201. Improving the Reading Skills of Young People with Duchenne Muscular Dystrophy in Preparation for Adulthood

Abstract

Duchenne muscular dystrophy (DMD) is a progressive genetic condition that affects both muscle and brain. Children with DMD are at risk of psycho-social difficulties such as poor academic achievement and behavioural and socio-emotional problems. This article by Janet Hoskin and Angela Fawcett, both from the University of Swansea, describes how 34 participants with DMD took part in a 36-week online literacy intervention which was delivered in partnership between home and school. The key objective was to improve reading skill. Participants were re-tested at 36 weeks for single word and text level reading, comprehension, fluency, processing and timed single word reading. Pre and post results indicated that children who followed the intervention for 36 weeks made significant improvement in their single word reading (p = <0.0001), timed single word reading (p = <0.0001) and text level reading (p = <0.004). They also made significant improvement in their fluency and comprehension scores. The results showed that children with DMD and related literacy difficulties benefit from a regular, structured and systematic synthetic phonics programme. With young people with DMD increasingly living into adulthood, early literacy intervention is particularly important to ensure optimum career and training opportunities.

Abstract

AIMS: Immune checkpoint inhibitors (ICIs) are used in incurable urothelial cancers, both in chemo-naïve and platinum-refractory patients. Efficacy and toxicity data published outside controlled clinical trials are limited. We report overall survival, progression-free survival and toxicities of ICIs in locally advanced (LABC) or metastatic bladder cancer (MBC). We aimed to develop and validate a prognostic model for these patients.MATERIALS AND METHODS: A multicentre real-world individual patient-level data study (n = 272) evaluating ICIs in the first-line platinum-ineligible or platinum-refractory setting for LABC/MBC between March 2017 and February 2020 was undertaken. Cox regression analyses evaluated the association of prognostic factors with overall survival. Data were split to create a training (n = 208) and validation (n = 64) cohort. The backward elimination method with a P-value cut-off of 0.05 was used to develop a reduced prognostic model using the training data set. The concordance index and assessment of observed versus predicted survival probabilities were used to evaluate the final model.RESULTS: The median follow-up was 18.9 (15.8-21.5) months. The median overall survival and progression-free survival in the training cohort were 9.2 (95% confidence interval 7.4-10.5) and 4.5 months (3.5-5.7), respectively. The most common grade 1/2 adverse events recorded were fatigue (47.8%) and infection (19.9%). Five key prognostic factors found in the training set were low haemoglobin, high neutrophil count, choice of immunotherapy favouring pembrolizumab, presence of liver metastasis and steroid use within 30 days of treatment. The concordance index for the training and validation cohorts was 0.66 (standard error = 0.05) and 0.64 (standard error = 0.04), respectively, for the final model. A nomogram was developed to calculate the expected survival probabilities based on risk factors.CONCLUSIONS: Real-world data were used to produce a validated prognostic model for overall survival in LABC/MBC treated with ICIs. This model could assist in patient stratification, interpreting and framing future trials incorporating PD-1/PD-L1 inhibitors in LABC/MBC.

216. Femmephobia versus gender norms: Examining women’s responses to competing and contradictory gender messages

Abstract

Abstract:While women experience pressure to be more feminine/less masculine, recent years have documented their simultaneous experiences of feeling pressured to be more masculine/less feminine. This phenomenon can be linked to the concept of femmephobia – the way femininity is devalued and regulated across bodies, identities, and various social locations. Despite the accumulating research documenting the existence of femmephobia across identities, how women respond to these experiences remains unexplored. Thus, the current study examined women’s (N = 97) responses to femmephobia. Participants were asked to describe whether (and how) they attempted to modify any of the feminine expressions they felt were targeted in the femmephobic incident they reported. Results of the thematic analysis supported the hypothesized response types of amplification, suppression, and no change to feminine expressions following the femmephobic incident. We found that women overwhelmingly suppressed their femininity in response to femmephobia. Strikingly, while some women reported not making any changes, almost none of the respondents reported amplifying their feminine expressions in response to a femmephobic incident (and the few who did were all LGBTQ+). We theorize these findings in the context of women’s gender policing more broadly, specifically the tension between gender norms (pressure to be more feminine/less masculine) and femmephobia (pressure to be less feminine/more masculine). We argue that the frequency of suppressing femininity in response to femmephobia suggests that, when faced with contradictory gender messages, femmephobia may be more influential than considered by previous research.

225. An Examination of the Balanced School Day Schedule

Principals are always looking for effective ways to enhance the learning environment. The Balanced School Day (BSD) schedule, an alternative to the traditional schedule, reframes the school day by eliminating recess and lunch breaks and providing two scheduled nutrition breaks. Advantages and disadvantages to the schedule have been anecdotally reported. A two-year evaluation was completed to verify these claims. The BSD showed some favourable results (e.g., amount of instructional and transition time, school cleanliness), some positive trends (e.g., student concentration and behaviour in the hallway), and some mixed results (e.g., supervision time and outdoor time). Although the evaluation did not provide a direct assessment of the schedule's impact on student achievement, some teachers and parents felt that students were achieving better. Tracking will need to take place over the next few years to examine student achievement within these schools. (Contains 4 tables and 1 figure.)

Abstract

Myriad research examines benefits and drawbacks of working from home, both pre- and post-pandemic. Our research looks at how work from home mandates due to the COVID-19 pandemic were implemented, primarily by those who also had caregiver roles to fulfill. We used a convenience sample, drawing from full-time college faculty at a mid-sized state college in Florida, gathering information from caregivers and non-caregivers for comparative purposes. As we analyzed our data, we considered two additional concepts: Elmore's backward mapping, which asks us to consider how employer mandates are implemented and will assist us in making policy recommendations, and Smith's Standpoint, which allowed us to consider our own different gendered experiences as we analyzed survey responses. Our findings reveal that there are some employees well suited to working from home, while others are less enthusiastic about this initiative, and that a significant factor is household caregiving responsibilities, often considered to be the domain of women. Our insights shed light on differences in caregiver-employee statuses; we hope to help guide effective institutional policy should there be a need for workplaces to shut down again and to encourage administrators to consider faculty who may be a good work from home fit should it be considered for everyday work.
