202. Switching to a Second Thiopurine in Adult and Elderly Patients With Inflammatory Bowel Disease: A Nationwide Study From the ENEIDA Registry.

- Abstract
Background and Aims Although commonly used in inflammatory bowel disease [IBD], thiopurines frequently cause intolerance, and switching to a second thiopurine has only been reported in some small series. Ours aims in this study were to evaluate the safety of switching to a second thiopurine in a large cohort, and to assess the impact of age on tolerance. Methods Adult IBD patients from the ENEIDA registry, who were switched to a second thiopurine due to adverse events [excluding malignancies and infections], were identified. At the beginning of thiopurine treatment, patients were divided by age into two groups: 18–50 and over 60 years of age. The rate and concordance of adverse events between the first and second thiopurines, treatment intolerance, and persistence with the second thiopurine were evaluated. Results A total of 1278 patients [13% over 60 years of age] were switched to a second thiopurine. At 12 months, the cumulative probability of switch intolerance was 43%, and persistence with treatment was 49%. Independent risk factors of switch intolerance were age over 60 years (odds ratio [OR] 1.49; 95% confidence interval [CI] 1.07–2.07; p = 0.017) , previous gastrointestinal toxicity [OR 1.4; 95% CI 1.11–1.78; p = 0.005], previous acute pancreatitis [OR 6.78; 95% CI 2.55–18.05; p <0.001], and exposure to the first thiopurine <6 months [OR 1.59; 95% CI 1.14–2.23; p = 0.007]. Conclusions In a large series in clinical practice, switching to a second thiopurine proved to be a valid strategy. Tight monitoring of elderly IBD patients switching to a second thiopurine because of adverse events is recommended. [ABSTRACT FROM AUTHOR]

204. Effectiveness and Safety of the Switch from Remicade® to CT-P13 in Patients with Inflammatory Bowel Disease.

- Abstract
Background and Aims To evaluate the clinical outcomes in patients with IBD after switching from Remicade
® to CT-P13 in comparison with patients who maintain Remicade®. Methods Patients under Remicade® who were in clinical remission with standard dosage at study entry were included. The 'switch cohort' [SC] comprised patients who made the switch from Remicade® to CT-P13, and the 'non-switch' cohort [NC] patients remained under Remicade®. Results A total of 476 patients were included: 199 [42%] in the SC and 277 [58%] in the NC. The median follow-up was 18 months in the SC and 23 months in the NC [ p < 0.01]. Twenty-four out of 277 patients relapsed in the NC; the incidence of relapse was 5% per patient-year. The cumulative incidence of relapse was 2% at 6 months and 10% at 24 months in this group. Thirty-eight out of 199 patients relapsed in the SC; the incidence rate of relapse was 14% per patient-year. The cumulative incidence of relapse was 5% at 6 months and 28% at 24 months. In the multivariate analysis, the switch to CT-P13 was associated with a higher risk of relapse (HR = 3.5, 95% confidence interval [CI] = 2–6). Thirteen percent of patients had adverse events in the NC, compared with 6% in the SC [ p < 0.05]. Conclusions Switching from Remicade® to CT-P13 might be associated with a higher risk of clinical relapse, although this fact was not supported in our study by an increase in objective markers of inflammation. The nocebo effect might have influenced this result. Switching from Remicade® to CT-P13 was safe. [ABSTRACT FROM AUTHOR]- Published

206. Pulmonary artery pseudoaneurysm arising from primary lung neoplasm: A proposed mechanism.

- Abstract
Pulmonary artery pseudoaneurysms (PAPs) are rare and life-threatening occurrences. We present a 57-year-old male patient with squamous cell lung cancer, who presented with hemoptysis. Bronchoscopy did not reveal ongoing bleeding. Imaging showed a left lower lobe tumor, a cavitary lesion communicating with the bronchus, and a pulmonary artery pseudoaneurysm. Successful embolization of the originating segmental branch of the pulmonary artery was performed. The pathogenesis of PAPs associated with primary lung malignancies remains poorly understood. We propose a four-step mechanism involving primary tumor expansion, central cavitary necrosis, direct arterial invasion, inflammatory response, vessel wall damage, pseudoaneurysm formation, and subsequent filling of the former cavitary lesion. This case emphasizes the importance of considering PAPs in primary lung malignancies, particularly in male patients with squamous cell pathology. Understanding the proposed pathogenic mechanism could lead to early detection, prompt intervention, and improved outcomes. [ABSTRACT FROM AUTHOR]

La evidencia de que el seguimiento de las recomendaciones sobre el cribado y tratamiento de la infección tuberculosa no evita totalmente la aparición de tuberculosis en pacientes con enfermedad inflamatoria intestinal, y el uso reciente de nuevos fármacos biológicos y de nuevos inmunomoduladores, ha llevado al Grupo Español de Trabajo en Enfermedad de Crohn y en Colitis Ulcerosa a actualizar sus recomendaciones para la prevención de la tuberculosis en los pacientes con enfermedad inflamatoria intestinal. Se revisan los métodos de diagnóstico de la infección tuberculosa latente, los distintos escenarios en los que se va a realizar el cribado, las estrategias para disminuir el riesgo de tuberculosis una vez iniciado el tratamiento biológico, las pautas de quimioprofilaxis de la infección tuberculosa latente y el manejo de la tuberculosis activa durante el tratamiento biológico. Finalmente, se resumen las recomendaciones en el texto y en un algoritmo.

209. Efficacy of Long-lasting Insecticidal Nets With Declining Physical and Chemical Integrity on Aedes aegypti(Diptera: Culicidae)

Fitting long-lasting insecticidal nets (LLIN) as screens on doors/windows has a significant impact on indoor-adult Aedes aegypti(L.), with entomological reductions measured in a previous study being significant for up to 2 yr post-installation, even in the presence of pyrethroid-resistant Aedespopulations. To better understand the mode of LLIN protection, bioassays were performed to evaluate the effects of field deployment (0, 6, and 12 mo) and damage type (none, central, lateral, and multiple) on LLIN efficacy. Contact bioassays confirmed that LLIN residual activity (median knockdown time, in minutes, or MKDT) decreased significantly over time: 6.95 (95% confidence interval [CI]: 5.32–8.58) to 9.24 (95% CI: 8.69–9.79) MKDT at 0- and 12-mo age, respectively, using a pyrethroid-susceptible Aedesstrain. Tunnel tests (exposing human forearm for 40 min as attractant) showed that deployment time affected negatively Aedespassage inhibition from 54.9% (95% CI: 43.5–66.2) at 0 mo to 35.7% (95% CI: 16.3–55.1) at 12 mo and blood-feeding inhibition from 65.2% (95% CI: 54.2–76.2) to 48.9% (95% CI: 26.4–71.3), respectively; both the passage/blood-feeding inhibition increased by a factor of 1.8–2.9 on LLINs with multiple and central damages compared with nets with lateral damage. Mosquito mortality was 74.6% (95% CI: 65.3–83.9) at 0 mo, 72.3% (95% CI: 64.1–80.5) at 6 mo, and 59% (95% CI: 46.7–71.3) at 12 mo. Despite the LLIN physical integrity could be compromised over time, we demonstrate that the remaining chemical effect after field conditions would still contribute to killing/repelling mosquitoes.

Abstract Rationale The tuberculin skin test (TST) and interferon ? release assays (IGRAs) are commonly used for latent tuberculosis infection (LTBI) screening. Unexpectedly high TST positivity rates have been reported in patients with rheumatic diseases, and methotrexate is frequently used in this population. We hypothesized that methotrexate use could be associated with false-positive TST results. Objectives To investigate whether treatment with methotrexate and other factors are associated with false-positive TST results in patients with rheumatic diseases. Methods Prospective single-center study conducted between April 2013 and March 2016. Adult patients with rheumatic diseases were evaluated with a TST and two IGRAs for LTBI screening. We compared TST and IGRA results in patients treated and not treated with methotrexate and analyzed for factors associated with positive TST results. Conclusions Our data suggest false-positive TST results associated with methotrexate therapy. Thus, we recommend against using the TST for LTBI screening in patients receiving methotrexate and the preferential use of IGRAs in such patients. Measurements and Main Results We studied 393 patients with rheumatic diseases, including ankylosing spondylitis (ASP, n = 90), rheumatoid arthritis (RA; n = 120), psoriatic arthritis (PA, n = 126), and other disorders (n = 57). The rate of TST positivity varied across the groups: ASP 22.2%, RA 25%, PA 35.7%, and other disorders (22.8%). Positivity rates were lower with IGRAs. Methotrexate use was associated with a statistically significant two-fold increase in the risk of a positive TST and a dose\x96 response relationship was observed. 211. Serial Tuberculin Skin Tests Improve the Detection of Latent Tuberculosis Infection in Patients With Inflammatory Bowel Disease.

Aim To assess the likelihood of detecting latent tuberculosis infection [LTBI] by the positive conversion of a serial tuberculin skin test [TST] at 1 year in inflammatory bowel disease [IBD] patients with negative baseline two-step TST. Methods In this multicentre prospective cohort study, we evaluated rate and predictors of conversion of TST at 1 year in patients with negative baseline TST. We also evaluated management of patients who had a positive TST at baseline or a conversion at 1 year. In all patients we assessed TB cases occurring during follow-up. Results Of the 192 IBD patients receiving anti-tumour necrosis factor [TNF] and 220 IBD controls not receiving anti-TNF, 35 [8.5%, 95% CI 5.7–11.3] had positive conversion (median TST induration 13 mm, interquartile range [IQR] 9–16). Ten anti-TNF cohort patients [5.2%, 95% CI 2.5–9.5] versus 25 controls [11.4%, 95% CI 7.5–16.3] had TST conversion [ p = 0.029]. In multivariate analysis, conversion was associated with smoking habit (odds ratio [OR] 2.19, 95% CI 1.08–3.97; p = 0.028). Anti-TNF-treated patients had a lower conversion rate [OR 0.41, 95% CI 0.20–0.83; p = 0.013]. The likelihood of conversion correlates with fewer immunosuppressive therapies between baseline TST and TST at 1 year [ p = 0.042]. One case of active TB [isoniazid-resistant strain] occurred in a patient with positive baseline TST receiving anti-TNF [0.05 events/100 patient-years]. Conclusions Serial TST at 1 year can detect LTBI in IBD patients receiving anti-TNF therapy with negative baseline TST. Serial TST seems to be advisable to reduce the risk of TB cases associated with inability to detect LTBI in pre-treatment screening. [ABSTRACT FROM AUTHOR]

Cada vez es más frecuente que se produzcan peticiones de pacientes o de sus representantes relativas a que determinada información no quede registrada en la historia clínica o que, si ya lo está, se elimine. Sin duda esto es debido a que saben que a los historiales clínicos acceden numerosos profesionales que en muchas ocasiones no guardan una relación estrictamente asistencial con ellos y que de manera generalizada se copian antecedentes médicos que de forma innecesaria se reproducen en los diversos informes de alta o de asistencia. El problema produce situaciones de conflicto cuando los datos objeto de controversia hacen referencia a aspectos clínicos especialmente sensibles para la intimidad personal y familiar, como ocurre con las técnicas de reproducción asistida. Por ello, la pregunta que cabe formular es: ¿quién decide qué datos deben constar en la historia clínica y en función de qué criterios debe tomarse esa decisión?

Aims: To determine risk factors for active tuberculosis in patients with inflammatory bowel diseases. Methods: Retrospective, case-control study at 4 referral hospitals in Spain. Cases developed tuberculosis after a diagnosis of inflammatory bowel disease. Controls were inflammatory bowel disease patients who did not develop tuberculosis. For each case, we randomly selected 3 controls matched for sex, age (within 5 years) and time of inflammatory bowel disease diagnosis (within 3 years). Inflammatory bowel disease characteristics, candidate risk factors for tuberculosis and information about the tuberculosis episode were recorded. Multivariate analysis and a Chi-squared automatic interaction detector were used. Results: Thirty-four cases and 102 controls were included. Nine of the 34 cases developed active tuberculosis between 1989 and 1999, and 25 became ill between 2000 and 2012. Multivariate regression showed an association between active tuberculosis and anti-TNF (tumor necrosis factor) therapy in the previous 12 months (OR 7.45; 95% CI, 2.39-23.12; p = 0.001); hospitalization in the previous 6 months (OR 4.38; 95% CI, 1.18-16.20; p = 0.027); and albumin levels (OR 0.88; 95% CI, 0.81-0.95; p = 0.001). The median time between the start of biologic therapy and the onset of active tuberculosis was 13 (interquartile range, 1-58) months. Tuberculosis developed after a year of anti-TNF therapy in 53%, and late reactivation occurred in at least 3 of 8 patients. The Al-Si cast alloy family is widely used in the production of complex castings for various applications and known for its very good castability and high strength-to-weight ratio. However, early cracking under tensile loading is sometimes a limiting factor. Among other parameters, it is yet controversial whether grain boundaries are dominant strengthening factor in cast alloys, instead of dendrite/eutectic boundaries. This study presents the effect of secondary dendrite arm spacing (SDAS) and grain size on crack initiation and propagation of Al-Si cast alloys under tensile loading. The Al-10Si (wt.%) alloy with modified Si morphology was cast using inoculants (Al-5Ti-B master alloy) under different cooling rates to obtain a range of grain sizes (from below 138 µm to above 300 µm) and SDAS (6, 15 and 35 µm). Conventional tensile test as well as in-situ tensile test in a scanning electron microscope, equipped with an electron backscatter diffraction (EBSD) was carried out to understand the deformation mechanisms of the alloy. Observation of slip bands within the dendrites showed that in modified Si structure, the interdendritic (eutectic) area takes more portion of the strain during plastic deformation. Besides, only a few cracks were initiated at the grain boundaries; they were mostly initiated from dendrite/eutectic interface. All cracks propagated trans-granularly. Hall-Petch calculations also showed a strong relationship between SDAS and flow stress of the cast alloy. Although statistically correct, there was no physically meaningful relationship between the grain size and the flow stress. Nevertheless, formation of identical slip bands in each grain could be an evidence for the marginal effect of the grain size on the overall strength development of the alloy. Consequently, among other effects, the combinational dominant effect of SDAS and modest effect of grain size shall be considered for modification of the Hall-Petch equation for precise prediction of mechanical properties of cast alloys. This study aims to establish objective anatomical criteria to obtain accurate and reliable measurements of the right liver lobe (RLL) length by ultrasound while considering sex and anthropometry of the patient. Thirty‐three (n= 33) adult participants underwent two‐dimensional (2D) and panoramic (PAN) ultrasound imaging of the RLL in the anterior axillary region. Each measurement was performed in the standard oblique plane and in a tested craniocaudal (CC) plane by two independent observers. Pearson analysis was conducted to evaluate correlation between measurements and intraclass correlation coefficient for inter‐rater reliability. Two‐tail paired ttest was used to compare groups. Statistical significance was attained at p< 0.05. The proposed CC 2D RLL length was 13.0 ± 1.5 cm for women and 14.1 ± 1.3 cm for men whereas PAN RLL size was 13.7 ± 1.8 cm for women and 15.0 ± 1.1 cm for men. A strong correlation between the proposed 2D CC measurement of the RLL length with PAN CC measurements was found (r= 0.87). Inter‐rater reliability for 2D CC measurements showed high reliability (intraclass correlation coefficient 0.96). Craniocaudal measurement of RLL by ultrasound in anterior axillary region is a precise and reliable measurement that may allow for the routine monitoring of RLL length across time.

Trichomonas vaginalisis a sexually transmitted, eukaryotic parasite that causes trichomoniasis, the most common nonviral, sexually transmitted disease in the USA and worldwide. Little is known about the molecular mechanisms involved in the host immune response to this widespread parasite. Here we report that T. vaginalisinduces NLRP3 inflammasome activation in human macrophages, leading to caspase-1 activation and the processing of pro-IL-1β to the mature and bioactive form of the cytokine. Using inhibitor-based approaches, we show that NLRP3 activation by T. vaginalisinvolves host cell detection of extracellular ATP via P2X7receptors and potassium efflux. In addition, our data reveal that T. vaginalisinflammasome activation induces macrophage inflammatory cell death by pyroptosis, known to occur via caspase-1 cleavage of the gasdermin D protein, which assembles to form pores in the host cell membrane. We found that T. vaginalis-induced cytolysis of macrophages is attenuated in gasdermin D knockout cells. Lastly, in a murine challenge model, we detected IL-1β production in vaginal fluids in response to T. vaginalisinfection in vivo. Together, our findings mechanistically dissect how T. vaginaliscontributes to the production of the proinflammatory IL-1β cytokine and uncover pyroptosis as a mechanism by which the parasite can trigger host macrophage cell death.

Although genetics clearly influences the onset of menarche, the association of age at menarche (AAM) with variants in genes related to energy homeostasis remains unexplored. Our aim was to analyze the relationship of the Q223R polymorphism in the leptin receptor gene (LEPR(Q223R)) with AAM in a population-based sample of healthy pubertal girls. The study included 338 Spanish girls aged between 11 and 17 yr. Data were collected on AAM. The Q223R polymorphism in LEPR was detected by TaqMan allelic discrimination assays. Girls carrying the RR genotype had a significantly younger AAM (11.5 yr) than those carrying the QR (11.9 yr) or QQ (12.0 yr) genotype (P0.05). Furthermore, we found a significantly higher frequency of the RR genotype in girls with an AAM of 11 yr or younger than in girls with an AAM older than 12 yr (23.9% vs. 7.8%, χ(2) = 11.17, P = 0.0008). Also, the RR genotype frequency in girls with an AAM between 11 and 12 yr was significantly higher than in girls with an AAM older than 12 yr (16.8% vs. 7.8%, χ(2) = 3.97, P = 0.0046). The Q223R polymorphism in the LEPR gene is associated with variations in AAM among Spanish girls, with the RR genotype being related to earlier onset.

To examine the relationship of three common polymorphisms in the leptin receptor (LEPR) gene, implicated in the regulation of body weight, with leptin levels and obesity-related phenotypes in a population-based sample of healthy pubertal children in Spain.The study included 806 boys and girls aged 12-16 whose anthropometrical data and body composition were recorded. Serum leptin levels were determined by ELISA. The LEPR Q223R, K109R and K656N polymorphisms were determined by TaqMan® allelic discrimination assays.When analysing the Q223R polymorphism, we observed that female carriers of the RR genotype had significantly higher plasma leptin levels (18.2 vs. 15.1 ng/mL p = 0.016) and significantly higher mean BMI values (22.5 vs. 21.3 Kg/m² p = 0.032) than QR carriers. Furthermore, the frequency of the RR genotype in overweight-obese girls was significantly higher than that found in normal-weight girls. No significant differences were observed in boys. Neither boys nor girls showed significant differences when comparing leptin levels, anthropometric variables or body composition by K109R or K656N genotype.The fact, that the Q223R polymorphism in the LEPR gene is significantly associated with leptin levels and BMI only in girls, suggests a sex-specific influence of this polymorphism on these variables.

Background and Aim: Sensitivity of tuberculin skin test [TST] during screening for latent tuberculosis infection [LTBI] is affected by steroid and/or immunosuppressant therapy. The aim of this study was to compare performance of the two-step TST in inflammatory bowel disease patients immediately before anti-tumour necrosis factor [TNF] therapy as part of routine screening for LTBI vs control patients when the TST was carried out at an early stage. Methods: In this multicentre prospective controlled study, we evaluated the performance of two-step TST with 5-mm threshold. Factors associated with TST results were determined by logistic regression. Results: We evaluated 243 candidates for anti-TNF therapy and 337 control patients. Overall, 105 patients [18.1%] had an induration = 5 mm in the first TST or in TST retest. LTBI was diagnosed in 25% of patients by TST retest. Twenty-eight [11.5%] anti-TNF group patients vs 77 [22.8%] control patients had a positive TST (odds ratio [OR] 0.44, 95% confidence interval [CI] 0.28-0.70; P < 0.001]. In multivariate analysis, positive TST was associated with higher age [OR 2.63, 95% CI 1.21-5.72; P < 0.001] and 5-aminosalicylate therapy [OR 1.86, 95% CI 1.14-3.05; P = 0.013]. Negative TST was associated with steroid therapy [OR 0.36, 95% CI 0.16-0.83; P = 0.016], immunosuppressant therapy [OR 0.36, 95% CI 0.21-0.62; P < 0.001], or steroids + immunosuppressant therapy [OR 0.20, 95% CI 0.07-0.59; P = 0.004]. Conclusions: The sensitivity of routine TST performed just before starting anti-TNF therapy is low. TST performed at an early stage enables screening in the absence of immunosuppressive treatment and thus maximises the diagnostic yield of TST for detecting LTBI. [ABSTRACT FROM AUTHOR]

There are many medicinal products that, although having shown efficacy and safety in different ophthalmological indications, they are not authorized or commercially available for ophthalmic administration. This implies, on one hand, that they must be used according to legislation that regulates the availability of medicines in special situations and, on the other hand, that they must be prepared in the pharmacy services for ophthalmic administration, according to quality criteria to ensure its effectiveness, stability and sterility. This document gathers the consensus between the Spanish Society of Ophthalmology and the Spanish Society of Hospital Pharmacy about these selected preparations which have shown enough evidence in their efficacy and safety for their ophthalmic use (off label) and ophthalmic administration. This document includes recommendations about its use according to the current legislation. In addition, with the aim of harmonizing the preparation of intraocular injections in the hospital pharmacy services, general recommendations are set in this document to ensure the compliance with standards established in the Spanish Guideline for GoodPreparation Practices of Medicinal Products in Hospital Pharmacies.These recommendations include sections such as the area of preparation, material, technique, packaging, stability, quality control, prescription and traceability of intraocular preparations.

AbstractThe design of long-span timber floors, which are able to bear overloaded public spaces, requires the use of beams with large depths to meet the requirements of the serviceability limit state. This article studies the use of a self-tensioning system that, in combination with a pre-stressing, allows designing wooden structural floors that are able to remain practically horizontal in service load situations with depths that are no greater than 1/30 of the span. Several situations are studied, with spans between 9 and 18 m and variable loads of 3.0 and 5.0 kN/m2. These situations obtain deflections below 1/1000 of the span under a total load set-up.

The patient's questionnaire included 285 items regarding diagnosis, treatment, and follow-up, whereas the physician's questionnaire added 32 specific items for clinicians. Relevance of items was scored from 1 (not at all important) to 9 (very important). Recommendations were based on very highly rated items. Results Most items were rated consistently by patients and physicians with some exceptions. Patients emphasized the choice of moderate flare-ups to be followed at the hospital setting rather than in primary care as well as to be informed on aspects, such as diet and impact of UC on their health and lifestyle. Treatment-related efficacy, safety, and improved quality of life were relevant for both patients and physicians. Physicians believed that to reduce pill burden was a crucial aspect to improve patient's satisfaction. Patients preferred oral administration treatment over rectal treatment. Conclusions The study identified specific aspects related to the management of mild-to-moderate UC that both patients and physicians considered to be highly relevant for patient satisfaction. Recommendation based on these factors would be important at the time of designing and implementing strategies targeting satisfaction in UC patients. [ABSTRACT FROM AUTHOR]
