BackgroundFloatation-REST (Reduced Environmental Stimulation Therapy), an intervention that attenuates exteroceptive sensory input to the nervous system, has recently been found to reduce state anxiety across a diverse clinical sample with high levels of anxiety sensitivity (AS). To further examine this anxiolytic effect, the present study investigated the affective and physiological changes induced by Floatation-REST and assessed whether individuals with high AS experienced any alterations in their awareness for interoceptive sensation while immersed in an environment lacking exteroceptive sensation.MethodsUsing a within-subject crossover design, 31 participants with high AS were randomly assigned to undergo a 90-minute session of Floatation-REST or an exteroceptive comparison condition. Measures of self-reported affect and interoceptive awareness were collected before and after each session, and blood pressure was measured during each session.ResultsRelative to the comparison condition, Floatation-REST generated a significant anxiolytic effect characterized by reductions in state anxiety and muscle tension and increases in feelings of relaxation and serenity (p < .001 for all variables). Significant blood pressure reductions were evident throughout the float session and reached the lowest point during the diastole phase (average reduction >12 mm Hg). The float environment also significantly enhanced awareness and attention for cardiorespiratory sensations.ConclusionsFloatation-REST induced a state of relaxation and heightened interoceptive awareness in a clinical sample with high AS. The paradoxical nature of the anxiolytic effect in this sample is discussed in relation to Wolpe's theory of reciprocal inhibition and the regulation of distress via sustained attention to present moment visceral sensations such as the breath.

Two main views have informed the literature on the psy-chology of emotion in the past few decades. On one side,cognitivists identify emotions with processes such as judg-ments, evaluations and appraisals. On the other side, advo-cates of non-cognitive approaches leave the “intellectual” as-pects of emotional experience out of the emotion itself, in-stead identifying emotions with embodied processes involv-ing physiological changes. Virtually everyone on either sideof the cognitive/non-cognitive divide identify William James’view, also known as the James-Lange theory, fully on the non-cognitivist side. But this is a mistake. Re-interpreting James’writings in its scientific context, this paper argues that he actu-ally rejected the cognitive/non-cognitive divide, such that hisview of emotions did not fit either side—that is, James was nota “Jamesian” in the sense the term is used in the literature.

The world view of rodents is largely determined by sensation on two length scales. One is within the animal's peri-personal space; sensorimotor control on this scale involves active movements of the nose, tongue, head, and vibrissa, along with sniffing to determine olfactory clues. The second scale involves the detection of more distant space through vision and audition; these detection processes also impact repositioning of the head, eyes, and ears. Here we focus on orofacial motor actions, primarily vibrissa-based touch but including nose twitching, head bobbing, and licking, that control sensation at short, peri-personal distances. The orofacial nuclei for control of the motor plants, as well as primary and secondary sensory nuclei associated with these motor actions, lie within the hindbrain. The current data support three themes: First, the position of the sensors is determined by the summation of two drive signals, i.e., a fast rhythmic component and an evolving orienting component. Second, the rhythmic component is coordinated across all orofacial motor actions and is phase-locked to sniffing as the animal explores. Reverse engineering reveals that the preBötzinger inspiratory complex provides the reset to the relevant premotor oscillators. Third, direct feedback from somatosensory trigeminal nuclei can rapidly alter motion of the sensors. This feedback is disynaptic and can be tuned by high-level inputs. A holistic model for the coordination of orofacial motor actions into behaviors will encompass feedback pathways through the midbrain and forebrain, as well as hindbrain areas.

BackgroundPaclitaxel is one of the most commonly used drugs to treat breast cancer. Its major dose-limiting toxicity is paclitaxel-induced peripheral neuropathy (PIPN). PIPN persists into survivorship and has a negative impact on patient's mood, functional status, and quality of life. No interventions are available to treat PIPN. A critical barrier to the development of efficacious interventions is the lack of understanding of the mechanisms that underlie PIPN. Mitochondrial dysfunction has been evaluated in preclinical studies as a hypothesized mechanism for PIPN, but clinical data to support this hypothesis are limited. The purpose of this pilot study was to evaluate for differential gene expression and perturbed pathways between breast cancer survivors with and without PIPN.MethodsGene expression in peripheral blood was assayed using RNA-seq. Differentially expressed genes (DEG) and pathways associated with mitochondrial dysfunction were identified between survivors who received paclitaxel and did (n = 25) and did not (n = 25) develop PIPN.ResultsBreast cancer survivors with PIPN were significantly older; more likely to be unemployed; reported lower alcohol use; had a higher body mass index and poorer functional status; and had a higher number of lower extremity sites with loss of light touch, cold, and pain sensations and higher vibration thresholds. No between-group differences were found in the cumulative dose of paclitaxel received or in the percentage of patients who had a dose reduction or delay due to PIPN. Five DEGs and nine perturbed pathways were associated with mitochondrial dysfunction related to oxidative stress, iron homeostasis, mitochondrial fission, apoptosis, and autophagy.ConclusionsThis study is the first to provide molecular evidence that a number of mitochondrial dysfunction mechanisms identified in preclinical models of various types of neuropathic pain including chemotherapy-induced peripheral neuropathy are found in breast cancer survivors with persistent PIPN and suggest genes for validation and as potential therapeutic targets.

The article is devoted to the study of the linguistic expression of the phenomenon of pain in the texts of F.M. Dostoevsky. The material of the analysis is the texts of works of fiction, journalism and letters of the Great Russian writer. The study of Dostoevskys texts is preceded by an analysis of entries in explanatory and specialized dictionaries and encyclopedias, including articles in the Dictionary of Dostoevskys Language. The relevance of the study is determined by the approaching bicentennial of F.M. Dostoevsky, in whose works the phenomenon of pain has become one of the important subjects of the image. The specificity of this work lies in the reliance on linguistic methods of studying the phenomenon of pain while maintaining the interdisciplinary focus of the research as a whole. Various associative connections of the word pain and its derivatives in the contexts of the writers works of different genres are being studied. At the initial stage, the selection of material is carried out by the method of continuous sampling using the DiaWin software system of the Department of Experimental Lexicography of the V.V. Vinogradov Russian Language Institute of the Russian Academy of Sciences. The material thus obtained, is subjected to a multilevel analysis, in course of which the specificity of the conceptualization of pain in language and culture is revealed and the features of the depiction and interpretation of the phenomenon of pain in the discourse of Dostoevsky and his characters are revealed. Conclusions are formulated about the stable associative connections of the lexeme pain, the specificity of the linguistic depiction of various states of pain in the writers texts is revealed, the original interpretations of the concept of pain in the discourses of heroes-ideologists are analyzed. As a result, the analyzed phenomenon appears in multidimensional illumination. The general structure of the frame is established. The relationship between external symptoms and internal sensations of physical and mental pain is revealed. It is postulated that the explication of the concept of pain by a native speaker is fraught with significant difficulties, since pain is introspective and subjectively experienced. This special state is experienced and lived on. The contextual combinations of meanings of the word pain in Dostoevskys texts are highlighted. The specificity of the figurative representation of pain situations in the writers works is revealed. The increments of meaning characteristic of Dostoevskys contexts are analyzed. As a result, pain is interpreted as a complex phenomenon of language and culture and an important concept in the discourse of F.M. Dostoevsky.

Background: To evaluate the relationship between measures of neuromuscular impairment and limb accelerations (LA) collected during sleep among individuals with chronic spinal cord injury (SCI) to provide evidence of construct and concurrent validity for LA as a clinically meaningful measure.Methods: The strength (lower extremity motor score), sensation (summed lower limb light touch scores), and spasticity (categorized lower limb Modified Ashworth Scale) were measured from 40 adults with chronic (≥ 1 year) SCI. Demographics, pain, sleep quality, and other covariate or confounding factors were measured using self-report questionnaires. Each participant then wore ActiGraph GT9X Link accelerometers on their ankles and wrist continuously for 1-5 days to measure LA from movements during sleep. Regression models with built-in feature selection were used to determine the most relevant LA features and the association to each measure of impairment.Results: LA features were related to measures of impairment with models explaining 69% and 73% of the variance (R²) in strength and sensation, respectively, and correctly classifying 81.6% (F1-score = 0.814) of the participants into spasticity categories. The most commonly selected LA features included measures of power and frequency (frequency domain), movement direction (correlation between axes), consistency between movements (relation to recent movements), and wavelet energy (signal characteristics). Rolling speed (change in angle of inclination) and movement smoothness (median crossings) were uniquely associated with strength. When LA features were included, an increase of 72% and 222% of the variance was explained for strength and sensation scores, respectively, and there was a 34% increase in spasticity classification accuracy compared to models containing only covariate features such as demographics, sleep quality, and pain.Conclusion: LA features have shown evidence of having construct and concurrent validity, thus demonstrating that LA are a clinically-relevant measure related to lower limb strength, sensation, and spasticity after SCI. Palatal sensation is important for articulation, feeding, and swallowing. However, palatal sensation in patients with cleft palate (CP) after palatoplasty has been investigated only inadequately because of the complexity and high costs of objective evaluation. This study compared the somatosensory evoked magnetic fields (SEFs) induced by electrical stimulation of the palates of patients with CP after palatoplasty and the palatal sensory thresholds (PSTs) of the stimulation with those of healthy (control) subjects. The CP group comprised 12 patients with unilateral cleft lip and palate (UCLP). The control group comprised 31 control subjects. No significant difference in intensity was found between them. Nevertheless, the PSTs in the UCLP group were significantly higher than those in the control group at all sites (p < 0.05). We infer that the electrical signals transmitted from palatal sensory receptors of patients with CP might be amplified by a compensation mechanism in the central nervous system. SEFs provide an effective method for objective evaluation of palatal sensation in patients with CP after palatoplasty. Evaluation of SEFs during palatal sensory stimulation in patients with CP after palatoplasty might lead to better corrective surgical methods that also preserve palatal sensation. • SEF latency in the palate is longer in UCLP patients than in healthy subjects. • SEF intensity shows no difference between UCLP patients and healthy subjects. • PSTs are higher in UCLP patients than in healthy subjects at all palate sites. • SEF measurement is an effective objective method for evaluating palatal sensation. Introduction: Autonomic nervous system functioning is theorised to underpin individual differences in sensory processing, with a small amount of supporting evidence. Despite recognition of the need to measure the balance between sympathetic and parasympathetic activity, most studies have measured only one of these systems. This study investigated whether sensory processing patterns are related to autonomic balance for young adults at rest and in response to a sensory-based stressor. Method: In this cross-sectional laboratory study, 100 young adults completed the adolescent/adult sensory profile. Heart rate variability and pre-ejection period were recorded before, during and after a coldpressor task. Results: Relationships between sensory processing patterns and autonomic balance varied by gender. In males, low sensory thresholds were generally related to a state of reciprocal sympathetic activation. In females, low thresholds were associated with greater autonomic reactivity in response to the coldpressor. Conclusion: This study showed, for the first time in young adults, that sensory processing patterns may be linked to patterns of autonomic balance. Therapists should be aware that young men with low sensory thresholds may be more likely than others to exhibit a resting state of sympathetic hyperarousal and further research should investigate whether this impacts their daily functioning and wellbeing. This essay considers the appeal of the 'water cave' scene in Boucicault's The Colleen Bawn to nineteenth-century spectators by focusing on its iconography and visual address. The written archive is of limited value on this question since contemporary commentators struggled to explain its popularity. Recent scholarly accounts have identified this and other sensation scenes as vehicles for eliciting spectator 'affect', meaning involuntary somatic responses, prompted in this case by the spectacle of physical danger. While this approach has been useful in seeking readings beyond those embedded in language and text, it has tended to confine the discussion to a narrow range of extreme affective states associated with anxiety and terror. This essay considers interpretations arising out of the 'affective turn' before proposing a new direction. Working across a range of images, I address the specific visual nature of the 'water cave' scene, drawing out a more subtle and multi-faceted set of ideas than those with which sensation drama is generally associated, and proposing an alternative reading to do with feminine power and transfiguration. Objective: Enhancement by using filler have become the material of choice for soft-tissue augmentation. Cosmetic procedures using fillers are easier, longer lasting, less immunogenic and can be broken down by hyaluronidase. However, early and delayed complications, ranging from minor to severe, can occur following filler injection. Main common complications are those that occurs earlier after injections, main goal is to discover the relation of the lip fillers with the loss of superficial sensation of lips after fillers as its translated into numbness and tenderness sensations. as an evaluation for these types of complications a survey had been designed in questionnaire form to evaluate (200) patients that had went through lip filler injections. Review of early onset of both tenderness and numbness is inserted in ranged pain scale type to evaluate the specific symptom the patient had, the lasting of the symptoms and the outcomes. Method: One specific module consisting of several questions regarding the specific site of filler injection, dose of dermal filler injections and the patient's symptoms will be obtained to understand the consequences of the filler injections and their relationship with the patient's sensation at site of injection, by a part of a preliminary review of n (20) patients. The review will be conducted by collecting answers from patients who had went through dermal fillers. Result: A total of 200 women participated in this survey from all over Iraq. Among them higher graduated from college (92.5). Their age mostly was in range of (30-39). Conclusion: In this paper we tried to investigate whether the amount of filler injected and the possibility of patient being participations were from Sulaymaniyah (37.0). Introduction: We investigate the construct validity, test re‐test reliability, and responsiveness of the Wrist Position Sense Test (WPST) for children with hemiplegic cerebral palsy (CP). Methods: Twenty‐eight children with spastic hemiplegic CP [mean age 10.8 years; SD 2.4 years] and 39 typically developing (TD) children [mean age 11 years; SD 2.9 years] participated in a cross‐sectional study to investigate construct validity and association with an upper limb activity measure, the Box and Block Test (BBT). Twenty‐two TD children were tested at a second time‐point to examine reliability. Test responsiveness was determined by random allocation of 17 children with CP to a treatment (n = 10) or control (n = 7) group with assessments completed at four time‐points. Results: Significantly greater differences were observed in mean error of indicated wrist position (p < 0.01) in children with CP at baseline (M = 21.6°, SD = 21.6°) than in TD children (M = 12.8°, SD = 11.0°). Larger WPST errors were associated with poorer performance on the BBT (p < 0.01) indicating a substantial association, and there were no consistent differences between time‐points indicating test re‐test reliability within a TD population. The WPST demonstrated responsiveness to intervention with a statistically significant reduction in mean error following treatment (p < 0.001), not seen in the control group (p = 0.28). Conclusion: The WPST demonstrated construct validity in this preliminary study. Scores were associated with an upper limb activity measure, and scores changed significantly following somatosensory training. These findings support further research and future psychometric investigation of the WPST in children with CP. About 69% of the 299,000 Americans living with spinal cord injury (SCI) experience long-term debilitating neuropathic pain. New treatments are needed because current treatments do not provide enough pain relief. We have found that insular-opercular brain network alterations may contribute to neuropathic pain and that restoring this network could reduce neuropathic pain. Here, we outline a study protocol using a physical therapy approach, cognitive multisensory rehabilitation (CMR), which has been shown to restore OP1/OP4 connections in adults post stroke, to test our hypothesis that CMR can normalize pain perception through restoring OP1/OP4 connectivity in adults with SCI and relieve neuropathic pain. To compare baseline brain function via resting-state and task-based functional magnetic resonance imaging in adults with SCI versus uninjured controls, and to identify changes in brain function and behavioral pain outcomes after CMR in adults with SCI. In this phase I randomized controlled trial, adults with SCI will be randomized into two groups: Group A will receive 6 weeks of CMR followed by 6 weeks of standard of care (no therapy) at home. Group B will start with 6 weeks of standard of care (no therapy) at home and then receive 6 weeks of CMR. Neuroimaging and behavioral measures are collected at baseline, after the first 6 weeks (A: post therapy, B: post waitlist), and after the second 6 weeks (A: post-therapy follow-up, B: post therapy), with follow-up of both groups up to 12 months. In scientific work, sonification (i.e., the use of nonspeech audio to convey information) is primarily thought of as a novel way to communicate post hoc research findings to lay audiences but only rarely, if ever, as a component of the research itself. This article argues that, rather than any inherent epistemological limitations of sound as a medium of scientific reasoning, this framing reflects a sociohistorical tendency to "silence" experimental techniques as they become widely adopted—both in terms of the literal silencing of noisy instrumentation and the elision of the role of sound (along with other nonvisual sensory media) in received discovery accounts. This tendency is well-illustrated by the history of electrophysiology research, particularly the case of the electrical penetration graph (EPG), an instrument used by entomologists to study how insects feed on plants. Combining participant observation, interviews, and analysis of EPG technical literature, this article recovers the unacknowledged role of sound in EPG's exploratory stages and gradual emergence as a standard laboratory technique as well as its gradual silencing as a consequence of this standardizing process. This article attends to "questionnaires" in linguistic fieldwork defined by the inclusion of sensory stimuli. It shows that such non-verbal protocols have been used to help elucidate and compare semantic content, which has generally been subordinated to formal analysis in the history of linguistics. To explain and exemplify this relationship, I target the color questionnaire developed by Hugo Magnus, which included ten standardized color chips and a long list of interview questions on language use. Magnus's questionnaire (Fragebogen) decoupled perception and denotation in its day, though similar protocols have recently been employed to restore the connection between meaning and form. The effect of postoperative sensation on quality-of-life (QoL) following nipple-sparing mastectomy (NSM) with implant-based reconstruction is not well described. We evaluated the impact of breast and nipple sensation on patient QoL by using BREAST-Q. Patients undergoing NSM with implant reconstruction from 2008 to 2020 were mailed a survey to characterize their postoperative breast and nipple sensation. BREAST-Q metrics were compared between totally numb patients and those with sensation. A total of 349 patients were included. Overall, 131 (38%) responded; response rates regarding breast and nipple sensation were 36% (N = 124/349) and 34% (N = 117/349). Median time from surgery to survey completion was 6 years. The majority had bilateral procedures (101, 77%), including direct-to-implant (99, 76%) and tissue expander (32, 24%) reconstruction. Regarding breast sensation, the majority of patients reported their reconstructed breasts as totally numb (47, 38%) or much less sensation than before surgery (59, 48%). Regarding nipple sensation, the majority of patients reported their nipples were totally numb (67, 57%) or had much less sensation than before surgery (37, 32%). Total numbness of reconstructed breasts resulted in a significantly lower chest physical well-being (mean score: 73.5 vs. 81.2, respectively, P = 0.048). Total numbness of postoperative nipple(s) resulted in significantly lower chest physical (mean score: 74.8 vs. 85.2, respectively, P = 0.007), psychosocial (mean score 77.4 vs. 84.4, respectively, P = 0.041), and sexual well-being (mean score: 55.7 vs. 68.3, respectively, P = 0.002). Long-term breast and nipple sensation are significantly diminished after NSM with implant reconstruction. In an indoor kitchen environment, proper installation of supplying air system is essential for maintaining optimal human thermal comfort level. Current thermal comfort investigation has been accomplished during "various cooking periods (breakfast, lunch, snacks, dinner)" inside the air‐conditioned kitchen environment of railway pantry car in India. The study goals to improve thermal comfort inside the pantry kitchen by implementing proper installation of the supply air system. A field experiment and computational fluid dynamics approach has been carried out during the summer season. Also, standard effective temperature (SET) index was utilized to determine the thermal comfort status. Three proposed design modified cases of pantry cars (based on supplying air system concepts) have been executed followed by validation and comparison with the existing case model. Results revealed modified Case III as a better design concept (indicating better air supply circulation and temperature decrease at the desired location) compared to the existing and other two proposed cases. Simultaneously, the SET index range was 26.5–28.6°C; indicating "comfortable thermal sensation throughout the cooking period." This study's outcomes suggest improvements in thermal comfort and energy savings of "air‐conditioned" pantry car kitchens in "Indian Railways" that could be referred directly. [ABSTRACT FROM AUTHOR]

Background: Substance abuse of a family member can be effective as an observational model regarding the tendency to substance abuse in other family members. The present study aimed to determine the effectiveness of mindfulness-based stress reduction (MBSR) intervention in distress tolerance and sensation-seeking in adolescents with an addicted parent in Ahvaz. Methods: This was a pre-test-post-test clinical trial study with a control group. The statistical population of the research comprised all adolescents with an addicted parent in Ahvaz City, Iran, in the school year 2019-2020. In this research, 30 adolescents with an addicted parent were selected through convenience sampling and equally divided into experimental and control groups (n=15). The distress tolerance scale and the sensationseeking questionnaire were used to collect data. The experimental group underwent MBSR intervention for eight 60-minute sessions. The control group received no intervention. The data were analyzed using analysis of covariance through SPSS software, version 23. Results: The results suggested that MBSR was effective in increasing distress tolerance and reducing sensation seeking of adolescents with an addicted parent in Ahvaz (P<0.001). Conclusion: Accordingly, this treatment can be used to reduce distress and sensation seeking in adolescents with an addicted parent and decrease their stress. [ABSTRACT FROM AUTHOR]

Background: There is currently no consensus on the efficacy of operative reinnervation at the time of post-mastectomy reconstruction. This review compares postoperative sensation between innervated and non-innervated flaps in breast reconstruction through systematic review and meta-analysis., Methods: EMBASE, Ovid, and CENTRAL were searched from inception to December 6, 2023. The primary outcome was level of sensation postoperatively. Secondary outcomes included presence of sensation, breast-related complications, operative time, and quality of life. Meta-analysis was conducted using mean difference (MD) or standardized mean difference (SMD) for continuous outcomes and odds radio (OR) for dichotomous outcomes. Quality of evidence was appraised using RoB-2/ROBINS-I. Certainty of evidence was assessed with Grading of Recommendations, Assessment, Development and Evaluations., Results: Twenty-eight studies were included; studies were observational studies except one randomized controlled trial. Common flaps were deep inferior epigastric perforator (11/28, 39.3%) and transversus rectus abdominis muscle (9/28, 32.1%). Innervated flaps were significantly associated with improved level of postoperative sensation (SMD: -0.94, 95% CI: [-1.35; -0.53], I 2 : 78%, p < 0.01), presence of postoperative sensation (OR: 11.4, 95% CI: [5.09; 24.5], I 2 : 0%, p < 0.01), and improved postoperative BREAST-Q scores (MD: 8.11, 95% CI: [4.33; 11.89], I 2 : 27%, p < 0.01). However, certainty of evidence was low/very low for all outcomes. Most studies had moderate risk of bias (56.5%). There were no statistically significant differences in breast-related complications or operative time., Conclusions: Operative reinnervation in breast reconstruction is significantly associated with improved level and presence of postoperative sensation and BREAST-Q scores without significant increase in breast-related complications/operative time. It has been established that near infrared (NIR) spectroscopy has the potential of estimating sensory traits given the direct spectral responses that these properties have in the NIR region. In sweetpotato, sensory and texture traits are key for improving acceptability of the crop for food security and nutrition. Studies have statistically modelled the levels of NIR spectroscopy sensory characteristics using partial least squares (PLS) regression methods. To improve prediction accuracy, there are many advanced techniques, which could enhance modelling of fresh (wet and un-processed) samples or nonlinear dependence relationships. Performance of different quantitative prediction models for sensory traits developed using different machine learning methods were compared. Overall, results show that linear methods; linear support vector machine (L-SVM), principal component regression (PCR) and PLS exhibited higher mean R 2 values than other statistical methods. For all the 27 sensory traits, calibration models using L-SVM and PCR has slightly higher overall R 2 (x¯ = 0.33) compared to PLS (x¯ = 0.32) and radial-based SVM (NL-SVM; x¯= 0.30). The levels of orange color intensity were the best predicted by all the calibration models (R 2  = 0.87 - 0.89). The elastic net linear regression (ENR) and tree-based methods; extreme gradient boost (XGBoost) and random forest (RF) performed worse than would be expected but could possibly be improved with increased sample size. Lower average R 2 values were observed for calibration models of ENR (x¯ = 0.26), XGBoost (x¯ = 0.26) and RF (x¯ = 0.22). The overall RMSE in calibration models was lower in PCR models (X = 0.82) compared to L-SVM (x¯ = 0.86) and PLS (x¯ = 0.90). ENR, XGBoost and RF also had higher RMSE (x¯ = 0.90 - 0.92). Effective wavelengths selection using the interval partial least-squares regression (iPLS), improved the performance of the models but did not perform as good as the PLS. Background: Individuals living with intra-articular temporomandibular disorders (IA-TMDs) often report limitations with mouth opening activities. While clinical measures such as active range of motion (AROM) and movement quality are often used to assess mouth opening function, it is unclear if and how these relate to patient-reported limitations and whether other factors such as kinesiophobia influence mouth opening activities in those with IA-TMDs., Objectives: Compare clinical measures of mouth opening function in those with IA-TMDs to asymptomatic controls. In those with an IA/TMD, explore relationships between patient-reported mouth opening limitations, and mouth opening function and kinesiophobia., Design: Cross-sectional study., Method: Clinical mouth opening function (AROM, movement quality, pain on movement/10, stiffness on movement/10) was compared between groups (n = 30 IA-TMD, n = 30 controls). Within the IA-TMD group, correlations between patient-reported mouth opening limitations (Patient specific functional scale), kinesiophobia (Tampa Scale for Kinesiophobia for Temporomandibular Disorders) and clinical measures of mouth opening function were explored., Results: Impairments in AROM (-4 mm, p = 0.04, d = 0.5), movement quality (p < 0.01, φ = 0.6), pain on movement (p < 0.01, d = 0.8) and stiffness on movement (p < 0.01, d = 1.6) were observed in the IA-TMD group compared to controls. Patient-reported mouth opening limitations and kinesiophobia were significantly correlated (r = -0.48, p < 0.01); no correlation was found between patient-reported limitations and clinical mouth opening measures (r < 0.3, p > 0.05)., Conclusions: Mouth opening function is impaired in IA-TMD. However, kinesiophobia appears more related to patient-reported mouth opening limitations than clinical impairments. Background: Repetitive transcranial magnetic stimulation (rTMS) targeting the primary somatosensory cortex (S1) has a potential effect on somatosensory functioning following a stroke. However, S1-rTMS was combined with peripheral therapies in previous trials. Moreover, these studies have commonly targeted the ipsilesional S1 with excitatory rTMS paradigms., Methods: This double-blind, randomized trial (registration number: ChiCTR2200059098) investigated two forms of paradigms, that is ipsilesional excitatory and contralesional inhibitory rTMS, as a stand-alone treatment in post-stroke somatosensation. Patients in the acute and subacute phases of stroke were randomly assigned to either contralesional 1-Hz or ipsilesional 10-Hz rTMS group and received 10 daily sessions of treatment in two consecutive weeks., Results: Results indicate that the contralesional inhibitory and ipsilesional excitatory stimulation were equally effective in improving somatosensory functioning. Moreover, this effect was most prominent in deep sensations and subjective sensations. Introduction: Ischemic stroke (IS) is one of the three main fatal disorders and is a major health challenge. 4-methylumbelliferone (4-MU) is one of the coumarin derivatives (7-hydroxy-4-methylcoumarin) with antioxidant and anti-inflammatory impact. This study was conducted to elucidate the neuroprotective effects and anti-inflammatory impact of 4-MU in a rodent model of IS., Methods: The IS model was induced by middle cerebral artery occlusion (MCAO) for 1 hour and reperfusion was established for 24 hours. 44 Male Wistar rats were divided into four groups: 1) Sham, 2) MCAO, 3) MCAO + Vehicle, and 4) MCAO + 4-MU (25 mg/kg). Evaluation of neurological deficit was performed using Garcia's score. 2,3,5-triphenoyl-2H-tetrazolium chloride (TTC) staining was employed to measure infarct size. Nissl staining was applied to determine neuronal loss. Moreover, western blotting was utilized to detect the expression of the proteins relevant to the TLR4/NF-κB/NLRP3 axis (p-NF-κB p65, TLR4, NLRP3, IL-1β, IL-10, IL-18, ASC, and Caspase-1)., Results: It was observed that MCAO caused neurological deficit (P<0.0001), infarct (P<0.0001), and neuronal loss (P<0.002); up-regulated NLRP3 (P<0.0001), TLR4 (P<0.0001), p-NF-κB p65 (P<0.0005), IL-1β (P<0.0014), IL-18 (P<0.0001), ASC (P<0.0027), and Caspase-1 (P<0.0052); and reduced IL-10 concentrations (P<0.0024). Administration of 4-MU (25 mg/kg) quickly after reperfusion reduced neurological deficit (P<0.0001), infarct size (P<0.0001), neuronal loss (P<0.0058), and down-regulated NLRP3 (P<0.0257), TLR4 (P<0.0001), p-NF-κB p65 (P<0.0075), IL-1β (P<0.0106), IL-18 (P<0.0005), ASC (P<0.0072), and Caspase-1 (P<0.0315), and increased IL-10 concentrations (P<0.0215)., Conclusion: These results indicate that 4-MU can attenuate injury after MCAO by suppressing the TLR4/NF-κB/NLRP3 axis. Our findings show that 4-MU can be considered a novel therapeutic compound to cure IS.

Background: Sleep paralysis (SP) is a rapid eye movement (REM) parasomnia that occurs during the transition between wakefulness and sleep. During this brief state, a person is conscious but unable to move or speak, often experiencing hallucinations. It is 'isolated' when it occurs without other symptoms of narcolepsy or sleep disorders. Despite its prevalence, much remains unknown about its clinical features and associated beliefs., Objectives: This study examines the frequency of self-reported SP experiences and its associated demographic characteristics, along with participants' beliefs about the condition., Methods: A cross-sectional survey was conducted among visitors at a shopping mall, using the Unusual Sleep Experiences Questionnaire (USEQ) to assess the features of SP episodes and the common beliefs surrounding them., Results: A total of 350 participants were surveyed (mean age = 25.5 ± 9.30 years; 55.4 % females). Participants who experienced SP at least once in their life time (39.1 %) reported feeling pressure on their chest (67.2 %), an inability to open their eyes (71.5 %), and difficulty speaking (81.0 %) during the episode. The majority of the individuals reported having their first episode at a young age (16-20 years). Most people (24.5 %) were unaware that this condition was known as sleep paralysis, and many (23.5 %) thought that it was just 'a dream'., Conclusion: Isolated sleep paralysis episodes are fairly prevalent, with many people experiencing their first episode at a young age. A typical episode entails a sense of pressure on the chest and difficulty in vocalizing. Objective: To evaluate the accuracy of the Ipswich touch test compared to the 10-g monofilament test for identifying of loss of protective sensation in Thai patients with diabetes mellitus., Methods: A cross-sectional observational study was conducted on Thai patients with diabetes mellitus who attended routine annual foot check-ups in an outpatient diabetes clinic. The loss of protective sensation was assessed by the Ipswich touch test and compared with the Semmes-Weinstein 10-g monofilament test. Sensitivity, specificity, predictive values and likelihood ratios were calculated to measure the accuracy of the Ipswich touch test against 10-g monofilament as a reference standard. The interrater reliability of the Ipswich touch test was assessed by two raters., Results: In a study of 283 diabetic patients, 10-g monofilament detected a 25 % prevalence of loss protective sensation. The Ipswich touch test demonstrated a good diagnostic accuracy, with a sensitivity of 70.8 %, specificity of 98.6 %, and an area under the curve of 0.85 when compared to 10-g monofilament. Positive and negative predictive values were 94.4 % and 90.8 % respectively. The positive likelihood ratio was 49.82, and the negative likelihood ratio was 0.30. Interrater reliability, assessed with two raters in a subset of 93 participants, yielded a kappa of 0.88, indicating almost perfect agreement., Conclusions: The Ipswich Touch Test demonstrated good accuracy and interrater reliability compared to the standard 10-g monofilament, thus establishing its effectiveness as a valuable diagnostic tool for identifying loss of protective sensation among diabetic patients. Purpose: Common peroneal nerve (CPN) neuropathy is the most common lower extremity mononeuropathy. When delayed or no recovery from CPN neuropathy is suspected, surgical CPN decompression (CPND) is considered to relieve symptoms. This study aimed to evaluate patient outcomes post-CPND performed by a single surgeon at a tertiary medical center., Methods: Patient outcomes after CPND performed by a single surgeon were reviewed. Motor, sensation, and pain scores post-CPND were assessed in 47 of the 46 patients. Patient demographics, including age, concomitant morbidities, time from injury to surgery, and body mass index (BMI), were also analyzed for correlations with outcomes after CPND by logistic regression., Results: 29/34 patients with impaired motor function improved by at least one motor grade, 19/42 with altered sensation reported restored normal sensation, and 31/37 reported improved pain after CPND. No correlation of patient demographic factors with motor or pain improvement after CPND was observed. Background: Radial forearm free flap phalloplasty (RFFF) is a set of complex reconstructive procedures aimed at creating an aesthetic and functional penis in transgender patients. Sensory recovery in the neophallus and donor site is crucial for optimizing outcomes, but the few prior studies that exist assess neophallus sensation at limited locations and time points. The purpose of this study was to prospectively quantify sensory outcomes in the neophallus and donor site following RFFF phalloplasty., Methods: Sensation testing occurred prospectively over February 2019-January 2021 on Stage 1 RFFF phalloplasty patients using the Pressure Specified Sensory Device (PSSD). On the neophallus, one-point discrimination (1PS) pressure threshold and lengthwise sensory recovery were measured at six circumferential locations proximally to distally. On the donor site, 1PS was measured at three locations on the donor hand., Results: Nineteen patients were included (average age 34.0 years old, range 18-53 years). Among patients that received neophallus testing (n = 13), eight had at least two follow-up appointments. Six of these patients had sensation as of their most recent measurement (75.0%), with an average of 73 days to regain sensation. There was a significantly greater proportion of patients with sensation at the right ventral (80.0% after 3 months vs. 11.1%-60.0% before 3 months, p = 0.024) and right lateral (100.0% after 3 months vs. 11.1%-60.0% before 3 months, p = 0.004) aspects of the neophallus over time. Pressure required to elicit sensation decreased by 18.0% from 1 week-1 month postoperatively to 3-7.7 months postoperatively in the right ventral neophallus (96.2 g/mm 2  ± 11.3 g/mm 2 to 56.6 ± 39.9 g/mm 2 , p = 0.037). Among patients that received donor site testing (n = 11), mixed effects regression analysis with random intercepts demonstrated significant changes in the thumb (3.4 g/mm 2 ± 1.4 g/mm 2 , p < 0.05) and webspace (13.5 g/mm 2 ± 4.9 g/mm 2 , p < 0.01) that returned to baseline at 3 months postoperatively (1.7 g/mm 2  ± 1.0 g/mm 2 , p > 0.05, and 2.3 g/mm 2  ± 4.0 g/mm 2 , p > 0.05, respectively)., Conclusion: This pilot study demonstrates that quantitative sensory testing can be used to monitor post-phalloplasty sensory changes. Recovery was significantly associated with contralateral (i.e, right side in a left forearm RFF) aspects of the neophallus, suggesting a possible pattern of circumferential sensory innervation via RFFF sensory nerves. Numerous studies from different international groups have demonstrated that sensations can be propagated along acupuncture channel pathways. The propagated sensation along the channel pathway (PSCP) can be elicited by electroacupuncture (EA), transcutaneous electrical nerve stimulation (TENS), manual acupuncture (MA), and heat applied to distal acupuncture points (acupoints). Nitric oxide (NO) levels were reported to be elevated in the gracile nucleus and skin regions near to the EA sites, with higher levels at acupoints associated with an enhanced expression of NO synthase and transient receptor potential vanilloid type 1. The stimuli, EA, MA, TENS, and heat, have been used to elicit axonal reflexes, which increase local release of NO and neuropeptides such as calcitonin gene related peptide. Furthermore, the sensation of PSCP along the body surface occurs only ipsilaterally to the stimulated acupoints in various human studies, which does not support the involvement of the spinal-thalamic pathway, which would involve cross over transmission of the signals. The gracile nucleus receives ascending input from the sciatic nerve and responds to somatosensory stimulation mainly on the ipsilateral side via the dorsal column pathway. EA at Zusanli (ST36) increases NO release and expression of NO synthase mainly in the ipsilateral side of the gracile nucleus, while the cardiovascular effects and analgesic responses to EA at ST36 are changed by influences of l-arginine-derived NO synthesis in the ipsilateral gracile nucleus in rats. The stimuli-induced release of NOergic molecules and neuropeptides exist high levels in the acupoints, which contain rich neuronal components and blood vessels. Enhanced NOergic molecules at acupoints cause axon reflexes during the stimuli, which elevate cutaneous blood flow. Elevated NOergic molecules and local blood flow may spread over acupoints one after another along the meridian lines differing from nerve pathways following the stimuli to induce PSCP. The same types of stimulation also elicit NO release in the gracile nucleus, which contributes to the somatosensory signal transduction of PSCP through the dorsal medulla-thalamic pathways. Other substances such as serotonin and catecholamines are proposed to mediate responses and certain effects of acupuncture-like stimulation but their mechanisms are poorly-understood. In this review we summarize the current understanding of the neurobiological processes of PSCP research with an emphasis on recent developments of NO mediating stimulation-evoked axon reflexes and somatosensory signal transduction for PSCP perceptions through the dorsal medulla-thalamic pathways. Purpose of Review: To review findings related to phantom genital sensation, emphasizing phantom sensation in the transgender and gender diverse (TGD) population. We discuss prevalence, presentation and potential implications for sensory outcomes in genital gender-affirming surgery., Recent Findings: There is a high prevalence of phantom genital sensations in the TGD population. The prevalence varies by body part, approaching 50% in the most frequently reported transgender phantom - the phantom penis. Unlike genital phantoms that occur after trauma or surgery which are often painful, transgender phantoms are typically neutral and often erogenous in experience. Phantom sensation in the TGD population can be an affirming experience and important part of sexual well being and embodiment., Summary: Recent studies have begun to characterize the prevalence and presentations of phantom genital sensations in TGD people, informing our evolving understanding of the sensory experiences of the transgender and gender diverse population. Targeting integration of these centrally-mediated phantom genital sensations with the peripherally generated sensation from genital stimulation may represent one potential avenue to improve sensation and embodiment following genital gender-affirming surgical procedures. Background: To prevent rehospitalization for heart failure (HF), patients need to be able to perceive physical changes that occur at the onset of HF exacerbation and seek early help., Objective: The aim of this study was to evaluate the effect of a self-monitoring intervention on patients' perceptions of physical sensations during daily activities in the context of HF via a randomized controlled trial., Methods: Participants (N = 70) were randomly assigned to the intervention (received daily activity record-based self-monitoring intervention support; group A) or control (only explained the measured results from the records; group B) group. Group A reflected on and described the physical sensations in their daily activities within 1 month after discharge. Outcome measures were assessed at 1 month after the intervention using the European Heart Failure Self-care Behavior Scale, Evaluation Scale for Self-Monitoring by patients with Heart Failure, clinical events, physical activity, and sleep., Results: There was no significant difference in the change in the "asking for help" subscale score of the European Heart Failure Self-care Behavior Scale between the groups (+0.7 vs +0.4 points, P = .716). Group A had improved score on the self-monitoring subscale related to "concern about how movements affect body" from baseline (from 12.7 to 14.0 points, P = .026). There was no significant effect of self-monitoring intervention support on the first rehospitalization related to HF and all-cause death (log-rank χ 2 = 0.432, P = .511). Background: The fitting of a prosthesis after lower limb amputation is associated with several challenges. Skeletal stump-prosthesis interfaces and selective nerve transfer can partially overcome these but are also associated with new impairments that emphasize the necessity of innovative approaches. The concept of so-called spare part surgery with the use of fillet flaps could play an important role in this respect., Objective: An overview of the classical prosthesis-associated discomforts, advantages and disadvantages of treatment strategies and presentation of alternative surgical concepts., Material and Methods: A selective literature search was carried out considering the experiences of the authors and perspectives with respect to the advantages and disadvantages of the surgical treatment options. Furthermore, a clinical case is presented., Results and Conclusion: The transfer of the sole of the foot as a fillet flap to the weight-bearing region of the amputation stump offers a number of benefits, such as creating a fully weight-bearing stump, prevention of neuralgia, preserved sensation and conservation of the body image. As long as the calcaneal region is not impaired, this technique can be performed in amputations below as well as above the knee. The question of whether parts of the bone should be included in the transfer must be individually evaluated for each patient. Taxonomies and ontologies for the characterization of everyday sounds have been developed in several research fields, including auditory cognition, soundscape research, artificial hearing, sound design, and medicine. Here, we surveyed 36 of such knowledge organization systems, which we identified through a systematic literature search. To evaluate the semantic domains covered by these systems within a homogeneous framework, we introduced a comprehensive set of verbal sound descriptors (sound source properties; attributes of sensation; sound signal descriptors; onomatopoeias; music genres), which we used to manually label the surveyed descriptor classes. We reveal that most taxonomies and ontologies were developed to characterize higher-level semantic relations between sound sources in terms of the sound-generating objects and actions involved (what/how), or in terms of the environmental context (where). This indicates the current lack of a comprehensive ontology of everyday sounds that covers simultaneously all semantic aspects of the relation between sounds. Such an ontology may have a wide range of applications and purposes, ranging from extending our scientific knowledge of auditory processes in the real world, to developing artificial hearing systems.

Tinnitus is an auditory sensation without external acoustic stimulation or significance, which may be lived as an unpleasant experience and impact the subject’s quality of life. Tinnitus loudness, which is generally low, bears no relation to distress. Factors other than psychoacoustic (such as psychological factors) are therefore implicated in the way tinnitus is experienced. The aim of this article is to attempt to understand how tinnitus can, like chronic pain, generate a ‘crisis’ in the process of existence, which may go as far as the collapse of the subject. The main idea put forward in the present article is that tinnitus may be compared to the phenomenon of pain from the point of view of the way it is experienced. Although the analogy between tinnitus and pain has often been made in the literature, it has been limited to a parallel concerning putative physiopathological mechanisms and has never really been explored in depth from the phenomenological point of view. Tinnitus is comparable to pain inasmuch as it is felt, not perceived: it springs up (without intention or exploration), abolishes the distance between the subject and the sensation (there is only a subject and no object), and has nothing to say about the world. Like pain, tinnitus is formless and abnormal and can alter the normal order of the world with maximum intensity. Finally, tinnitus and pain enclose the subject within the limits of the body, which then becomes in excess. Tinnitus may be a source of suffering, which affects not only the body but a person’s very existence and, in particular, its deployment in time. Plans are thus abolished, so time is no longer ‘secreted’, it is enclosed in an eternal present. If the crisis triggered by tinnitus is not resolved, the subject may buckle and collapse (depression) when their resources for resisting are depleted. The path may be long and winding from the moment when tinnitus emerges to when it assaults existence and its eventual integration into a new existential norm where tinnitus is no longer a source of disturbance.

Background: Current therapeutic interventions for dysesthesias caused by spinal cord dysfunctions are ineffective. We propose a novel intervention using transcutaneous electrical nerve stimulation (TENS) for dysesthesias, and we present an in-depth case series. Patients and methods: Conventional high-frequency TENS and the novel dysesthesia-matched TENS (DM-TENS) were applied to 16 hands of nine patients with spinal cord dysfunction. The dysesthesia-matched TENS' stimulus intensity and frequency matched the intensity and somatosensory profile of the patients' dysesthesias. The Short-Form McGill Pain Questionnaire version-2 (SF-MPQ2) and quantitative sensory testing (QST) were applied during electrical stimulation/no stimulation. We determined intraclass correlation coefficients (ICCs) to evaluate the reliability of the setting and the effects on the dysesthesias and the change in subjective dysesthesia between each patient's baseline without TENS and DM-TENS. Results: We were able to apply electrical stimulation matching the patients' subjective dysesthesia for 14 hands (eight patients). TENS could not be applied for the remaining patient due to severe sensory deficits. Compared to the patients' baseline and high-frequency TENS, the DM-TENS provided significant decreases in tingling/pinsand-needles and numbness on the SF-MPQ2, and it significantly improved the dynamic and static mechanical detection on QST. Regarding the reliability of the dysesthesia-matched TENS settings, the ICCs (1,5) were intensity, 0.95; frequency, 1.00; and effect on dysesthesia, 0.98. Conclusion: DM-TENS improved the dysesthesias and mechanical hypoesthesia caused by spinal cord dysfunction. The effectiveness of DM-TENS particularly for tingling and numbness was clearly higher and was reliable within the patients. Objectives: To summarize the current literature on lower urinary tract electrical sensory assessment (LUTESA), with regard to current perception thresholds (CPTs) and sensory evoked potentials (SEPs), and to discuss the applied methods in terms of technical aspects, confounding factors, and potential for lower urinary tract (LUT) diagnostics. Methods: The review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) statement. Medline (PubMed), Embase and Scopus were searched on 13 October 2020. Meta‐analyses were performed and methodological qualities of the included studies were defined by assessing risk of bias (RoB) as well as confounding. Results: After screening 9925 articles, 80 studies (five randomized controlled trials [RCTs] and 75 non‐RCTs) were included, comprising a total of 3732 patients and 692 healthy subjects (HS). Of these studies, 61 investigated CPTs exclusively and 19 reported on SEPs, with or without corresponding CPTs. The recording of LUTCPTs and SEPs was shown to represent a safe and reliable assessment of LUT afferent nerve function in HS and patients. LUTESA demonstrated significant differences in LUT sensitivity between HS and neurological patients, as well as after interventions such as pelvic surgery or drug treatments. Pooled analyses showed that several stimulation variables (e.g. stimulation frequency, location) as well as patient characteristics might affect the main outcome measures of LUTESA (CPTs, SEP latencies, peak‐to‐peak amplitudes, responder rate). RoB and confounding was high in most studies. Conclusions: Preliminary data show that CPT and SEP recordings are valuable tools to more objectively assess LUT afferent nerve function. LUTESA complements already established diagnostics such as urodynamics, allowing a more comprehensive patient evaluation. The high RoB and confounding rate was related to inconsistency and inaccuracy in reporting rather than the technique itself.
