207. Cardiac Involvement in Trichinellosis: A Case of Left Ventricular Thrombosis

We report the case of a 42-year-old woman who was admitted to the hospital for fever, chills, nausea, vomiting, fatigue, myalgia, and general muscle weakness. All these symptoms had occurred 3 weeks after the ingestion of inadequately cooked pork meat, subsequently confirmed to be infested with Trichinella spiralis . Laboratory results showed mild leukocytosis, inflammation, and mild liver and muscle cytolytic syndrome, all suggestive of trichinellosis. Echocardiography showed apical hypokinesis and an apical mass (likely a thrombus). The immunologic assessment for the presence of Trichinella antigens was positive. The outcome was favorable after treatment with an anticoagulant, an antiaggregant, prednisone, and mebendazole. Follow-up controls showed the absence of any symptoms and thrombus, with only mild electrocardiogram modifications still present.

The purpose of this study is to examine various aspects of patentability and protection of inventions with special reference to pharmaceutical inventions both in the European Union and Turkey.A well-functioning patent system is crucial for sustainable development in all fields of industry. Especially, the pharmaceutical industry is a field which relies almost exclusively on patent protection. Effective protection and implementation of patent rights is essential to encourage investment in pharmaceutical research and development.In the first section of this study, a brief explanation about the general characteristics of intellectual property rights is given. Due to intensive and fragmented European intervention in the field of intellectual property rights; legislative E.C. harmonisation measures and international treaties in fields of copyright, trademarks, industrial designs, integrated circuits and geographical indications are mentioned. Principal patent related provisions of Paris Convention, Strasbourg Agreement Concerning the International Patent Classification and TRIPS Agreement are introduced.In the following part important characteristics of the European Patent Convention and the procedure for obtaining a European patent including post-grant opposition procedure and appeal procedure are scrutinized. Requirements for patentability in respect of European patents; that is, novelty, inventive step and industrial application are described in detail. There is a presentation of the functioning and the advantages of the Patent Cooperation Treaty System and the developments on establishing Community Patent. The time-consuming and expensive procedure for drug development and special cases concerning the patentability of pharmaceutical product and process inventions are also discussed in detail. In the last part, general aspects of Turkish patent legislation and patent protection with special reference to pharmaceutical inventions in Turkey are studied. Features that appeared for the first time in the field of patent protection in Turkish legislation after the enactment of the Decree Law No.551, are mentioned.ÖZETBu çalışmanın amacı, özellikle Avrupa Birliği ve Türkiye'deki farmasötik buluşlarla ilgili olarak, buluşların patentlenebilirliğinin ve korunmasının çeşitli yönlerini incelemektir.İyi işleyen bir patent sistemi, sanayinin tüm alanlarında sürdürülebilir bir gelişme için çok önemlidir. Özellikle ilaç sanayinin büyük ölçüde patent korumasına bağlı bir alan olduğu söylenebilir. Farmasötik araştırma ve geliştirmeye yatırımı cesaretlendirmek için patent haklarının etkin korunması ve uygulanması asıldır.Bu çalışmanın birinci bölümünde , sınai mülkiyet haklarının genel nitelikleri hakkında kısa bir açıklama yer almaktadır. Sınai mülkiyet hakları alanındaki yoğun ve ayrıntılı Avrupa müdahalesinin varlığı nedeniyle; fikir ve sanat eserleri, markalar, endüstriyel tasarımlar, entegre devreler ve coğrafi işaretler hakkındaki Avrupa Topluluğu mevzuatı uyum tedbirlerinden ve uluslararası sözleşmelerden bahsedilmektedir. Paris Sözleşmesi, Milletlerarası Patent Sınıflandırmasıyla İlgili Strasburg Anlaşması ve TRIPS'teki patentlerle ilgili başlıca hükümler tanıtılmaktadır.Bir sonraki bölümde Avrupa Patent Sözleşmesinin önemli nitelikleri, ve ayrıca itiraz ve şikayet (temyiz) usullerini de içeren Avrupa patenti verilmesi usulü dikkatle incelenmektedir. Avrupa patentleri açısında patentlenebilirlik şartları; yenilik, buluş basamağı, sanayide uygulanabilirlik detaylı biçimde tanımlanmaktadır. Patent İşbirliği Anlaşmasının işleyişi, avantajları ve Topluluk patentine ilişkin gelişmelerden bahsedilmektedir.Zaman alan ve pahalı olan ilaç geliştirme prosedürü ve farmasötik ürün ve usul buluşlarının patentlenebilirliğine ilişkin özel durumlar detaylıca tartışılmaktadır.Son bölümde, Türk patent mevzuatının genel yönleri ve patent koruması üzerinde durulmakta, özellikle Türkiye'deki farmasötik buluşlar irdelenmektedir. 551 Sayılı Kanun Hükmünde Kararnamenin yürürlüğe girmesinde sonra Türk mevzuatında, patent koruması alanında ilk defa ortaya çıkan özelliklerden bahsedilmektedir

Atrial fibrillation (AF) is frequently found in association with rheumatic mitral valve disease. To study the interrelation of factors contributing to the risk of AF in patients with mitral stenosis, we examined a cardiac catheterization database of a series of 314 patients. Patients with AF were older, 53.4 ± 6.1 years versus 51.7 ± 7.2 years (p = 0.03), and had a lower cardiac index, 2.3 ± 0.6 L/min/m2 versus 2.6 ± 0.7 L/min/m2 (p = 0.0002), than patients in sinus rhythm at catheterization. The mitral valve area was significantly smaller in patients with AF than in patients in sinus rhythm, 1.2 ± 0.5 cm2 versus 1.6 ± 0.7 cm2 (odds ratio 1.40/0.25 cm2 decrease; p = 0.0001) as was mitral valve index. The pressure-AF association with the highest statistical significance was seen with mean right atrial pressure, 10.6 ± 4.9 mm Hg versus 7.6 ± 3.8 mm Hg (odds ratio 2.24; p < 0.0001). Other variables with significant positive associations by univariate analysis were pulmonary artery wedge pressure, pulmonary artery mean pressure, and pulmonary resistance. When stepwise logistic multiple regression analysis was performed, the results indicated that both severity of mitral stenosis and increased right atrial pressure were independently associated with AF in this population with mitral stenosis. After adjustment was performed for these variables, age was not independently associated with AF. (Am Heart J 1998:135:138-45.)

Introduction: To investigate the existence of peripheral and optic neuropathies in asymptomatic individuals with hepatitis C infection. Methods: Thirty consecutive patients who were followed in a hepatitis C outpatient clinic were recruited for electrophysiological evaluation together with 30 age- and gender-compatible healthy controls. All patients had a detailed neurological examination. The information regarding the disease duration and management with interferons were collected. Nerve conduction studies and visual evoked potentials (VEP) were recorded in all subjects. The results of the patient and control groups were statistically compared. Results: Of the patients with hepatitis C infection, 16 were females and 14 males. The mean age was 57.5 years, and the average disease duration was 6.43 years. The P100 latencies in the patient group were within normal limits, while the amplitudes were meaningfully small by comparison with the controls. There were some abnormalities in the nerve conduction studies of 15 patients. Sensorial neuropathy was detected in two patients, sensorimotor polyneuropathy in four, carpal tunnel syndrome in seven, and carpal tunnel syndrome and sensorimotor polyneuropathy as comorbid states in another two patients. The nerve conduction studies and VEP parameters were entirely normal in the control group. Conclusion: Hepatitis C-related neurological abnormalities may occur both in the central and peripheral nervous system. Mononeuritis multiplex, sensorial axonal neuropathy, and multiple mononeuropathies are some of the presentations of the peripheral nervous system involvement. The mode of infection is considered to be via vasculitic mechanisms. In addition, optic neuropathy is a known complication of interferon treatment. Autoantibodies, cytokines, chemokines, and cryoglobulins are accused to play roles in the pathogenesis. In this study, we investigated the involvement of the peripheral nervous system and optic nerves in a group of patients with hepatitis C. The results were in favor of peripheral nerve injury of various types and optic neuropathy of the axonal type. [ABSTRACT FROM AUTHOR]

During a period of 20 years, 8 patients with atrial fibrillation, without other cardiac abnormalities, resistant to electroconversion, but successfully defibrillated with quinidine, were examined. All were males, aged 40 to 50 years, working with electric current and exposed for many years to frequent electric shocks. The authors consider that chronic exposure to electric current induces a special form of atrial fibrillation, resistant to electric conversion. The inefficiency of the electroshock therapy in such cases might be explained by the increase of transthoracic impedance consecutive to repeated electrocutions.

The aim of this paper was to test the antifungal activity of volatile oils extracted from Anethi fructus, Anethi flores and Anethi folium using disk diffusion method, compared with an antifungal drug, miconazole, against Candida albicans reference strain. Three tests were made of antifungal activity, in identical conditions, at intervals of 1 week. We also tested the antifungal activity of presumed active components in the form of oily solutions, in concentrations equal to those of volatile oil from flower and leaf respectively, as well as the antifungal activity of volatile oil entrapped in a liposomal formulation. It was found that both flowers and fruits showed good antifungal activity against C. albicans strain, while the volatile oil of the leaves was not active. It was also observed that oily solution containing the appropriate concentration of carvone in the fruit oil has antifungal activity, but oily solutions containing appropriate concentrations of limonene from flowers, respectively fruits do not have antifungal activity. Mărculești Agricultural Research-Development Station is located in the south-east of the country, in the center of the Bărăgan plain. Throughout the years, it has developed and perfected the cultivation technologies for cereals, technical plants and fodder, in the pedoclimatic conditions of the area. According to Köppen climatic classification, Bărăgan region is part of the BSax province, the mean value for annual temperature is 10.9 0C and the mean annual value for precipitation is 480 mm. Fertile soils of chernozem type with a relatively good supply of nutrients are characteristic of this region. The present paper presents the climatic evolution in the South-eastern part of Bărăgan, based on comparison of the values of temperature and precipitation measured at the location during 2017-2022 with the multiannual mean values of these parameters. The results of the data analysis showed an increase in temperature and a decrease in precipitation, which are most likely to have negative effects on yields. Precipitations are of great importance for the autumn cultures because they provide the quick and uniform growing of the plants, and they ensure the accumulation of moisture in the soil during the winter season. Considering the obtained results, the following recommendations can be made: changes of the agricultural technology used for the main cultures in Bărăgan - autumn cereals, rape, maize and sunflower, early spring sowing, and the choice of hybrids cultivars with a medium vegetation period. Objective: The aim of this study was to assess the prevalence of no-reflow after coronary recanalization in anterior acute myocardial infarction with ST-T segment elevation. Methods: In a cohort of 80 consecutive patients with anterior acute myocardial infarction who were treated with primary angioplasty, we analysed angiographic and clinical characteristic of patients and the prevalence of pre-infarction angina. Successful angioplasty was defined as Thrombolysis In Myocardial Infarction (TIMI) grade 3 flow and residual stenosis less than 50% after the angioplasty procedure. The phenomenon of no-reflow is defined as inadequate myocardial perfusion through a given segment of the coronary circulation without angiographic evidence of mechanical vessel obstruction. Results: Successful angioplasty was achieved in 63 patients (78.75%). Mean age and gender were not different from patients with and without TIMI grade 3 flow. The no-reflow phenomenon was more frequently observed in patients with diabetes (29.41% vs 4.76%, p 0.0001). TIMI grade III flow was higher in the patients with pre-infarction angina (42.86% vs 11.76% p 0.02). Conclusion: The no reflow phenomenon in patients with anterior acute myocardial infarction treated with primary angioplasty is present in 21.25% of patients. Pre-infarction angina is associated with preservation of the microvasculature, reflected by reduced no-reflow. [ABSTRACT FROM AUTHOR]

- Abstract
Apium graveolens is a medicinal and aromatic plant used in pharmaceutical industry, cosmetic industry, gastronomy and traditional medicine. Aerial parts of Apium graveolens (stems and leaves) are rich in bioactive compounds (flavonoids, phenols, furanocoumarins, volatile oils, sesquiterpene alcohols, fatty acids, etc.). It is known for its hepatoprotective, antioxidant, anticancer, antidiabetic, anti-inflammatory, antimicrobial, analgesic, anti-spasmolytic, anti-infertility, cardiotonic activities. This is why studies in this direction have not yet ceased and are increasingly pursuing the discovery of new, unconventional methods for identifying, quantifying and isolating bioactive compounds from Apium graveolens waste. The aim of this study was to determine and correlate the total phenolics, total flavonoids, antioxidant activity and extraction yield from ethanolic extracts (70%) of stems and leaves waste with different unconventional extraction methods. Apium graveolens aerial parts waste were obtained after aqueous extraction of the leaves and stems. The extraction yield was expressed as function of freeze-dried extracts, and the extraction methods were ultrasound assisted extraction, microwave assisted extraction and accelerated solvent extraction. Regarding the chosen extraction methods, numerous studies have shown, especially in the case of microwave assisted extraction, the presence of bioactive compounds in plant waste, in a reasonable percentage, therefore this study aimed to identify an optimal extraction method also in the case of Apium graveolens aerial parts waste. Total phenols content was determined by Folin-Ciocalteu spectrophotometric method, the flavonoids content waste determined by aluminium chloride spectrophotometric method and the antioxidant activity was studied using DPPH (2,2-Diphenyl-1-picrylhydrazyl) spectrophotometric method. The findings of this work indicated a positive dependence between the extraction yield of the waste material and the considerable content of bioactive compounds still present (phenols and flavonoids) for all considered extraction methods.

Catalysis represents the cornerstone of chemistry, since catalytic processes are ubiquitous in almost all chemical processes developed for obtaining consumer goods. Nanocatalysis represents nowadays an innovative approach to obtain better properties for the catalysts: stable activity, good selectivity, easy to recover, and the possibility to be reused. Over the last few years, for the obtaining of new catalysts, classical methods—based on potential hazardous reagents—have been replaced with new methods emerged by replacing those reagents with plant extracts obtained in different conditions. Due to being diversified in morphology and chemical composition, these materials have different properties and applications, representing a promising area of research. In this context, the present review focuses on the metallic nanocatalysts' importance, different methods of synthesis with emphasis to the natural compounds used as support, characterization techniques, parameters involved in tailoring the composition, size and shape of nanoparticles and applications in catalysis. This review presents some examples of green nanocatalysts, grouped considering their nature (mono- and bi-metallic nanoparticles, metallic oxides, sulfides, chlorides, and other complex catalysts). Kronik hastalıklar hakkında farkındalık ve bilgilenme, erken tanı ve düzenli takip ile uzun vadedeki olumsuz sonuçların önlenmesinde ilk basamağı oluşturur. Bu çalışmada epilepsi, Parkinson hastalığı ve multipl sklerozun nörolojik hastalığı olmayan bir grupta ne denli tanındığı, bilgi düzeyinin yeterli olup olmadığı, yanlış bilgi ve inanışların ne düzeyde olduğu bir anket çalışması ile araştırıldı. Genel hastane ortamından rastgele alınan, 109’u kadın olan 18-85 yaş arası 230 bireye hastalığı tanıyıp tanımadıkları ve tanıyorlarsa neler bildikleri soruldu. Epilepsi oldukça iyi tanınan bir hastalıkken Parkinson hastalığı grubun yaklaşık yarısı, multipl skleroz 1/3’ü tarafından duyulmuştu. Bu bireylerin hastalık hakkında doğru bilgi sahibi olma oranları epilepsi ve multipl skleroz için %30’un altında, Parkinson hastalığı için ise % 80 bulundu. Anketi yanıtlayanlar en çok sağlık çalışanları ya da TV’den bilgilendiklerini ifade etmişlerdi. Anket grubunda en çok dikkati çeken bireylerin hastalıklar hakkında bilgili olduklarını düşünmelerine karşın birçok konuda kararsızlık ifade etmeleriydi. Sonuç olarak kronik nörolojik hastalıklar hakkında toplumsal bilgilendirme kampanyalarının artarak sürmesinin gerekli olduğu kanaatine varılmıştır, Awareness and sufficient knowledge of chronic diseases constitute the first step for preventing the long term consequences with early diagnosis and intervention in addition to regular followups. In this study the awareness and level of knowledge, wrong information or beliefs about epilepsy, Parkinson’s disease and multiple sclerosis were evaluated in a group of subjects without any neurological condition by means of a semi-structured hundred thirty persons randomly picked in a general hospital ward, were inquired on whether they were familiar with these diseases, and if they do what they knew. They were 18-85 years of age and 109 were females. Epilepsy was a quite wellknown disease while Parkinson’s disease was heard by only the half and multiple sclerosis one third. The level of sufficient knowledge was less than 30% for epilepsy and multiple sclerosis, however 80% for Parkinson’s disease. The respondents declared to possess knowledge professionals or TV. The most intriguing point was the high level of uncertainty of the respondents although they felt themselves sufficiently informed. Conclusively the informative campaigns on chronic neurological diseases must increasingly survive in order to achieve a better disease management

The presented project aims to provide practical solutions for ceramic and paper artifacts, based on new antifungal recipes, in order to reduce the impact of biodegradation of cultural heritage artifacts. The developed recipes, specific for each type of support material (ceramic and paper), will be based on the use of soft nanomaterials, applicable in the form of xerogels or spraying. The recipes will take into consideration the interaction between the developed materials and the support materials.
