Prognosis in relapsed metastatic head and neck squamous cell cancer (RM-HNSCC) is dismal. Platinum based chemotherapy in combination with Cetuximab is used in first-line setting, while no further validated options are available at progression. Immunotherapy has produced durable clinical benefit in some patients with RM-HNSCC although the premises are several patients are nonresponders. Studies are ongoing to determine predictive factors and the ideal setting/combination of novel immunotherapies. In this paper, we discuss the past and present of immunotherapy in head and neck cancer and provide an up-to-date information regarding the potential ways to improve immunotherapy outcomes in HNSCC.

This article starts from a personal vision according to which any adolescent could develop personally and socially, anywhere and anytime, if he had an environment with offering learning experiences, he would acquire development tools, which he could use throughout life and would benefit from the support of the main educational partners (family, school, community) in his training. Starting from this vision, between December 2018 and June 2019, we initiated and carried out an exploratory study, within the Department of Educational Sciences, at the Faculty of Psychology and Educational Sciences of the "Babeş-Bolyai" University, Cluj-Napoca under coordination of the University Professor Vasile Chiş, who aimed to identify the educational needs of adolescents in Romania, for the design and implementation of educational programs, for their personal and social development. In the study, we asked teenagers to fill an online questionnaire, in which they were encouraged to answer questions openly and honestly, the answers being anonymous and the lack of answer variants being considered to be correct or wrong. Over 750 respondents provided answers to the questions “What would you like to learn if you could choose” and “What would you most like to do in life, if you had the time, energy, money, self-confidence and support of your family?". The results of the study open the discussion on the need to approach education from a multilateral and personalized perspective, depending on the individual needs of the beneficiaries. In the context in which education for well-being begins in the first years of family life, continues in school, can be extended to non-governmental organizations and is supported by the community and businesses throughout life, this work reflects the current needs of education, anchored to the future aspirations of adolescents in Romania. Based on the results of this study can be outlined a portrait of the Romanian adolescent, ideas for personalizing education and solutions for educational agents to design and implement programs and projects adapted to the educational needs of today's adolescents, tomorrow's adults.

The article discusses the issues of logical inference for abnormal truth-values of premises in logics with vector semantics from the VTF-logic class. Such situations as strict lies, uncertainty, and complete contradiction are considered in this article. It is shown that the truth of the conclusion in this case can take an interval value and the nature of this interval is preserved throughout the inference. This feature can be used to detect knowledge base artefacts during dynamic verification of knowledge bases.

BackgroundAge is considered as one of the most important risk-factor for many types of solid and hematological cancers, as their incidence increases with age in parallel to the ever-growing elderly population. Moreover, cancer incidence is constantly increasing as a consequence of the increase in life expectancy that favors the process of cellular senescence. Geriatric assessment has been increasingly recognized as predictive and prognostic instrument to detect frailty in older adults with cancer. In particular, the G8 score is a simple and reproducible instrument to identify elderly patients who should undergo full geriatric evaluation. Due to their frailty, elderly patients may be often under-treated and a therapeutic choice based also on a comprehensive geriatric assessment (CGA) is recommended. With these premises, we aim to test the impact of the CGA based interventions on the quality of life (QoL) of frail elderly onco-hematological patients, identified by the G8 screening, candidate for innovative target directed drugs or treatments including the combination of radiotherapy and chemotherapy (RT + CT).MethodsPatients aged > 65 years, candidate to target directed agents or to RT + CT treatments are screened for frailty by the G8 test; those patients classified as frail (G8 ≤ 14) are randomized to receive a CGA at baseline or to conventional care. The primary endpoint is QoL, assessed by EORTC QLQ-C30C. As collateral biological study, the potential prognostic/predictive role of T-cell senescence and myeloid derived suppressor cells (MDSC) are evaluated on plasma samples.DiscussionThis trial will contribute to define the impact of CGA on the management of frail elderly onco-hematologic patients candidate to innovative biological drugs or to integrated schedules with the association of RT + CT. Furthermore, the use of plasma samples to assess the potential prognostic value of imbalance of immune-competent cells is expected to contribute to the individualized care of elderly patients, resulting into a fine tuning of the therapeutic strategies.Trial registrationClinicalTrials.gov ID:NCT04478916. registered July 21, 2020 – retrospectively registered.

A method for operational forecasting of fires is proposed that enables the sequential implementation of five procedures. The method development is necessary to predict early fires in premises in order to take measures to prevent them from escalating into an uncontrolled combustion phase ‒ a fire. As a result of research, it was found that a short-term forecast of the recurrence of increments of the air conditions by one step, based on the current measure of recurrence, is an effective indicator of early fires in premises. At the same time, it was found that before the moment of ignition of the material, the state of the air environment is characterized by dynamic stability, which is described by an irregular and time-dependent random change in the recurrence of the states of the vector of current increments of the state of the air environment. The values of the indicated levels of recurrence of the state increments are determined by the probability levels of 0.67 and 0.1, respectively. The probability of recurrence of state increments of 0.67 is characteristic of a larger number of measured states. When the material is ignited, the dynamics of the probability of recurrence of state increments change abruptly. There is a transition from two to one level of recurrence, close to zero probability ‒ the loss of dynamic stability (in the region of count 250). Further dynamics are characterized by the appearance of separate random recurrent increments corresponding to the instability of the air environment in the premises. In the course of the experiment, it was found that the accuracy of predicting a fire by the proposed method ranges from 4.48 % to 12.79 %, which generally indicates its efficiency. The obtained data prove useful in the development of new systems that early warn of fire in premises, as well as in the modernization of existing systems and means of fire protection of premises

Pediatric cancers are not the equivalent of adult cancers occurring at a younger age and the prospect of immunotherapy in children has not been received with the same enthusiasm as in the adult setting. Although most pediatric malignancies are considered immunologically cold, we are learning more about PD-L1 expression, tumor mutational burden, and microsatellite instability in several pediatric cancers. The side effects of immunotherapy are an important consideration. Immune checkpoint inhibitors (ICIs) engender a unique constellation of inflammatory toxicities known as immune-related adverse events (irAEs). Three early-phase trials—KEYNOTE-051, iMATRIX, and ADVL1412—were the first to describe irAEs in pediatric patients and ICIs were well tolerated. There was concern about unknown late irAEs in pediatric patients, as they have much more time to develop than in adult or elderly patients. Academic clinicians, biopharmaceutical companies, and parents’ advocates concluded that no benefit could be expected from further monotherapy trials employing other ICIs with the same mechanism of action until more scientific knowledge becomes available. On the other hand, ICIs could be useful in combination with other therapies to prevent the functional inactivation of several pathways in the hostile microenvironment. Future clinical studies on ICIs in children need to build on strong biological premises, taking into account the distinctive immunobiology of pediatric cancers vis-à-vis ICI-responsive adult cancers. We need to gain and share experiences of new therapies for managing pediatric patients with cancer, clarifying to what extent we can extrapolate the data obtained in adults.

Background Cryptosporidium can be an important human health risk, predominantly causing gastroenteritis. With increased public attendance at commercial and open farms, there is a need to improve the understanding of Cryptosporidium risk on premises that are visited by the public. Methods This study was designed to explore the animal premises-related and animal sampling-related data routinely collected, during 2009-2019, from human outbreak sampling investigations where animal contact was suggested as a source of Cryptosporidium. Results The results from the 23 eligible investigations indicated a diverse population of animals on the premises and that sheep and cattle, including bottle feeding, were frequently identified as contacts made by the human cases on these premises. Faecal samples from cattle and sheep were found to have a relatively high proportion of positive results and frequently matched the Cryptosporidium species and strain identified in the outbreak cases. Generally, investigations where no positive samples were detected had fewer samples collected. Conclusion The findings support the advice to prioritise sampling of groups of animals which have been identified as being contacted by the human cases, and to use statistically valid sample size calculations for the number of samples to collect at each investigation.

Background School injuries are recognized as a preventable public health problem. No study has been conducted to find the incidence of school injuries in Sri Lankan schools up to date. Methods A sample of 820 students were recruited using multi-stage cluster sampling from an education division in Kandy. A well-structured, pre-tested, interviewer-administered questionnaire was used to gather information on unintentional school injuries that took place in the preceding 1 month. Injury severity was assessed using the Abbreviated Injury Score (AIS). The statistical significance of the associations was tested using the chi-square test. Results The event-based injury incidence rate was 25.37 per 100 students per month (95% CI: 22.04–29.06). A majority (31.8%) of the injury events occurred during a sports event. Some contributory factors identified were overexertion (32.2%), starvation (19.7%), and lack of maintenance of the premises (15.4%). Common injury types were abrasions (33.5%) and lacerations (14.7%). Lower extremities (50.7%) were affected most. The majority of injuries (94.7%) fell to AIS 1, whilst 5.3% fell to AIS 2. Being a male (χ2=22.6, p Conclusions & Learning Outcomes Event-based school injury incidence is very high in the study area. Recommendations can be made to adopt safety measures during sports, proper maintenance of the school premises and improvement of first aid facilities.

IntroductionAssessment of response to therapy in paediatric patients with Hodgkin lymphoma (HL) by 18F-fluorodeoxyglucose positron emission tomography/CT has become a powerful tool for the discrimination of responders from non-responders. The addition of volumetric and texture analyses can be regarded as a valuable help for disease prognostication and biological characterisation. Based on these premises, the Hodgkin Lymphoma Study Group of the Associazione Italiana Ematologia Oncologia Pediatrica (AIEOP) has designed a prospective evaluation of volumetric and texture analysis in the Italian cohort of patients enrolled in the EuroNet-PHL-C2.Methods and analysisThe primary objective is to compare volumetric assessment in patiens with HL at baseline and during the course of therapy with standard visual and semiquantitative analyses. The secondary objective is to identify the impact of volumetric and texture analysis on bulky masses. The tertiary objective is to determine the additional value of multiparametric assessment in patients having a partial response on morphological imaging.The overall cohort of the study is expected to be round 400–500 patients, with approximately half presenting with bulky masses. All PET scans of the Italian cohort will be analysed for volumetric assessment, comprising metabolic tumour volume and total lesion glycolysis at baseline and during the course of therapy. A dedicated software will delineate semiautomatically contours using different threshold methods, and the impact of each segmentation techniques will be evaluated. Bulky will be defined on contiguous lymph node masses ≥200 mL on CT/MRI. All bulky masses will be outlined and analysed by the same software to provide textural features. Morphological assessment will be based on RECIL 2017 for response definition.Ethics and disseminationThe current study has been ethically approved (AIFA/SC/P/27087 approved 09/03/2018; EudraCT 2012-004053-88, EM-04). The results of the different analyses performed during and after study completion the will be actively disseminated through peer-reviewed journals, conference presentations, social media, print media and internet.

The study of cultures involves researching the concepts of beliefs and norms in order to reveal the content of the rules that coordinate human behavior, a task often pursued through an approach based on psycho-socio-cultural premises (PSCP; Díaz-Guerrero, 2002). The basis of the ethnopsychology of Mexicans is formed by the original premises of the family (Díaz-Guerrero, 1994), which lack norms or beliefs to regulate the behavior of couples, the current gender situation, or aspects of postmodern life. The goal of the present study was to investigate PSCPs in a manner that would include norms and beliefs. In order to do this, items were taken from 4 inventories and administered to a non probabilistic purposive sample of 1624 men and women of various educational levels, living in 6 different regions of Mexico. The analysis of variance and the correlation analysis (r) conducted reveal the predominance of sexism, machismo and marianismo, which are indicative of a stronger attachment to the culture in people belonging to more traditional ecosystems, basically people with lower education and males. The efficiency of systems for maintaining comfortable conditions in the room with the use of mathematical modeling was evaluated. The comparative analysis of the technical and economic analysis of efficiency of various heating systems is resulted and the most economic effect from use of ceramic electric heaters is defined. The modeling of the ceramic electric heater and temperature and heat fluxes of the electric heater itself is carried out. [Resumen] El presente Trabajo Fin de Grado tiene como objeto realizar la adecuación de un local, sin acondicionar, en uno de pública concurrencia destinado a restaurante, en la provincia de A Coruña. Se pretende con este trabajo dar solución a las necesidades de un establecimiento de este tipo, cumpliendo con la normativa vigente actualmente. El trabajo se compone de cinco tomos: TOMO I: MEMORIA. ObjectiveTo understand more about the relationship between economic deprivation, types of premises, food hygiene scores and rates of gastrointestinal illness in the UK.DesignData were extracted from the UK Food Standards Agency for about 300 000 UK premises which had hygiene scores based on visits from local authority food safety officers. These scores were analysed by type of premises, deprivation and local authority. Local authority-level average scores were mapped and compared with rates of laboratory-detected gastrointestinal illness from the Health Protection Agency.SettingUK.SubjectsUK premises (n 311 458) from 341 local authority areas that sell or produce food.ResultsThere was a modest but statistically significant relationship between average food hygiene score and deprivation, which was caused by deprived areas having more of the categories of premises with significantly lower hygiene scores; these were pub/club (n 40 525), restaurant/café/canteen (n 73 052), small retailer (n 42 932) and takeaway (n 36 708). No relationship was established between local authority average food hygiene scores and rates of laboratory-detected gastrointestinal illness; however, this result does not preclude a relationship between food hygiene and rates of gastrointestinal illnesses, as laboratory-detected illness rates make up only a small proportion of actual rates of illness in the community.ConclusionsCertain types of UK premises are more likely to have low hygiene scores, which means that they should be targeted more for enforcement. This paper is providing an overview of global economy, highlighting following aspects: premises of globalization process in economy, their effects and subsequent reactions which occur, for a proper management of adverse effects. Our approach facilitates understanding of globalization process that begins to be part of our life. We would like to describe globalization, as a phenomenon capable to determine pulse of national economic activities. Globalization is an important issue and the aim of our paper is to describe globalization as a whole, with its lights and its shadows. According to this dual description, in this way, becoming a known process, "survival" ways can be discovered. Globalization is irreversible and we have to "assimilate" it as a model of thinking and to manage it in our own interest, individually or nationally, and before considering it a good or bad intruder in our lives, we must know it. Acute Kidney Injury (AKI) is a frequent complication in critically ill patients with Coronavirus disease 2019 (COVID-19), and it has been associated with worse clinical outcomes, especially when Kidney Replacement Therapy (KRT) is required. A condition of hypercoagulability has been frequently reported in COVID-19 patients, and this very fact may complicate KRT management. Sustained Low Efficiency Dialysis (SLED) is a hybrid dialysis modality increasingly used in critically ill patients since it allows to maintain acceptable hemodynamic stability and to overcome the increased clotting risk of the extracorporeal circuit, especially when Regional Citrate Anticoagulation (RCA) protocols are applied. Notably, given the mainly diffusive mechanism of solute transport, SLED is associated with lower stress on both hemofilter and blood cells as compared to convective KRT modalities. Finally, RCA, as compared with heparin-based protocols, does not further increase the already high hemorrhagic risk of patients with AKI. Based on these premises, we performed a pilot study on the clinical management of critically ill patients with COVID-19 associated AKI who underwent SLED with a simplified RCA protocol. In most studies, the affiliation of the manager (family-affiliated or non-family affiliated) and supposedly related behavior (agent or steward) is considered the sole antecedent to explain a family business’ (non) professionalization of managerial controls. This paper, based on Luhmann’s new system theory, examines whether a family’s decision premises influence the design of managerial controls in family firms in addition to a manager’s family affiliation status. Using survey data of 135 large and medium-sized Brazilian family firms and testing the hypotheses with SEM, this study provides evidence that a family’s decision premises significantly influence the design of managerial controls in family firms. This study provides evidence that when a family’s intention to transfer the firm to next generation (TGO) is high, more formal controls, as well as controls of a more participative nature are adopted in a family firm. Moreover, the results do not indicate that the level of family involvement in management affects the design of controls in firms with high TGO. The results only showed a significant relationship between a family’s intention to control and influence (FCI) the firm and the absence of participative controls. In addition, these findings also illustrate that each single family-induced decision premise has the potential to explain family firm behavior, since each of the two premises considered in our study is related to a different design of the controls adopted by the family firm.

Smoking has become a comman practice in society today. Various initiatives have been taken to reduce smoker populations in the country but almost all doesn’t work. The introduction and imposition of a ban on smoking in public spaces especially on food premises introduced by the Ministry of Health Malaysia have received positive and negative responses from consumers. This study was conducted to determine the level of consumers’ awareness of the implications of smoking on health; and the level of consumers’ acceptance of smoking ban instructions on food premises. At the same time, the study also aimed at determining the food premis owners’ perceptions of no smoking instructions at food premises. A total of 500 consumers at the food premises and 50 food premis owner were selected as a sample. Findings indicate that the level of knowledge about the implications of smoking on health was high (mean = 3.54) and that respondents’ level of acceptance of smoking ban instructions was also high (mean = 3.13). The analysis also showed that there was a significant relationship between the level of respondents’ knowledge of the implications of smoking and the prohibition of smoking imposed on food premises with a value of r = 0.728 at a significant level p = 0.000 level. Meanwhile, most food premis owners say that smoking bans on food premises do not affect their business performance, and Pearson’s correlation findings also indicate that there is a weak relationship between smoking bans and business performance.

The purpose of the work - analysis of the efficiency of the heating system of the classroom with the use of ceramic electric heaters of the building № 22 NTUU "KPI of Igor Sikorsky ». The results of the literary search of technical and economic preconditions and prospects of introduction of ceramic electric heaters in Ukraine are given. The efficiency of systems for maintaining comfortable conditions in the room with the use of mathematical modeling was evaluated. The comparative analysis of the technical and economic analysis of efficiency of various heating systems is resulted and the most economic effect from use of ceramic electric heaters is defined. The modeling of the ceramic electric heater and temperature and heat fluxes of the electric heater itself is carried out. The obtained results can be offered to increase the level of energy efficiency of the building № 22 NTUU "KPI of Igor Sikorsky ».
