The concept of "thought-transference" holds a special position within the psychoanalytic field. It first emerged in the margins of the discipline as an object of the psychical sciences. It was then extracted by psychoanalysis from the darkness of occultism, Freud considered thought-transference as the kernel of truth in occult beliefs. In this article, we propose to review the role of thought-transference from Freud to the present. This article is divided into two sections, the first of which illustrates how the concept was interpreted by Freud, then taken over by successive generations of analysts. The second section turns to contemporary works that have addressed the topic through three lines of inquiry: its ontological possibility, its conditions of emergence, and its clinical implications. Our aim is thus to shed light on the heuristic role of thought-transference and how it has been the locus of innovative psychoanalytic discoveries. [ABSTRACT FROM AUTHOR]

Kidney transplantation is the ideal treatment modality for patients with end-stage kidney disease, with excellent outcomes post-transplant compared with dialysis. However, kidney transplant recipients are at increased risk of infections and cancer because of the need for immunosuppression. Kidney transplant recipients have approximately two to three times greater risk of developing cancer than the general population, and cancer is a major contributor to morbidity and mortality. Most of the increased risk is driven by viral-mediated cancers such as post-transplant lymphoproliferative disorder, anogenital cancers, and Kaposi sarcoma. Nonmelanoma skin cancer is the most frequent type of cancer in kidney transplant recipients, likely due to an interaction between ultraviolet radiation exposure and decreased immune surveillance. Occurrence of the more common types of solid organ cancers seen in the general population, such as breast, prostate, lung, and colorectal cancers, is not, or is only mildly, increased post-transplant. Clinical care and future research should focus on prevention and on improving outcomes for important immunosuppression-related malignancies, and treatment options for other cancers occurring in the transplant setting. The goal of this study was to evaluate the effect of the Model for End-Stage Liver Disease (MELD)–based allocation system on mortality, bleeding, and transfusion requirement in orthotopic liver transplantation (OLT). OLTs were studied for this observational study (before-and-after observational cohort study). One community hospital. The study comprised 686 patients who underwent 750 consecutive OLTs. None. Patients who underwent OLT in the MELD era had an adjusted lower 1-year mortality (adjusted odds ratio 0.45 [0.24-0.83]) compared with patients who underwent OLT the pre-MELD era. No significant difference in 1-month mortality was observed. Other variables with a significant effect on 1-year mortality in multivariate analysis were preoperative international normalized ratio, intraoperative use of a phlebotomy, total intraoperative volume of crystalloid infused, and retransplantation. Blood loss was greater in the MELD era (median difference 200 mL; p < 0.001), as were red blood cell, fresh frozen plasma, and cryoprecipitate transfusions. More patients in the MELD era received at least 1 transfusion (27% v 20%; p = 0.024). The MELD allocation system did not affect 1-month mortality, but a decrease in 1-year mortality was demonstrated. Blood loss and transfusions increased during OLTs performed in the MELD era. The role of other variables should be explored further to explain postoperative morbidity and mortality. The article focuses on the contributions that English-speakers made to Quebec's Mauricie region in the early twentieth-century. The authors used oral and written sources to examine this history. They cover both winter and summer sports and focus on two individuals. Doctor Maurice Cambell was a founder of the curling club in Cap-de-la-Madeleine. The Englishman Duncan Breeze, who came to the city of Trois-Rivières in 1919, was editor of the St. Maurice Valley Chronicle and shared stories about hunting and fishing.

Objective Orthotopic liver transplantation (OLT) frequently is associated with major blood loss and considerable transfusion requirements. The goal of this study was to define the risk factors for multiple transfusions and major bleeding during OLT and to help identify higher risk patients that could benefit from targeted interventions. Design OLTs were studied for this observational cohort study. Setting Community hospital. Participants A total of 800 consecutive OLTs were studied. Intervention No intervention. Measurements and Main Results Baseline and intraoperative data were gathered. Multivariate logistic regression analyses were performed to find variables associated with 2 outcomes: transfusion of more than 2 units of red blood cells (RBC) and bleeding ≥900 mL. Two nomograms were developed to predict individual risks. The overall intraoperative RBC transfusion was 0.6 ± 1.4 units on average, and 61 surgeries (7.6%) received more than 2 units of RBC (4.5 ± 1.9). Some variables were associated with the outcomes: 5 were associated with transfusion of more than 2 units of RBC (patient’s height, starting hemoglobin concentration, starting bilirubin value, the use of a phlebotomy, and central venous pressure [CVP] at the time of vena cava clamping) and 3 with blood loss of ≥900 mL (starting hemoglobin value, Child-Turcotte-Pugh score, and CVP at the time of vena cava clamping). Preclamping CVP showed the strongest association with both outcomes. Nomograms were developed to predict the individual OLT recipients’ risk of requiring more than 2 units RBC and suffering from major bleeding. Among the variables associated with multiple RBC transfusions and major bleeding, 3 can lead to interventions: baseline hemoglobin value, the use of a phlebotomy, and the preclamping CVP. Conclusion Some variables were able to predict the risk of multiple transfusions and major bleeding in this low bleeding liver transplantation population. Further studies based on these variables should be done to better define the role of targeted interventions in higher risk liver transplant recipients. [ABSTRACT FROM AUTHOR]

Objective The use of cerebral near-infrared spectroscopy (NIRS) has become widespread in cardiac surgery after research demonstrated an association between perioperative cerebral desaturations and postoperative complications. Somatic NIRS desaturation also is associated with an increased risk of postoperative complications and mortality. The objective of this study was to explore the trends of both somatic and cerebral NIRS during liver transplantation. Design A prospective, single-site, observational case series. Setting Tertiary care center. Participants The study comprised 10 patients undergoing liver transplantation. Interventions NIRS sensors were placed on the forehead (cerebral regional oxygen saturation [rSO 2 ]) and on the right arm and right leg (somatic rSO 2 ) to measure tissue perfusion. Desaturation was defined as a 20% decrease of baseline values for 15 seconds. Measurements and Main Results In all patients, parallel changes in both cerebral and somatic rSO 2 values were observed during phlebotomy, bleeding, transfusion, portal vein clamping, and the use of vasoactive agents. Induction of anesthesia increased cerebral rSO 2 more than it did somatic values. However, ascites removal, abdominal manipulation, and clamping of the inferior vena cava (IVC) were associated with nonparallel changes in cerebral and somatic rSO 2 . Ascites removal was associated with increased somatic leg rSO 2 , and IVC clamping and abdominal hypertension were associated with a significant reduction in somatic leg rSO 2 . Somatic leg desaturation instead of arm or cerebral desaturation was associated with more postoperative complications. Conclusions The use of combined NIRS monitoring allows for the identification of the source of somatic or cerebral desaturation. Compromised venous flow from the IVC from clamping or abdominal compartment syndrome typically is associated with the appearance of more pronounced leg than arm desaturation. [ABSTRACT FROM AUTHOR]

The interpretations of the surface electromyography (sEMG) signals from the trunk region are strongly distorted by the heart activity (ECG), especially in case of low-amplitude EMG responses analyses. Many methods have been investigated to resolve this nontrivial problem, by using advanced data processing on the overall sEMG recorded signal. However, if they reduce ECG artifacts, those cancellation methods also deteriorate noiseless parts of the signal. This work proposes an original ECG cancellation method designed to limit the deterioration of sEMG information. To do that, the proposed techniques does not directly attempt to remove the ECG, but is based on two main steps: the localization of ECG and the cancellation of ECG but only where heart pulses have been detected. The phase of localization efficiently extracts the ECG contribution by combining the discrete wavelet transforms (DWT) and the method of independent component analysis (ICA). And finally, this phase takes advantage of quasi-periodic properties of ECG signals to accurately detect pulses localization with an original algorithm based on the fast Fourier transform (FFT). Intensive simulations were achieved in terms of relative errors, coherence and accuracy for different levels of ECG interference. And the correlation coefficients computed from the paraspinal muscles EMG signals of 12 healthy participants were also used to evaluate the developed method. The results from simulation and real data demonstrate that the proposed method accurately detects pulses positions and efficiently removes the ECG from EMG signals, even when both signals are strongly overlapped, and greatly limits the deterioration of the EMG. Background The Infant Jarvik 2015 is an implantable axial-flow ventricular assist device (VAD) that has undergone the major evolutionary design modifications to improve hemocompatibility. This study was conducted in anticipation of data submission to the US Food and Drug Administration to obtain Investigational Device Exemption approval. Methods The VAD was implanted via a left thoracotomy in Barbado sheep (n = 10, 26 (19-34] kg). Anticoagulation was maintained with coumadin, with a target international normalized ratio of greater than the individual sheep's baseline values. The VAD was managed at the highest possible speed as clinically tolerable. Complete necropsy was performed at the end of the study. Results There were 2 early mortalities: tension pneumothorax (n = 1) and shower emboli of the fragmented myocardium (n = 1). The remaining 8 sheep (2 with 30-day and 6 with 60-day protocols) completed the anticipated study duration in excellent condition, with the 6 completing 60-day sheep showing appropriate weight gain during support. There were no signs of clinically significant hemolysis, with the final plasma-free hemoglobin of 2 (1-17) mg/dL. Necropsy showed old renal infarction in 7 sheep. Although thromboembolism can be the potential etiology, given the mild anticoagulation regimen, other sources of emboli were identified in 2 sheep (graft coating material and fragmented myocardium). Flow study demonstrated favorable increase in flow (up to 3.0 L/min) in proportion to change in pump speed. Conclusions This study has demonstrated that the Infant Jarvik 2015 VAD is capable of maintaining its functionality for an extended period of time with minimal hemolysis. [ABSTRACT FROM AUTHOR]

Management of the airway may be challenging in patients undergoing occipito-cervical spine fusions (OCF). Changes in the occipito-cervical angle (dOC2A) of fusion after surgery may result in acute airway obstruction, dyspnea and/or dysphagia. Objectives of the study were to review the airway management of patients during posterior OCF, determine the incidence, nature and risk factors for postoperative airway adverse events (AEs), and to determine the relationship between airway AEs and the change in dOC2A. In this retrospective cohort of 59 patients, following extubation in the operating room (OR), there were no complications in 43 (73%) patients (Group 1). Sixteen (27%) patients (Group 2) had airway complications; 4 requiring reintubation and 12 having delayed extubation. The number of vertebral levels fused (>6), presence of difficult intubation and duration of surgery (>5 h) were significantly associated with AEs. There was no significant difference in the dOC2A between the groups (−1.070 ± 5.527 versus −4.375 ± 10.788, p = 0.127). Airway management in patients undergoing OCF poses a challenge for the anesthesiology and surgical teams. The incidence of AEs was 27%. The decision to extubate immediately after surgery needs to be individualized. Factors such as difficult intubation, number of vertebral levels fused and duration of surgery have to be considered. A significant correlation between dOC2A and postoperative AEs could not be established. Risk factors for postoperative AEs are multifactorial and prospective evaluation of these factors is indicated. The Infant Jarvik ventricular assist device (VAD; Jarvik Heart, Inc., New York, NY) has been developed to support the circulation of infants and children with advanced heart failure. The first version of the device was determined to have elevated hemolysis under certain conditions. The objective of this work was to determine appropriate modifications to the Infant Jarvik VAD that would result in acceptably low hemolysis levels. In vitro hemolysis testing revealed that hemolysis was related to the shape of the pump blade tips and a critical speed over which hemolysis would occur. Various design modifications were tested and a final design was selected that met the hemolysis performance goal. The new version was named the Jarvik 2015 VAD . Chronic in vivo tests, virtual fit studies, and a series of other performance tests were carried out to assess the device’s performance characteristics. In vivo test results revealed acceptable hemolysis levels in a series of animals and virtual fit studies showed that the device would fit into children 8 kg and above, but could fit in smaller children as well. Additional FDA-required testing has been completed and all of the data are being submitted to the FDA so that a clinical trial of the Jarvik 2015 VAD can begin. Development of a Jarvik VAD for use in young children has been challenging for various reasons. However, with the hemolysis issue addressed in the Jarvik 2015 VAD, the device is well-poised for the start of the PumpKIN clinical trial in the near future. [ABSTRACT FROM AUTHOR]
