BackgroundThe coronavirus disease-2019 (COVID-19) pandemic significantly affected management of cardiovascular disease around the world. The effect of the pandemic on volume of cardiovascular diagnostic procedures is not known.ObjectivesThis study sought to evaluate the effects of the early phase of the COVID-19 pandemic on cardiovascular diagnostic procedures and safety practices in Asia.MethodsThe International Atomic Energy Agency conducted a worldwide survey to assess changes in cardiovascular procedure volume and safety practices caused by COVID-19. Testing volumes were reported for March 2020 and April 2020 and were compared to those from March 2019. Data from 180 centers across 33 Asian countries were grouped into 4 subregions for comparison.ResultsProcedure volumes decreased by 47% from March 2019 to March 2020, showing recovery from March 2020 to April 2020 in Eastern Asia, particularly in China. The majority of centers cancelled outpatient activities and increased time per study. Practice changes included implementing physical distancing and restricting visitors. Although COVID testing was not commonly performed, it was conducted in one-third of facilities in Eastern Asia. The most severe reductions in procedure volumes were observed in lower-income countries, where volumes decreased 81% from March 2019 to April2020.ConclusionsThe COVID-19 pandemic in Asia caused significant reductions in cardiovascular diagnostic procedures, particularly in low-income countries. Further studies on effects of COVID-19 on cardiovascular outcomes and changes in care delivery are warranted.

The speed-accuracy tradeoff suggests that responses generated under time constraints will be less accurate. While it has undergone extensive experimental verification, it is less clear whether it applies in settings where time pressures are not being experimentally manipulated (but where respondents still vary in their utilization of time). Using a large corpus of 29 response time datasets containing data from cognitive tasks without experimental manipulation of time pressure, we probe whether the speed-accuracy tradeoff holds across a variety of tasks using idiosyncratic within-person variation in speed. We find inconsistent relationships between marginal increases in time spent responding and accuracy; in many cases, marginal increases in time do not predict increases in accuracy. However, we do observe time pressures (in the form of time limits) to consistently reduce accuracy and for rapid responses to typically show the anticipated relationship (i.e., they are more accurate if they are slower). We also consider analysis of items and individuals. We find substantial variation in the item-level associations between speed and accuracy. On the person side, respondents who exhibit more within-person variation in response speed are typically of lower ability. Finally, we consider the predictive power of a person's response time in predicting out-of-sample responses; it is generally a weak predictor. Collectively, our findings suggest the speed-accuracy tradeoff may be limited as a conceptual model in its application in non-experimental settings and, more generally, offer empirical results and an analytic approach that will be useful as more response time data is collected.

Chronic psychosocial stress is increasingly being recognised as a risk factor for sporadic Alzheimer's disease (AD). The hypothalamic-pituitary-adrenal axis (HPA axis) is the major stress response pathway in the body and tightly regulates the production of cortisol, a glucocorticoid hormone. Dysregulation of the HPA axis and increased levels of cortisol are commonly found in AD patients and make a major contribution to the disease process. The underlying mechanisms remain poorly understood. In addition, within the general population there are interindividual differences in sensitivities to glucocorticoid and stress responses, which are thought to be due to a combination of genetic and environmental factors. These differences could ultimately impact an individuals' risk of AD. The purpose of this review is first to summarise the literature describing environmental and genetic factors that can impact an individual's HPA axis reactivity and function and ultimately AD risk. Secondly, we propose a mechanism by which genetic factors that influence HPA axis reactivity may also impact inflammation, a key driver of neurodegeneration. We hypothesize that these factors can mediate glucocorticoid priming of the immune cells of the brain, microglia, to become pro-inflammatory and promote a neurotoxic environment resulting in neurodegeneration. Understanding the underlying molecular mechanisms and identifying these genetic factors has implications for evaluating stress-related risk/progression to neurodegeneration, informing the success of interventions based on stress management and potential risks associated with the common use of glucocorticoids.

- Abstract
Background Alzheimer’s disease (AD) clinical trials require cognitive test scores that assess change in cognitive function accurately. Here, we propose new composite cognitive test scores to detect earlier stages of AD accurately by using the full neuropsychological testing battery (in ADNI) and a manifold learning dimension reduction technique namely UMAP. Method Data for this study included N=1585 ADNI participants ([492 cognitively normal (CN), 804 mild cognitively impaired (MCI), 289 AD; aged 73.8±7.1; 708 females]; Table 1). Subjects with 3 or more follow‐up sessions were included. Cognitive test scores with more than 60% missing data were excluded. Missing data within included test scores were imputed using the MissForest algorithm. A linear mixed model using all follow‐up data was applied to calculate the random slope (rate of change) and random intercept for each cognitive score and for each subject. The scores and demographic measurements: age, gender, years of education and APOE‐ɛ4 status were used to inform the UMAP. Levels for the output variable were defined as: 1) stable CN, 2) CN who progressed to MCI or probable dementia due to AD, 3) stable MCI, 4) MCI who progressed to dementia AD and 5) dementia due to AD. The model calculated two composite scores. These cognitive stages were predicted using Support Vector Machine (SVM) analysis of both the new composite scores and the traditional clinical rating measures of Clinical Dementia Rating (CDR) and Mini‐Mental State Examination (MMSE). Result Predicting cognitive stages using the proposed composite scores show a highly significant improvement with a 0.981 accuracy and 0.976 reliability (evaluated by Cohen's kappa coefficient), compared to using the combination of CDR and MMSE scores covaried for demographics, which had 0.660 accuracy and 0.567 reliability. Background The development of cognitive endpoints that can accurately assess changes in cognition over short time frames is crucial for clinical trials and research of Alzheimer’s disease (AD). Understanding the changing influence of contributing test scores on composites throughout the disease course provides the opportunity to optimise cognitive composite scores for different stages of AD. Method AIBL participants with declining cognitive performance were included in this study N=1275 [688 cognitively unimpaired (CU), 277 mild cognitively impaired (MCI), 310 AD; aged 73±9; 718 females]). Two cognitive composite scores (Episodic Memory (EM) and PACC) and their component test scores (California Verbal Learning Test‐II Delayed Recall (CVLT‐II DR), Logical Memory Delayed Recall (LMII), Rey Complex Figure Test 30 minute delayed recall (RCFT‐DR) and CVLT‐II DR, LMII, Digit Symbol Substitution Test (DS), MMSE, respectively) were evaluated. We first examined the relationship between each of component tests score for each composite. We then compared the extent to which longitudinal trajectories of each component test score and each cognitive composite score differed at each disease stage. Result CVLT‐II DR contributed the most to the EM composite followed by RCFT‐DR and LMII with the influence remaining unchanged across each disease stage. For PACC, CVLT‐II DR contributed the most to the initial decline, with MMSE and LMII contributing similar amounts and DS contributing the least. CVLT‐II DR contributed substantially to changes in PACC earlier in the disease course but MMSE drove the PACC change in later stages of disease. Initially, both composites follow similar longitudinal trajectories. However, the EM composite reaches a floor not observed for the PACC. Conclusion Understanding the temporal contribution of component tests scores on cognitive composites could provide improved cognitive endpoints tailored to use. - Abstract
Background: The Australian Imaging, Biomarkers and Lifestyle (AIBL) Study commenced in 2006 as a prospective study of 1,112 individuals (768 cognitively normal (CN), 133 with mild cognitive impairment (MCI), and 211 with Alzheimer’s disease dementia (AD)) as an ‘Inception cohort’ who underwent detailed ssessments every 18 months. Over the past decade, an additional 1247 subjects have been added as an ‘Enrichment cohort’ (as of 10 April 2019). Objective: Here we provide an overview of these Inception and Enrichment cohorts of more than 8,500 person-years of investigation. Methods: Participants underwent reassessment every 18 months including comprehensive cognitive testing, neuroimaging (magnetic resonance imaging, MRI; positron emission tomography, PET), biofluid biomarkers and lifestyle evaluations. Results: AIBL has made major contributions to the understanding of the natural history of AD, with cognitive and biological definitions of its three major stages: preclinical, prodromal and clinical. Early deployment of Aβ-amyloid and tau molecular PET imaging and the development of more sensitive and specific blood tests have facilitated the assessment of genetic and environmental factors which affect age at onset and rates of progression. Conclusion: This fifteen-year study provides a large database of highly characterized individuals with longitudinal cognitive, imaging and lifestyle data and biofluid collections, to aid in the development of interventions to delay onset, prevent or treat AD. Harmonization with similar large longitudinal cohort studies is underway to further these aims.

Background: Genetic variation in Spondin-1, specifically rs11023139, has been associated with reduced rates of cognitive decline in individuals with Alzheimer’s disease. Objective: The aim of this study was to assess whether the association was present in cognitively normal older adults. Methods: Longitudinal cognitive decline was investigated using linear mixed modelling in a cohort of 590 cognitively normal older adults enrolled in the Australian Imaging, Biomarkers and Lifestyle Study. Results: No independent effect of Spondin-1 rs11023139 on cognitive decline was observed. However, significant associations were observed for the interaction between Apolipoprotein E (APOE) ɛ4 and rs11023139 in individuals with high amyloid-β burden. APOE ɛ4/rs11023139-A carriers declined significantly faster than APOE ɛ4/rs11023139-G_G carriers in measures of global cognition (p = 0.011) and verbal episodic memory (p = 0.020). Conclusion: These results suggest that carriage of the Spondin-1 rs11023139-A allele significantly contributes to a worsening of cognitive performance in APOE ɛ4 cognitively normal older adults with a high neocortical amyloid-β burden.

Item does not contain fulltext, The idea that the sheer exposure to intensive price competition boosts the competitiveness of entire economies and increases social welfare and economic growth has cast a spell on politicians and academics alike, particularly since the ascendancy of neoliberal ideas from the 1980s onwards. The proliferation of competition regimes around the world is testimony to the prevailing idea that capitalist competition is worthwhile to be safeguarded and fostered through state regulation. This chapter traces the origins and evolution of US and European competition laws and enforcement practices against the backdrop of capitalist development and the rise of neoliberalism. Adopting a critical political economy perspective, the chapter critiques the atomistic and reductionist social scientific precepts that serve to legitimize the neoliberal type of competition regulation, and argues that prioritization of price competition ultimately deflates labour. It will be argued that despite the growing critique on competition rules in the wake of the 2008 crisis, and a climate of renewed protectionism and lingering trade wars, as well as more recently, the emergency rescue packages in response to the crisis triggered by the COVID-19 pandemic, EU and US competition laws continue to be immersed with an orthodox neoliberal orientation.

Age-related decrease in testosterone levels is a potential risk factor for cognitive decline in older men. However, observational studies and clinical trials have reported inconsistent results on the effects of testosterone on individual cognitive domains. Null findings may be attributed to factors that studies have yet to consider. In particular, individual variations in polyglutamine (CAG) length in the androgen receptor (AR) gene could alter androgenic activity in brain regions associated with cognitive processes including memory and executive functions. However, the role of AR CAG repeat length as a moderator of the relationship between testosterone levels and cognition has not been investigated. Therefore, we aimed to examine the relationship between baseline calculated free testosterone (cFT) levels, change in cFT levels over 18 months and CAG repeat length on cognitive performance in memory, executive function, language, attention and processing speed domains. These relationships were examined in 304 cognitively normal older male participants of the Australian Imaging, Biomarkers and Lifestyle (AIBL) Study of Ageing. In the attention and processing speed domain, a short CAG repeat length appears to exacerbate the effects of low baseline cFT levels that are also lower than expected at follow-up. These results highlight that individual variations in AR CAG repeat length should be considered in future studies and clinical trials that examine the complex relationship between testosterone and cognition.

- Abstract
Massive black holes at the centers of galaxies can launch powerful wide-angle winds that, if sustained over time, can unbind the gas from the stellar bulges of galaxies. These winds may be responsible for the observed scaling relation between the masses of the central black holes and the velocity dispersion of stars in galactic bulges. Propagating through the galaxy, the wind should interact with the interstellar medium creating a strong shock, similar to those observed in supernovae explosions, which is able to accelerate charged particles to high energies. In this work we use data from the Fermi Large Area Telescope to search for the gamma-ray emission from galaxies with an ultrafast outflow (UFO): a fast (v similar to 0.1 c), highly ionized outflow, detected in absorption at hard X-rays in several nearby active galactic nuclei (AGN). Adopting a sensitive stacking analysis we are able to detect the average gamma-ray emission from these galaxies and exclude that it is due to processes other than UFOs. Moreover, our analysis shows that the gamma-ray luminosity scales with the AGN bolometric luminosity and that these outflows transfer similar to 0.04% of their mechanical power to gamma-rays. Interpreting the observed gamma-ray emission as produced by cosmic rays (CRs) accelerated at the shock front, we find that the gamma-ray emission may attest to the onset of the wind-host interaction and that these outflows can energize charged particles up to the transition region between galactic and extragalactic CRs.

- Abstract
We present the first Fermi-Large Area Telescope (LAT) solar flare catalog covering the 24th solar cycle. This catalog contains 45 Fermi-LAT solar flares (FLSFs) with emission in the gamma-ray energy band (30 MeV-10 GeV) detected with a significance of >= 5 sigma over the years 2010-2018. A subsample containing 37 of these flares exhibits delayed emission beyond the prompt-impulsive hard X-ray phase, with 21 flares showing delayed emission lasting more than two hours. No prompt-impulsive emission is detected in four of these flares. We also present in this catalog observations of GeV emission from three flares originating from active regions located behind the limb of the visible solar disk. We report the lightcurves, spectra, best proton index, and localization (when possible) for all FLSFs. The gamma-ray spectra are consistent with the decay of pions produced by >300 MeV protons. This work contains the largest sample of high-energy gamma-ray flares ever reported and provides a unique opportunity to perform population studies on the different phases of the flare and thus allowing a new window in solar physics to be opened.

- Abstract
Recent observations made with the High Altitude Water Cherenkov (HAWC) telescope of pulsar wind nebula (PWN) suggest that the diffusion in their vicinity is characterised with a lower diffusion constant than that predicted for propagation in the interstellar medium from observations of cosmic-ray (CR) fluxes at Earth. In this contribution, it is shown that models with the slow diffusion region localised about PWN can successfully explain the HAWC observations of the Geminga PWN and still retain consistency with other CR measurements. Parameter exploration shows that the size of the smaller diffusion zone has implications for the both the PWN emission at lower energies observable by the Fermi Large Area Telescope and the predicted positron flux at the Earth. Unless the Geminga PWN is unique, there are likely many small regions with slow diffusion throughout the Milky Way. - Abstract
Background: The coronavirus disease-2019 (COVID-19) pandemic significantly affected management of cardiovascular disease around the world. The effect of the pandemic on volume of cardiovascular diagnostic procedures is not known. Objectives: This study sought to evaluate the effects of the early phase of the COVID-19 pandemic on cardiovascular diagnostic procedures and safety practices in Asia. Methods: The International Atomic Energy Agency conducted a worldwide survey to assess changes in cardiovascular procedure volume and safety practices caused by COVID-19. Testing volumes were reported for March 2020 and April 2020 and were compared to those from March 2019. Data from 180 centers across 33 Asian countries were grouped into 4 subregions for comparison. Results: Procedure volumes decreased by 47% from March 2019 to March 2020, showing recovery from March 2020 to April 2020 in Eastern Asia, particularly in China. The majority of centers cancelled outpatient activities and increased time per study. Practice changes included implementing physical distancing and restricting visitors. Although COVID testing was not commonly performed, it was conducted in one-third of facilities in Eastern Asia. The most severe reductions in procedure volumes were observed in lower-income countries, where volumes decreased 81% from March 2019 to April 2020. Conclusions: The COVID-19 pandemic in Asia caused significant reductions in cardiovascular diagnostic procedures, particularly in low-income countries. Further studies on effects of COVID-19 on cardiovascular outcomes and changes in care delivery are warranted.

The HAWC Collaboration released the 2HWC catalog of TeV sources, in which 19 show no association with any known high-energy (HE; E > 10 GeV) or very-high-energy (VHE; E > 300 GeV) sources. This catalog motivated follow-up studies by both the MAGIC and Fermi-LAT observatories with the aim of investigating gamma-ray emission over a broad energy band. In this paper, we report the results from the first joint work between HAWC, MAGIC and Fermi-LAT on three unassociated HAWC sources: 2HWC J2006+341, 2HWC J1907+084* and 2HWC J1852+013*. Although no significant detection was found in the HE and VHE regimes, this investigation shows that a minimum 1 degree extension (at 95% confidence level) and harder spectrum in the GeV than the one extrapolated from HAWC results are required in the case of 2HWC J1852+013*, while a simply minimum extension of 0.16 degrees (at 95% confidence level) can already explain the scenario proposed by HAWC for the remaining sources. Moreover, the hypothesis that these sources are pulsar wind nebulae is also investigated in detail., 12 pages, 3 figures. Accepted for publication in MNRAS

- Abstract
Local interstellar spectra (LIS) of primary cosmic ray (CR) nuclei, such as helium, oxygen, and mostly primary carbon are derived for the rigidity range from 10 MV to ∼200 TV using the most recent experimental results combined with the state-of-the-art models for CR propagation in the Galaxy and in the heliosphere. Two propagation packages, GALPROP and HelMod, are combined into a single framework that is used to reproduce direct measurements of CR species at different modulation levels, and at both polarities of the solar magnetic field. The developed iterative maximum-likelihood method uses GALPROP-predicted LIS as input to HelMod, which provides the modulated spectra for specific time periods of the selected experiments for model-data comparison. The interstellar and heliospheric propagation parameters derived in this study are consistent with our prior analyses using the same methodology for propagation of CR protons, helium, antiprotons, and electrons. The resulting LIS accommodate a variety of measurements made in the local interstellar space (Voyager 1) and deep inside the heliosphere at low (ACE/CRIS, HEAO-3) and high energies (PAMELA, AMS-02).
