Abstract

Graft-versus-host disease assessment has been shown to be a challenge for healthcare professionals, leading to the development of the eGVHD App (www.uzleuven.be/egvhd). In this study, we formally evaluated the accuracy of using the App compared to traditional assessment methods to assess graft-versus-host disease. Our national multicenter randomized controlled trial involved seven Belgian transplantation centers and 78 healthcare professionals selected using a two-stage convenience sampling approach between January and April 2017. Using a 1:1 randomization stratified by profession, healthcare professionals were assigned to use either the App ("APP&") or their usual graft-versus-host disease assessment aids ("No APP") to assess the diagnosis and severity score of ten expert-validated clinical vignettes. Our main outcome measure was the difference in accuracy for graft-versus-host disease severity scoring between both groups. The odds of being correct were 6.14 (95% CI: 2.83-13.34) and 6.29 (95% CI: 4.32-9.15) times higher in favor of the "APP" group for diagnosis and scoring, respectively (p<0.001). App-assisted graft-versus-host disease severity scoring was significantly superior for both acute and chronic graft-versus-host disease, with an Odds Ratio of 17.89 and 4.34 respectively (p<0.001) and showed a significantly increased inter-observer agreement compared to standard practice. Despite a mean increase of 24 minutes (95% CI: 20.45-26.97) in time needed to score the whole graft-versus-host disease test package in the"APP" group (p<0.001), usability feedback was positive. The eGVHD App showed superior graft-versus-host disease assessment accuracy compared to standard practice and has the potential to improve the quality of outcome data registration in allogeneic stem cell transplantation.

Abstract

In 2005, the National Institutes of Health (NIH) consensus conference published a series of papers recommending methods to improve the conduct of clinical trials in chronic GVHD. Although the NIH recommendations were primarily aimed at strengthening research, several papers addressed issues relevant for clinical practice, particularly diagnosis, severity scoring, and ancillary and supportive care practices. We conducted an international survey to assess the uptake of these recommendations, identify barriers to greater use and document the use and perceived effectiveness of available treatments. The response rate for the American survey of 1387 practitioners was 21.8%, and it was 24.6% for 407 centers surveyed in Europe, Asia, Australia and Africa. Most respondents were familiar with the NIH consensus recommendations (94-96%) and used them in practice. Multiple barriers to greater use were reported. Besides lack of time (55-62%), unfamiliarity with the recommendations, scarcity of evidence supporting the impact of recommendations on outcomes, insufficient training/experience in chronic GVHD management and inaccessibility of subspecialists were also endorsed. Systemic corticosteroids were reported to be the most effective treatment for chronic GVHD, but many others were perceived to have moderate or great success. Therapeutic management of steroid-refractory chronic GVHD was identified as the highest priority for research.

Abstract

Introduction: Even though considerable effort has been invested into identifying a reliable biomarker for chronic Graft vs Host Disease (cGVHD), thus far the search has not been successful. Glycoprotein YKL40 has been investigated in various conditions and has been found to be a marker of an ongoing inflammation as well as oncogenic transformation. In a cross-sectional study we investigated the level of YKL-40 in patients suffering from cGVHD in reference to transplanted patients without cGVHD (controls). Material (or patients) and methods: The study was conducted in UHC Zagreb from July 2013 to October 2015. A total of 56 transplanted patients were included, 35 cGVHD patients and 21 controls. Median age was 45 years (range 9- 60) in patient group and 40 years (range 16-59) in control group, there were 18 and 10 females and 17 and 11 males, respectively. Median time from transplant to study enrollment was 463 days (range 61-7853) in patient group, and 428 days (range 190-1770) for controls. Median time from cGVHD diagnosis to enrollment was 154 days (range 1-7125). There were 17 and 8 patients that received myeloablative, 18 and 13 received reduced conditioning in patient and control group, respectively. There was no difference between the source of transplanted cells between groups, 15 and 8 patients that received bone marrow, 20 and 13 received PBSC in patient and control group, respectively. Using global NIH score to estimate disease severity 4 patients were considered to have mild, 15 moderate and 16 severe cGVHD. YKL-40 levels were measured by ELISA and compared between cGVHDpatients and controls. We also tested for correlation in reference to disease severity and activity in cGVHD patients (estimated using Global NIH score, Clinicians impression of activity and Intensity of immunosuppression). Results: YKL- 40 levels were found to be significantly higher in cGVHD patients in reference to controls (median 707 (71-4000) and 314 ng/ml (105-2790) respectively, (P = 0.003). Even if patients with myelofibrosis (condition shown to increase the level of YKL-40) were excluded from analysis (total 6 patients, 3 in cGVHD group and 3 controls), the difference between groups remained statistically significant (P = 0.017). Furthermore, YKL-40 levels were found to correlate significantly with disease severity estimated using global NIH score. YKL-40 levels also positively correlated with Clinician’s impression of activity (P = 0.016) but not with Intensity of immunosuppression (P = 0.085). When tested for correlation with CRP as a measure of active inflammation, no statistically significant correlation was found. Conclusion: Although the number of patients included in this cross-sectional study is limited, our results show that YKL-40 glycoprotein might be a simple and inexpensive biomarker of cGVHD and its activity. However, further investigation is warranted with validation in a larger group of patients, as well as longitudinal testing of YKL-40 glycoprotein levels in patients at risk of developing cGVHD.

Abstract

Introduction: Extracorporeal photochemotherapy (ECP), as a safe and non-toxic immunotherapeutic method that is able to mediate patient's immune system without generalized immunosuppression, has been used in our center since 2007. After the establishment of multidiciplinary Center for cGVHD and long- term follow-up at University Hospital Center (UHC) Zagreb in 2013. all patients referred to ECP were scored according to established cGVHD related grading scales and measurements in collaboration with the National Cancer Institute, USA, and patients previously treated with ECP were rescored. Material (or patients) and methods: The aim of the study was to report our 9 year experience with clinical and immunomodulatory effect of ECP, as well as adverse reactions associated with ECP.The influence of the EPC treatment on the levels of T-lymphocyte subsets, B lymphocytes, and NK cells in blood was evaluated. Study was performed throughout the period of 2007-2015 in UHC Zagreb on group of 13 patients (7 male, 6 female) who were treated with off-line ECP. MNC were collected with Cobe Spectra and Optia cell separators. After the addition of psoralen products were irradiated with UVA on Macogenic device. Patients' median age was 32 years (range, 12-69 years). The patients suffered from generalized sclerodermatous skin changes, impaired join mobility and joint pain. In 3 patients the symptoms of oral disease have developed. Peripheral blood samples taken before and after leukapheresis, and samples from leukapheresis bag were analyzed for WBC, Htc, MNC, and platelet counts. Number of T-lymphocyte subsets (CD3+, CD3+4+, CD3+8+, CD4+8+ ratio) B- lymphocytes (CD 19+), and NK cells (CD56+) in patient's peripheral blood were taken monthly. Blood counts and parameters of MNC were measured by means of analyzer Advia 120, Bayer, USA. The levels of B, T lymphocytes and NK cells were evaluated by the use of flow cytometry technique, Becton Dickinson, Facs Calibur, USA. Results: In 13 patients with cGVHD, scored according to NIH classification as 11 severe and 2 moderate, 664 ECP procedures were performed. The median number of ECP procedures performed per patient was 49 (range 12-131). Clinical response to ECP is typically delayed until 2 to 3 months. Overall response, defined as either a complete response (CR) or a partial response according to NIH criteria, was obtained in 8 of 13 patients (61, 5%), and CR, in 4 of 13 (30, 7%). The effect of ECP in patients with skin and joint involvement was mostly benefitcal, as well as in all patients who suffered from the oral disease. At last follow up, 10 patients were alive and well and 3 patients died. In patients who responded well to ECP, CD4+/CD8+ ratio and number of NK cells were normalized. In general, ECPs were well tolerated, and main issue was adequate venous access for long term ECP treatment. No increased incidence of infections and no serious adverse reactions have been observed so far. Conclusion: ECP is safe procedure that may be beneficial in treatment of cGVHD and can be recommended for patients who do not respond to conventional therapy. The specific influence of ECP on T-cell subsets leads to the suggestion that interactions between T-cell subsets may participate in the process of ECP.

234. Glycolytic metabolism of pathogenic T cells enables early detection of GVHD by 13C-MRI

Abstract

Graft-versus-host disease (GVHD) is a prominent barrier to allogeneic hematopoietic stem cell transplantation (AHSCT). Definitive diagnosis of GVHD is invasive, and biopsies of involved tissues pose a high risk of bleeding and infection. T cells are central to GVHD pathogenesis, and our previous studies in a chronic GVHD mouse model showed that alloreactive CD4+ T cells traffic to the target organs ahead of overt symptoms. Because increased glycolysis is an early feature of T-cell activation, we hypothesized that in vivo metabolic imaging of glycolysis would allow noninvasive detection of liver GVHD as activated CD4+ T cells traffic into the organ. Indeed, hyperpolarized 13C-pyruvate magnetic resonance imaging detected high rates of conversion of pyruvate to lactate in the liver ahead of animals becoming symptomatic, but not during subsequent overt chronic GVHD. Concomitantly, CD4+ T effector memory cells, the predominant pathogenic CD4+ T-cell subset, were confirmed to be highly glycolytic by transcriptomic, protein, metabolite, and ex vivo metabolic activity analyses. Preliminary data from single-cell sequencing of circulating T cells in patients undergoing AHSCT also suggested that increased glycolysis may be a feature of incipient acute GVHD. Metabolic imaging is being increasingly used in the clinic and may be useful in the post-AHSCT setting for noninvasive early detection of GVHD.

235. Post-transplant multimorbidity index and quality of life in patients with chronic graft-versus-host disease—results from a joint evaluation of a prospective German multicenter validation trial and a cohort from the National Institutes of Health

Abstract

Comorbidity after allogeneic hematopoietic stem cell transplantation (alloHSCT) impairs quality of life (QoL), physical functioning, and survival. We developed a new standardized measure to capture comorbidity after transplantation, the Post-transplant Multimorbidity Index (PTMI) in a cohort of 50 long term survivors. We subsequently evaluated the content validity and impact on survival and QoL within a multicenter trial, including 208 patients (pts) after alloHSCT, who were prospectively evaluated applying the FACT-BMT, the Human Activity Profile (HAP), the SF-36 v.2, PTMI and the Hematopoietic Cell Transplantation-Comorbidity Index (HCT-CI). The most prevalent comorbidities were compensated arterial hypertension (28.4%), ambulatory infections (25.5%), iron overload (23%), mild renal function impairment (20%), and osteoporosis (13%). Applying the PTMI 13% of patients had no comorbidity, while 37.1% had 1–3 comorbidities, 27.4% had 4–6 comorbidities, and 13.5% had > 6 comorbidities. Chronic graft-versus-host disease (cGvHD) was significantly associated with the PTMI, while age and prior acute GvHD were not. In contrast, the HCT-CI was not associated with the presence of cGvHD. cGvHD was significantly associated with depression (r= 0.16), neurological disease (r= 0.21), osteoporosis (r= 0.18) and nonmelanoma skin cancer (r= 0.26). The PTMI demonstrated strong measurement properties and compared to the HCT-CI captured a wider range of comorbidities associated with cGvHD.

236. Clinical characterization and cytokine profile of fatigue in hematologic malignancy patients with chronic graft-versus-host disease

Abstract

Limited information is available regarding clinical and biological properties of fatigue in patients with chronic graft-versus-host disease (cGvHD). Patients with moderate-to-severe cGvHD per NIH criteria were enrolled on a cross-sectional study and categorized as “fatigued” if SF-36 vitality score was <40. Clinical and laboratory parameters of fatigued (n= 109) and nonfatigued patients (n= 72) were compared. In univariate analysis, walk velocity, NIH joint-fascia score, human activity profile, and SF-36 physical and mental health self-report scales were correlates of fatigue. No cGvHD biomarkers were associated with fatigue. NIH joint score, Lee sleep and depression questions, and PG-SGA activities and function score jointly predicted fatigue. Though higher rates of depression and insomnia were reported in the fatigued group, antidepressant or sleep aid use did not differ between groups. Survival ratio was not significantly different by fatigue status. Pathophysiology of fatigue in patients with cGvHD is complex and may involve mechanisms unrelated to disease activity. Patients with cGvHD experiencing fatigue had higher rates of untreated depression and insomnia, highlighting the need to focus clinical management of these conditions to improve health-related quality of life.

237. Ocular surface indicators and biomarkers in chronic ocular graft-versus-host disease: a prospective cohort study

Abstract

This longitudinal cohort study compared ocular surface indicators in forty allogeneic hematopoietic stem cell transplant (HSCT) subjects with twenty healthy controls at baseline and identified changes in ocular graft-versus-host disease (oGVHD). Outcome measures included: Ocular Surface Disease Index (OSDI), tear osmolarity, Schirmer’s test, Oxford corneal staining score, tear break-up time (TBUT), and tear and serum biomarkers (IFN-γ, IL-10, MMP-9, IL-12, IL-13, IL-17α, IL-1β, IL-2, IL-4, IL-6, IL-8, CXCL10, MCP-1, MIP-1α, RANTES, TNF-α). At baseline the HSCT group had higher median Oxford corneal staining score (1.7 vs. 0.0; P< 0.0001), higher tear TNF-α (20.0 vs. 11.2 pg/mL; P< 0.0001), lower tear RANTES (70.4 vs. 190.2 pg/mL; P< 0.0001), higher serum IL-8 (10.2 vs. 4.5 pg/mL; P= 0.0008), and higher serum TNF-α (8.7 vs. 4.2 pg/mL; P< 0.0001). The incidence of oGVHD was 62% and associated changes included increased Oxford corneal staining score (4.6 vs. 1.8, P= 0.0001), decreased Schirmer’s test (3.0 vs. 10.0; P< 0.0001), and decreased TBUT (4.7 vs. 9.0 s; P= 0.0004). Baseline differences in ocular surface indicators suggest a tendency toward ocular dryness in individuals with hematologic disorders preparing for HSCT. Individuals who developed oGVHD showed changes in corneal staining score, Schirmer’s test, and TBUT.
