The work reports the observed fatigue damage of C45 steel specimens tested in a previous work under multiaxial loading conditions and its relationship with the thermal energy dissipation which has been used in the last decades to estimate the uniaxial fatigue behavior of metals. For this purpose, fatigue data relevant to thin-walled samples made of quenched and tempered C45 steel tested under completely reversed combined axial and torsional cyclic loadings with different biaxiality ratios and phase-shift angles have been analysed. The analyses of crack paths at the initiation point of failure were performed after a 50% stiffness loss that corresponded to a crack size ranging from 7 to 15 mm; afterwards, the characteristic crack paths of each loading condition were analysed by using a digital microscope to identify the orientation of the crack initiation plane. After having broken all fatigue tested specimens under static tensile loading, the fracture surfaces were inspected close to the crack initiation point using a digital microscope. Despite the stress states and fatigue damage mechanisms dependent on the load condition, the Q parameter applied to the present experimental results proved to correlate all multiaxial fatigue test results in a single fatigue scatter band

This paper presents WHETGEO and its 1D deployment: a new physically based model simulating the water and energy budgets in a soil column. The purpose of this contribution is twofold. First, we discuss the mathematical and numerical issues involved in solving the Richardson–Richards equation, conventionally known as the Richards equation, and the heat equation in heterogeneous soils. In particular, for the Richardson–Richards equation (R2) we take advantage of the nested Newton–Casulli–Zanolli (NCZ) algorithm that ensures the convergence of the numerical solution in any condition. Second, starting from numerical and modelling needs, we present the design of software that is intended to be the first building block of a new customizable land-surface model that is integrated with process-based hydrology. WHETGEO is developed as an open-source code, adopting the object-oriented paradigm and a generic programming approach in order to improve its usability and expandability. WHETGEO is fully integrated into the GEOframe/OMS3 system, allowing the use of the many ancillary tools it provides. Finally, the paper presents the 1D deployment of WHETGEO, WHETGEO-1D, which has been tested against the available analytical solutions presented in the Appendix.

BackgroundAlmost 44 million people are currently living with dementia worldwide. This number is set to increase threefold by 2050, posing a serious threat to the sustainability of healthcare systems. Overuse of antipsychotic drugs for the management of the symptoms of dementia carries negative consequences for patients while also increasing the health expenditures for society. Supportive care (SC) interventions could be considered a safer and potentially cost-saving option. In this paper we provide a systematic review of the existing evidence regarding the cost-effectiveness and cost-utility of SC interventions targeted towards persons living with dementia and their caregivers.MethodsA systematic literature review was performed between February 2019 and December 2021 through searches of the databases PubMed (MEDLINE), Cochrane Library, CENTRAL, Embase and PsycINFO. The search strategy was based on PRISMA 2020 recommendations. We considered studies published through December 2021 with no lower date limit. We distinguished between five categories of SC strategies: cognitive therapies, physical activity, indirect strategies (organisational and environmental changes), interventions primarily targeted towards family caregivers, and multicomponent interventions.ResultsOf the 5,479 articles retrieved, 39 met the inclusion criteria. These studies analysed 35 SC programmes located at different stages of the dementia care pathway. Eleven studies provided evidence of high cost-effectiveness for seven interventions: two multicomponent interventions; two indirect interventions; two interventions aimed at caregivers of community-dwelling persons with dementia; one community-based cognitive stimulation and occupational programme.ConclusionWe find that the most promising SC strategies in terms of cost-effectiveness are multicomponent interventions (targeted towards both nursing home residents and day-care service users), indirect strategies (group living and dementia care management at home), some forms of tailored occupational therapy, together with some psychosocial interventions for caregivers of community-dwelling persons with dementia. Our results suggest that the adoption of effective SC interventions may increase the economic sustainability of dementia care.

BackgroundIntensive care unit (ICU) survivors have reduced long-term survival compared to the general population. Identifying parameters at ICU discharge that are associated with poor long-term outcomes may prove useful in targeting an at-risk population. The main objective of the study was to identify clinical and biological determinants of death in the year following ICU discharge.MethodsFROG-ICU was a prospective, observational, multicenter cohort study of ICU survivors followed 1 year after discharge, including 21 medical, surgical or mixed ICUs in France and Belgium. All consecutive patients admitted to intensive care with a requirement for invasive mechanical ventilation and/or vasoactive drug support for more than 24 h following ICU admission and discharged from ICU were included. The main outcome measure was all-cause mortality at 1 year after ICU discharge. Clinical and biological parameters on ICU discharge were measured, including the circulating cardiovascular biomarkers N-terminal pro-B type natriuretic peptide, high-sensitive troponin I, bioactive-adrenomedullin and soluble-ST2. Socioeconomic status was assessed using a validated deprivation index (FDep).ResultsOf 1570 patients discharged alive from the ICU, 333 (21%) died over the following year. Multivariable analysis identified age, comorbidity, red blood cell transfusion, ICU length of stay and abnormalities in common clinical factors at the time of ICU discharge (low systolic blood pressure, temperature, total protein, platelet and white cell count) as independent factors associated with 1-year mortality. Since the early 2000s, an ever-increasing subset of missense pathogenic variants in the ACTG1 gene has been associated with an autosomal-dominant, progressive, typically post-lingual non-syndromic hearing loss (NSHL) condition designed as DFNA20/26. ACTG1 gene encodes gamma actin, the predominant actin protein in the cytoskeleton of auditory hair cells; its normal expression and function are essential for the stereocilia maintenance. Different gain-of-function pathogenic variants of ACTG1 have been associated with two major phenotypes: DFNA20/26 and Baraitser–Winter syndrome, a multiple congenital anomaly disorder. Here, we report a novel ACTG1 variant [c.625G>A (p. Val209Met)] in an adult patient with moderate-severe NSHL characterized by a downsloping audiogram. The patient, who had a clinical history of slowly progressive NSHL and tinnitus, was referred to our laboratory for the analysis of a large panel of NSHL-associated genes by next generation sequencing. An extensive review of previously reported ACTG1 variants and their associated phenotypes was also performed.

Objective To investigate risk factors and subphenotypes associated with long term symptoms and outcomes after hospital admission for covid-19.Design Prospective, multicentre observational study.Setting 93 hospitals in France.Participants Data from 2187 adults admitted to hospital with covid-19 in France between 1 February 2020 and 30 June 2021.Main outcome measures Primary endpoint was the total number of persistent symptoms at six months after hospital admission that were not present before admission. Outcomes examined at six months were persistent symptoms, Hospital Anxiety and Depression Scale, six minute walk test distances, 36-Item Short Form Health Survey scores, and ability to resume previous professional activities and self-care. Secondary endpoints included vital status at six months, and results of standardised quality-of-life scores. Additionally, an unsupervised consensus clustering algorithm was used to identify subphenotypes based on the severity of hospital course received by patients.Results 1109 (50.7%) of 2187 participants had at least one persistent symptom. Factors associated with an increased number of persistent symptoms were in-hospital supplemental oxygen (odds ratio 1.12, 95% confidence interval 1 to 1.24), no intensive care unit admission (1.15, 1.01 to 1.32), female sex (1.33, 1.22 to 1.45), gastrointestinal haemorrhage (1.51, 1.02 to 2.23), a thromboembolic event (1.66, 1.17 to 2.34), and congestive heart failure (1.76, 1.27 to 2.43). Three subphenotypes were identified: including patients with the least severe hospital course (based on ventilatory support requirements). Although Hospital Anxiety and Depression Scale scores were within normal values for all groups, patients of intermediate severity and more comorbidities had a higher median Hospital Anxiety and Depression Scale score than did the other subphenotypes. Patients in the subphenotype with most severe hospital course had worse short form-36 scores and were less able to resume their professional activity or care for themselves as before compared with other subphenotypes.Conclusions Persistent symptoms after hospital admission were frequent, regardless of acute covid-19 severity. However, patients in more severe subphenotypes had a significantly worse functional status and were less likely to resume their professional activity or able to take care of themselves as before.Trial registration NCT04262921.

IntroductionAfter COVID-19 restrictions, hybrid solutions were established that combined smart working and work in presence. Workplace conditions significantly impact employees' lives, particularly in terms of meeting their needs and promoting their wellbeing. Based on a socio-ecological and multilevel methodology, the UP150 concept (Proactive Office 150) represents a possible innovative solution to meet employees' needs and valorize flexible work. It encourages physical exercise and active breaks during the typical workday by using particular architectural modifications, a dedicated App, and physical activity professionals as wellness coaches. The present study is the last step of the preliminary actions planned to check the benefits of the UP150 concept and aims to explore the workers' perceptions after experiencing this project.MethodsThe qualitative analysis of a preliminary survey (concerning information about the company structure and workers' habits) performed before conducting a randomized controlled trial intervention study and the analysis of the semi-structured interviews after 8 weeks of a UP150 experience served as datasets for this study and have been examined and discussed.ResultsIn the preliminary survey, the young (under 40) and generally active (57% of the workers) reported being motivated to exercise but inhibited by a lack of time and a heavy workload. After 8 weeks at a modified workplace designed in accordance with the motive behind the UP150, the workers displayed noticeable positive perceptions and appreciation.DiscussionThe qualitative analysis confirmed and supported the effectiveness of the UP150 concept that previous research had already found in quantitative parameters related to employees' motor efficiency, psychophysical status, and amount of physical activity. Participants reported beneficial perceived effects on their wellness and psychophysical status following a UP150 experience. Moreover, the concept improved social relationships and increased motivation. In conclusion, the UP150 concept efficiently fostered a positive perception of physical exercise and directed the employees toward the assumption of healthy behaviors fitting the physical literacy paradigm.

This paper presents a critical analysis of image-based 3D reconstruction using neural radiance fields (NeRFs), with a focus on quantitative comparisons with respect to traditional photogrammetry. The aim is, therefore, to objectively evaluate the strengths and weaknesses of NeRFs and provide insights into their applicability to different real-life scenarios, from small objects to heritage and industrial scenes. After a comprehensive overview of photogrammetry and NeRF methods, highlighting their respective advantages and disadvantages, various NeRF methods are compared using diverse objects with varying sizes and surface characteristics, including texture-less, metallic, translucent, and transparent surfaces. We evaluated the quality of the resulting 3D reconstructions using multiple criteria, such as noise level, geometric accuracy, and the number of required images (i.e., image baselines). The results show that NeRFs exhibit superior performance over photogrammetry in terms of non-collaborative objects with texture-less, reflective, and refractive surfaces. Conversely, photogrammetry outperforms NeRFs in cases where the object’s surface possesses cooperative texture. Such complementarity should be further exploited in future works.

Metabolic syndrome is a serious health condition reaching epidemic proportions worldwide and is closely linked to an increased risk of cardiovascular problems. The lack of appropriate treatment paves the way for developing new therapeutic agents as a high priority in the current research. In this study, we evaluated the protective effects of Capsicum baccatum red pepper on metabolic syndrome scenarios induced by an ultra-processed diet in rats. After four months, the ultra-processed diet increased central obesity, triglycerides, total cholesterol, LDL-cholesterol plasma levels, and impaired glucose tolerance. The oral administration of C. baccatum concomitantly with the ultra-processed diet avoided the accumulation of adipose tissue in the visceral region, reduced the total cholesterol and LDL fraction, and improved glucose homeostasis, factors commonly associated with metabolic syndrome. The data presented herein reveal an important preventive action of C. baccatum in developing metabolic disorders among animals fed a hypercaloric diet, significantly reducing their cardiometabolic risk. Allied with the absence of toxic effects after chronic use, our study suggests C. baccatum red pepper as a secure and enriched source of bioactive compounds promising to protect against pathological processes associated with metabolic syndrome.

Augmented Reality (AR) is already transforming many fields, from medical applications to industry, entertainment and heritage. In its most common form, AR expands reality with virtual 3D elements, providing users with an enhanced and enriched experience of the surroundings. Until now, most of the research work focused on techniques based on markers or on GNSS/INS positioning. These approaches require either the preparation of the scene or a strong satellite signal to work properly. In this paper, we investigate the use of visual-based methods, i.e., methods that exploit distinctive features of the scene estimated with Visual Simultaneous Localization and Mapping (V-SLAM) algorithms, to determine and track the user position and attitude. The detected features, which encode the visual appearance of the scene, can be saved and later used to track the user in successive AR sessions. Existing AR frameworks like Google ARCore, Apple ARKit and Unity AR Foundation recently introduced visual-based localization in their frameworks, but they target mainly small scenarios. We propose a new Mobile Augmented Reality (MAR) methodology that exploits OPEN-V-SLAM to extend the application range of Unity AR Foundation and better handle large-scale environments. OBJECTIVE: This study evaluated the prevalence of obesity and associated factors among older adults. METHODS: This was a household-based cross-sectional study involving 282 individuals aged ≥ 60 years recruited in the city of Veranópolis, Brazil, using random probabilistic sampling. Participants underwent a clinical oral examination and completed a structured questionnaire. Obesity was determined based on body mass index (BMI). Individuals were divided into two groups based on the presence (BMI ≥ 30 kg/m2) or absence of obesity (BMI < 30 kg/m2). Uni- and multivariate analyses were performed using Poisson regression with robust variance. RESULTS: The prevalence of obesity was 34% (n = 96). Each 1-year increase in age resulted in a 3.09% decrease in the likelihood of being classified as having obesity (prevalence ratio [PR] = 0.969; 95% confidence interval [95%CI] 0.949 – 0.989). Older adults that reported walking < 5 or ≥ 5 times per week were 39.65 and 37.20% less likely to be classified as obese. The PRs of obesity in former and non-smokers were 4.40 and 5 times higher, respectively, than in current smokers (p < 0.05). Older adults with no access to dental care were 51.72% (p = 0.013) more likely to present with obesity. CONCLUSIONS: There was a high prevalence of obesity among older adults. Obesity was associated with lower age, smoking status, no access to dental care, and a lack of physical activity.
