Abstract

One of the most critical but poorly understood processes during cardiovascular development is the establishment of a functioning coronary artery (CA) system. Due to the lack of suitable imaging technologies, it is currently impossible to visualize this complex dynamic process on living human embryos. Furthermore, due to methodological limitations, this intriguing process has not been unveiled in living animal embryos, too. We present here, to the best of our knowledge, the first in vivo images of developing CAs obtained from the hearts of chick embryos grown in shell-less cultures. The in vivo images were generated by optical coherence tomography (OCT). The OCT system used in this study is a mobile fiber-based time-domain real-time OCT system operating with a center wavelength of 1330 nm, an A-scan rate of 4 kHz, and a typical frame rate of 8 frames/s. The axial resolution is 17 mu m (in tissue), and the lateral resolution is 30 mu m. The OCT system is optimized for in vivo chick heart visualization and enables OCT movie recording with 8 frames/s, full-automatic 3D OCT scanning, and blood flow visualization, i.e., Doppler OCT imaging. Using this OCT system, we generated in vivo OCT recordings of chick embryo hearts to study the process of connection of the future right coronary artery (RCA) to the aorta. Recordings were made at three critical stages during development: day 8 (no clear connection yet), day 9 (established connection of RCA with the aorta with clear blood flow) and day 10 (further remodeling of the established RCA).

209. Evaluation of dyspnoea in a sample of elderly subjects recruited from general practice

Abstract

The objectives of this study were to investigate the cause of dyspnoea in a sample of elderly individuals and to assess the diagnostic yield of a three-step examination algorithm for the evaluation of dyspnoea paired with a cost analysis. A total of 152 subjects were examined. A predefined diagnostic approach in three steps was carried out to find the cause of dyspnoea. Step 1 included lung spirometry and ECG; step 2 included lung diffusion capacity, echocardiography, haemoglobin and thyroid function; and step 3 included cardiac magnetic resonance imaging, chest radiography and exercise test. Of 129 subjects with dyspnoea, 68 (53%) had signs of lung disease, 27 (21%) had heart disease, a total of 43 (33%) were obese, 20 (16%) were obese without other causes of dyspnoea and five (4%) had general physical deconditioning. Twelve per cent had none of the above-mentioned potential causes of dyspnoea. Steps 1, 1 + 2 and 1 + 2 + 3 revealed a cause of dyspnoea in 39%, 63%, and 73% of subjects respectively. The cost per diagnosed case at steps 2 and 3 was twice and 3.5 times the cost per diagnosed case at step 1. In this sample of elderly subjects, a potential cause of dyspnoea was identified in most cases, the most frequent being lung disease followed by heart disease and obesity. These data shed light on the diagnostic yield that can be expected from a relatively simple diagnostic approach, including the most frequent recommended initial screening tests. As expected, the incremental nature of this algorithm translated into incremental costs per diagnosis achieved.

Abstract

The present CERN PS low-energy antiproton complex involves 4 machines to collect, cool, decelerate and supply experiments with up to 1010 antiprotons per pulse and per hour of momenta ranging from 0.1 to 2 GeV/c. In view of a possible future physics programme requiring low energy antiprotons, mainly to carry out studies on antihydrogen, a simplified scheme providing at low cost antiprotons at 100 MeV/c has been studied. It requires only one machine, the present Antiproton Collector (AC) converted into a cooler and decelerator (Antiproton Decelerator, AD) and delivering beam to experiments in the hall of the present Antiproton Accumulator Complex (AAC) [1]. This paper describes the feasibility study of such a scheme [2].

Abstract

A three-month prospective surveillance study was undertaken in four dialysis centres to establish the prevalence of Staphylococcus aureus carriage in a Danish population of patients on haemodialysis (HD) or on continuous ambulatory peritoneal dialysis (CAPD). General data such as sex, age, diagnosis, number of months in dialysis, hospital and ward were registered on a precoded form. Standardized nose and four skin swabs (axillae, groins, perineum) were performed on the first day of the survey. After one and two months, nose swabs were collected. Infections were registered and cultures were sent for phage-typing together with the S. aureus strains isolated from the swabs; 59.5% of HD patients and 51.2% of CAPD patients carried S. aureus. Permanent carriage was most frequent (P < 0.00009), primarily in the nose (44.0 and 34.9%, respectively in HD and CAPD). Skin carriage alone was rare (2.4 and 4.7%). Approximately one third (36.6 and 40.7%) of infections were caused by S. aureus. Although diabetics were not significantly more frequent carriers (60.5%) than non-diabetics (55.0%), the incidence of infection was much higher (26.3% vs. 10.3%, P = 0.004). In CAPD, peritonitis and tunnel/exit-site infections predominated (81.4%), often caused by S. aureus (34.8%). More than two thirds of the infections in HD patients were related to intravascular catheterization. The most serious infection was septicaemia, in all cases due to S. aureus. S aureus infections occurred significantly more frequently among carriers (P = 0.005), and more than half the patients were infected by the same or possibly the same strain as they carried in the nose or on skin. Different regimens for the elimination of S. aureus carriage in dialysis patients are discussed. A policy for risk assessment of patients should be developed, and the elimination of S. aureus carriage before dialysis should be encouraged. Controlled trials comparing the cost-effectiveness of recommended regimens to eliminate carriage in HD/CAPD patients are needed. Nose swabs are reliable indicators of carriage in dialysis patients.

Abstract

Udgivelsesdato: 1996-Dec-6 TAL1/SCL is a basic helix-loop-helix (bHLH) oncoprotein that is expressed in several cell lines including many hematolymphoid cells, but not in T- and B-lineage cells. The TAL1 gene was originally discovered as being transcriptionally activated by chromosomal rearrangements in T-cell acute lymphoblastic leukemia (T-ALL). Here we have shown that TAL1 and the ubiquitously expressed murine bHLH transcription factor ALF1 formed heterodimers that, compared with ALF1 homodimers, had a more restricted E-box specificity and bound preferentially to the glucocorticoid-responsive E-box (Egre) motif (AACAGATGGT). Overexpression of the dominant inhibitory HLH protein Id1 in NIH3T3 cells reduced the transcriptional activity mediated by ALF1 homodimers, whereas the transcriptional activity mediated by TAL1/ALF1 heterodimers was resistant to Id overexpression. Our results show that ALF1 may serve as a dimerization partner for the bHLH oncoprotein TAL1 and form a complex with a distinctive DNA binding property. These findings support the hypothesis that the leukemic characteristics of the TAL1 oncoprotein could be mediated by activation of a set of target genes as heterodimeric complexes with ubiquitously expressed bHLH transcription factors such as ALF1 and that a principal role of TAL1 might be to neutralize an Id-mediated inactivation.

Dual X-ray absorptiometry (DXA) performs noninvasive assessment of bone and soft tissue with high precision. However, soft tissue algorithms assume that 73.2% of the lean body mass is water, a potential source of error in fluid retention. We evaluated DXA (model QDR-2000; Hologic Inc, Waltham, MA), bioelectrical impedance analysis (BIA), and simple anthropometry in 19 patients (9 women and 10 men, mean age 46 y) before and after hemodialysis, removing 0.9-4.3 L (x: 2.8L) of ultrafiltrate. Abstract

Optical coherence tomography (OCT) images of basal cell carcinomas (BCCs) have been acquired using a compact handheld probe with an integrated video camera allowing the OCT images to be correlated to a skin surface image. In general the healthy tissue of the skin has an obvious stratified structure, whereas the cancerous tissue shows a more homogenous structure. Thus it was demonstrated that it is possible to distinguish BCCs from healthy tissue by means of OCT. Furthermore different histological types of BCC were identified. Comparison of OCT images taken prior to and immediately after photodynamic therapy clearly shows the tissue response to the treatment, and indicates local oedema in the treated area.
