This retrospective study aims to assess cognitive involvement in pre-AIDS, not drug abuser subjects and to determine whether CD4 status or disease stage best correlates with cognitive changes that may portend development of ADC. 328 cases were analyzed. no differences in psychometric performance in relation to CDC stage were found. instead , patients with CD4<200/ul performed worse overall, with a statistically significant difference for Digit Symbol, Corsi Test, Block Design and HIVDA Scale. even if cognitive decline is not evident in the early phase of HIV infection, CD4 count seems the more sensitive early indicator of cognitive changes adequately pointed out by the HIVDA Scale, which could be considered a useful screening tool for cognitive deficit.

In patients with AIDS, cerebral infection due to cytomegalovirus (CMV) results in two distinct neuropathological patterns: microglial nodular encephalitis (MGNE) and ventriculoencephalitis (VE). In order to identify clinical features to facilitate the differential diagnosis of these two forms of CMV encephalopathy in living patients, we retrospectively reviewed the clinical records of 18 patients with MGNE or VE diagnosed at autopsy. We identified the following clinical features as distinguishing the two encephalopathies: (1) MGNE manifests earlier than VE; (2) the onset of MGNE is acute, whereas the onset of VE is insidious; (3) the onset of MGNE is marked by confusion and delirium, which do not occur in VE; (4) VE is frequently associated with radiculopathy, which is absent in MGNE; and (5) VE is associated with more marked alterations in cerebrospinal fluid (high protein levels and pleocytosis). The early neurological manifestations of MGNE should prompt a search for systemic CMV infection, which may lead to earlier treatment.

viene riportato, per la prima volta, il caso clinico di un paziente affetto da lesione ischemica talamica destra causante un disturbo selettivo a carico della prassia del disegno caratterizzato, dal punto di vista qualitativo, da rimpicciolimento e semplificazione del modello. Tali aspetti sono tipici dell'aprassia costruttiva di tipo frontale piuttosto che di quella secondaria a lesione emisferica destra. la peculiarità di tale caso risiede nella presenza di errori nella produzione del disegno dovuti a deficit di programmazione motoria e non di tipo visuo-spaziale, verosimilmente dovuti al coinvolgimento delle connessioni talamo-frontali, we report , for the first time, the case of a patient affected with a right thalamic ischemic lesion wich has determined a selective damage of the constructional praxia. From a qualitative point of view , the design was smaller and more simple than the model. The above mentioned aspects are typical of a frontal type constructional apraxia, more than a praxic disorder due to right emispheric lesion. The peculiaritu of the present case is therefore the presence of errors due to a motor planning deficit and not to a visuo-spatial one, probably due to the involvement of the thalamo-frontal connections.

A 65-year-old man with IgG lambda multiple myeloma developed severe polyneuropathy with prominent thermal -pain sensory impairment and autonomic failure . although the clinical presentation suggested amyloid neuropathy, nerve biopsy showed the immunohistochemical and ultrastructural features typical of light chain deposition disease (LCDD). A precise morphologic and clinical description of LCDD neuropathy is given for the first time in the present report

The WCST is considered a fiontal lest, mainly suitable tu evaluate aspects of 'problem solving'. The several evaluation criteria are due to the different modalities of the test used in the neuropsychological field. AIM: to consider some modalities of the test in order to define which could offer more informations when applied to two degenerative pathologies: PD and AD. METHODS: The modalities proposed by Grant-Berg(GB), Milner(M), Taylor(T) and Heaton(H) have been chosen. 44 AD pts and 31 PD pts have been enrolled For each patient two control subjects with the same age, have been chosen among 546 sbjs.(S) enrolled to obtain the WCST's normal values. The parameters(prt) St(stages), Rc( corrected answers). El( total errors), Ep(perseverative errors), Enp(non perseverative errors), U( 'special' answers) were considered for GB,M and T. In addition to the above mentioned prt, the following were considered for H too. Rp( perseverative answers), %Ep(% of perseverative errors), NTPS( cards to complete the first St). CLR and %CLR ( nc and % of answers of conceptual level), FMS (failure to mantain the set) and I Tl. (learning to learn measure) The statistical analysis requires the study of the frequency distribution, indicator of central tendency, indicator of dispersion, analysis of variance. RESULTS 1)AD vs S: AD performance was statistically worse in the following prt:-St(in all modalities);-Rc(in T and M);-U(in T, M, GB ): -Enp (inH) ;-Et (inT.M andH);-NTPS,CLR,%CLR inH. 2)PD vs S no differences were found in the four modalities. The analysis of the patients divided according to a therapeutic criteria(14 pts with a<1? mnths and 17 pts with a >12 mnths L-dopa therapy) has shown a worse statistically significant performance for the prt St(in T and H) and Rp,Ep,%Ep,%CLR in H in longer therapy pts. DISCUSSION: the GB modality is more applicable in AD and the H more useful in PD.

A screening dementia scale is very useful for: -ADC early diagnosis; -ADC follow-up (in relation to the pharmachological therapy). AIM: to verify the suitability of HIVDA. METHODS: from 1.3 to 31.12 1996 HIV+ pts in all stages of the illness were enrolled satisfying the defined inclusion criteria. These pts underwent a wide and complete neuropsychological evaluation and neurological examination in order to classify them under the following diagnosis. Moderate Dementia (MD) and Severe Dementia (SD) according to the criteria of the AAN .Border-Line(BL) and Normal(N). The pts were then evaluated by another examiner unaware of the previous neuropsychological diagnosis with the HIVDA which is composed of two parts: the anamnestic part (AP1 and AP2) and the psychometric part (PP) HIVDA scale is the result of a step by step implementation from the Blessed scale. RESULTS: 153 pts were enrolled. 68 were classified as N, 24 as BL, 26 as MD, 35 as SD. CD4 number was the only statistically different data among the general characteristics . HIVDA A and P total scores are the following: N BL MD SD HIVDA Ax(sd) 2.80(202) 6.34(4.24) 8.03(4.66) 13.00(4.01)HIVDA P 33.78(314)30.07(2.72)27.66(3.60)19.26(4.28) p<0.0001 HIVDA A (A1 anamnestic, A2 behavioral) and P( information, memory, concentration, frontal functions) subitems show a statistically difference too DISCUSSION HIVDA seems to point out scores that agree with the neuropsychological diagnosis: such scores look quite precise both in BL case and in dementia grading. These results confirm the utility to compute the sensitivity, specificity and predictive values of this scale.

fifthy-eight HIV-positive drug abusers and 22 HIV-positive nondrug abusers at stages II-III and IV of the Centers for Disease Control classification were evaluated neuropsychologically. The study confirmed previous finding that drug abuse has a negative influence on cognitive function. It also emerges that seropositivity affects cognitive function, althoug the poor performance of group II-III patients compared to group IV may be explained by factors related to seropositivity (anxiety and panic) rather than the disease itself. It is concluded that disease-related factors probably determine cognitive performance in the earlier stages of HIV infection.

Scopo dello studio è identificare nei soggetti con infezione da HIV non affetti da AIDS la presenza o meno di disturbi neuropsicologici, i parametri indicativi di gravità di malattia (conta dei linfociti CD4 o presenza di sintomatologia) associati a deficit cognitivi, i fattori maggiormente implicati in una deflessione della performance nei test psicometrici. Lo studio è stato condotto in 20 centri italiani di Malattie infettive da medici precedentemente addestrati sulle metodiche di somministrazione e di valutazione di una breve batteria neuropsicologica. i 503 soggetti arruolati sono stati suddivisi in base ai livelli ematici di CD4 (<-200/ul; 201-500/ul; >500/ul) e alla presenza di sintomatologia (suddivisi in : gruppo1=soggetti asintomatici senza linfoadenopatia, gruppo2=soggetti con linfoadenopatia e gruppo3= soggetti con sintomi senza AIDS). é stata evidenziata una deflessione, che non ha mai raggiunto i valori patologici , di 4 test psicometrici nel gruppo a più basso livello di CD4, e di un solo test nel gruppo di soggetti sintomatici. nell'analisi multivariata, il livello ematico dei linfociti CD4 inoltre risulta l'unico fattore tra quelli considerati (presenza di sintomi, fattore di rischio per infezione da HIV, livello di linfociti CD4) correlabile con l'esito dei test psicometrici. lo studio sembra evidenziare nelle fasi pre-AIDS l'assenza di compromissioni cognitive di rilievo; tuttavia , con livelli di CD4 <200/ul, si possono riscontrare deflessioni psicometriche che rendono necessaria una valutazione neuropsicologica di screening ed una osservazione longitudinale.

L'AIDS-dementia complex(ADC) rappresenta la patologia neurologica più frequente in corso di infezione da HIV. Dall'esordio della sindrome da immunodeficienza acquisita ad oggi è stato possibile monitorare il decorso di tale patologia che ha rappresentato nel corso di questi anni diverse modificazioni della sua storia naturale in relazione prevalentemente all'introduzione della terapia antiretrovirale. Molti degli aspetti dell'ADC sono ancora oggetto di studio e allo stato attuale sussistono diverse problematiche legate alla definizione sia del profilo clinico che del profilo neuropsicologico. Il nostro contributo si inserisce a questo livello e pone come duplice obiettivo quello di offrire da una parte una chiarificazione dell'aspetto propriamente clinico dell'ADC e dall'altra una indicazione metodologica per quanto riguarda l'affronto neuropsicologico del paziente HIV positivo in tutte le fasi della malattia. Verranno inoltre citati anche gli aspetti legati all'epidemiologia , all'anatomia patologica non chè alla terapia dell'ADC in funzione essenzialmente di una ulteriore comprensione di questi due obiettivi., AIDS demetia complex (ADC) represents the most common neurological disease of HIV infection. From immunodeficiency syndrome onset to nowadays it has been possible to check the course of this neurological disease, wich has shown during these years several chianges in its natural history, mainly related to antiretroviral therapy introduction. Several aspects of ADC are still studied and at present many problems about the clinical and neuropsychological profile exist. At this level our own contribution has a double purpose , to clarify the ADC clinical features and to offer a methodological guide -line to face, from a neurpsychological point of view, the HIV seropositive patient in every stage of the disease. The ADC epidemiological , pathological and therapeutical aspects will also be discussed to better understand the above mentioned aims.

in campo neuro-AIDS la diagnosi differenziale tra la PML e l'encefalopatia da HIV sembra essere abbastanza definita. tuttavia quando la forma di encefalopatia da virus HIV si manifesta nella variante HIVL (leucoencefalopatia da HIV) molto frequentemente nelle fasi iniziali della malattia neurologica si evidenziano difficoltà di diagnosi differenziale principalmente con la PML. Obiettivi: lo studio si propone di identificare criteri di diagnosi differenziale tra PML e HIVL. Metodi: sono stati valutati i profili neurologici, neuroradiologici e infettivologici di soggetti con diagnosi anatomopatologica di PML e HIVL, senza presenza di coinfezioni cerebrali. Risultati. i soggetti inclusi nello studio sono risultati essere 10 con PML e 9 con HIVL. da un punto di vista clinico la PML ha un esordio subacuto. HIVL sucronico; la PML presenta prevalentemente deficit neurologici di tipo corticale mentre HIVL prevalentemente di tipo sottocorticale. dal punto di vista neuropsicologico la PML presenta deficit prevalentemente fasici, la HIVL deficit mnesici con rallentamento ideo-motorio. radiologicamente la PML presenta una sofferenza focale della sostanza bianca periferica mentre in HIVL vi è un diffuso coinvolgimento della sostanza bianca centrale. la sopravvivenza in PML risulta inferiore rispetto a HIVL. discussione. l'utilizzo dei suddetti criteri clinici nella fase iniziali della patologia cerebrale può costituire un ausilio per un corretto approccio diagnostico ed un tempestivo intervento terapeutico., IN NEURO--AIDS FILED THE DIFFERENTIAL DIAGNOSIS BETWEEN PML AND HIV ENCEPHALOPATHY SEEM QUITE DEFINITE. NEVERTHELESS SOME DIFFICULTIES ARISE, MAINLY AT THE BEGINNING OF THE DISEASE, WHEN HIV ENCEPHALOPATHY MANIFESTS ITSELF AS HIV LEUKOENCEPHALOPATHY (HIVL). THE PRESENT STUDY AIMS TO IDENTIFY SOM E DISTINGUISHING CRITERIA BETWEEN PML AND HIVL. METHODS: SUBJECTS WITH PATHOLOGICALLY PROVED PML OR HIVL WITHOUT CEREBRAL COINFECTIONS WERE NEUROLOGICALLY , NEURORADIOLOGICALLY AND IMMUNOLOGICALLY EVALUATED. RESULTS: 10 PATIENTS WERE AFFECTED BY PML WHILE 9 WERE AFFECTED BY HIVL. FROM A CLINICAL POINT OF VIEW PML ONSET IS SUBACUTE WHILE HIVL ONE IS INSIDIOUS; PML IS CHARACTERIZED BY NEUROLOGICAL SIGNS OF CORTICAL INCOLVEMENT WHILE HIVL NEUROLOGICAL DEFICITS ARE MAINLY SUBCORTICAL. FROM A NEUROPSYCHOLOGICAL POINT OF VIEW PML PRESENTS MAINLY LANGUAGE DEFICITS , WHILE HIVL PATIENTS HAVE MEMORY DEFICITS AND IDEOMOTOR SLOWNESS. CONCERNING NEURO-IMAGING , PML WHITE MATTER LESIONS ARE FOCAL AND AT THE CORTICO-SUBCORTICAL JUNCTION, WHILE HIVL WHITE MATTER INVOLVEMENT IS DIFFUSE AND CENTRALLY CONFINED. EVENTUALLY SURVIVAL IS LOWER IN PML THAN IN HIVL. DISCUSSION:THE ABOVE MENTIONED DISTINGUISHING CLINICAL CRITERIA COULD HELP , AT THE BEGINNING OF THE NEUROLOGICAL DISEASE, TO ACHIEVE A CORRECT DIAGNOSIS AND THEN A TIMELY THERAPEUTICAL APPROACH.

L'identificazione in campo neuro-AIDS di possibilità terapeutiche alternative a quelle farmacologiche può determinare una nuova modalità di cura e di recupero funzionale senza l'implicazione di ulterirori interferenze tossiche. In tal senso l'attenzione per le lesioni focali cerebrali, che costituiscono una discreta parte delle patologie neurologiche HIV correlate , risulta interessante poichè esse si manifestano clinicamente con quadri caratterizzati sia da deficit motori che deficit neuropsicologici di tipo focale suscettibili di trattamento riabilitativo. L'obiettivo dello studio consiste nel valutare la frequenza e la tipologia dei deficit neuropsicologici e le caratteristiche cliniche , immunitarie e di sopravvivenza , al fine di valutare l'opportunità di un approccio riabilitativo. I soggetti che rispondevano ai criteri d'inclusione sono stati sottoposti ad esame neurologico standardizzato e a batteria neuropsicologica . Sono stati selezionati 60 (40%9 soggetti con deficit neuropsicologici focali. Il deficit neuropsicologico più frequente è risultato essere l'afasia, ed in particolare l'afasia di tipo non fluente. Per quanto concerne l'etiolgia , la toxoplasmosi spicca per frequenza. Il confronto tra il tempo medio di sopravvivenza dei soggetti viventi (18 mesi) e deceduti (4 mesi) è risultato significativo. alla luce di tali risultati è possibile affermare che un approccio riabilitativo cognitivo può trovare una sua applicabilità nei soggetti affetti da infezione da HIV.

Allo scopo di verificare se soggetti sieropositivi in fase pre AIDS presentavano alterazioni di tipo neurologico sono stati valutati 132 soggetti suddivisi nei seguenti gruppi: sieropositivi (HIV+) tossicodipendenti (TD+), sieronegativi (HIV-) non tossicodipendenti (TD-), HIV+TD- e HIV-TD+. tutti i soggetti sono stati sottoposti ad esame neurologico standardizzato costituito complessivamente da 73 items valutati con punteggio 0 quando il segno patologico era assente e 1 quando era presente. All'interno dei 4 gruppi è risultata diversa la frequenza di comparsa di 3 parametri: il riflesso del muso, il tremore e la coordinazione. tali parametri si sono evidenziati con maggior frequenza nel gruppo di soggetti HIV+TD+. é stata inoltre utilizzata l'analisi di regressione logistica per valutare l'influenza della sieropositività , della tossicodipendenza e l'assunzione di sostanze alcooliche sui risultati ottenuti. La tossicodipendenza si è rivelata essere l'unico fattore di influenzamento sui risultati ottenuti. Si ribadisce pertanto che nelle fasi pre AIDS le alterazioni neurologiche riscontrate non sono attribuibili al virus HIV ma piuttosto all'uso cronico di sostanze tossiche

Objective:to examine the involvement of cognitive function in HIV-seropositive drug users (DU) in a pre-AIDS state. Design: fifty-six HIV-positive DU were prospectively evaluated. They belongined to groups II, III and IV (subgroups A, C2 and E) of the 1987 Centers for Disease Control and Prevention classification, with anamnesis negative for neurological pathology. HIV-negative DU (n=19) and non-DU (n=27) were used as controls. infection with HIV and use of toxic drugs were considered variables of influence on cognitive function. Method: subjects underwent neuropsychological evaluation by tests designed to explore cortical and subcortical function. results: HIV-positive DU showed worse performace scores at the psychometric tests than HIV-negative non-DU, but there was no difference when compared with HIV-negative DU. Ex-DU showed better performance than active DU. No difference with regard to degree of disease evolution was observed among HIV-positive individuals (i.e., groups II and III versus group IV). Conclusions: there was no evidence of cognitive deficits in Hiv-positive individuals in non-AIDS phases to indicate early involvemente by HIV at the cerebral level. progression of the disease, prior to the AIDS phase, did not determine a worsening of intellectual performance. Instead , cognitive function was affected by the chronic and current use of toxic substances. In HIV-positive DU, a decline in cognitive function was found to be attributable to the chronic use of toxic substances rather than HIV infection.

A soli due anni di distanza dalla segnalazione del primo caso di AIDS si rese evidente che molti pazienti affetti da tale patologica presentavano un quadro di decadimento intellettivo. Divenne presto chiaro che il virus HIV tipo 1 agiva direttamente sul SNC determinando esso stesso un quadro di encefalopatia da HIV (HIVE).Il termine AIDS dementia Complex (ADC) è oggi utilizzato per descrivere tale compromissione cognitiva. In questo articolo di revisione dello stato attuale delle conoscenze intorno al problema vengono descritte brevemente l'epidemiologia, l'ezio-patogenesi, l'anatomia patologica , gli aspetti clinici e diagnostici delll'ADC. Viene riservata una discussione più particolareggiata relativamente alla terapia farmacologica e riabilitativa, quest'ultima rivolta agli aspetti motori, cognitivi e comportamentali.

A case of reversible anterior bilateral opercular syndrome (Foix-Chavany-Marie syndrome) secondary to cerebral toxoplasma abscesses is described in a patient with AIDS. The symptoms regressed following antitoxoplasma and antiedema drug therapy. Although this is the first reported AIDS-related case, the syndrome is likely to recur in AIDS sufferers in whom multifocal cerebral lesions are common.

Twenty-five HIV-seropositive drug abusers (DA+ HIV+) (groups II-III and IV [A, C2 and E] of the CDC classification) were evaluated by use of the WAIS scale to determine any possible involvement of cognitive functions in the not yet overt phases of AIDS. The results were compared with those obtained in two control populations composed of 19 seronegative drug abusers (DA+ HIV-) and 24 healthy subjects (DA- HIV-) to evaluate, in addition to the disease, the possible effect of the use of alcohol and toxic substances on cognitive performance. In spite of the small number of subjects, the study indicated that drug abuse is the main factor, among those analyzed, in determining a decline in cognitive functions.

It is still unknown what effect early HIV infection has on the central nervous system and thus on the mental functions in patients who have not progressed to clinically significant AIDS.

Cognitive impairment was evaluated by means of the Luria-nebraska neuropsychological Battery (LNNB) in 24 drug-free patients with early Alzheimer type dementia (DAT) 24 multinfarct dementia (MD) patients and 24 normal controls. The LNNB clinical scales indicates a lower degree of cognitive impairment and qualitative deficit differences in the MID patientsas compared to the DAT group. the latter showed ore impairment on the all clinical scales. the localisation scale results showed right hemisphere functional deficits for both patient groups. DAT patients, however, had greater impairment of left hemisphere frontal and parietal area functions
