Dental insurance is insurance designed to pay the costs associated with dental care. The Foreign Direct Investment (FDI) bill which was put forward in the winter session of the Lok Sabha (2008) focused on increasing the foreign investment share from the existing 26% to 49% in the insurance companies of India. This will allow the multibillion dollar international insurance companies to enter the Indian market and subsequently cover all aspects of insurance in India. Dental insurance will be an integral a part of this system. Dental insurance is a new concept in Southeast Asia as very few countries in Southeast Asia cover this aspect of insurance. It is important that the dentists in India should be acquainted with the different types of plans these companies are going to offer and about a new relationship which is going to emerge in the coming years between dentist, patient and the insurance company.

Background: Depressed scars in the neck pose a cosmetic problem. There is a need to fill the lost tissue volume defect between the surface and deeper tissues. It is preferable that the filling is done by autologous tissue which is available in substantial amount in the adjoining area. There should be no donor site morbidity. Platysma muscle flap meets these criteria. Materials and Methods: Platysma muscle flap was advanced into the defect after excision of depressed scar. The procedure was done under local anesthetic in two patients. Result: The result was a ′good scar′ with scar lying in the transversely oriented neck lines. Conclusions: Platysma muscle flap has a definitive role in revision surgery of depressed scars in neck as it provides an ideal tissue for lost tissue volume.

Angiokeratomas (AK) are a group of capillary vascular malformations encompassing several unrelated conditions, predominantly seen on the lower extremities. AK can be classified into localized and diffuse forms. The diffuse forms comprise Fabry′s disease, which is the result of α-galactosidase deficiency. The cutaneous AK includes the Mibelli type, the Fordyce type, angiokeratoma circumscriptum (AC), and ′solitary and multiple papular angiokeratoma′. We describe a case of AC with unusual features including late onset and palmar affection. Amongst the possible therapeutic options, cryotherapy was considered. However, the patient was lost to follow up.

A patient presenting with fever and purpura often presents a diagnostic challenge for the dermatologist. Acute hemorrhagic edema of infancy (AHEI) is a rare acute cutaneous leukocytoclastic vasculitic syndrome of infants leading to this presentation. We present an 18-month-old girl with AHEI who presented with fever, edema, and purpuric lesions involving the face, ears and extremities with uneventful complete recovery.

Aggressive angiomyxoma is a rare, locally invasive mesenchymal tumor occurring usually in women of reproductive age, which carries a high risk for local relapse; hence the need to differentiate it from the other mesenchymal tumors occurring in this region. We describe a case of a 44-year-old female presenting with a large pedunculated swelling on the right labia majora.

A 38-year - old female came to us with sudden eruptions of painful edematous lesions which appeared pseudovesicular on cutaneous examination. Histopathology supported the diagnosis of sweet′s syndrome and she responded to a combination of dapsone and oral steroids, after having relapsed on self-discontinuation of monotherapy with dapsone.

Superficial angiomyxomas (SA) are clinically variable, infrequent, benign skin lesions. They are usually located on the trunk, but can also appear on the lower limbs, head, and neck. The present case, hence, is unusual, because the lesion developed on the toe. They are diagnosed histologically, and are characterized by being poorly delimited tumors with the presence of interstitial myxoid material and scanty cellularity. We discuss the clinical and histological features of digital SA as well as a brief outline of other myxoid lesions.

Disabling pansclerotic morphea (DPM) of childhood is a rare generalized type of localized scleroderma (LS) that is known to follow an aggressive course with pansclerotic lesions leading to severe joint contractures and consequent immobility. Mortality is due to complications of the disease such as bronchopneumonia, sepsis, or gangrene. There is no specific laboratory finding. Treatment protocols are still evolving for this severe recalcitrant disorder. Extracutaneous manifestations are rarely reported in DPM. We present the case of a 7-year-old girl with DPM with severe extracutaneous manifestations in the form of gastrointestinal and vascular disease, whose disease progressed rapidly. In spite of treatment with methotrexate, corticosteroids, and PUVA therapy, she ultimately succumbed to her illness due to sepsis.

Dermoscopy is a non-invasive tool which helps in the visualization of skin structures and patterns not visible to the naked eye. This study evaluates the effectiveness of dermoscopy in diagnosing primary syphilis, which typically presents as a painless ulcer at the site of Treponema pallidum inoculation. In this study, dermoscopic patterns such as vascular structures and background colour changes were analyzed in patients with confirmed primary syphilis. Results indicate that primary syphilis lesions often display unique dermoscopic features, including polymorphic vessels and orange, red-purple backgrounds. These patterns can facilitate the early detection and diagnosis of syphilis, improving clinical outcomes through earlier intervention. Background: Inadvertent intra-arterial injection of sclerosants is an uncommon adverse event of both ultrasound-guided and direct vision sclerotherapy. This complication can result in significant tissue or limb loss and significant long-term morbidity., Objectives: To provide recommendations for diagnosis and immediate management of an unintentional intra-arterial injection of sclerosing agents., Methods: An international and multidisciplinary expert panel representing the endorsing societies and relevant specialities reviewed the published biomedical, scientific and legal literature and developed the consensus-based recommendations., Results: Actual and suspected cases of an intra-arterial sclerosant injection should be immediately transferred to a facility with a vascular/interventional unit. Digital Subtraction Angiography (DSA) is the key investigation to confirm the diagnosis and help select the appropriate intra-arterial therapy for tissue ischaemia. Emergency endovascular intervention will be required to manage the risk of major limb ischaemia. This includes intra-arterial administration of vasodilators to reduce vasospasm, and anticoagulants and thrombolytic agents to mitigate thrombosis. Mechanical thrombectomy, other endovascular interventions and even open surgery may be required. Lumbar sympathetic block may be considered but has a high risk of bleeding. Systemic anti-inflammatory agents, anticoagulants, and platelet inhibitors and modifiers would complement the intra-arterial endovascular procedures. For risk of minor ischaemia, systemic oral anti-inflammatory agents, anticoagulants, vasodilators and antiplatelet treatments are recommended., Conclusion: Inadvertent intra-arterial injection is an adverse event of both ultrasound-guided and direct vision sclerotherapy. Medical practitioners performing sclerotherapy must ensure completion of a course of formal training (specialty or subspecialty training, or equivalent recognition) in the management of venous and lymphatic disorders (phlebology), and be personally proficient in the use of duplex ultrasound in vascular (both arterial and venous) applications, to diagnose and provide image guidance to venous procedure. The toxic impact of environmentally relevant nominal sub-lethal concentration of lead nitrate (23 mg/L) on the gills of Cyprinus carpio after 30 days of exposure was assessed. Structural alterations were analyzed through histopathology, and the DNA damage rate in peripheral erythrocytes was evaluated by alkaline comet assay. A significant deviation in the gill histoarchitecture was observed compared to the control group. Significant changes, including the curling of secondary lamellae, loss of filaments, necrosis, hyperplasia, hypertrophy of cells of primary filament, and vacuolization, were found in the fish exposed to nominal sub-lethal concentration of lead nitrate. Moreover, the comet assay showed lead nitrate-induced DNA damage, evidenced by the length of the DNA "tail" in the exposed fish. The findings of this study strongly indicated that lead nitrate, even at sub-lethal levels, could significantly alter the overall physiology of the fish. Thin films of Bismuth Tri-iodide BiI 3 were synthesized on a glass substrate by the thermal vacuum deposition technique. Films were exposed to gamma radiations of two doses 10 Gy and 50 Gy. Structural and optical properties of films were studied through X-ray Diffraction and UV spectroscopy both before and after the gamma irradiation. The effect of gamma radiations on microstructure parameters i.e. crystallite size and the micro-strain were studied qualitatively and quantitively. The observed increase in crystallite size and decrease in micro-strain is due to localized heating of crystallites which results in the merging of small crystallites and their assembly leading to the formation of larger crystallites as evident from X-ray Diffraction after gamma irradiation. Effect on the optical constants namely refractive index and extinction coefficient for different doses was also enumerated to throw light on the change in optical density of this material on gamma irradiation. It is observed that the refractive index n increases with an increase in the doses of gamma irradiation. The increase in optical density could be attributed either to the displacement of atoms from lattice sites to the interstitial sites or to the self-assembly behavior of the crystallites on irradiation. The absorption coefficient follows Urbach's empirical relation and exhibits exponential dependency on photon energy close to the band edge. Chronic Limb Threatening Ischemia (CLTI) is a challenging clinical problem associated with high morbidity and mortality. Endovascular interventions have been the cornerstone of treatment whenever possible. It is estimated that CLTI represents < 10% of all Peripheral Artery Disease patients, yet 50% of the patients end up either with a major amputation of the lower limbs or die of cardiovascular causes within one year period, especially in those with unsuccessful revascularization or "no-option" CLTI. Cell-based therapeutics, especially bone marrow-derived mesenchymal stromal cells have emerged as a potential, promising, and novel alternate therapeutic modality in the management of CLTI, bolstered with positive results in numerous research, including randomized and nonrandomized trials. REGENACIP ® is one such BM-MSC therapy approved by Central Drugs Standard Control Organization in India for the management of "no-option" Atherosclerotic Peripheral Arterial disease / Buerger's disease patients with established critical limb ischemia in Rutherford Grade III-5 or III-6, not eligible for or have failed traditional revascularization treatment, with rest pain and / or ulcers in the affected limb. Drugs with lower permeability and water solubility provide major challenges for producing safe and efficient formulations. The current work aims to prepare ICs of the drug phenothiazine and β-cyclodextrin via physical, microwave, freeze-drying, and kneading methods. Many analytical methods, such as 1 H NMR, ROESY, FT-IR, DSC, SEM, and XRD, were then used to confirm the formation of inclusion complexes. The natural polysaccharide-based hydrogel comprising pectin and pullulan was synthesized in air and optimized through various parameters. In order to maximize the reaction parameters, Response Surface Methodology design was employed for experimental optimization. We use FT-IR, TGA, SEM, EDX, and XRD to investigate hydrogel formation. At 37 °C, an investigation was carried out on the in vitro controlled release of PN at pH 2, 7, and 7.4. The analysis of drug release data revealed that PM and KM exhibited an initial burst release of drugs, with the MW and FD method proving to be the most suitable approach for achieving precise ICs of PN and β-CD for sustained drug release.
