Glomerular damage in nephrotoxic nephritis is mediated by circulating cells (neutrophils, platelets, and macrophages) infiltrating the glomerular tuft and responsible for the inflammatory reactions. It has been suggested that inflammatory cells may participate in glomerular inflammation through the synthesis and release of eicosanoids derived from the metabolism of arachidonic acid. On the other hand, old data and recent evidence indicate that eicosanoids derived from renal arachidonic acid metabolism play an important role in maintaining hemodynamics, especially in disease conditions. We wanted to evaluate the differential role of arachidonic acid metabolites derived from circulating or renal cells on the evolution of nephrotoxic nephritis. We used a nonselective cyclooxygenase inhibitor, aspirin, which blocks eicosanoid synthesis both in circulating cells and in renal tissue, as compared with a selective cyclooxygenase inhibitor, sulindac, which inhibits arachidonic acid metabolism in circulating cells, partially sparing renal cyclooxygenase. Aspirin, at a dosage that almost completely inhibits both circulating cell and renal arachidonate metabolites, worsens the morphologic expression of rabbit nephrotoxic nephritis and negatively influences the clinical course of the disease. Sulindac, at a dose that suppresses circulating platelet cyclooxygenase activity by 90%, but only partially affects renal prostaglandin synthesis, prevents extracapillary proliferation and reduces proteinuria without negative influence on glomerular hemodynamics.

The expulsion of runaway electrons (REs) during different types of tokamak instabilities is analysed by means of a Cherenkov probe inserted into the scrape-off layer of the FTU tokamak. One such type of instability, the well-known tearing mode, is involved in disruptive plasma termination events, during which the risk of RE avalanche multiplication is highest. The second type, known as anomalous Doppler instability, influences RE dynamics by enhancing pitch angle scattering. Three scenarios are analysed here, characterised by different RE generation rates and mechanisms. The main conclusions are drawn from correlations between the Cherenkov probe and other diagnostics. In particular, the Cherenkov probe permits the detection of fast electron expulsion with a high level of detail, presenting peaks with 100% signal contrast during tearing mode growth and rotation, and sub-peak structures reflecting the interplay between the magnetic island formed by the tearing mode, RE diffusion during island rotation and the geometry of obstacles in the vessel. Correlations between the Cherenkov signal, hard x-ray emission and electron cyclotron emission reveal the impulsive development of the anomalous Doppler instability with instability rise time in the microsecond scale resolved by the high time-resolution of the Cherenkov probe.

Background: Remodelling of the heart in patients with pulmonary arterial hypertension (PAH) involves both the right and left ventricles. The objective of the present study was to analyse the dimensions of the right and left ventricles as assessed by echocardiography, and to compare this with haemodynamics and exercise capacity inpatients with PAH.Materials and methods: Transthoracic echocardiogram, exercise capacity as assessed by the 6-minute walk test and right-heart catheterization were performed within 1 week in 66 consecutive patients with PAH. Univariate correlation and multiple regression analyses were performed to relate echocardiographic and clinical variables.Results: Left ventricular end-diastolic area index had the best correlation with cardiac index (r = 0.62; p < 0.001) and 6-minute walk distance(r = 0.56; p < 0.001). The ratio of the right to left ventricular area correlated with right atrial pressure (r = 0.62; p < 0.001). Right ventricular percent change in area correlated with cardiac index (r = 0.51; p < 0.001). No correlations were observed between the right and left ventricular dimensions.Conclusions: Left ventricular dimensions and the ratio of the right to left ventricular area correlated with resting haemodynamics and exercise capacity in patients with pulmonary arterial hypertension. These correlations were stronger than those observed with right ventricular dimensions alone. Left ventricular dimensions are important for the non-invasive evaluation of severity of patients with pulmonary arterial hypertension. [ABSTRACT FROM AUTHOR]

Summary Here, CD40L expression and cytokine production have been analysed in peripheral blood cells from orthotopic liver transplantation (OLT) recipients treated with ribavirin for recurrent chronic hepatitis C. The study included 18 OLT recipients treated with ribavirin, eight control OLT recipients and 10 healthy controls. FACS analysis showed that baseline expression of CD40L was not different between ribavirin-treated patients and controls. In contrast, after stimulation with both HCV core antigen and phorbol myristate acetate (PMA) plus ionomycin (IO), the expression of CD40L on CD4 lymphocytes was significantly higher in the ribavirin group compared with controls. In the ribavirin group, the increased expression of CD40L significantly correlated with reduction of HCV RNA levels with respect to pretreatment values. Finally, ribavirin treatment was not associated with modification of PMA-IO-induced cytokine production by T lymphocytes and interleukin (IL)-1β and tumour necrosis-α (TNF)-α production by CD40L-stimulated monocytes. In conclusion, these data indicate that ribavirin upmodulates CD40L expression on CD4 T cells, a property which may account in part for its ability to enhance the antiviral activity of interferon-α in the treatment of chronic HCV infection. The Radial Neutron Camera (RNC) is an ITER diagnostic system devoted to the radial measurement of the plasma neutron emissivity during ITER operation. In particular, plasma core measurements will be performed using 48 detectors located in the Ex-Port RNC subsystem and viewing the plasma through 16 lines-of-sight (LOS). Since discrepancies concerning the position of the LOS and the related optical path can occur during RNC installation and ITER operation, a Position Monitoring System (PMS) is foreseen to evaluate the misalignment and provide a correction tool for the neutrons count rate. During the design of the PMS for the RNC, optic fiber-based displacement sensors have been identified as the best solution to measure the displacements from reference positions along the radial, vertical and toroidal directions. Potential issues that could arise during the installation and operation of such sensors have been evaluated; MCNP calculations have been performed to determine the radiation level at the position of the PMS components. Finally, mechanical tests have been carried out at ENEA to evaluate the performance of the sensors: results indicate compliance with the specifications provided by the producer and the ITER RNC design requirements. Quantification of dysfunctional but viable myocardium has high prognostic value for improvement of left ventricular (LV) function after coronary artery bypass grafting (CABG). Dobutamine stress echocardiography (DSE) can assess viable myocardium by segmental wall motion changes during stress. However, analysis of wall motion is subjective with only moderate interinstitutional agreement (70%) and frequently overestimates functional improvement after CABG. In contrast, calculation of ejection fraction (EF) is less subjective and allows a more precise quantification of global contractile reserve. The aim of the study was to compare the prognostic value of EF response and segmental wall motion changes during DSE for the prediction of LV functional recovery after CABG. Forty patients underwent DSE before CABG. EF responses were assessed at rest, low-dose dobutamine, and at peak stress using the biplane disk method. Wall motion was scored using a 16-segment 5-point model. Resting radionuclide ventriculography (RNV-LVEF), performed before and 8 +/- 2 months after CABG, was used as an independent reference. Five patients were excluded because of perioperative infarction or poor echo images. In 11 of 35 patients, RNV-LVEF recovered >5%. Improvement in EF during dobutamine infusion predicted RNV-LVEF recovery after CABG significantly better than segmental wall motion changes (72% vs 53%, p = 0.03). A biphasic EF response (i.e., improvement in > or =10% at low dose and subsequent worsening at peak stress) had the highest predictive value (80%) for late functional recovery. In conclusion, EF response to dobutamine infusion was superior to segmental wall motion changes in predicting RNV-LVEF recovery after CABG. [ABSTRACT FROM AUTHOR]

Within the EUROfusion MST1 work package, a series of experiments has been conducted on AUG and TCV devices to disentangle the role of plasma fueling and plasma shape for the onset of small ELM regimes. On both devices, small ELM regimes with high confinement are achieved if and only if two conditions are fulfilled at the same time. Firstly, the plasma density at the separatrix must be large enough (), leading to a pressure profile flattening at the separatrix, which stabilizes type-I ELMs. Secondly, the magnetic configuration has to be close to a double null (DN), leading to a reduction of the magnetic shear in the extreme vicinity of the separatrix. In the frame of the design activities of the Divertor Tokamak Test (DTT) facility the development of a comprehensive set of neutron and gamma-ray diagnostics is on-going in order to enable measurements of: neutron yield, neutron yield rate, neutron emissivity over a poloidal section through the plasma; neutron emission spectrum; runaway electrons induced bremsstrahlung radiation and gamma-ray emission from reactions between fast ions and plasma impurities. The present paper provides an overview of the DTT neutron and gamma-ray diagnostics and describes the present status of their design including main components and interfaces, detector types and performances. The ASDEX Upgrade (AUG) programme, jointly run with the EUROfusion MST1 task force, continues to significantly enhance the physics base of ITER and DEMO. Here, the full tungsten wall is a key asset for extrapolating to future devices. The high overall heating power, flexible heating mix and comprehensive diagnostic set allows studies ranging from mimicking the scrape-off-layer and divertor conditions of ITER and DEMO at high density to fully non-inductive operation (q95 = 5.5, ) at low density. Higher installed electron cyclotron resonance heating power 6 MW, new diagnostics and improved analysis techniques have further enhanced the capabilities of AUG. Stable high-density H-modes with MW m−1 with fully detached strike-points have been demonstrated. The ballooning instability close to the separatrix has been identified as a potential cause leading to the H-mode density limit and is also found to play an important role for the access to small edge-localized modes (ELMs). Density limit disruptions have been successfully avoided using a path-oriented approach to disruption handling and progress has been made in understanding the dissipation and avoidance of runaway electron beams. ELM suppression with resonant magnetic perturbations is now routinely achieved reaching transiently . This gives new insight into the field penetration physics, in particular with respect to plasma flows. Modelling agrees well with plasma response measurements and a helically localised ballooning structure observed prior to the ELM is evidence for the changed edge stability due to the magnetic perturbations. The impact of 3D perturbations on heat load patterns and fast-ion losses have been further elaborated. Progress has also been made in understanding the ELM cycle itself. Here, new fast measurements of and Er allow for inter ELM transport analysis confirming that Er is dominated by the diamagnetic term even for fast timescales. New analysis techniques allow detailed comparison of the ELM crash and are in good agreement with nonlinear MHD modelling. The observation of accelerated ions during the ELM crash can be seen as evidence for the reconnection during the ELM. As type-I ELMs (even mitigated) are likely not a viable operational regime in DEMO studies of 'natural' no ELM regimes have been extended. Stable I-modes up to have been characterised using -feedback. Core physics has been advanced by more detailed characterisation of the turbulence with new measurements such as the eddy tilt angle—measured for the first time—or the cross-phase angle of and fluctuations. These new data put strong constraints on gyro-kinetic turbulence modelling. In addition, carefully executed studies in different main species (H, D and He) and with different heating mixes highlight the importance of the collisional energy exchange for interpreting energy confinement. A new regime with a hollow profile now gives access to regimes mimicking aspects of burning plasma conditions and lead to nonlinear interactions of energetic particle modes despite the sub-Alfvénic beam energy. This will help to validate the fast-ion codes for predicting ITER and DEMO. • PROTO-SPHERA is a new concept of spherical torus with closed flux surfaces and a force-free Screw Pinch with open flux surfaces. • A two colour interferometer is the diagnostic system to measure the line density and to characterize this uncommon plasma. • For the high densities and its rate variation which can provoke a beam refraction a CO2 laser is the wavelength choice. • A two-colour vibration compensation system is foreseen by using a He-Ne laser. • The interferometer was assembled in a laboratory, a test interferometric signal has been observed. PROTO-SPHERA (Spherical Plasma for Helicity Relaxation Assessment) is a new concept of torus that aims to produce a spherical torus with closed flux surfaces and a force-free Screw Pinch with open flux surfaces, fed by electrodes. In order to verify the stability of the configuration and the technical components of the PROTO-SPHERA initial arc, the initial Phase-1 experiment was built, and its plasma, in the year 2108, has reached both the total current (10 kA) and total duration (1 s) targets. A two colour interferometer is one of the diagnostic system proposed to provide PROTO-SPHERA with the measurement of its electron density, which is currently of the order of ∼1020 m−3. It will allow to obtain line density, necessary to characterize this uncommon plasma, as integrate measurement along the equatorial plane. A portable Runaway Electron Imaging and Spectrometry System (REIS) was developed in ENEA-Frascati to measure synchrotron radiation spectra from in-flight runaway electrons in tokamaks. The REIS is a wide-angle optical system collecting simultaneously visible and infrared emission spectra using an incoherent bundle of fibers, in a spectral range that spans from 500 nm to 2500 nm, and visible images using a CCD color microcamera at a rate of 25 frames/s. The REIS system is supervised and managed using a dedicated LabVIEW program to acquire data simultaneously from three spectrometers every 20 ms (configurable down to 10 ms). An overview of the REIS architecture and acquisition system and resulting experimental data obtained in FTU are presented and discussed in this paper. [ABSTRACT FROM AUTHOR]

The new multichannel X-mode reflectometer installed on ASDEX Upgrade measures the plasma density profile evolution at different positions in front of the ICRF antenna. The reflectometer operates in the extended U-band (40–68 GHz) microwave region, measuring density profiles up to 2 × 10 19 m −3 with magnetic fields between 1.5 T and 2.7 T. In this heterodyne reflectometer architecture, the signal reflected by the plasma is down-shifted and quadrature detectors demodulate its in-phase and quadrature (IQ) components. In this work we analysed the acquired IQ signals from the different reflectometer antennas, and describe the waveguide dispersion calibration and filtering of the raw signal. The effect of spurious reflections, such as the multiple reflections from the ICRF antenna metal straps, are analysed and taken into account on the data processing software. In high plasma density and high magnetic field scenarios, both the lower and upper X-mode cut off frequencies are detected in the probing range. The first fringe (FF) reflection of the upper cut off indicates the start of the plasma signal and must be determined robustly to reduce density profile errors. Existing FF estimation algorithms based on signal amplitude and spectral information may still fail in the presence of both upper and lower cut off reflections. [ABSTRACT FROM AUTHOR]

New ICRF (Ion Cyclotron Range of Frequency) antennas, targeted to reduce impurity generation in the all-metal ASDEX Upgrade were designed and fabricated at different sites in an international cooperation between IPP, ASIPP (Hefei, China) and ENEA (Frascati, Italy). Installation and test were done by a multinational team at IPP. The antenna further integrates a powerful reflectometer system, designed and built by ENEA and IST (Portugal). The project, with the cooperation of four institutes in four countries on two continents was successfully completed within budget and schedule boundaries. Test results, gathered during recent experiment campaign, reveal that the new antennas have achieved their goal: A significant reduction of ICRF heating induced W impurity generation at the antennas. An overview of recent results obtained at the tokamak ASDEX Upgrade (AUG) is given. A work flow for predictive profile modelling of AUG discharges was established which is able to reproduce experimental H-mode plasma profiles based on engineering parameters only. In the plasma center, theoretical predictions on plasma current redistribution by a dynamo effect were confirmed experimentally. For core transport, the stabilizing effect of fast ion distributions on turbulent transport is shown to be important to explain the core isotope effect and improves the description of hollow low-Z impurity profiles. The L–H power threshold of hydrogen plasmas is not affected by small helium admixtures and it increases continuously from the deuterium to the hydrogen level when the hydrogen concentration is raised from 0 to 100%. One focus of recent campaigns was the search for a fusion relevant integrated plasma scenario without large edge localised modes (ELMs). Results from six different ELM-free confinement regimes are compared with respect to reactor relevance: ELM suppression by magnetic perturbation coils could be attributed to toroidally asymmetric turbulent fluctuations in the vicinity of the separatrix. Stable improved confinement mode plasma phases with a detached inner divertor were obtained using a feedback control of the plasma β. The enhanced Dα H-mode regime was extended to higher heating power by feedback controlled radiative cooling with argon. The quasi-coherent exhaust regime was developed into an integrated scenario at high heating power and energy confinement, with a detached divertor and without large ELMs. Small ELMs close to the separatrix lead to peeling-ballooning stability and quasi continuous power exhaust. Helium beam density fluctuation measurements confirm that transport close to the separatrix is important to achieve the different ELM-free regimes. Based on separatrix plasma parameters and interchange-drift-Alfvén turbulence, an analytic model was derived that reproduces the experimentally found important operational boundaries of the density limit and between L- and H-mode confinement. Feedback control for the X-point radiator (XPR) position was established as an important element for divertor detachment control. Stable and detached ELM-free phases with H-mode confinement quality were obtained when the XPR was moved 10 cm above the X-point. Investigations of the plasma in the future flexible snow-flake divertor of AUG by means of first SOLPS-ITER simulations with drifts activated predict beneficial detachment properties and the activation of an additional strike point by the drifts. Objective The objective of the study was to investigate the relationship between structural connectivity ( SC) and functional connectivity ( FC) in the cingulum in bipolar disorder ( BD) and its various phases. Method We combined resting-state functional magnetic resonance imaging and probabilistic tractographic diffusion tensor imaging to investigate FC and SC of the cingulum and its portions, the SC- FC relationship, and their correlations with clinical and neurocognitive measures on sustained attention in manic ( n = 21), depressed ( n = 20), and euthymic ( n = 20) bipolar patients and healthy controls ( HC) ( n = 42). Results First, we found decreased FC between the anterior and posterior parts of the cingulum in manic patients when compared to depressed patients and HC. Second, we observed decreased SC of the cingulum bundle, particularly in its anterior part, in manic patients when compared to HC. Finally, alterations in the cingulum FC (but not SC) correlated with clinical severity scores while changes in the cingulum SC (but not FC) were related with neurocognitive deficits in sustained attention in BD. Conclusion We demonstrate for the first time a reduction in FC and concomitantly in SC of the cingulum in mania, which correlated with psychopathological and neurocognitive parameters, respectively, in BD. A multichannel reflectometer will be built for the new three-straps ICRF antenna of ASDEX Upgrade (AUG), to study the density behavior in front of it. Ten different accesses to the plasma are available for the three reflectometer channels that can be interchanged without breaking the machine vacuum. Frequency is scanned from 40 GHz to 68 GHz, in 10ms, which corresponds to a cut-off density ranging from 1018 1019m??3 in the Right cut-off of the X-mode propagation, for standard toroidal magnetic field values of AUG. The diagnostic performances of photodetectors based on Chemical Vapor Deposition (CVD) single crystal diamonds that were installed on one of the equatorial ports of the FTU tokamak during the last six months of operation of the machine are reported. Examples of plasma fast events have been collected in several different plasma conditions, confirming the fast response capabilities of diamond detectors and the high S/N ratio. During the Runaway Control and Mitigation experiments, for example, the so-called Anomalous Doppler Instabilities were observed as sharp peaks of Ly-α emission caused by fast electrons hitting the wall, as a result of RE beam instabilities, perfectly correlated in time with other magnetic diagnostics and fast EC polychromator signals. Fast oscillations in the plasma emission are also observed in the presence of tearing modes, while other interesting observations relate to pellet ablation processes. The CVD diamond detectors were installed on FTU in view of their possible use for replacement of the Si photodiodes currently adopted for Soft X-Ray tomography (SXT) on future devices with harsher radiation environments. In fact, it was realized that the relatively flat response of the UV diamond sensor opened the possibility of using these detectors as bolometers. The first encouraging results have prompted launching an R&D program for the development of full-fledged diamond bolometers, which will be especially well suited for the coverage of the divertor and edge regions in high performance devices, in the energy range from 10 eV to about 20 keV.

The paper reports the results of laboratory testing of dense sandy gravel carried out on large dimension undisturbed samples, obtained by means of in situ freezing technique. The specimens, 290 mm in diameter and 600 mm high, have been subjected to undrained cyclic triaxial tests carried out in isotropically and anisotropically consolidated conditions aimed at investigating sandy gravel susceptibility to cyclic mobility and plastic strain accumulation during earthquake induced vibrations. The quality of the undisturbed samples was corroborated by comparing the field shear wave velocities against those inferred from bender element tests, carried out in the triaxial apparatus after the specimen consolidation. The cyclic tests were supported by a series of monotonic undrained and drained triaxial tests performed on isotropically and anisotropically consolidated specimens to help in their more rational interpretation. The tests yielded valuable information as to the susceptibility of the sandy gravel in hands to cyclic mobility and plastic strain accumulation during earthquake loading. [ABSTRACT FROM AUTHOR]
