202. New vertebral fractures after vertebroplasty: Two year results from a randomised placebo-controlled trial.

Abstract

Background/Purpose: We previously reported the results of a randomized participant and outcome assessor blinded controlled trial that found no beneficial effect of vertebroplasty (VP) over placebo for people with acute osteoporotic fractures up to 24 months and no between-group differences in incident clinical vertebral fractures. A secondary aim was to determine the effect of VP on the risk of further radiologically apparent vertebral fracture at 12 and 24 months. Method(s): The trial was registered (ACTRN012605000079640), and conducted between April 2004 and October 2010. Eligible participants had one or two acute osteoporotic vertebral compression fractures of at least grade 1, confirmed by history, radiograph and the presence of bone oedema. They were randomly assigned to VP (n=38) or placebo (n=40). Cement volume and leakage were recorded for the VP group during the cement injection and on imaging post-procedure. Plain thoracolumbar radiographs were taken at baseline, 12 and 24 months. Two independent radiologists assessed these for new and progressed fractures at the same, adjacent and non-adjacent levels. Discrepancies were resolved by consensus at the completion of the review process. A total sample of 164 participants (82/group) was needed to show a three-fold increase in the risk of further vertebral fractures at 24 months assuming a 10% event rate in the placebo group. Recruitment difficulties led to trial enrolment being terminated early after reaching sufficient numbers to address the primary efficacy outcomes but insufficient to assess the safety outcomes. Result(s): Baseline characteristics were similar between the two groups (31 females in each group, mean (SD) age, median (interquartile range) duration of symptoms and one or more prior vertebral fractures was 74.2 (14.0), 9.0 (3.8 to 13.0) and 18 (47%) in the VP group, and 78.9 (9.5), 9.5 (3.0 to 17.0) and 21 (52%) in the placebo group). Abstract

Background: Although Methotrexate (MTX) is one of the most commonly prescribed disease-modifying drugs in JIA no questionnaire exists that assesses the knowledge of parents about this drug. A 60-item questionnaire was recently developed to measure rheumatoid arthritis (RA) patients’ knowledge about MTX; the Methotrexate in Rheumatoid Arthritis Knowledge Test (MiRAK; Ciciriello et al. (Arthritis Rheum 62:10–1009, 2010)). This study aimed to adapt the MiRAK for parents of children with JIA.\ud \ud Methods: Adaption of the MiRAK involved: 1) email consultations with clinicians working in the field of paediatric rheumatology (Panel 1) to ascertain the potential adaptations of the MiRAK from a clinical perspective, 2) synthesis of clinicians’ suggestions by a panel of experts, researchers and MiRAK developers (Panel 2) to reach consensus on which items needed to be modified and create a draft Methotrexate in Juvenile Idiopathic Arthritis Knowledge Test (MiJIAK), 3) a review of the draft by 5 parents of children with JIA (Panel 3) using the cognitive ‘think-aloud’ method, 4) a second consultation with Panel 2 to review parents’ suggestions and determine the final items.\ud \ud Results: A total of 9 items remained unchanged, e.g. “Methotrexate is effective at relieving joint stiffness”, 19 were deemed inappropriate in the paediatric setting and deleted, e.g. “It is safe to become pregnant 3 weeks after methotrexate has been stopped”, 32 underwent editorial changes largely to indicate that the questionnaire was about the children with JIA, e.g. “If you forget to give a dose of Methotrexate, you can still take it the next day” became “If your child misses a dose of Methotrexate, they can still take it the next day”, and 1 new item was added. A new 42-item questionnaire was produced and was found to be well understood by parents of children with JIA.\ud \ud Conclusions: The systematic modification of the MiRAK, a patient-centred MTX knowledge questionnaire, has generated a comprehensive new questionnaire for use in the JIA setting. The wide consultation process, including cognitive testing, has ensured the tool is both relevant and acceptable to clinicians and will therefore be a valuable addition in understanding the parents’ perspective of this treatment in JIA.

Abstract

The eastern variety of Kmhmu' described in this study has a rich phoneme inventory of 36 consonants and 10 distinctive vowel qualities with contrastive vowel length. There is no contrast of vowel phonation or tone, although voicing and glottalisation do contrast in initial consonants. Word and syllable structure are described along with word formation patterns. Previous descriptions of this variety of Kmhmu' differ in their interpretation of the final voiceless palatal continuant and in their interpretation of vowel length preceding final /Ɂ h ... / (Smalley 1961, Preisig 1990, Suksavang Simana et al. 1994 and Suwilai 2002). This study presents data and analysis which clarify these details. This analysis suggests that the voiceless palatal approximant in final position be interpreted as an approximant, rather than a fricative as others have analysed it, because it is realised with little turbulence. It also demonstrates how vowel length contrast is neutralised in syllables with final laryngeal consonants /Ɂ h ... /, where duration is consistently intermediate between long and short. The neutralisation of vowel length before final /.../ is thought to be due to the articulatory and auditory similarity to final /h/. [ABSTRACT FROM AUTHOR]

Abstract

SUMMARYThe minimal hormonal requirements for inducing the ovine β-lactoglobulin gene have been investigated using mammary gland explants from ewes in the first half of pregnancy. Quantification of β-lactoglobulin mRNA showed that a combination of insulin, Cortisol and prolactin was required to stimulate the expression of the gene and that this response could not be enhanced by the addition of oestrogen and thyroid hormone to the culture medium. Explants cultured in the presence of insulin, Cortisol and prolactin also demonstrated the capacity to synthesize the protein. Progesterone did not inhibit the induction of the gene, which is consistent with the increase in β-lactoglobulin mRNA observed in vivo in the mammary gland during the final 2 months of pregnancy when the circulating level of progesterone is elevated.

Abstract

Empowerment refers to an individual's feelings of being able to manage the challenges of the cancer experience and of having a sense of control over one's life. However, empowerment questionnaires that have been validated for the cancer setting are lacking. The objective of this study was to validate scales from the Health Education Impact Questionnaire (heiQ), which assesses the effects of health education programs among individuals with chronic conditions. The heiQ scales Social integration and support, Health service navigation, Constructive attitudes and approaches, Skill and technique acquisition, and Emotional distress were identified as key dimensions of empowerment for the cancer context.

220. Two-year results of a randomized placebo-controlled trial of vertebroplasty for acute osteoporotic vertebral fractures

Abstract

We previously reported the results of a randomized controlled trial that found no benefit of vertebroplasty over a sham procedure for acute osteoporotic vertebral fractures up to 6 months. We report here the 12-month and 24-month clinical outcomes of this trial. Eligible participants (n = 78) were randomly assigned to receive either vertebroplasty (n = 38) or a sham procedure (n = 40). Randomization was stratified by treatment center, sex, and symptom duration (<6 weeks or ≥6 weeks). Participants, investigators (except the treating radiologists), and outcome assessors were blinded to group assignments. Enrolment occurred between April 2004 and October 2008 with follow-up completed October 2010. The primary outcome was overall pain measured on a scale of 0 (no pain) to 10 (maximal imaginable pain). Secondary outcomes included pain at rest and at night, disability, quality of life, perceived recovery, and adverse events, including incident clinically apparent vertebral fractures. At 12 and 24 months, complete data were available for 67 (86%) and 57 (73%) participants, respectively. At 12 months participants in the active group improved by 2.4 ± 2.7 (mean ± SD) units in overall pain compared with 1.9 ± 2.8 units in the sham group, adjusted between-group mean difference (MD) 0.3 (95% confidence interval [CI], –0.9 to 1.5), whereas at 24 months participants in the active group had improved by 3.0 ± 3.1 units compared with 1.9 ± 3.0 units in the sham group, MD 1.1 (95% CI, –0.3 to 2.4). No significant between-group differences were observed for any of the secondary efficacy outcomes at 12 or 24 months. Abstract

OBJECTIVE: To estimate the global burden of hip and knee osteoarthritis (OA) as part of the Global Burden of Disease 2010 study and to explore how the burden of hip and knee OA compares with other conditions. METHODS: Systematic reviews were conducted to source age-specific and sex-specific epidemiological data for hip and knee OA prevalence, incidence and mortality risk. The prevalence and incidence of symptomatic, radiographic and self-reported hip or knee OA were included. Three levels of severity were defined to derive disability weights (DWs) and severity distribution (proportion with mild, moderate and severe OA). The prevalence by country and region was multiplied by the severity distribution and the appropriate disability weight to calculate years of life lived with disability (YLDs). As there are no deaths directly attributed to OA, YLDs equate disability-adjusted life years (DALYs). RESULTS: Globally, of the 291 conditions, hip and knee OA was ranked as the 11th highest contributor to global disability and 38th highest in DALYs. The global age-standardised prevalence of knee OA was 3.8% (95% uncertainty interval (UI) 3.6% to 4.1%) and hip OA was 0.85% (95% UI 0.74% to 1.02%), with no discernible change from 1990 to 2010. Prevalence was higher in females than males. YLDs for hip and knee OA increased from 10.5 million in 1990 (0.42% of total DALYs) to 17.1 million in 2010 (0.69% of total DALYs). CONCLUSIONS: Hip and knee OA is one of the leading causes of global disability. Methodological issues within this study make it highly likely that the real burden of OA has been underestimated. With the aging and increasing obesity of the world's population, health professions need to prepare for a large increase in the demand for health services to treat hip and knee OA.

229. SCOTROC 2A: Carboplatin followed by docetaxel or docetaxel gemcitabine as first-line chemotherapy for ovarian cancer

Abstract

The feasibility of sequential carboplatin followed by docetaxel-based therapy for untreated ovarian cancer was determined. Patients received four q3w cycles of carboplatin AUC 7, then four q3w cycles of either docetaxel 100 mg m−2 (day 1) (arm A); docetaxel 75 mg m−2 (day 8) and gemcitabine 1250 mg m−2 (days 1,8) (arm B) or docetaxel 25 mg m−2 and gemcitabine 800 mg m−2 (both given weekly (days 1,8,15)) (arm C). A total of 44 patients were randomised to each treatment arm. None of the arms demonstrated an eight cycle completion rate (70.5/72.7/45.5% in arms A/B/C, respectively), which was statistically greater than 60% (P=0.102, P=0.056, P=0.982) which was our formal feasibility criteria, although only the completion rate in arm C was clearly worse than this level. The overall response rate (ORR) after carboplatin was 65.7% in 70 evaluable patients. In evaluable patients, ORRs after docetaxel-based cycles were: arm A 84.0% (21 out of 25); arm B 77.3% (17 out of 22); arm C 69.6% (16 out of 23). At follow-up (median 30 months), median progression-free survival times were: arm A 15.5 months (95% CI: 10.5–20.6); arm B 18.1 months (95% CI: 15.9–20.3); arm C, 13.7 months (95% CI: 12.8–14.6). Neutropenia was the predominant grade 3–4 haematological toxicity: 77.8/85.7/54.4% in arms A/B/C, respectively. Dyspnoea was markedly increased in both gemcitabine-containing arms (P=0.001) but was worse in arm C. Although just failing to rule out eight cycle completion rates less than 60%, within the statistical limitations of these small cohorts, the overall results for arms A and B are encouraging. Larger phase III studies are required to test these combinations.

230. PALAEOKARST DEPOSITS IN CAVES: EXAMPLES FROM EASTERN AUSTRALIA AND CENTRAL EUROPE.

Abstract

Palaeokarst deposits are most commonly found in excavations, drill holes and naturally exposed at the Earth's surface. Some caves however intersect palaeokarst deposits. This occurs in large hypogene caves in the USA, thermal caves in Hungary and in many caves in eastern Australia. Palaeokarst deposits in caves respond to cave forming processes in the same way as hostrock: the palaeokarst deposits form cave walls. A range of palaeokarst deposits is exposed in caves including; filled tubes, walls composed of flowstone, large-scale bodies, breccia pipes, dykes, volcaniclastic palaeokarst and crystalline palaeokarst. As well as being exposed in cave walls, palaeokarst deposits can wholly or partly form speleogens (speleogens made from palaeokarst). Records of geological events not preserved elsewhere can occur in palaeokarst deposits in caves. These can be difficult to correlate with conventional geological histories. It is important to be able to distinguish between palaeokarst deposits, relict sediments and phantom rock (in-situ weathered rock, also called ghost rock; Vergari & Quinif 1997). Relict sediments can be distinguished from palaeokarst deposits because the cave walls bound relict sediments while palaeokarst deposits form the cave walls. Palaeokarst can be distinguished from phantom rock, as palaeokarst is unconformable with the hostrock, with structures in the hostrock not continuing across the boundary into the palaeokarst. Hostrock structures and textures do continue across the boundary between unaltered hostrock and phantom rock. Similarly, cave sediments are unconformable or disconformable with the hostrock while phantom rock is conformable with hostrock containing hostrock structures and textures. It has been difficult to explain why palaeokarst occurs in some caves and not others. One explanation worth considering is that palaeokarst deposits are not intersected by caves or sections of caves that contain large perennial streams and/or have undergone large-scale vadose fluvial development capable of escaping from the bounds of structural guidance, such as the caves in the Classical Karst. Knowledge about the relations between these self-management skills and different quality of life (QoL) outcomes has received little attention. It is also important to provide further evidence on its properties in non-English healthcare contexts, as the questionnaire is being used in cross-cultural research. Furthermore, in the Italian healthcare context, the relationship between the medical staff and the patients remains asymmetrical, with the latter having the role of passive recipients of medical prescriptions and services. The current study provided further evidence about the psychometric properties of the heiQ among Italian people with chronic conditions, specifically by assessing the factor structure, reliability, convergent/divergent and criterion validity (ie, the specific contribution of each of the self-management skills to QoL outcomes).Methods: Two hundred ninety-nine individuals with a chronic condition (mean age = 61.4 years, 50% females) completed the heiQ and the Medical Outcomes Study-Short Form (MOS SF-36). Confirmatory factor analyses, Composite Reliability Indices (CRI), bivariate correlations and linear regression analyses were computed.Results: A model with 8 correlated factors showed good fit, in a similar way to previous studies. CRI values were acceptable to good for all the subscales. Associations between some of the heiQ subscales and some of the MOS SF-36 subscales supported criterion validity. In particular, it was confirmed by the moderate associations between the constructive attitudes and approaches subscale and the MOS SF-36 vitality and perceived mental health and by the moderate correlations between the health directed activities subscale and the MOS SF-36 Vitality. In linear regressions, higher emotional distress predicted higher physical and mental QoL, while higher mental QoL was also associated with stronger constructive attitudes and approaches.Conclusion: The heiQ has robust properties across translations and it can also be used routinely in Italian healthcare contexts. The evidence that all the other self-management skills did not predict either physical or mental QoL could suggest that the self-management model of chronic diseases is still not sufficiently developed in the Italian context, where patients are passive recipients of medical instructions.Keywords: chronic diseases, self-management, health education, psychometric properties, health status, quality of life, diabetes, cardiovascular diseases

Abstract

BACKGROUND: Non-fatal health outcomes from diseases and injuries are a crucial consideration in the promotion and monitoring of individual and population health. The Global Burden of Disease (GBD) studies done in 1990 and 2000 have been the only studies to quantify non-fatal health outcomes across an exhaustive set of disorders at the global and regional level. Neither effort quantified uncertainty in prevalence or years lived with disability (YLDs). METHODS: Of the 291 diseases and injuries in the GBD cause list, 289 cause disability. For 1160 sequelae of the 289 diseases and injuries, we undertook a systematic analysis of prevalence, incidence, remission, duration, and excess mortality. Sources included published studies, case notification, population-based cancer registries, other disease registries, antenatal clinic serosurveillance, hospital discharge data, ambulatory care data, household surveys, other surveys, and cohort studies. For most sequelae, we used a Bayesian meta-regression method, DisMod-MR, designed to address key limitations in descriptive epidemiological data, including missing data, inconsistency, and large methodological variation between data sources. For some disorders, we used natural history models, geospatial models, back-calculation models (models calculating incidence from population mortality rates and case fatality), or registration completeness models (models adjusting for incomplete registration with health-system access and other covariates). Disability weights for 220 unique health states were used to capture the severity of health loss. YLDs by cause at age, sex, country, and year levels were adjusted for comorbidity with simulation methods. We included uncertainty estimates at all stages of the analysis. FINDINGS: Global prevalence for all ages combined in 2010 across the 1160 sequelae ranged from fewer than one case per 1 million people to 350,000 cases per 1 million people. Abstract

Objective: To investigate the impact of generic and diabetes-specific self-management programs offered in a real world context. Methods: A quasi-experimental design with 12-week follow-up compared Living with a Chronic Condition and Living with Diabetes. Self-report data collected included: Self-management Knowledge and Skills; Health Related Quality of Life (HRQOL); Depression; Social Isolation; Loneliness; Self-efficacy; and Health Behaviours. Results: Participants (N = 458) in the two programs differed on almost all baseline measures. Both demonstrated statistically significant improvements in Self-management Knowledge and Skills, as well as reductions in depression. In addition to younger age, low HRQOL, high self-efficacy and Positive and Active Engagement in Life, were the clinical factors most likely to lead to improvements in HRQOL and self-efficacy. Changes in different characteristics predicted different outcomes. Conclusion: Both generic and disease-specific programs led to improved outcomes, despite the two programs attracting significantly different participants. Referral patterns also differed but GP referral rates were low for both. Practice implications: Positive participant outcomes can be achieved in real life clinical settings. While younger people with a positive attitude may appear to gain more, it is important to encourage people from low socio-economic status to enter these programs so that social inequalities in health are not worsened.
