, initiated in patient sera by incubation with inulin, was equal to that of controls as determined by cleavage of Factor B and C3, and by the consumption of terminal components. Factor B, however, was more readily activated in patient than in control sera. This rapid alteration of Factor B did not lead to accelerated or more extensive activation of the terminal complement components via the alternative pathway when assessed by C3 cleavage and the consumption of terminal components. Thus, a complement deficiency was not found. The importance of the easily activated Factor B is undefined.

Postpartum hemolytic uremic syndrome is characterized by the findings of renal failure and microangiopathic hemolytic anemia in the puerperium. The case reported and literature reviewed demonstrate the need for early recognition and aggressive therapy in this rare and generally fatal condition.

Modern techniques of mechanical hemapheresis have made it possible to selectively remove vast quantities of lymphocytes and plasma immunoglobulins, and the concentration of these substances in donor blood can fall below the normal range. It is feared that this may lead to immunosuppression; a condition associated in some clinical settings with infections, malignancy and autoimmune diseases. Using primary immunodeficiency diseases and induced immunodeficiency states (for example, therapeutic lymphocytapheresis, chronic thoracic duct drainage and intestinal lymphangiectasia) as models to judge competency of the immune system, it can be predicted that body defense mechanisms can become defective when serum IgG levels are less than 200 mg/dl or the blood lymphocyte count is less than 1000/microliter. However, impaired immunologic function can occur in the presence of normal quantities of these substances in the blood stream; conditions that may be related either to imbalances of immune regulatory factors or to qualitative (rather than quantitative) abnormalities of the immune system. A number of investigators have documented the losses of lymphocytes and plasma immunoglobulins incurred by donors experiencing mechanical hemapheresis. In addition, both the immediate and long-term decreases in the concentration of these substances in donor blood have been reported. In summary, the immediate decreases in blood lymphocyte counts and serum immunoglobulin concentrations are of slight to moderate degree and are without known adverse effects. Less information is available regarding long-term alterations of the immune system, and little data have been collected from prospective studies in which large numbers of donors have been thoroughly evaluated by modern techniques. In general, results of many laboratory studies have been altered. However, these abnormalities have been transient for the most part, and it has been difficult to document clinically significant adverse effects. Thus, the quantities of blood lymphocytes and plasma immunoglobulins that can be removed from healthy donors without causing significant immediate or long-term harm is unknown. Bearing these limitations in mind, the following recommendations are suggested regarding the frequency of repeated mechanical plasma-and-cytapheresis. 1) The usual requirements for whole blood donation must be met if the frequency of mechanical hemapheresis does not exceed once every eight weeks.(ABSTRACT TRUNCATED AT 400 WORDS)

Although reactions to granulocyte transfusions in neonates are rarely reported, we observed a near-fatal pulmonary reaction, presumably due to white cell antibodies, in a neonate with Rh hemolytic disease. The hemolytic disease was being treated with exchange transfusions, and at 2 days after the infant's birth, bacterial sepsis was suspected and granulocyte transfusions were begun. The first granulocyte transfusion (Day 3) was uneventful. Five minutes after the beginning of the second granulocyte transfusion (Day 4), severe respiratory distress, hypotension, bradycardia, cyanosis, and acidosis suddenly occurred. The infant's serum obtained after the reaction contained granulocytotoxic and B-lymphocytotoxic antibodies that reacted with leukocytes from the second granulocyte donor. Antibodies could not be detected either in the initial infant serum or in maternal serum. However, an antileukocyte antibody was present in the serum of a parous woman donor. We used plasma from this woman to prepare reconstituted whole blood for the exchange transfusion that we performed immediately preceding the second granulocyte transfusion. Despite the sequence of events, an irrefutable cause-and-effect mechanism could not be established because the properties of the donor and neonatal antibodies were similar, but not identical. However, this catastrophic event emphasizes both the potential for adverse effects of granulocyte transfusions in neonates and the need for caution when transfusing blood from parous women.

Review of the literature, as it pertains to the use of therapeutic and prophylactic granulocyte transfusions in the treatment of children with cancer, leads to the conclusion that granulocyte transfusions are of definite value in only one clinical setting. That setting is the severely neutropenic patient with Gram-negative septicemia who has failed to improve in response to appropriate antibiotics and in whom prompt bone marrow recovery seems unlikely. The efficacy of therapeutic granulocyte transfusions in other situations has not been demonstrated. Most investigators agree that prophylactic granulocyte transfusions should not be used because the benefits are few and the risks are great.

In the second trimester of pregnancy in a 26-year-old woman, marked exacerbation of epileptic seizures occurred with somatomotor status epilepticus. The oral requirement of phenytoin varied, and up to 1,200 mg per day were needed to maintain a therapeutic plasma concentration during the second trimester. Intestinal malabsorption was shown to be a causal factor; 56 percent of the daily oral dose of phenytoin was found in the stool. Late in pregnancy and postpartum, therapeutic plasma concentrations of phenytoin were maintained with decreased daily oral doses. Intestinal absorption improved postpartum.
