Abstract

Introduzione: Nel corso della stagione influenzale, gli Operatori Sanitari (OS) rappresentano una delle categorie lavorative maggiormente investite dalla responsabilità di limitare la diffusione della patologia influenzale tra i pazienti ospedalizzati e contemporaneamente di proteggere se stessi garantendo la continuità dell'assistenza sanitaria nei periodi di maggiore assenteismo lavorativo e sovraccarico delle attività assistenziali (picco influenzale stagionale). La vaccinazione antinfluenzale, ormai riconosciuta sicura ed efficace, rappresenta ancora oggi una argomento di dibattito tra gli OS, nonostante le raccomandazioni dei Center for Disease Control and Prevention Statunitense ed Europeo a mantenere le coperture tra gli OS sopra il 75%. Numerosi studi confermano come i tassi di vaccinazione antinfluenzale rimangano generalmente bassi sia tra gli OS che in particolare tra i medici in formazione specialistica (MFS). I risultati del presente studio multicentrico cercano di indagare le attitudini e i determinanti associati con la scelta vaccinale antinfluenzale tra i MFS italiani per comprendere le cause e proporre delle soluzioni. Materiali e Metodi: Nel 2012 è stata condotta una survey tra i MFS iscritti a 18 Università Italiane (Bari, Bologna, Brescia, Catania, Catanzaro, Chieti, L'Aquila, Messina, Modena, Napoli Federico II, Palermo, Pavia, Parma, Roma Cattolica, Roma Tor Vergata, Siena, Torino, Verona). Ciascun partecipante è stato contattato tramite una mail in cui erano illustrate le motivazioni dello studio e veniva chiesto di rispondere ad un breve questionario online, anonimo ed autosomministrato che includeva domande sulle attitudini riguardanti la vaccinazione antinfluenzale. Il questionario è stato precedentemente elaborato in collaborazione con il gruppo di lavoro nazionale e successivamente testato in uno studio pilota condotto nell'Università capofila dello studio (Palermo). Results: 2.506 MFS su un totale di 10.854 hanno aderito allo studio (23.1%). L'adesione alla vaccinazione antinfluenzale per la stagione 2011/2012 è stata pari al 11,9%, dato in chiara diminuzione rispetto al 2010/2011 (21.,7%) ed al 2009/2010 (15.5%). Abstract

BACKGROUND:In 2012, the ItalianMinistry of Health issued the National Immunization Prevention Plan (Piano Nazionale Prevenzione Vaccinale, or PNPV 2012-2014), with the aim of harmonizing immunization strategies across the country and ensuring equitable access to infectious disease prevention to all citizens. The Plan defines the immunization standards all regions should comply with.OBJECTIVE AND METHODS:As new evidence has accumulated in the field of immunization, and the new National Immunization Prevention Plan is about to be launched, the aim of the current study is to: i. present immunization coverage data (2000-2014) for 14 vaccines included in the PNPV to be offered to the general population, ii. assess to what extent the PNPV coverage targets and objectives have been met, and iii. report on how the PNPV was transposed into regional immunization programs. Data are also available for the eight regions that piloted varicella immunization.RESULTS:The 2012-2014 PNPV first introduced a "lifecourse" approach to vaccination at the institutional level, and has been a milestone for prevention in the Italian health policy agenda. However, infant vaccine coverage rates have been decreasing over the last years, as has influenza immunization in the elderly. HPV vaccine coverage has been increasing for all birth cohorts, but is still far below the targets set in the Plan. Promising preliminary data show that pneumococcal and meningococcal C conjugate vaccines were well introduced in regional immunization schedules.CONCLUSION:The 2012-2014 PNPV objectives have only been partially met, due to several factors, in particular increase in vaccine hesitancy. Strengthened efforts are needed to promote immunization. The new National Immunization Prevention Plan should introduce new vaccines and extend immunization programs to other target populations on the basis of the most recent scientific evidence available. It is of crucial importance that interventions of proven efficacy be planned and implemented to contrast the growing phenomenon of vaccine hesitancy and ultimately increase immunization uptake.

Abstract

Neisseria meningitidis causes severe invasive meningococcal diseases (IMDs) in humans including meningitis and septicemia, responsible for serious clinical conditions and leading to life-long disabilities and death. Serogroup B dominates IMDs burden in Italy, accounting for over 60% of total cases. On January 2013 the European Medicine Agency (EMA) licensed the first serogroup B meningococcal (Men B) vaccine in Europe. A number of European countries and Regions have introduced the new Men B vaccine in their immunization schedule, including Italy. In this paper we present the state of art, related critical issues and future perspectives of Men B vaccine introduction in Italy, in the context of the most recent available epidemiological data. In particular, we systematically assess the ongoing processes in the 8 Italian regions and one autonomous province that have already introduced Men B vaccine. With the new National Vaccine Prevention Plan including active Men B vaccine offer about to be adopted, it is of fundamental importance to gather further evidence on Men B vaccine clinical effectiveness, duration of protection and cost-effectiveness. Italian regions are called to organize and manage Men B immunization programs. Careful consideration will need to be devoted on timing, doses, and co-administration with other vaccines but also to economic assessments and strengthened communication to the general public. Abstract

INTRODUCTION:Immunization practices of athletes is a topic of growing interest in preventive medicine. Several reasons contribute to support such statement including: the demonstrated decrease of the immune response of athletes, the recent increase in vaccine-preventable diseases, the increasing mobility of professional athletes and coaches and sports doctors' frequent lack of awareness of on the issue of athletes' immune prophylaxis.RESULTS:This review offers a concise and schematic framework to the immunization practices in athletes, based on the most recent scientific evidences and the vaccine schedules proposed by international organizations and scientific societies including the recent "Life Vaccination Schedule" proposed by four Italian Scientific Societies and Professional Associations.CONCLUSION:vaccination in athletes, if correctly managed, represents a powerful, costly and long lasting tool for athletes sport teams.

Abstract

Introduction: Understanding the cause of their cancer is important for many cancer patients. Childhood cancer survivors'/survivors' parents' beliefs about cancer etiology are understudied. We aimed to assess survivors'/parents' beliefs about what causes childhood cancer, compared with beliefs in the community. We also investigated the influence of clinical and socio-demographic characteristics on the participants' beliefs about cancer etiology. Methods: This two-stage study investigated the participants' beliefs, by using questionnaires assessing causal attributions related to childhood cancer (stage 1) and then undertaking telephone interviews (stage 2; survivors/survivors' parents only) to get an in-depth understanding of survivors'/survivors' parents beliefs. We computed multivariable regressions to identify factors associated with the most commonly endorsed attributions: bad luck/chance, environmental factors and genetics. We analyzed interviews using thematic analysis. Results: Six hundred one individuals (64.6% survivors and 35.4% survivors' parents) and 510 community comparisons (53.1% community adults, 46.9% community parents) completed the question on causal attributions. We conducted 87 in-depth interviews. Survivors/survivors' parents (73.9%) were more likely to believe that chance/bad luck caused childhood cancer than community participants (42.4%). Community participants more frequently endorsed that genetics (75.3%) and environmental factors (65.3%) played a major role in childhood cancer etiology (versus survivors' and survivors' parents: genetics 20.6%, environmental factors: 19.3%). Community participants, participants with a first language other than English, and reporting a lower quality of life were less likely to attribute bad luck as a cause of childhood cancer. Community participants, all participants with a higher income and higher education were more likely to attribute childhood cancer etiology to environmental factors. Conclusion: Causal attributions differed between survivors/survivors' parents and community participants. Most of the parents and survivors seem to understand that there is nothing they have done to cause the cancer. Understanding survivors' and survivors' parents' causal attributions may be crucial to address misconceptions, offer access to services and to adapt current and future health behaviors. Abstract

The exact role of environmental risk factors in the etiology of the neurodegenerative disease amyotrophic lateral sclerosis (ALS) is still unknown. Their hypothetical contribution ranges from a minimal impact to a major role. Among the environmental factors strictu sensu (i.e., not life-style factors) suspected to play a role in ALS etiology, we consider pesticides, the metalloid selenium, some heavy metals, magnetic fields and cyanobacteria. However, the possibility exists that these factors exert their activity only in genetically susceptible persons and only after long-term exposures, thus further hampering epidemiologic studies. The recent availability of powerful tools such as population-based ALS registries for case ascertainment and clustering detection, and of environmental modeling techniques and of geographical information systems, may yield unique opportunities for offering insight into the etiology of the disease.

Abstract

L’edizione italiana di questo volume rappresenta uno strumento molto utile per chi si interessa degli aspetti sanitari collegati agli ambienti acquatici nei quali si svolge attività ricreativa: viene quindi messo a disposizione del lettore il testo di riferimento più autorevole sugli aspetti igienico-sanitari propri della frequentazione di piscine e di ambienti simili. I contenuti derivano dalla revisione completa della letteratura scientifica sull’argomento, alla quale abbiamo contribuito con numerosi studi svolti sul nostro territorio nazionale, e permettono utili confronti tra diverse realtà a livello internazionale. È comunque da rilevare come si tratti di un documento senza alcuna valenza di tipo normativo o impositivo: vengono presentate infatti indicazioni, vere e proprie “linee-guida”, che possono essere adattate alle diverse realtà locali, con l’intento di contribuire alla salute pubblica in una prospettiva di autocontrollo che prevede una stretta collaborazione tra l’autorità sanitaria, i gestori e gli utenti. Ed è grazie alla coerenza tra la natura propositiva di queste linee-guida e l’interesse alla promozione della salute pubblica da parte di un gruppo di studiosi della SItI che ne è stata predisposta la traduzione italiana: il gruppo che ha svolto il lavoro è costituito da docenti universitari afferenti all’area dello sport (Scienze Motorie e Sportive), spesso molto vicini nelle proprie sedi regionali sia al mondo delle Federazioni sia a quello delle Aziende Unità Sanitarie Locali, e dai loro studenti, appassionati utilizzatori delle piscine. Queste linee-guida hanno quindi un primo obiettivo: quello di fornire un riferimento di base, già condiviso da esperti di molti paesi, sulle caratteristiche di qualità degli ambienti acquatici ad uso ricreativo, che possono variare in funzione dei diversi tipi di struttura. Quanto riportato è stato tratto dai numerosi studi scientifici che hanno riguardato, tra gli altri temi, la valutazione dei rischi per la salute e l’efficacia delle misure di prevenzione e di controllo, unitamente ad una descrizione completa delle possibilità di monitoraggio e di valutazione di salubrità in un’ottica di tutela della salute pubblica. L’adozione di queste linee-guida fornirà quindi un suggerimento per il metodo di lavoro che può essere adottato dai gestori e dai controllori locali, oltre che rappresentare un riferimento autorevole per i progettisti di impianti di piscina e per le industrie che operano nel settore.
