Dental caries, one of the most prevalent infectious diseases worldwide, affects approximately 80% of children and the majority of adults. Dental caries may result in endodontic disease, leading to dental pulp necrosis, periapical inflammation and bone resorption, severe pain, and tooth loss. Periapical inflammation may also increase inflammation in other parts of the body. Although many studies have attempted to develop therapies for this disease, there is still an urgent need for effective treatments. In this study, we applied a novel gene therapeutic approach using recombinant adeno-associated virus (AAV)-mediated RNAi knockdown of Cathepsin K (Ctsk) gene expression, to target osteoclasts and periapical bone resorption in a mouse model. We found that AAV-sh-Cathepsin K (AAV-sh-Ctsk) impaired osteoclast function in vivo and furthermore reduced bacterial infection-stimulated bone resorption by 88%. Reduced periapical lesion size was accompanied by decreases in mononuclear leukocyte infiltration and inflammatory cytokine expression. Our study shows that AAV-RNAi silencing of Cathepsin K in periapical tissues can significantly reduce endodontic disease development, bone destruction, and inflammation in the periapical lesion. This is the first demonstration that AAV-mediated RNAi knockdown gene therapy may significantly reduce the severity of endodontic disease. [ABSTRACT FROM PUBLISHER]

Abstract: Introduction: The present study investigated whether bacteria infecting the root canal can activate any infiltrating T cells to produce receptor activator of nuclear factor kappa B (NF-κB) ligand (RANKL). Methods: Using a mouse model of periapical lesion induced by artificial dental pulp exposure, the presence of RANKL-positive T cells and osteoclasts in the periapical lesion was examined by an immunohistochemical approach. The bacteria colonizing the exposed root canal were identified by 16S ribosomal RNA (rRNA) sequence analysis. The isolated endodontic bacteria were further immunized to normal mice, and soluble activator of NF-κB ligand (sRANKL) production by the T cells isolated from the immunized mice was evaluated by ex vivo culture system. Results: RANKL-positive T cells along with TRAP+ osteoclasts were identified in periapical bone resorption lesions. The gram-negative bacterium Pasteurella pnumotropica, which was most frequently detected from the root canal of exposed pulp, showed remarkably elevated serum immunoglobulin G (IgG)-antibody response in pulp-exposed mice compared with control nontreated mice. Immunization of mice with P. pneumotropica induced not only serum IgG-antibody but also primed bacteria-reactive T cells that produced sRANKL in response to ex vivo exposure to P. pneumotropica. Conclusions: T cells infiltrating the periapical region express RANKL, and the endodontic bacteria colonizing the root canal appear to induce RANKL expression from bacteria-reactive T cells, suggesting the possible pathogenic engagement of the immune response to endodontic bacteria in the context of developing bone resorptive periapical lesions. Background: This report is a further analysis of a study designed to determine clinical and microbial risk indicators for progressing periodontitis. Methods: One hundred ninety subjects who were periodontally healthy or had early signs of periodontitis (age range: 20 to 40 years) were monitored clinically at 6-month intervals followed by supragingival cleaning. At each visit, gingival crevicular fluid (GCF) and blood were collected for determination of interleukin (IL)-1β content (in GCF) and IL-1 genotype (in blood). Interproximal sites with a >1.5-mm increase in clinical attachment over 18 months were considered disease active. Characteristics were compared between active and inactive subjects. Results: IL-1β levels in GCF increased with the severity of disease and correlated well with clinical signs of incipient disease. However, the IL-1 genotype did not show any significant associations with disease or the extent of disease. Conclusion: Indicators of inflammation may be important clinical determinants of future periodontal disease progression, but the IL-1 genotype was not a risk indictor for early (slight) periodontitis as defined in this subject population. Abstract: Introduction: The objective of this study was to evaluate the antimicrobial effects of photodynamic therapy (PDT) on infected human teeth ex vivo. Methods: Fifty-two freshly extracted teeth with pulpal necrosis and associated periradicular radiolucencies were obtained from 34 subjects. Twenty-six teeth with 49 canals received chemomechanical debridement (CMD) with 6% NaOCl, and 26 teeth with 52 canals received CMD plus PDT. For PDT, root canal systems were incubated with methylene blue (MB) at concentration of 50 μg/mL for 5 minutes, followed by exposure to red light at 665 nm with an energy fluence of 30 J/cm2. The contents of root canals were sampled by flushing the canals at baseline and after CMD alone or CMD+PDT and were serially diluted and cultured on blood agar. Survival fractions were calculated by counting colony-forming units (CFUs). Partial characterization of root canal species at baseline and after CMD alone or CMD+PDT was performed by using DNA probes to a panel of 39 endodontic species in the checkerboard assay. Results: The Mantel-Haenszel χ2 test for treatment effects demonstrated the better performance of CMD+PDT over CMD (P = .026). CMD+PDT significantly reduced the frequency of positive canals relative to CMD alone (P = .0003). After CMD+PDT, 45 of 52 canals (86.5%) had no CFUs as compared with 24 of 49 canals (49%) treated with CMD (canal flush samples). The CFU reductions were similar when teeth or canals were treated as independent entities. Post-treatment detection levels for all species were markedly lower for canals treated by CMD+PDT than they were for those treated by CMD alone. Bacterial species within dentinal tubules were detected in 17 of 22 (77.3%) and 15 of 29 (51.7%) canals in the CMD and CMD+PDT groups, respectively (P = .034). Conclusions: Data indicate that PDT significantly reduces residual bacteria within the root canal system, and that PDT, if further enhanced by technical improvements, holds substantial promise as an adjunct to CMD. J Periodont Res 2010; 45: 757-763. © 2010 John Wiley & Sons A/S 18β-Glycyrrhetinic acid (GA) is a natural anti-inflammatory compound derived from licorice root extract ( Glycyrrhiza glabra). The effect of GA on experimental periodontitis and its mechanism of action were determined in the present study. Periodontitis was induced by oral infection with Porphyromonas gingivalis W83 in interleukin-10-deficient mice. The effect of GA, which was delivered by subcutaneous injections in either prophylactic or therapeutic regimens, on alveolar bone loss and gingival gene expressions was determined on day 42 after initial infection. The effect of GA on lipopolysaccharide (LPS)-stimulated macrophages, T cell proliferation and osteoclastogenesis was also examined in vitro. 18β-Glycyrrhetinic acid administered either prophylactically or therapeutically resulted in a dramatic reduction of infection-induced bone loss in interleukin-10-deficient mice, which are highly disease susceptible. Although GA has been reported to exert its anti-inflammatory activity via downregulation of 11β-hydroxysteroid dehydrogenase-2 (HSD2), which converts active glucocorticoids to their inactive forms, GA did not reduce HSD2 gene expression in gingival tissue. Rather, in glucocorticoid-free conditions, GA potently inhibited LPS-stimulated proinflammatory cytokine production and RANKL-stimulated osteoclastogenesis, both of which are dependent on nuclear factor-κB. Furthermore, GA suppressed LPS- and RANKL-stimulated phosphorylation of nuclear factor-κB p105 in vitro. These findings indicate that GA inhibits periodontitis by inactivation of nuclear factor-κB in an interleukin-10- and glucocorticoid-independent fashion. [ABSTRACT FROM AUTHOR]

Abstract: Introduction: T-regulatory (Treg, CD4+ FOXP3+) cells constitute a unique subpopulation of CD4+ T cells that inhibit T-cell responses and prevent disease development/exacerbation in models of autoimmunity. In the present study, we tested the hypothesis that Treg cells are induced in periapical lesions by dental pulp infection. Methods: In situ hybridization (ISH) was used to localize FOXP3+ cells on day 21 after pulp exposure of the first molar teeth and infection with bacteria from the oral environment. FOXP3/GFP knock-in transgenic mice were used to quantify FOXP3+ Treg cells that infiltrate into periapical lesions by flow cytometry on days 7, 14, and 21 after infection. Periodontal ligament from uninfected teeth served as a negative control. Results: ISH showed strong signals that showed the presence of FOXP3+ cells mainly at the periphery of periapical lesions. In contrast, no positive cells were present in the periodontal ligament of uninfected controls. Flow cytometry showed an increase in the number of FOXP3+ Treg beginning between day 7 and day 14 (0.69% of the infiltrate) after infection and increased to day 21 (0.94%) (p < 0.05 and p < 0.001, respectively, vs uninfected controls). Treg were also increased in number in draining cervical lymph nodes after pulpal infection. Conclusions: These results show that Treg cells are induced to infiltrate into periapical lesions by pulpal infection and suggest that they increase in a time-dependent manner. Abstract: We investigated the photodynamic effects of methylene blue on multispecies root canal biofilms comprising Actinomyces israelii, Fusobacterium nucleatum subspecies nucleatum, Porphyromonas gingivalis, and Prevotella intermedia in experimentally infected root canals of extracted human teeth in vitro. The 4 test microorganisms were detected in root canals by using DNA probes. Scanning electron microscopy showed the presence of biofilms in root canals before therapy. Root canal systems were incubated with methylene blue (25 μg/mL) for 10 minutes followed by exposure to red light at 665 nm with an energy fluence of 30 J/cm2. Light was delivered from a diode laser via a 250-μm diameter polymethyl methacrylate optical fiber that uniformly distributed light over 360 degrees. Photodynamic therapy (PDT) achieved up to 80% reduction of colony-forming unit counts. We concluded that PDT can be an effective adjunct to standard endodontic antimicrobial treatment when the PDT parameters are optimized. [Copyright &y& Elsevier]

Periodontal disease involves multi-bacterial infections accompanied by inflammatory bone resorption lesions. The abundant T and B lymphocyte infiltrates are the major sources of the osteoclast differentiation factor, receptor activator for nuclear factor-kB ligand (RANKL) which, in turn, contributes to the development of bone resorption in periodontal disease. In the present study, we found that the concentrations of RANKL and regulatory T cell (Treg)-associated cytokine, interleukin (IL)-10, in the periodontal tissue homogenates were correlated negatively, whereas RANKL and proinflammatory cytokine, IL-1β, showed positive correlation. Also, according to the fluorescent-immunohistochemistry, the frequency of forkhead box P3 (FoxP3)/CD25 double-positive cells was diminished strikingly in the bone resorption lesion of periodontal disease compared to healthy gingival tissue, while CD25 or FoxP3 single positive cells were still observed in lesions where abundant RANKL+ lymphocytes were present. Very importantly, few or no expressions of FoxP3 by the RANKL+ lymphocytes were observed in the diseased periodontal tissues. Finally, IL-10 suppressed both soluble RANKL (sRANKL) and membrane RANKL (mRANKL) expression by peripheral blood mononuclear cells (PBMC) activated in vitro in a bacterial antigen-specific manner. Taken together, these results suggested that FoxP3/CD25 double-positive Treg cells may play a role in the down-regulation of RANKL expression by activated lymphocytes in periodontal diseased tissues. This leads to the conclusion that the phenomenon of diminished CD25+FoxP3+ Treg cells appears to be associated with the increased RANKL+ T cells in the bone resorption lesion of periodontal disease. Abstract: The aims of this study were to investigate the effects of photodynamic therapy (PDT) on endodontic pathogens in planktonic phase as well as on Enterococcus faecalis biofilms in experimentally infected root canals of extracted teeth. Strains of microorganisms were sensitized with methylene blue (25 μg/ml) for 5 minutes followed by exposure to red light of 665 nm with an energy fluence of 30 J/cm2. Methylene blue fully eliminated all bacterial species with the exception of E. faecalis (53% killing). The same concentration of methylene blue in combination with red light (222 J/cm2) was able to eliminate 97% of E. faecalis biofilm bacteria in root canals using an optical fiber with multiple cylindrical diffusers that uniformly distributed light at 360 degrees. We conclude that PDT may be developed as an adjunctive procedure to kill residual bacteria in the root canal system after standard endodontic treatment. [Copyright &y& Elsevier]

We have previously shown that two-dimensional, high-resolution, micro-computed tomography is a rapid, reproducible, and noninvasive method for measuring periradicular bone resorption in mice, giving results virtually identical to histology. In this study, we determined whether a three-dimensional volumetric quantitation of bone resorption could be achieved and whether this correlates with two-dimensional measurements. Periradicular lesions were induced in the lower first molars of mice by pulp exposure and infection; unexposed teeth served as controls. Mandibles were harvested on day 21 and subjected to: (a) three-dimensional micro-computed tomography imaging; and (b) conventional histology. Using a three-dimensional model and a semiautomatic contouring algorithm, we determined three-dimensional void volume, void surface, void thickness, and the standard deviation of the thickness distribution. The results showed a significant correlation between lesion void volume and two-dimensional lesion area by histology (r2 = 0.73), as well as high correlations between void volume and void thickness (r2 = 0.86) and standard deviation of the void thickness (r2 = 0.87), but no relationship with void surface. These results show that three-dimensional analysis of micro-computed tomography images is highly correlated with two-dimensional cross-sectional measures of periradicular lesions. Nevertheless, micro-computed tomography allows assessment of additional microstructural features as well as subregional analysis of lesion development. [Copyright &y& Elsevier]

Bacterial infection of the dental pulp results in pulpal destruction, and subsequently stimulates an inflammatory cell response and destruction of bone in the periapex. Bacterial components, including lipopolysaccharides, induce the production of many polypeptide mediators, or cytokines, by inflammatory cells. These cytokines, which include macrophage-derived interleukin-1 beta, interleukin-1 alpha and tumor necrosis factor, and lymphocyte-derived lymphotoxin, have been shown to potently stimulate bone resorption and to inhibit reparative bone formation in vitro and in vivo. T helper (TH) and T suppressor (TS) cells were enumerated in developing rat periapical lesions in order to identify the cell type associated with the active phase of lesion formation. Experimental pulp exposures were made in all molar teeth of 29 Sprague-Dawley rats, and teeth were left open to the oral environment. On days 15, 20, 30, and 90 after induction, inflammatory cells were isolated from periapical lesions, and T helper (W3/25 +), T suppressor (OX8 +), and total T cells (W3/13 +) were enumerated by reactivity with monoclonal antibodies. During the early, most active phase of lesion development = (day 15), TH cells outnumbered TS cells (TH/TS 1. 7). However, by day 20, when lesion expansion had slowed, the TH/TS ratio was reversed (0. 9) and remained so for up to 90 days after induction (TH/TS = 0. 7). Total T cell numbers did not change over the period of observation. TH cells thus predominated during the active phase of lesion development, whereas TS cells were associated with the chronic phase. This dynamic TH/TS reversal indicates that immune responses which occur within periapical lesions are highly regulated. In addition, the predominance of TH during the active phase of lesion development suggests that TH-mediated activities maybe involved in the bone destruction and immune cell activation within periapical lesions. The ratio of helper (T4+) to suppressor (T8+) T cells was determined in a population of periodontitis patients and in normal subjects. Although patients as a group were found to have the same T4/T8 ratio as normal individuals. that subgroup of patients previously identified as T cell low responders to oral microorganisms exhibited a relative increase in circulating suppressor T cells. Conversely, unusually low T4/T8 ratios were predictive of low level T cell proliferative responses generated against 7/12 oral microorganisms tested. Patients with low T4/T8 ratios had significantly more redness and bleeding on probing than did those with high T4/T8 ratios, but did not differ with respect to plaque. pocket depth, attachment loss, or episodes of disease activity. The relationship between T cell proliferative responses to oral microorganisms and periodontal disease status was studied utilizing a standardized T cell/monocyte mixture previously shown to generate maximal proliferative responses in vitro. T cells from periodontally healthy and diseased subjects were found to respond equally well to all oral microorganisms tested. When subjects were grouped according to their T cell response profile by cluster analysis, a generalized high responder and generalized low responder group was defined. Low responders were found to have significantly more redness and bleeding on probing than high responders. No differences were found between the two groups with respect to suppuration, plaque, attachment level, pocket depth, or bone loss. Patients not falling into the high or low responder groups had intermediate proliferative responses and moderate amounts of inflammation. These findings indicate that T cell proliferative responses to oral microorganisms may identify subgroups of individuals with unique clinical features. [ABSTRACT FROM AUTHOR]
