Abstract

The management of unruptured intracranial aneurysms requires knowledge of the natural history of these lesions and the risks of repairing them.A total of 2621 patients at 53 participating centers in the United States, Canada, and Europe were enrolled in the study, which had retrospective and prospective components. In the retrospective component, we assessed the natural history of unruptured intracranial aneurysms in 1449 patients with 1937 unruptured intracranial aneurysms; 727 of the patients had no history of subarachnoid hemorrhage from a different aneurysm (group 1), and 722 had a history of subarachnoid hemorrhage from a different aneurysm that had been repaired successfully (group 2). In the prospective component, we assessed treatment-related morbidity and mortality in 1172 patients with newly diagnosed unruptured intracranial aneurysms.In group 1, the cumulative rate of rupture of aneurysms that were less than 10 mm in diameter at diagnosis was less than 0.05 percent per year, and in group 2, the rate was approximately 11 times as high (0.5 percent per year). The rupture rate of aneurysms that were 10 mm or more in diameter was less than 1 percent per year in both groups, but in group 1, the rate was 6 percent the first year for giant aneurysms (or =25 mm in diameter). The size and location of the aneurysm were independent predictors of rupture. The overall rate of surgery-related morbidity and mortality was 17.5 percent in group 1 and 13.6 percent in group 2 at 30 days and was 15.7 percent and 13.1 percent, respectively, at 1 year. Age independently predicted surgical outcome.The likelihood of rupture of unruptured intracranial aneurysms that were less than 10 mm in diameter was exceedingly low among patients in group 1 and was substantially higher among those in group 2. The risk of morbidity and mortality related to surgery greatly exceeded the 7.5-year risk of rupture among patients in group 1 with unruptured intracranial aneurysms smaller than 10 mm in diameter.

235. Multimodales zerebrales Monitoring bei schweren Schädel-Hirn-Trauma

Abstract

Die vorliegende Arbeit setzt sich mit der klinischen Anwendung von zwei neuen Monitoringparametern - Hirngewebe-PO2 in der weißen Substanz (PtiO2), und online intrakranielle Compliance (cICC) - im Rahmen des multimodalen zerebralen Monitorings bei Patienten mit schwerem Schädel-Hirn-Trauma auseinander. Bezüglich des PtiO2 konnte erstmalig eine Hypoxiegrenze von 8,5 mmHg durch vergleichende Messungen mit der jugular-venösen Oxymetrie ermittelt werden. Ferner konnte gezeigt werden, dass, bei intakter zerebraler Autoregulation, der PtiO2 bei einem zerebralen Perfusionsdruck (CPP) >60 mmHg über dem pathologischen Grenzwert liegt. Eine forcierte bzw. moderate Hyperventilation hingegen, induziert, trotz adäquatem CPP, eine Reduktion des PtiO2, die im individuellen Fall zur Unterschreitung des hypoxischen Grenzwerts führt. Das PtiO2-Verfahren ist somit v.a. dann indiziert, wenn eine Hyperventilationstherapie zur Kontrolle eines pathologisch erhöhten intrakraniellen Drucks (ICP) eingesetzt werden muss. PtiO2-Messwerte bedürfen aber einer kritischen Interpretation, sofern der PtiO2-Katheter in der Nähe einer Kontusion lokalisiert ist. Hier ist der PtiO2, als Ausdruck des perikontusionell reduzierten zerebralen Blutflusses, signifikant erniedrigt und somit nicht repräsentativ für die globale zerebrale Oxygenierung. Für die cICC konnte ebenfalls ein pathologischer Grenzwert angegeben werden (0,5 ml/mmHg). Die Dateninterpretation ist aber, durch die offensichtliche Abnahme der intrakraniellen Compliance mit zunehmendem Lebensalter, erschwert. Ferner bleibt die cICC bzgl. ihrer Datenqualität weit hinter etablierten Parametern zurück, so dass ihre routinemäßige Anwendung zum jetzigen Zeitpunkt nicht zu empfehlen ist. Basierend auf unseren Untersuchungen hat sich das PtiO2-Verfahren international als Langzeitmonitoring der zerebralen Oxygenierung etablieren können. Die cICC hingegen bedarf umfangreicher Systemänderungen, um eine frühe Risikoabschätzung bezüglich eines drohenden IC, The aim of our clinical and experimental studies was to evaluate two new monitoring parameters -brain tissue PO2 (PtiO2) of cerebral white matter, and online intracranial compliance (cICC) - in patients with severe traumatic brain injury by using a computerized multimodal cerebral monitoring system. By comparing PtiO2 with jugular vein oxygen saturation, we were able to establish the hypoxic PtiO2-threshold of 8.5 mmHg. Moreover, we demonstrated that in case of an intact cerebral autoregulation, PtiO2 was well above the hypoxic threshold as long as cerebral perfusion pressure (CPP) stayed above 60 mmHg. However, forced or moderate hyperventilation carried an individual risk of a PtiO2 reduction below the hypoxic threshold despite an adequate CPP. PtiO2 monitoring is therefore particularly indicated, if hyperventilation therapy is necessary for control of pathologically increased intracranial pressure (ICP). However, PtiO2-values needed critical interpretation, if catheters were placed close to contusions. In these situations, PtiO2 has been shown to be significantly reduced, presumably due to low peri-contusional blood flow. Thus, such PtiO2 measurements cannot be taken as representatives of global cerebral oxygenation. In cICC monitoring, a pathological threshold was accomplished (0.5 ml/mmHg). Due to a stepwise cICC reduction with increasing age, cICC data interpretation was aggravated, and cICC data quality was significantly reduced in comparison to other established monitoring parameters. Hence, a routine use of this device is currently not advisable. Based on ours results, the PtiO2-methode has been established internationally as an ideal tool for long-term monitoring of cerebral oxygenation. On the contrary, the cICC system needs extensive alterations in order to anticipate sufficiently pathological ICP rises.

Abstract

Background and purpose Occlusive cerebrovascular moyamoya disease ( MMD) is rare and has been characterized mainly in Asian countries, so far. In recent years, MMD has been increasingly reported worldwide, raising the question whether its clinical presentation would vary amongst different ethnic backgrounds. Here, a homogeneous series of 153 patients with MMD are reported and the specific clinical features of this rare disease amongst European Caucasians are highlighted. Methods A total of 153 European Caucasians with MMD who were treated in our institution between 1997 and 2014 were retrospectively identified. Demographic data, clinical symptoms, angiographic characteristics and functional hemodynamic studies were analyzed. Results Moyamoya disease presented with a female predominance of 2,9:1.,78% presented with a typical MMD, 17% with a unilateral MMD and 5% with an atypical MMD. 16% of our patients belonged to the pediatric population. Overall, 81% and 8.5% of our cohort presented initially with ischaemic and hemorrhagic manifestation, respectively. The rate of hemorrhagic manifestation of MMD amongst the pediatric group was slightly higher (12%). Angiographic analysis revealed steno-occlusive involvement of the posterior circulation in 34% with a higher involvement in pediatric patients (64%) compared to adults (28%). Conclusions The characterization of our homogeneous European Caucasian cohort reveals several significant differences compared to Asian cohorts. In contrast, MMD presents similarly amongst European and North American cohorts, suggesting that non-Asian MMD is characterized by distinct clinical features. [ABSTRACT FROM AUTHOR]

Abstract

Spontaneous subarachnoid hemorrhage (SAH) is usually caused by a ruptured cerebral aneurysm. Despite the use of initial four-vessel cerebral digital subtraction angiography (DSA), 15 % of all cases remain idiopathic. According to the initial computed tomographic scan, the spontaneous SAH can be divided into a perimesencephalic group associated with a benign nature and a nonperimesencephalic group with a similar clinical course as aneurysmal SAH. We present a case of a 49-year-old man with a de novo aneurysm formation of the anterior communicating artery with SAH 7 years after initial cryptogenic nonperimesencephalic SAH.
