Fibrobronchoscopy, first used in pneumology with a diagnostic and therapeutic purpose, has been recently used in the therapy of "difficult intubations". After trismus, complex fracture, serious maxillofacial malformations and wide neoplasia, classic nose--orotracheal intubation through direct laryngoscopy may turn out difficulty. Some years ago blind intubation was used, which was stressing for the operator and very dangerous for the patient. In the most serious cases, tracheotomy was used. Thanks to flexible fibroscopes of suitable lenght and reduced diameter, nosetracheal intubations guided by fibroscope can be carried out today. Remembering the indications about fibrobronchoscope use in oral surgery, the Authors describe tre method they have used. They also underline the facility of this technique which allows to carry out a sure and not traumatic intubation without resorting to lisive medicines without producing lacerations or tumefactions in laryngotracheal mucosa.

The authors study 84 patients that had damages of the duodenum and or pancreas and underwent different types of surgery. In 39 patients who had damage only in the duodenum; there were 2 (4.0%) duodenal fistulas, and 3 (6.0%) intraperitoneal abscess. In 38 patients who had injuries only in the pancreas; there were 6 (13.1%) acute pancreatities, pancreatic fistulas and 1 (2.2%) pancreatic pseudocyst. In 7 cases, injuries were found to both pancreas and duodenum. The author reports 1 case of duodenal fistula (14.3%) 1 case of acute pancreatitis (14.3%) and 3 cases of pancreatic fistula (43.0%). Only 7 patients died (8.3%), and of these 2 died for reasons not directly related to the operatory technic used. A simple suture can be performed in those cases where a complete section of the duodenum is unnecessary and there is no injury to the duodenal papilla. A burying suture of the stomas associated with a side-to-side duodenojejunal anastomosis should be preferred in the complete section of the duodenum localized beyond the ampulla of Vater. In all superficial wounds of small extension a suture of the pancreas should be performed. Distal pancreatectomy of body or tail should be made in the wounds with a possible lesion to the Wirsung duct and when there is an extensive glandular lesion. The associate wounds of duodenum and pancreas should be treated as if they were isolated lesions.

- Abstract
OBJECTIVE: of this paper is to compare observed values of immune parameters obtained in the CESAR study (The Central Europe Study of Air Pollution and Respiratory Health, funded by EC PHARE program) with ranges derived from other large population-based studies. STUDY DESIGN: Data were collected in healthy school children aged 9-11 years, in 6 countries: Bulgaria, the Czech Republic, Hungary, Poland, Romania and the Slovak Republic with the same standard approach in 1996. Random samples of 85 children per country, from 19 communities were selected from children having completed the health questionnaire, in total 495 children were analyzed. Lymphocyte subsets were determined by two-colour flow cytometric immunophenotyping using the lysed whole blood method (Becton-Dickinson). For determination of immunoglobulin concentration in sera nephelometric method (Behring Nephelometer system) was used. RESULTS: Medians, (5th-95th percentiles) of the lymphocyte subsets absolute count (x 10(9)/l) were as follows: CD19+ B cells 0.36 (0.13-0.66), total CD3+ T cells 1.74 (0.98-2.90), CD3+CD4+ helper-inducer T cells 0.95 (0.47-1.78), CD3+CD8+ suppressor/cytotoxic T cells 0.71 (0.38-1.22), CD3-CD16+56+ NK cells 0.36 (0.14-0.78), and for CD3+CD4+/CD3+CD8+ ratio 1.4 (0.8-2.4). Medians, (5th-95th percentiles) of percentages of lymphocyte subpopulations (%) were as follows: CD19+ B 13 (7-22), CD3+ T 70 (59-80), CD3+CD4+ T 38 (27-48), CD3+CD8+ T 28 (20-39), CD3-CD16+56+ NK cells 14 (6-27). Medians, (2.5th-97.5th percentiles) of the total immunoglobulin [g/l] were 11.7 (7.4-18.2) for IgG, 1.2 (0.5-2.5) for IgM, and 1.5 (0.5-3.4) for IgA. Based on the aspects of the size of the CESAR immune biomarker study and on the use of the standardized protocols we recommend to use the reference ranges on lymphocyte subsets and immunoglobulin in Europe as provided by this study.

230. Perfluoroalkyl substances and immune cell counts in adults from the Mid-Ohio Valley (USA)

Background: Although perfluoroalkyl substances (PFASs) may be immunotoxic, evidence for this in humans is scarce. We studied the association between 4 PFASs (perfluorohexane sulfonate [PFHxS], perfluorooctanoic acid [PFOA], perfluorooctane sulfonate [PFOS] and perfluorononanoic acid [PFNA]) and circulating levels of several types of immune cells. Methods: Serum PFASs and white blood cell types were measured in 42,782 (2005–2006) and 526 (2010) adults from an area with PFOA drinking water contamination in the Mid-Ohio Valley (USA). Additionally, the major lymphocyte subsets were measured in 2010. Ln(cell counts) and percentages of cell counts were regressed on serum PFAS concentrations (ln or percentiles). Adjusted results were expressed as the percentage difference (95% CI) per interquartile range (IQR) increment of each PFAS concentration. Results: Generally positive monotonic associations between total lymphocytes and PFHxS, PFOA, and PFOS were found in both surveys (difference range: 1.12–7.33% for count and 0.36–1.77 for percentage, per PFAS IQR increment), and were stronger for PFHxS. These associations were reflected in lymphocyte subset counts but not percentages, with PFHxS positively and monotonically associated with T, B, and natural killer (NK) cell counts (range: 5.51–8.62%), PFOA and PFOS with some T-cell phenotypes, and PFOS with NK cells (range: 3.12–12.21%), the associations being monotonic in some cases. Neutrophils, particularly percentage (range: −1.74 to −0.36), showed decreasing trends associated with PFASs. Findings were less consistent for monocytes and eosinophils. Conclusion: These results suggest an association between PFHxS and, less consistently, for PFOA and PFOS, and total lymphocytes (although the magnitudes of the differences were small). The increase in absolute lymphocyte count appeared to be evenly distributed across lymphocyte subsets since associations with their percentages were not significant.

Increasingly, the potential short and long-term impacts of climate change on human health and wellbeing are being demonstrated. However, other environmental change factors, particularly relating to the natural environment, need to be taken into account to understand the totality of these interactions and impacts. This paper provides an overview of ongoing research in the Health Protection Research Unit (HPRU) on Environmental Change and Health, particularly around the positive and negative effects of the natural environment on human health and well-being and primarily within a UK context. In addition to exploring the potential increasing risks to human health from water-borne and vector-borne diseases and from exposure to aeroallergens such as pollen, this paper also demonstrates the potential opportunities and co-benefits to human physical and mental health from interacting with the natural environment. The involvement of a Health and Environment Public Engagement (HEPE) group as a public forum of “critical friends” has proven useful for prioritising and exploring some of this research; such public involvement is essential to minimise public health risks and maximise the benefits which are identified from this research into environmental change and human health. Research gaps are identified and recommendations made for future research into the risks, benefits and potential opportunities of climate and other environmental change on human and planetary health.

We present and discuss a late-nineteenth century clinical case described by Professor Taruffi in a scientific paper titled “Scheletro con prosopoectasia e tredici vertebre dorsali” (Skeleton with prosopoectasia and thirteen thoracic vertebrae). Taruffi could not explain the disproportionate skeletal and visceral growth, and the case could therefore be considered an unrecognized case of acromegaly. The anatomopathological specimens and the wax model cited in the paper are currently hosted at the “Luigi Cattaneo” Anatomical Wax Collection of Bologna University; however, some inaccuracies and uncertainties as to their attribution to the same case have remained to this day. The skeletal remains were examined macroscopically to investigate any structural abnormalities and pathological changes. In addition, thanks to archival, museum inventory and literature research, we documented the systematic relationship between the paper and the samples and were able to ascribe the abnormally dilated dried stomach, currently displayed in a different showcase, to the same case. This is, to our knowledge, the first case of acromegaly in the history of medical literature which also includes a visceral specimen. As far as we know, there are no reports of the occurrence of severe gastromegaly in patients with acromegaly. In view of this rare association and, to date, endocrinological research, we hypothesize a further pathogenic mechanism by which acromegaly could have induced this massive dilatation. Taruffi’s work represents an immensely valuable scientific/artistic heritage and is still cited in contemporary endocrinological literature, demonstrating its relevant contribution to the historical evolution of the disease through the nineteenth and twentieth centuries.

Die stete Zunahme von Texten mehrsprachiger Autorinnen und Autoren gehört zu den innovativen Tendenzen der deutschen Gegenwartsliteratur. Hierfür hat sich der Begriff Migrationsliteratur eingebürgert. Zur Problematik des Begriffs gehört, dass er sich vor allem auf die erste Generation von Migranten bezieht und ein Ankommen von außen meint. Das trifft aber für spätere Generationen nicht mehr zu. Autorinnen und Autoren verschiedener Generationen machen sprachlich-kulturelle Divergenzen und Kontraste produktiv, wobei das Dynamische und Prozesshafte von Sprachgebrauch und -reflexion in verschiedenen Verfahren gezeigt wird. Thematisch geht es u. a. um translokale Biographien, in denen der Sprachwechsel als Verlusterfahrung erlebt werden kann, neben solchen, in denen Mehrsprachigkeit und Interkulturalität eine Ressource sind und zu neuen Perspektivierungen führen. Durch den Sprachwechsel und/oder das Schreiben mit einer anderen Sprache ‚im Ohr‘ unterlaufen die Repertoires die Dichotomie von sprachlich richtig und falsch, zudem entziehen sich die Texte vereinfachenden kulturellen Zuschreibungen bzw. sie spielen damit. Implizit klingt immer etwas Abwesendes mit: andere Räume oder Zeiten, Sprachen, Varietäten

Der verbale Ausdruck von Emotionen sowie die verschiedenen Strategien, mit denen Erzähler/innen ihre mündlichen Geschichten strukturieren und dramatische Höhepunkte markieren, stehen im Mittelpunkt der zwölf Beiträge dieses Bandes. Allen Analysen zugrunde liegt ein umfangreiches Oral History Korpus narrativer autobiographischer Interviews mit deutschsprachigen jüdischen Emigrant/inn/en, die in den 1930er Jahren von Mitteleuropa nach Palästina/Israel geflohen sind (das sog. Israelkorpus). Die individuellen Lebensberichte, Formulierungsverfahren und die damit verbundenen Identitätskonstruktionen werden aus den Forschungsperspektiven von (Dialog-)Linguistik, Kultur- und Geschichtswissenschaften untersucht. Besonders die große Variationsbreite an sprachlichen Darstellungen des Erlebten belegt eindrucksvoll, wie stark und unterschiedlich die durch den Erinnerungsprozess wachgerufenen Emotionen die Gestaltung des Erzählten beeinflussen.

n dem Beitrag „Abschied von den Eltern. Auseinandersetzungen mit dem Tod der Eltern im Israelkorpus“ geht Eva-Maria Thüne auf der Grundlage von sechs ausgewählten Beispielen der Frage nach, wie Interviewte den Abschied von ihren Eltern schildern bzw. was sie über den Tod ihrer Eltern sagen. Beim Erzählen einzelner sehr berührender Szenen wird der Erinnerungsprozess sowohl durch Emotionsbeschreibung als auch durch Emotionsausdruck gestaltet. Dies zeigt sich auf den Ebenen der Lexik, der Prosodie und des Paraverbalen. Durch die Analysen der Textstellen soll der mündliche Ausdruck von Trauer genauer beschrieben werden.

Il contributo analizza il linguaggio usato nel romanzo “Wie der Soldat das Grammofon repariert” (“La storia del soldato che riparò il grammofono”) di Sasa Stanisic, scrittore tedesco di origine bosniaca e uno dei rappresentanti della “letteratura migrante” (Migrationsliteratur) in lingua tedesca. Vengono esaminati, in primo piano, i diversi piani di “ibridazione” linguistica presenti nel romanzo, che trovano corrispondenza nella forma stessa, ibrida, del romanzo, caratterizzata da molteplici allusioni a diverse tradizioni letterarie e testuali, come il romanzo picaresco ed il romanzo epistolare, ma anche alla scritura e le tematiche al Nobel jugoslavo Ivo Andrić e al cinema di Emir Kusturica. La lingua tedesca utilizzata nel romanzo è consequenziale alla complessità della realtà descritta per varietà tanto di stili quanto di registri, testimoniando le molteplici culture dell’ex Jugoslavia ed i rapporti con il mondo tedescofono, che fanno da sfondo alle vicende del protagonista, emigrato in Germania a causa della guerra.

Il segno della poesia transculturale contemporanea in tedesco è stato visto da Andreas Blödorn (2006: 144) nell’Unterwegs-Sein (nel movimento, nell’essere in cammino), un movimento che nella prima generazione si è espresso spesso nella descrizione del cammino dal luogo d’origine verso la Germania, legato al processo di continua ricerca della lingua d’espressione. Nelle poesie qui prese in esame, infatti, vediamo questa scrittura iniziale di poesie come di biglietti di viaggio, andata e ritorno, sia per la strada reale, sia per il viaggio tra le lingue, che diventa movente creativo continuo dei poeti anche nella seconda generazione. In questo senso sono da interpretare le metafore del camaleonte (SAID), che beve e nuota in due fiumi. Tutte queste sono immagini della complessità e dell’ambiguità. In più di un caso le metafore usate oscillano tra l’aspetto dell’apertura e quello della divisione. Nella seconda generazione questo movimento si trasforma in un processo interiore, in un cambio di prospettiva, nello sguardo dell’altro (Çırak), si tratta sempre di più di un cammino immaginario che porta a un interrogarsi, in cui il bi- o plurilinguismo diventano la scia di collegamento tra aspetti esistenziali differenti che non si vogliono necessariamente conciliare. Questo vale anche per la tarda produzione di alcuni poeti della prima generazione, che fanno confluire tradizioni della lingua d’origine e del tedesco (p.es. Pazarkaya). Se nella prima generazione era in primo piano il tema della terra straniera sperimentata come Fremde, ‘paese straniero, estraneo’ e a tratti ostile, la seconda generazione sposta lo sguardo sul momento successivo all’arrivo, alla ricerca di una Heimat (‘casa’, ma anche ‘patria’), come nelle ultime poesie di Özdemir sul Palatinato. Non si tratta però di una Heimat identitaria, ma di un luogo in cui confluiscono lingue, provenienze e esperienze eterogenee. Per l’esperienza di scrittori e scrittrici nel territorio d’arrivo si potrebbe parlare di un concetto di “lingua casa”. La difficoltà di quest’ultimo punto ci ricorda un importante filone, e cioè quella della poesia civile, della denuncia della conflittualità sociale ancora latente che in SAID, Pazarkaya e Özdemir si fa esplicita, in Çırak affiora in immagini di diversità, come nell’essere mancina, in Bodrožić nello sguardo dall’alto sugli esseri umani che non capiscono. In tutti gli autori troviamo la riflessione sulla lingua, come ricerca della lingua materna o quella seconda, come posizione plurilingue ibrida o consolidata, mettendo in primo piano le costrizioni, i limiti, o ancora le possibilità di sperimentazione sintattica, o di introdurre un gioco, p.es. di tipo lessicale, come abbiamo visto in più casi. Sono processi di scrittura transculturale in cui l’immersione nella lingua e cultura tedesca apre una corrente d’ispirazione di sicuro impatto e novità.

- Abstract
Medulloblastoma (MB) is a common primary brain cancer in children. The sonic hedgehog (SHH) pathway is indispensable for the normal development of the cerebellum, and MB is often caused by persistent SHH activation owing to mutations in pathway components. Patched1 ( PTCH1 ) is the primary receptor for the SHH ligand and a negative regulator of the SHH signal transduction pathway. Mice heterozygous for the Ptch1 gene ( Ptch1
+/- ) are predisposed to MB development. Irradiation of newborn Ptch1+/- mice dramatically increases MB occurrence. A genetic background carrying the Ptch1 mutation significantly influences the risk of developing MB. This study aims to investigate the genetic background-related mechanisms that regulate radiation-induced cellular response and oncogenesis in the cerebellum. We employed multiple approaches, including: (a) analysis of cellular radiosensitivity in granule cell precursors (GCPs), the MB cells of origin, derived from Ptch1 mice with a genetic background that is sensitive (CD1) or resistant (C57Bl/6) to the induction of radiogenic MB; (b) identification of genes differentially expressed in spontaneous and radiation-induced MBs from these two mouse strains; (c) bioinformatic analysis to correlate the expression of radiation-induced genes with survival in MB patients; and (d) examining the expression of these genes in ex vivo MBs induced by single or repeated radiation doses. We have identified a potential gene expression signature- Trp53bp1 , Bax , Cyclin D1 , p21 , and Nanog -that influences tumor response. In ex vivo cultured spontaneous MBs, the expression levels of these genes increase after irradiation in CD1 mice, but not in mice with a C57Bl/6 genetic background, suggesting that this signature could predict tumor response to radiation therapy and help develop strategies for targeting DNA damage repair in tumors. A detailed understanding of the mechanisms behind genetic background-related susceptibility to radiation-induced oncogenic responses is crucial for translational research.- Published

246. Long Term Ventilation in Pediatric Central Apnea: Etiologies and Therapeutic Approach over a Decade.

- Abstract
Objective: This retrospective study aimed to analyze the clinical characteristics, ventilatory strategies, and effectiveness of ventilation in pediatric patients with central apneas treated at the Sleep Medicine and Long-Term Ventilation Unit of the Bambino Gesù Children's Hospital in Rome from 2012 to 2022., Methods: Among all ventilated patients at our Center from January 2012 to December 2022, we retrospectively included children with a cAHI ≥ 1 events/h on baseline poly(somno)graphic study. Additional parameters assessed included the underlying disease, type of ventilation (non-invasive vs. invasive), age at ventilation onset, ventilation mode, and transcutaneous capnometry parameters. To assess the effectiveness of ventilation on central apneas, we compared the cAHI at baseline and on ventilation., Results: Sixty-seven patients met the inclusion criteria for central apnea (cAHI > 1 events/h). Diagnoses included hypoxic-ischemic encephalopathy, 15 (22.4%); Ondine syndrome, 14 (20.9%); polymalformative syndrome, 10 (14.9%); Prader-Willi syndrome, 8 (11.9%); brain tumor, 6 (9.0%); Down syndrome, 4 (6.0%); ROHHAD syndrome, 2 (3.0%); other infrequent pathologies were, Arnold-Chiari II, primary central apnea, epilepsy, lisosomal diseases, hydrocephalus, myopathy, obesity, Rett Syndrome. Pressure-supported ventilation (PSV) was the most common mode used (45 out 67 patients, 67.2%), followed by pressure-controlled ventilation (PCV) (15 out 67 patients, 22.4%) and continuous positive airway pressure (CPAP) (7 out 67 patients, 10.4%). Statistically significant improvement (p < 0.05) in cAHI was observed in patients with polymalformative syndrome (3.5 vs. 0.3, p = 0.01), hypoxic-ischemic encephalopathy (3.1 vs. 0.1, p = < 0.01), and Prader-Willi syndrome (3.5 vs. 0.1, p = 0.03), while there was no significant improvementn in children with brain tumor (6.2 vs. 1.5, p = 0.21)., Conclusion: Central apneas are present in children with various underlying pathologies. Ventilatory strategies tailored to the specific diagnosis and severity of central apneas yield significant improvements in cAHI. PSV was the preferred ventilation mode in this study and there was notable effectiveness across different diagnostic categories. PCV was employed in most severe cases. 247. Coagulative Biomarkers Differently Predict Clinical Outcomes in Invasive Infections Caused by Neisseria Meningitidis and Streptococcus Pneumoniae.

- Abstract
Background: Coagulation plays a crucial role in innate immune response to invasive infections. Coagulative biomarkers might predict clinical outcomes differently, depending on etiology., Methods: A retrospective study was conducted during a 79-month period, recruiting 90 patients with meningitis or bloodstream infection caused by Neisseria meningitidis (n = 47) or Streptococcus pneumoniae (n = 43), median age 19 and 58 years, respectively. Biomarkers were assessed within 24 hours., Results: For N. meningitidis: in univariate analysis, increasing D-dimer was associated with in-hospital mortality (odds ratio [OR] 1.360; 95% confidence interval [CI], 1.063-1.889); in multivariate regression, increasing D-dimer was predictive (OR 1.037; 95% CI, 1.001-1.074) of the composite outcome (in-hospital mortality or amputations or hearing loss or neurological sequelae); protein C showed a clear trend toward lower levels in nonsurvivors (26% vs 48%) and in patients with the composite outcome (32% vs 51%); activated partial thromboplastin time (aPTT) was significantly prolonged in nonsurvivors (51.3 vs 35.3 seconds, P = .003), confirmed in univariate analysis (OR 1.122; 95% CI, 1.031-1.253). For S. pneumoniae: antithrombin was significantly lower in nonsurvivors (70% vs 81%, P = .038), confirmed in univariate analysis (OR 0.961; 95% CI, 0.921-0.997). For overall population: in multivariate regression, increasing age was associated with mortality (OR 1.043; 95% CI, 1.010-1.077), and S. pneumoniae etiology with the composite outcome (OR 6.024; 95% CI, 1.798-20.180)., Conclusions: For invasive infections caused by N. meningitidis, D-dimer is a biomarker capable of predicting unfavorable clinical outcomes; a potential role is suggested for aPTT prolongation and protein C decrease, and, in case of S. pneumoniae infections, for antithrombin decrease., (Copyright © 2024 The Authors. 248. Efficacy and perceived satisfaction of 3-year SLIT in children with allergic rhinitis and asthma: a pilot study.

- Abstract
Background: Despite the presence of robust evidence, there is very limited data on the efficacy of allergen immunotherapy (AIT) for selected patients. Accordingly, we aimed to evaluate the efficacy and perceived satisfaction of a 3-year course of sublingual immunotherapy (SLIT) in a paediatric population with allergic rhinitis and/or asthma., Methods: A pilot, monocentre, retrospective cohort study was performed. One hundred fifty-three children who fulfilled the criteria for allergic rhinitis and asthma and were either mono- or poly-sensitized were enrolled. A standardized questionnaire assessing perceived efficacy, use of rescue medication, disease control, number of exacerbations, quality of life, and perceived satisfaction was administered to each patient., Results: Seventy patients (49 males, 21 females; mean age, 14.3±1.9 years) were included in the final analysis. All 70 patients received SLIT for up to 3 years, with 100% adherence to the treatment throughout the study. Significant improvements in symptoms and quality of life were reported (p<0.01). There was also a significant decrease in disease severity, use of rescue medication, and sleep disturbances (p<0.01). Additionally, a significant improvement in school performance was also recorded (p<0.01). Of the enrolled patients, 60 out of 70 (85.7%) reported being very satisfied, 6 out of 70 (8.57%) were much satisfied, and 4 out of 60 (5.71%) were satisfied., Conclusions: We were the first to demonstrate the efficacy and perceived satisfaction of a 3-year SLIT in a paediatric population, with 100% treatment adherence throughout the study., Competing Interests: EC and FF work in Lofarma. An association between the leukocyte count and the risk of developing myocardial infarction has been well known for many years; however, only recently did Mendelian randomization studies show that a high neutrophil count is a causal risk factor for atherosclerotic cardiovascular disease. On the other hand, experimental studies show that depletion of circulating neutrophils impairs plaque development. Clopidogrel, an antiplatelet agent, is widely used in combination with aspirin to reduce the incidence of ischemic events in patients treated with coronary stenting. Chronic treatment with this drug reduces inflammatory markers and neutrophil numbers, rarely causing severe leukopenia. The purpose of this review is to present recent evidence showing the link between neutrophil number and the development of cardiovascular diseases and to discuss how the clopidogrel-induced reduction in the neutrophil count may be a beneficial off-target effect of this drug., (Copyright © 2024 Italian Federation of Cardiology - I.F.C. 250. Derivation and Validation of the PRECISE-HBR Score to Predict Bleeding After Percutaneous Coronary Intervention.

- Abstract
Background: Accurate bleeding risk stratification after percutaneous coronary intervention (PCI) is important for treatment individualization. However, there is still an unmet need for a more precise and standardized identification of high bleeding risk patients. We derived and validated a novel bleeding risk score by augmenting the PRECISE-DAPT score with the Academic Research Consortium for High Bleeding Risk (ARC-HBR) criteria., Methods: The derivation cohort comprised 29,188 patients undergoing PCI, of whom 1136 (3.9%) had a Bleeding Academic Research Consortium (BARC) 3 or 5 bleeding at 1 year, from four contemporary real-world registries and the XIENCE V USA trial. The PRECISE-DAPT score was refitted with a Fine-Gray model in the derivation cohort and extended with the ARC-HBR criteria. The primary outcome was BARC 3 or 5 bleeding within 1 year. Independent predictors of BARC 3 or 5 bleeding were selected at multivariable analysis (p<0.01). The discrimination of the score was internally assessed with apparent validation and cross-validation. The score was externally validated in 4578 patients from the MASTER DAPT trial and 5970 patients from the STOPDAPT-2 total cohort., Results: The PRECISE-HBR score (age, estimated glomerular filtration rate, hemoglobin, white-blood-cell count, previous bleeding, oral anticoagulation, and ARC-HBR criteria) showed an area under the curve (AUC) for 1-year BARC 3 or 5 bleeding of 0.73 (95% CI, 0.71-0.74) at apparent validation, 0.72 (95% CI, 0.70-0.73) at cross-validation, 0.74 (95% CI, 0.68-0.80) in the MASTER DAPT, and 0.73 (95% CI, 0.66-0.79) in the STOPDAPT-2, with superior discrimination than the PRECISE-DAPT (cross-validation: Δ AUC, 0.01; p=0.02; MASTER DAPT: Δ AUC, 0.05; p=0.004; STOPDAPT-2: Δ AUC, 0.02; p=0.20) and other risk scores. In the derivation cohort, a cut-off of 23 points identified 11,414 patients (39.1%) with a 1-year BARC 3 or 5 bleeding risk ≥4%. An alternative version of the score, including acute myocardial infarction on admission instead of white-blood-cell count, showed similar predictive ability., Conclusions: The PRECISE-HBR score is a contemporary, simple 7-item risk score to predict bleeding after PCI, offering a moderate improvement in discrimination over multiple existing scores. Further evaluation is required to assess its impact on clinical practice.
