Abstract

Tricyclic antidepressant overdose is a frequent diagnosis in the emergency room. It is responsible for a significant percentage of hospital admissions for observation and treatment. This is due to its cardiac (as well as neurologic) toxicity, and the difficulty in predicting its clinical gravity. The authors present this paper with two objectives in mind: 1) information about a common and significant cardiological emergency; 2) presentation of the methodology concerning systematic reviews of the literature. In this second (of four) articles, the clinical presentation and diagnosis of tricyclic antidepressant overdose are presented (in the first article--published in the previous issue of the Journal--we discussed the epidemiological aspects). In the remaining two papers we will present its management, prognosis and prevention. NOTE: This is the second of a series of four articles on the cardiac toxicity of tricyclic antidepressant overdose; the next two articles will be presented in the September and October issues of the Revista.

Abstract

Fine coal from Copelmi (Brazil) and Cerrejon (Colombia) has been used to study flotation for the recovery of carbonaceous matter and reduction of ash and contaminants. The effect of air velocity on yield, organic recovery, ash recovery and water recovery was evaluated. The two coals were floated using a negative bias in a 5 cm diameter conventional flotation column 5 m high using air velocities ranging from 0.6 to 1.5 cm/s. For all runs, a constant negative bias of -0.25 cm/s and a pulp concentration of 8% w/w were used. For Cerrejon coal, 300 g/ton of Aerofroth 65 frother and 907 g/ton of kerosene collector was used, whereas for Copelmi coal, 600 g/ton of frother and 1 815 g/ton of collector was used. Results showed that for the Colombian coal, yield, organic recovery, ash recovery and water recovery decreased with the increasing air rate. For Brazilian coal, however, the air rate had no significant effect. Contains fulltext : 97896.pdf (publisher's version ) (Closed access), BACKGROUND: Sleep disorders are common in multiple system atrophy (MSA), but the prevalence of excessive daytime sleepiness (EDS) is not well known. OBJECTIVE: To assess the frequency and associations of EDS in MSA. DESIGN: Survey of EDS in consecutive patients with MSA and comparison with patients with Parkinson disease (PD) and individuals without known neurologic disease. SETTING: Twelve tertiary referral centers. PARTICIPANTS: Eighty-six consecutive patients with MSA; 86 patients with PD matched for age, sex, and Hoehn and Yahr stage; and 86 healthy subject individuals matched for age and sex. MAIN OUTCOME MEASURES: Epworth Sleepiness Scale (ESS), modified ESS, Sudden Onset of Sleep Scale, Tandberg Sleepiness Scale, Pittsburgh Sleep Quality Index, disease severity, dopaminergic treatment amount, and presence of restless legs syndrome. RESULTS: Mean (SD) ESS scores were comparable in MSA (7.72 [5.05]) and PD (8.23 [4.62]) but were higher than in healthy subjects (4.52 [2.98]) (P < .001). Excessive daytime sleepiness (ESS score >10) was present in 28% of patients with MSA, 29% of patients with PD, and 2% of healthy subjects (P < .001). In MSA, in contrast to PD, the amount of dopaminergic treatment was not correlated with EDS. Disease severity was weakly correlated with EDS in MSA and PD. Restless legs syndrome occurred in 28% of patients with MSA, 14% of patients with PD, and 7% of healthy subjects (P < .001). Multiple regression analysis (with 95% confidence intervals obtained using nonparametric bootstrapping) showed that sleep-disordered breathing and sleep efficiency predicted EDS in MSA and amount of dopaminergic treatment and presence of restless legs syndrome in PD. CONCLUSIONS: More than one-quarter of patients with MSA experience EDS, a frequency similar to that encountered in PD. In these 2 conditions, EDS seems to be associated with different causes.

Abstract

The present study evaluated the tooth/noncarious cervical lesion restoration interface when using different adhesive systems and resin composites, submitted to thermal cycling (TC), using optical coherence tomography (OCT). Noncarious cervical lesion (NCCL) preparations (0.7 mm depth × 2 mm diameter) were performed on 60 human third molars and randomly divided into six groups, according to the adhesive system and resin composite used: group 1 = Adper Single Bond 2 (SB2) + Aelite LS Posterior (AP); group 2 = SB2 + Venus Diamond (VD); group = SB2 + Filtek Z250XT (Z250); group 4 = Clearfil SE Bond (CSE) + AP; group 5 = CSE + VD; group 6 = CSE + Z250. Selective enamel etching was performed for 30 seconds on groups 4, 5, and 6, while groups 1, 2, and 3 were etched for 30 seconds in enamel and 15 seconds in dentin. All groups were evaluated using OCT before and after TC (n=10). Images were analyzed using Image J software; enamel and dentin margins were separately evaluated. Data from OCT were submitted to PROC MIXED for repeated measurements and Tukey Kramer test (a = 0.05). No marginal gaps were observed in etched enamel, either before or after TC, for all adhesive and resin composite systems. A significant interaction was found between adhesive system and TC for the dentin groups; after TC, restorations with CSE showed smaller gaps at the dentin/restoration interface compared with SB2 for all resin composites. Increased gap percentages were noticed after TC compared with the gaps before TC for all groups. In conclusion, TC affected marginal integrity only in dentin margins, whereas etched enamel margins remained stable even after TC. Dentin margins restored with CSE adhesive system showed better marginal adaptation than those restored with SB2. Resin composites did not influence marginal integrity of NCCL restorations. Item does not contain fulltext, OBJECTIVE: To assess the prevalence, nature, and associated phenotypes of PINK1 gene mutations in a large series of patients with early-onset (<50 years) parkinsonism. METHODS: The authors studied 134 patients (116 sporadic and 18 familial; 77% Italian) and 90 Italian controls. The whole PINK1 coding region was sequenced from genomic DNA; cDNA was analyzed in selected cases. RESULTS: Homozygous pathogenic mutations were identified in 4 of 90 Italian sporadic cases, including the novel Gln456Stop mutation; single heterozygous truncating or missense mutations were found in another 4 Italian sporadic cases, including two novel mutations, Pro196Leu and Gln456Stop. Pathogenic mutations were not identified in the familial cases. Novel (Gln115Leu) and known polymorphisms were identified with similar frequency in cases and controls. In cases carrying single heterozygous mutation, cDNA analysis detected no additional mutations, and revealed a major pathogenic effect at mRNA level for the mutant C1366T/Gln456Stop allele. All patients with homozygous mutations had very early disease onset, slow progression, and excellent response to l-dopa, including, in some, symmetric onset, dystonia at onset, and sleep benefit, resembling parkin-related disease. Phenotype in patients with single heterozygous mutation was similar, but onset was later. CONCLUSIONS: PINK1 homozygous mutations are a relevant cause of disease among Italian sporadic patients with early-onset parkinsonism. The role of mutations found in single heterozygous state is difficult to interpret. Our study suggests that, at least in some patients, these mutations are disease causing, in combination with additional, still unknown factors.

Abstract

Cette étude porte sur l'évolution des stocks des éléments biogéniques C, N et P (organiques et minéraux, particulaires et dissous) de la lagune de la Barra (état de Rio de Janeiro, Brésil). Elle a été réalisée de la mi-printemps 1990 à la mi-automne 1991, conjointement avec des études sur le métabolisme du carbone et sur les communautés planctoniques. La première phase de la période étudiée, qui correspond au début de la saison chaude, a été marquée par un appauvrissement du milieu en azote par rapport au phosphore (rapport molaire NOP/POP du seston = 5,7). La perte de N, probablement due au processus de dénitrification, a conduit à une floraison de #Synechocystis aqualitis f. salina$. Ces organismes, mieux adaptés à la pénurie de N, auraient réduit leur sécrétion en azote organique dissous et aussi favorisé le recyclage de l'azote dans le milieu pélagique au détriment du milieu benthique. Selon certains indices, ils auraient fixé de l'azote moléculaire dissous; mais ce processus, s'il a effectivement eu lieu, est resté limité. La pénurie en N s'est aggravée au cours de leur développement. Ils ont été conduits à produire des carbohydrates en excès et à les excréter (les teneurs des eaux en COD ont atteint 4000 micromoles/l) puis, lors de leur phase finale de déclin, en prenant une couleur rouge-marron, ils auraient détruit leurs phycobilipigments pour en puiser leurs ultimes réserves d'azote intracellulaire. L'emprunt de cette voie secondaire aurait conduit à la production de toxines qui serait à l'origine d'une mortalité massive d'un poisson herbivore, #Brevoortia tyrannus$, en février. A l'exception d'une brève période de floraison de #Chlorophyceae$ quelques jours après la mortalité, la grande quantité de NH4+ libérée par la décomposition des poissons a été mobilisée, dans un premier temps, par le phytoplancton, puis transférée trois ou quatre semaines plus tard vers les niveaux trophiques supérieurs. Abstract

Background and Purpose The function of the endocannabinoid system ( ECS) in renal tissue is not completely understood. Kidney function is closely related to ion reabsorption in the proximal tubule, the nephron segment responsible for the re-absorption of 70-80% of the filtrate. We studied the effect of compounds modulating the activity of cannabinoid ( CB) receptors on the active re-absorption of Na+ in LLC- PK1 cells. Experimental Approach Changes in Na+/ K+-ATPase activity were assessed after treatment with WIN55,212-2 ( WIN), a non-selective lipid agonist, and haemopressin ( HP), an inverse peptide agonist at CB1 receptors. Pharmacological tools were used to investigate the signalling pathways involved in the modulation of Na+ transport. Key Results In addition to CB1 and CB2 receptors and TRPV1 channels, the mRNAs encoding for enzymes of the ECS were also expressed in LLC- PK1. WIN (10−7 M) and HP (10−6 M) altered Na+ re-absorption in LLC- PK1 in a dual manner. They both acutely (after 1 min) increased Na+/ K+- ATPase activity in a TRPV1 antagonist-sensitive way. WIN's stimulating effect persisted for 30 min, and this effect was partially blocked by a CB1 antagonist or a PKC inhibitor. In contrast, HP inhibited Na+/ K+- ATPase after 30 min incubation, and this effect was attenuated by a CB1 antagonist or a PKA inhibitor. Conclusion and Implications The ECS is expressed in LLC- PK1 cells. Both CB1 receptors and TRPV1 channels regulate Na+/ K+- ATPase activity in these cells, and are modulated by lipid and peptide CB1 receptor ligands, which act via different signalling pathways.
