201. Vibrio cholerae O1 experiences mild bottlenecks through the gastrointestinal tract in some but not all cholera patients

Abstract

ABSTRACT Vibrio cholerae O1 causes the diarrheal disease cholera, and the small intestine is the site of active infection. During cholera, cholera toxin is secreted from V. cholerae and induces a massive fluid influx into the small intestine, which causes vomiting and diarrhea. Typically, V. cholerae genomes are sequenced from bacteria passed in stool, but rarely from vomit, a fluid that may more closely represents the site of active infection. We hypothesized that V. cholerae O1 population bottlenecks along the gastrointestinal tract would result in reduced genetic variation in stool compared to vomit. To test this, we sequenced V. cholerae genomes from 10 cholera patients with paired vomit and stool samples. Genetic diversity was low in both vomit and stool, consistent with a single infecting population rather than coinfection with divergent V. cholerae O1 lineages. The amount of single-nucleotide variation decreased from vomit to stool in four patients, increased in two, and remained unchanged in four. The variation in gene presence/absence decreased between vomit and stool in eight patients and increased in two. Pangenome analysis of assembled short-read sequencing demonstrated that the toxin-coregulated pilus operon more frequently contained deletions in genomes from vomit compared to stool. However, these deletions were not detected by PCR or long-read sequencing, indicating that interpreting gene presence or absence patterns from short-read data alone may be incomplete. Overall, we found that V. cholerae O1 isolated from stool is genetically similar to V. cholerae recovered from the upper intestinal tract.IMPORTANCEVibrio cholerae O1, the bacterium that causes cholera, is ingested in contaminated food or water and then colonizes the upper small intestine and is excreted in stool. Shed V. cholerae genomes from stool are usually studied, but V. cholerae isolated from vomit may be more representative of where V. cholerae colonizes in the upper intestinal epithelium. V. cholerae may experience bottlenecks, or large reductions in bacterial population sizes and genetic diversity, as it passes through the gut. Passage through the gut may select for distinct V. cholerae mutants that are adapted for survival and gut colonization. We did not find strong evidence for such adaptive mutations, and instead observed that passage through the gut results in modest reductions in V. cholerae genetic diversity, and only in some patients. These results fill a gap in our understanding of the V. cholerae life cycle, transmission, and evolution.

Abstract

Objective Microbial keratitis (MK) is a significant cause of blindness in sub-Saharan Africa. We investigated the feasibility of using a novel corneal impression membrane (CIM) for obtaining and processing samples by culture, PCR and whole-genome sequencing (WGS) in patients presenting with suspected MK in Malawi.Methods and analysis Samples were collected from patients presenting with suspected MK using a 12 mm diameter polytetrafluoroethylene CIM disc. Samples were processed using culture and PCR for Acanthamoeba, herpes simplex virus type 1 (HSV-1) and the bacterial 16S rRNA gene. Minimum inhibitory concentrations of isolates to eight antimicrobials were measured using susceptibility strips. WGS was used to characterise Staphylococcus aureus isolates.Results 71 eyes of 71 patients were included. The overall CIM isolation rate was 81.7% (58 positive samples from 71 participants). 69 (81.2%) of isolates were Gram-positive cocci. Coagulase-negative Staphylococcus 31.8% and Streptococcus species 14.1% were the most isolated bacteria. Seven (9.9%) participants were positive for HSV-1. Fungi and Acanthamoeba were not detected. Moxifloxacin and chloramphenicol offered the best coverage for both Gram-positive and Gram-negative isolates when susceptibility was determined using known antimicrobial first quartile concentrations and European Committee on Antimicrobial Susceptibility Testing breakpoints, respectively. WGS identified known virulence genes associated with S. aureus keratitis.Conclusions In a resource-poor setting, a CIM can be used to safely sample the cornea in patients presenting with suspected MK, enabling identification of causative microorganisms by culture and PCR. Although the microbiological spectrum found was limited to the dry season, these preliminary results could be used to guide empirical treatment.

Abstract

Tuberculosis (TB) caused 1.5 million deaths in 2020, making it the leading infectious killer after COVID-19. Bacille Calmette-Guerin (BCG) is the only licensed vaccine against TB but has sub-optimal efficacy against pulmonary TB and reduced effectiveness in regions close to the equator with high burden. Efforts to find novel vaccines are hampered due to the need for large-scale, prolonged, and costly clinical trials. Controlled human infection models (CHIMs) for TB may be used to accelerate vaccine development by ensuring only the most promising vaccine candidates are selected for phase 3 trials, but it is not currently possible to give participants Mycobacterium tuberculosis as a challenge agent. This study aims to replicate and refine an established BCG CHIM at the Liverpool School of Tropical Medicine. Participants will receive an intradermal injection with licensed BCG vaccine (Statens Serum Institut strain). In phase A, participants will undergo punch biopsy two weeks after administration, paired with minimally invasive methods of skin sampling (skin swab, microbiopsy, skin scrape). BCG detection by classical culture and molecular methods will be compared between these techniques and gold standard punch biopsy. Techniques meeting our pre-defined sensitivity and specificity criteria will be applied in Phase B to longitudinally assess intradermal BCG growth two, seven and fourteen days after administration. We will also measure compartmental immune responses in skin, blood and respiratory mucosa in Phase B. This feasibility study will transfer and refine an existing and safe model of BCG controlled human infection. Longitudinal BCG quantification has the potential to increase model sensitivity to detect vaccine and therapeutic responses. If successful, we aim to transfer the model to Malawi in future studies, a setting with endemic TB disease, to accelerate development of vaccines and therapeutics relevant for underserved populations who stand to benefit the most. Abstract

Summary: Phosphate (Pi) serves countless metabolic pathways and is involved in macromolecule synthesis, energy storage, cellular signaling, and bone maintenance. Herein, we describe the coordination of Pi uptake and efflux pathways to maintain mammalian cell Pi homeostasis. We discover that XPR1, the presumed Pi efflux transporter, separately supervises rates of Pi uptake. This direct, regulatory interplay arises from XPR1 being a binding partner for the Pi uptake transporter PiT1, involving a predicted transmembrane helix/extramembrane loop in XPR1, and its hitherto unknown localization in a subset of intracellular LAMP1-positive puncta (named “XLPVs”). A pharmacological mimic of Pi homeostatic challenge is sensed by the inositol pyrophosphate IP8, which functionalizes XPR1 to respond in a temporally hierarchal manner, initially adjusting the rate of Pi efflux, followed subsequently by independent modulation of PiT1 turnover to reset the rate of Pi uptake. These observations generate a unifying model of mammalian cellular Pi homeostasis, expanding opportunities for therapeutic intervention.

Abstract

Summary: The canonical mechanism behind tamoxifen’s therapeutic effect on estrogen receptor α/ESR1+ breast cancers is inhibition of ESR1-dependent estrogen signaling. Although ESR1+ tumors expressing wild-type p53 were reported to be more responsive to tamoxifen (Tam) therapy, p53 has not been factored into choice of this therapy and the mechanism underlying the role of p53 in Tam response remains unclear. In a window-of-opportunity trial on patients with newly diagnosed stage I–III ESR1+/HER2/wild-type p53 breast cancer who were randomized to arms with or without Tam prior to surgery, we reveal that the ESR1-p53 interaction in tumors was inhibited by Tam. This resulted in functional reactivation of p53 leading to transcriptional reprogramming that favors tumor-suppressive signaling, as well as downregulation of oncogenic pathways. These findings illustrating the convergence of ESR1 and p53 signaling during Tam therapy enrich mechanistic understanding of the impact of p53 on the response to Tam therapy.

Summary: Background: Consumption of fibre, fruits and vegetables have been linked with lower colorectal cancer (CRC) risk. A genome-wide gene-environment (G × E) analysis was performed to test whether genetic variants modify these associations. Methods: A pooled sample of 45 studies including up to 69,734 participants (cases: 29,896; controls: 39,838) of European ancestry were included. Abstract

Bayesian adaptive experimental design is a form of active learning, which chooses samples to maximize the information they give about uncertain parameters. Prior work has shown that other forms of active learning can suffer from active learning bias, where unrepresentative sampling leads to inconsistent parameter estimates. We show that active learning bias can also afflict Bayesian adaptive experimental design, depending on model misspecification. We analyze the case of estimating a linear model, and show that worse misspecification implies more severe active learning bias. At the same time, model classes incorporating more "noise" - i.e., specifying higher inherent variance in observations - suffer less from active learning bias. Finally, we demonstrate empirically that insights from the linear model can predict the presence and degree of active learning bias in nonlinear contexts, namely in a (simulated) preference learning experiment.

Abstract

Consider a Brownian motion on the circumference of the unit circle, which jumps to the opposite point of the circumference at incident times of an independent Poisson process of rate $\lambda$. We examine the problem of coupling two copies of this `jumpy Brownian motion' started from different locations, so as to optimise certain functions of the coupling time. We describe two intuitive co-adapted couplings (`Mirror' and `Synchronous') which differ only when the two processes are directly opposite one another, and show that the question of which strategy is best depends upon the jump rate $\lambda$ in a non-trivial way. More precisely, we use the theory of stochastic control to show that there exists a critical value $\lambda^\star = 0.083\dots$ such that the Mirror coupling minimises the mean coupling time within the class of all co-adapted couplings when $\lambda<\lambda^\star$, but for $\lambda\ge \lambda^\star$ the Synchronous coupling uniquely maximises the Laplace transform $\mathbb{E}[e^{-\gamma T}]$ of all coupling times $T$ within this class. 210. Principal Effects on Academic Progress over Time and the Potential Effects of School Context and Principal Leadership Practices

Abstract

We used interrupted time series to examine the causal effect of principals on school performance over time and whether the trajectory of performance is affected by school context and leadership practices. Results were based on a unique dataset that links responses from the National Schools and Staffing Survey with 10 years of California Academic Performance Index (API) data. These data and methods address a need identified in the literature to examine the effects of principals with more rigorous research designs. Results indicated that improved academic performance does not occur immediately but accrues over time. We estimated both the first-year impact of a principal and the subsequent impact over time. The effect size increased from 0.04 in the first year to 0.21 by year three. Exploratory analyses showed that school context and leadership practices account for a substantial portion of the variation in progress among schools: 25% and 40%, for context and practices, respectively. Although the Schools and Staffing Survey afforded an opportunity to examine teacher and principal responses to academic performance over time, continued study is warranted to systematically and more concretely identify specific leadership practices that impact academic progress over time.

Abstract

Pines (n=121 species) are important elements of forest ecosystems. They are economically and ecologically valuable and are often at the center of efforts to manage forests to reduce the risk of wildland fires. The pattern and process of pine seed dispersal and seedling establishment have important implications for maintaining healthy forests. 75% of pines are dispersed by wind, and 25% are dispersed by scatter-hoarding birds and rodents. Among the wind-dispersed pines, there are about 20 species that attract the attention of seed-caching animals that gather seeds and cache them in soil, so these species are dispersed by a combination of wind and animals. Animal-dispersed pines often occur in semi-arid ecosystems. The seeds cached by animals are a dynamic resource. Animals pilfer each other's caches, move them to new sites and recache them. Some seed reside in dozens of different cache sites between seed maturation and seed germination. Many pines are adapted to fire. This involves serotinous cones (about 24 species), which are dense, woody, and lignified and remain closed at maturity. Serotiny establishes a canopy seed bank that can persist for several decades. Shortly after fire, these cones open and shed seeds onto the burned landscape. Pines often mast, producing large crops of seeds at intervals of several years. These large cone crops satiate the appetites of specialist seed predators, resulting in increased seedling establishment and also increases the effectiveness of seed dispersal. In the past, pines have responded to climate change by shifting geographic ranges, and some pines appear to be responding to warming climates in a similar way. Abstract

Objective To estimate the prevalence of individual chronic conditions and multimorbidity among adults admitted to hospital in countries in sub-Saharan Africa. Methods We systematically searched MEDLINE[R], Embase[R], Global Index Medicus, Global Health and SciELO for publications reporting on patient cohorts recruited between 1 January 2010 and 12 May 2023. We included articles reporting prevalence of pre-specified chronic diseases within unselected acute care services (emergency departments or medical inpatient settings). No language restrictions were applied. We generated prevalence estimates using random-effects meta-analysis alongside 95% confidence intervals, 95% prediction intervals and [I.sup.2] statistics for heterogeneity. To explore associations with age, sex, country-level income status, geographical region and risk of bias, we conducted pre-specified meta-regression, sub-group and sensitivity analyses. Findings Of 6976 identified studies, 61 met the inclusion criteria, comprising data from 20 countries and 376 676 people. None directly reported multimorbidity, but instead reported prevalence for individual conditions. Among medical admissions, the highest prevalence was human immunodeficiency virus infection (36.4%; 95% CI: 31.3-41.8); hypertension (24.4%; 95% CI: 16.7-34.2); diabetes (11.9%; 95% CI: 9.9-14.3); heart failure (8.2%; 95% CI: 5.6-11.9); chronic kidney disease (7.7%; 95% CI: 3.9-14.7); and stroke (6.8%; 95% CI: 4.7-9.6). Conclusion Among patients seeking hospital care in sub-Saharan Africa, multimorbidity remains poorly described despite high burdens of individual chronic diseases. Prospective public health studies of multimorbidity burden are needed to generate integrated and context-specific health system interventions that act to maximize patient survival and well-being. Nous avons retenu les articles traitant de la prevalence de certaines maladies chroniques predefinies au sein des services de soins intensifs, sans selection prealable (aux urgences ou en milieu hospitalier). Aucune restriction de langue n'a ete appliquee. Nous avons ensuite estime le taux de prevalence au moyen d'une meta-analyse a effets aleatoires, tout en appliquant des intervalles de confiance de 95%, des intervalles de prevision de 95% et une statistique I2 pour l'heterogeneite. Enfin, en vue d'examiner les liens avec l'age, le sexe, le statut de revenu du pays, la situation geographique et le risque de biais, nous avons mene des analyses predefinies de meta-regression, de sous-groupe et de sensibilite. Resultats Sur les 6976 etudes identifiees, 61 correspondaient aux criteres d'inclusion et contenaient des informations relatives a 20 pays et 376 676 personnes. Incluimos articulos que informaban sobre la prevalencia de enfermedades cronicas predefinidas en el marco de los servicios de cuidados intensivos no seleccionados (salas de urgencias o centros medicos hospitalarios). No se aplicaron restricciones linguisticas. Generamos estimaciones sobre la prevalencia utilizando un metaanalisis de efectos aleatorios, junto con intervalos de confianza del 95%, intervalos de prediccion del 95% y estadistica I2 de heterogeneidad. Para analizar la relacion con la edad, el sexo, el nivel de ingresos del pais, la zona geografica y el riesgo de sesgo, llevamos a cabo una metarregresion predefinida y un analisis de subgrupos y sensibilidad. Resultados De los 6976 estudios seleccionados, 61 cumplian los criterios de inclusion, y englobaban datos de 20 paises y 376 676 personas. Ninguno registraba multimorbilidad de manera directa, sino prevalencia de enfermedades individuales. Abstract

Abstract Cancers of the same tissue-type but in anatomically distinct locations exhibit different molecular dependencies for tumorigenesis. Proximal and distal colon cancers exemplify such characteristics, with BRAF V600E predominantly occurring in proximal colon cancers along with increased DNA methylation phenotype. Using mouse colon organoids, here we show that proximal and distal colon stem cells have distinct transcriptional programs that regulate stemness and differentiation. We identify that the homeobox transcription factor, CDX2, which is silenced by DNA methylation in proximal colon cancers, is a key mediator of the differential transcriptional programs. Cdx2-mediated proximal colon-specific transcriptional program concurrently is tumor suppressive, and Cdx2 loss sufficiently creates permissive state for BRAF V600E -driven transformation. Human proximal colon cancers with CDX2 downregulation showed similar transcriptional program as in mouse proximal organoids with Cdx2 loss. Developmental transcription factors, such as CDX2, are thus critical in maintaining tissue-location specific transcriptional programs that create tissue-type origin specific dependencies for tumor development.

Abstract

The geographical exchange of non-native species can be highly asymmetrical, with some world regions donating or receiving more species than others. Several hypotheses have been proposed to explain such asymmetries, including differences in propagule pressure, source species (invader) pools, environmental features in recipient regions, or biological traits of invaders. We quantified spatiotemporal patterns in the exchange of non-native insects between Europe, North America, and Australasia, and then tested possible explanations for these patterns based on regional trade (import values) and model estimates of invader pool sizes. Europe was the dominant donor of non-native insect species between the three regions, with most of this asymmetry arising prior to 1950. This could not be explained by differences in import values (1827–2014), nor were there substantial differences in the sizes of modelled invader pools. Based on additional evidence from literature, we propose that patterns of historical plant introductions may explain these asymmetries, but this possibility requires further study.

Abstract

AR Sco is a binary system that contains a white and red dwarf. The rotation rate of the white dwarf has been observed to slow down, analogous to rotation-powered radio pulsars; it has thus been dubbed a "white dwarf pulsar". We previously fit the traditional radio pulsar rotating vector model to the linearly polarised optical data from this source, constraining the system geometry as well as the white dwarf mass. Using a much more extensive dataset, we now explore the application of the same model to binary phase-resolved optical polarimetric data, thought to be the result of non-thermal synchrotron radiation, and derive the magnetic inclination angle $\alpha$ and the observer angle $\zeta$ at different orbital phases. We obtain a $\sim 10^{\circ}$ variation in $\alpha$ and $\sim 30^{\circ}$ variation in $\zeta$ over the orbital period. The variation patterns in these two parameters is robust, regardless of the binning and epoch of data used. We speculate that the observer is detecting radiation from an asymmetric emission region that is a stable structure over several orbital periods.
