(1) Background: Vitiligo is characterized by white patches on the skin caused by loss of melanocyte activity or the absence of these cells. The available treatments minimize the symptoms by retarding the process of skin depigmentation or re-pigmenting the affected regions. New studies are required for a better comprehension of the mechanisms that trigger the disease and for the development of more efficient treatments. Studies have suggested an autoimmune feature for vitiligo, based on the occurrence of other autoimmune diseases in vitiligo patients and their relatives, and on the involvement of genes related to the immune response. (2) Methods: We evaluated, by massive parallel sequencing, polymorphisms of the HLA-G gene in vitiligo patients and control samples, to verify if variants of this gene could influence the susceptibility to vitiligo. (3) Results: We detected an association with non-segmental vitiligo regarding the haplotype Distal-010101a/G*01:01:01:01/UTR-1, adjusting for population stratification by using ancestry-informative markers (AIMs). (4) Conclusions: It remains unclear whether the HLA-G variants associated with vitiligo were detected because of the high linkage disequilibrium (LD) with HLA-A*02, or if the HLA-A variants previously reported as associated with vitiligo were detected because of the high LD with HLA-G*01:01:01:01/UTR-1, or if both genes jointly contribute to vitiligo susceptibility. Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) Esta pesquisa pretende esclarecer um tema ainda pouco mapeado pela historiografia, a história dos núcleos urbanos do Vale do Paraíba durante o apogeu do café no século XIX. Faremos isso através de Pindamonhangaba, cidade que sofreu marcante transformação no período com investimentos significativos de capital no tecido urbano a partir de 1840. Historiamos esse processo para poder compreender as especificidades e traços, tanto do ponto de vista da arquitetura como das formas de produção do espaço urbano edificado. A riqueza do café foi fundamental para a consolidação de um projeto urbanístico promovido pelas elites que visava cingir a paisagem com construções em conformidade com os estilos arquitetônicos trazidos pelos mestres de obras estrangeiros. Atuando no núcleo urbano estava a Câmara Municipal, dirigindo obras, fiscalizando, fazendo leis. Além das obras públicas, havia os suntuosos palacetes e os novos sobrados e interessa-nos entender em que medida já divulgavam os novos modos de morar. Diante disso, a presente pesquisa tem como objetivo entender como a Câmara Municipal de Pindamonhangaba mediou às transformações do espaço público dentre os anos de 1840, com a construção do cemitério público, até 1880, quando chegou a ferrovia. Pretendemos contribuir com o debate sobre a urbanização no Brasil império, entendendo como foi esse processo na cidade antes da chegada da ferrovia. A Câmara Municipal, composta principalmente por fazendeiros e profissionais liberais, exerceu importante função, como o levantamento de verbas, fiscalização e contratação de mão de obra para a construção do espaço urbano, escravos, arquitetos, trabalhadores não especializados. Ricos fazendeiros decidiram investir suas fortunas em Pindamonhangaba e não em São Paulo ou no Rio de Janeiro, como a maioria fazia. Interessava um projeto de poder simbólico local que acreditamos vale a pena ser esmiuçado. 2018/21759-9

Estudos sucessivos de nosso laboratório sugeriram que a concentração intermediária de 500ppm na ração poderia ser citotóxica e potencialmente cancerígena ao urotélio de ratos. A proposta deste estudo foi avaliar as alterações uroteliais induzidas pela exposição subcrônica e crônica (20 e 84 semanas, respectivamente) ao diuron na concentração de 500ppm. Cento e trinta e dois ratos Wistar machos foram distribuídos em grupos controle ou diuron e avaliados após 20 ou 84 semanas de exposição. Os rins e as bexigas foram processados para análise histológica, as bexigas para análise de microscopia eletrônica de varredura e para avaliação da expressão dos genes Gpc3, Ugt1a6, Nfe2l2, Tgfb1 e Gstm1 por RT-PCR quantitativo em tempo real. Apenas após 84 semanas de exposição, a incidência de hiperplasia simples foi significativamente maior na bexiga urinária (10/18) e na pelve renal (23/25) nos animais expostos ao diuron quando comparado aos do grupo controle (2/22 e 9/30, respectivamente). Em ambos os períodos de exposição, o diuron induziu diminuição de expressão do gene Gpc3, envolvido na proliferação celular, e aumento de expressão do gene Ugt1a6, relacionado com o metabolismo de xenobióticos. A expressão do gene Nfe2l2, relacionado ao estresse oxidativo, aumentou apenas no período de 20 semanas. Neste estudo, embora na 20ª semana a concentração de diuron 500ppm tenha se comportado morfologicamente como nível de efeito adverso não observável (NOAEL), as alterações na expressão gênica mostraram perturbações relacionadas à proliferação celular, estresse oxidativo e metabolismo de xenobióticos. Nossos resultados sugerem que a exposição dietética ao diuron 500ppm por um longo período foi citotóxica, suficiente para a indução de alterações transcricionais e para o desenvolvimento de hiperplasia urotelial, podendo ser considerada como o menor nível de efeito adverso observável (LOAEL) para exposição crônica em ratos. Durante a última década, nosso laboratório tem realizado estudos sobre a toxicidade urotelial do diuron PT fornecido pela ração a ratos Wistar machos. Porém, o diuron na forma de produto formulado (PF) ou comercial, composto por 50% de diuron PT e 50% de ingredientes inertes, não tem sido avaliado em ratos. Em lagartos P. sicula machos expostos ao diuron PF à 50% na concentração de 1100ppm na ração ou na água de beber, durante 3 semanas, houve toxicidade direta no sistema reprodutivo com alteração na espermatogênese, morfologia do epidídimo e diminuição dos níveis plasmáticos dos hormônios sexuais, sendo os efeitos mais severos nos grupos que receberam diuron na água de beber. O presente estudo objetivou comparar o potencial tóxico do diuron PF ou PT à 1250ppm oferecido pela ração ou água de beber no urotélio e nos parâmetros reprodutivos de ratos. Cinquenta ratos Wistar machos de seis semanas de idade foram distribuídos em cinco grupos (ração basal, ração com PF, água de beber com PF, ração com PT, água potável com PT) e expostos ao diuron à 1250ppm por 13 semanas. Na eutanásia foram avaliados a morfologia, concentração, motilidade e viabilidade dos espermatozoides. Os rins, fígado, baço, testículos e epidídimos foram retirados e pesados; os rins e testículos processados para histologia. A bexiga foi seccionada ao meio e processada para análise histológica e para microscopia eletrônica de varredura (MEV). Todos os grupos expostos ao diuron tiveram diminuição do peso corpóreo superior a 10%, comparados ao controle, indicando que a máxima dose tolerada (MDT) foi excedida. O consumo de ração e de água foi menor em todos os grupos expostos ao diuron. Não houve diferença nos pesos absolutos e relativos de órgãos entre os grupos. Em todos os grupos expostos ao diuron, os achados microscópicos incluíram hiperplasia urotelial simples (HS) na pelve renal e na bexiga à MEV. Diuron PF e PT induziram alterações na morfologia do esperma e diminuição da motilidade, concentração e viabilidade dos espermatozoides. Esse estudo mostrou que o diuron é nocivo ao urotélio e aos parâmetros reprodutivos de ratos Wistar machos, independentemente de estar na forma técnica (PT) ou formulada (PF) ou se administrado pela ração ou pela água de beber. Wistar rats exposed during two years to 2500ppm dietary diuron technical product (TP) (3-(3,4- dichlorophenyl)-1,1-dimethylurea) developed high incidence of urothelial tumors, being classified by the U.S. Environmental Protection Agency as a ‘‘known/likely human carcinogen’’. During the last decades our laboratory conducted studies on the urothelial toxicity of the diuron (TP) provided through diet to male Wistar rats. The diuron formulated product (FP) composed by 50% diuron TP and 50% inert ingredients, has not been evaluated in rats. The diuron (FP) was harmful to reproductive system of male P. sicula lizards exposed to 1100ppm during 3 weeks; the toxic effects were more prominent in the groups receiving diuron FP in drinking water. This study aimed to compare the potential toxicity of diuron TP or FP 1250ppm offered through diet or drinking water on the urothelium and reproductive parameters of rats. Fifty six-week old Wistar male rats were allocated to five groups (basal diet, FP diet, FP drinking water, TP diet, TP drinking water) and exposed to 1250ppm diuron for 13 weeks. At the end, body weight and weight of kidneys, liver, spleen, testes and epididymis, histopathology of the kidneys and testes and sperm parameters were evaluated. Urinary bladders were bisected and processed for histologic and scanning electron microscopic (SEM) analyses. At the end of the study, all groups exposed to diuron had decreased body weight gain compared to control greater than 10%, indicating that for this experiment the dose exceeded the maximum tolerated dose (MTD). Food and drinking water consumption was lower in the groups exposed to diuron. There were no differences in the absolute and relative organ weights among the groups. In all groups exposed to diuron, microscopic findings included simple urothelial hyperplasia (SH) in the kidney pelvis and in the urinary bladder. By SEM, the urinary bladder had superficial cytotoxicity in all groups exposed to diuron. Diuron FP and TP induced changes in the sperm morphology and concentration, and decreased sperm motility and viability. This study showed that diuron FP as well as diuron TP is harmful to the urothelium and to the reproductive parameters of male Wistar rats, regardless of whether in a diet or drinking water.

Diante disso, conclui-se que a identificação do padrão morfológico não é suficiente para o estabelecimento do prognóstico, e que existe a necessidade da identificação de novos biomarcadores imunoistoquímicos que indiquem moléculas sinalizadoras importantes para proliferação e sobrevivência das células neoplásicas, e que possam ser alvos de modalidades terapêuticas oncológicas. A ocorrência de alterações dos mecanismos regulatórios genéticos da apoptose pode resultar em uma hiperativação das proteínas antiapoptóticas, com a perda da sensibilidade aos sinais de morte fisiológica, evitando-se a ocorrência da apoptose e resultando, desse modo, na preservação de células geneticamente alteradas com consequente aumento no seu número. Esse é um evento crítico para o início do crescimento tumoral, com repercussões no prognóstico e na resistência ao tratamento oncológico. Dessa forma, o entendimento da maquinaria das vias do processo apoptótico é de crucial importância na avaliação prognóstica do processo neoplásico, pois suas moléculas são importantes biomarcadores da proliferação e sobrevivência celular. Objetivos: Avaliar a relação entre as proteínas inibidoras do processo apoptótico (IAPs) e seu inibidor Smac/DIABLO no carcinoma mamário ductal invasivo, sem outra especificação (SOE). Analisar, também, sua correlação com os marcadores imunoistoquímicos usuais, aplicados para a avaliação prognóstica, índice apoptótico e taxa de sobrevida dos pacientes. Material e Métodos: Foram confeccionados blocos de parafina de TMA com tecido de câncer mamário ductal invasivo sem outra especificação (SOE), extraído de pacientes operadas no Hospital das Clínicas da Faculdade de Medicina de Botucatu, entre 1980 e 2000. Estes foram submetidos à reação imunoistoquímica para Survivina, XIAP, Smac, Caspase 3 clivada (índice apoptótico) e Ki67 (índice de proliferação celular), RE, RP, HER2, P53. Dois patologistas realizaram a leitura das lâminas e obteve-se escores de intensidade de imunoexpressão e/ou percentuais de positividade celular. Esses escores foram correlacionados entre as proteínas apoptóticas, com outros marcadores imunoistoquímicos prognósticos usuais, com os dados de sobrevida dos pacientes, com o grau histológico e com o perfil molecular especifico do carcinoma ductal invasivo. Resultados: Ocorreu uma correlação positiva entre Smac, Survivina e XIAP, houve maior intensidade de imunoexpressão dos marcadores de apoptose nos tumores de maior grau e uma correlação de imunoexpressão inversa entre Smac e um status hormonal positivo. Um estímulo pró-apoptótico pareceu ser dominante no carcinoma mamário de subtipo molecular HER2, quando comparado a outros subtipos. XIAP foi a única proteína apoptótica estudada que guardou correlação com a taxa de mortalidade. Conclusão: A detecção de uma correlação estatística significante entre as proteínas apoptóticas, corrobora a integração múltipla que ocorre entre essas durante o processo apoptótico no carcinoma mamário invasivo, sem outra especificação (SOE). Estudos adicionais das proteínas apoptóticas com uma maior amostragem de pacientes serão necessários para o estabelecimento de valores prognósticos mais acurados de sobrevida. Most of malignant breast tumors exhibit morphological aspect of invasive ductal carcinoma, not otherwise specified (NOS) carcinoma, that can present different prognosis. It concludes that the identification of the morphological type of the breast cancer is not enough to establish tumor prognosis. So there is the need to recognize new immunohistochemistry biomarkers that can identify important molecular signaling proteins of neoplastic cell proliferation and survival that can be used as target of oncological therapeutic. Changes in these regulatory mechanisms that favors the hiperactivation of antiapoptotic proteins can set the loss of the physiological signs of cell death avoiding the occurrence of apoptosis and preserving genetic modified cells. This is a critical event to the beginning of tumor growth which may further lead to repercussions in prognosis and cancer resistance to therapy. The knowledge about the different pathways to apoptosis is crucial to evaluate the prognosis of the neoplastic process. Objectives: To study the relation between the inhibitors of apoptosis proteins (IAPs) and their antagonist, the protein SMAC/DIABLO, in invasive ductal breast carcinoma and also evaluate their correlation with the usual prognostic immunohistochemical markers, Apoptotic Index and clinical prognostics factors. Design: TMA paraffin blocks were made with invasive ductal breast carcinoma tissue samples of patients operated at the Botucatu School of Medicine Hospital from 1980 to 2000. Those samples were submitted to immunohistochemical reactions to Survivin, XIAP, cleaved Caspase 3 (apoptotic index) and Ki67 (cell proliferation index), RE, RP, HER2, P53. Two experienced pathologists analyzed the microscope slides and retrieved intensity scores and/or the percentage of positive cells of immune-expression. Those scores were interrelated and with IA, anti-cleaved caspase 3, the other usual immunohistochemical biomarkers and survival patients records, as well as the specific type and grade of the invasive ductal carcinoma. Results: There was a positive correlation between SMAC, Survivina and XIAP, as well as a higher immunoexpression intensity of apoptosi’s markers with high grade tumor, and a inverse immunoexpression correlation between SMAC and positive progesterone hormonal status. The pro-apoptotic stimulus seemed be dominant in breast carcinoma HER2 molecular subtype when compared to other subtypes.A pro-apoptotic stimulus seemed be dominant in breast carcinoma HER2 molecular subtype when compared to other subtypes.XIAP was the only one apoptotic protein studied that played correlation with mortality rate. Conclusion: The detection of a statistically significant correlation between the apoptotic proteins corroborates the multiple integration that occurs between these during the apoptotic process in invasive mammary carcinoma with no other specification. Further studies of apoptotic proteins in a large survey will be necessary to establish more accurate values of prognostic survival.

Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) O propósito de nossa investigação foi analisar a criação da Colônia Militar do Jataí e dos Aldeamentos Indígenas no vale do rio Tibagi, no norte da Província do Paraná. A referida região, até então pouco conhecida e habitada por grupos indígenas, sofreria, a partir de 1855, um processo de expansão das suas fronteiras de povoamento. Nosso recorte temporal abarca o período de 1845 a 1897, com o intuito de abranger o período que compreende desde as primeiras viagens de exploração realizadas pelo empreendedor João da Silva Machado na região, até o momento de emancipação da Colônia Militar do Jataí (1897). A criação de colônias militares fez parte de uma política imperial, oficializada no ano de 1850, e visava estabelecer a ordem e o controle sobre a população considerada perigosa: índios, escravos fugidos e criminosos. Naquele momento também era iminente a preocupação em consolidar as fronteiras do território nacional e constituir um país soberano. Nesse sentido, foram estabelecidas mais de vinte desses empreendimentos em regiões limítrofes com outros países ou, ainda, no centro das matas, para proteger vias de comunicação fluviais e terrestres. Somente na Província do Paraná foram instaladas três colônias militares, sendo a de Jataí a primeira. Logo, nossa investigação diz respeito não apenas à história regional, mas, sobretudo, trata de um momento de transformações políticas e econômicas no conjunto do Império do Brasil, que pretendeu viabilizar a consolidação do Estado. The purpose of our research is to analyze the creation of the Military Colony of Jataí and of the Indigenous Villages in the valley of the Tibagi River, in the north of the province of Parana, in the nineteenth century. That region was little known until then and inhabited by indigenous groups, and has suffered, after 1855, a process of expansion their boundaries with the start of the settlement on the area. Our timeframe cover the period from 1845 to 1897, in order to include since the first voyages of exploration undertaken by the entrepreneur João da Silva Machado to the region, until the moment of emancipation of the MC of Jataí. The creation of military colonies was part of an Imperial policy, made official in 1850, and aimed to establish order and control over the population considered dangerous: indians, runaway slaves and criminals. At that time, was also imminent the concern to consolidate the boundaries of national territory for constitute a sovereign country. In that sense, more than twenty of these enterprises (Military Colonies) have been established near borders with other countries or in the middle of the woods, in order to protect the communications through rivers and land. Only in the province of Parana were installed three military colonies (The MC of Jataí has been the first). Therefore, our research does not address only to regional history, but, above all, rescues a time of political and economic transformations in the Empire of Brazil, which were intended to enable the consolidation of the national State. Different scanning electron microscopy (SEM) processing methods resulted in differing observations of early urothelial changes induced by exposure to this herbicide. This study evaluated the influence of SEM processing methods on the detection of urothelial alterations induced by diuron. Male Wistar rats were divided into 3 groups: control (basal diet), 7.1% Sodium Saccharin (NaS) (positive control) and 2500 ppm diuron, and fed for 7 days or 15 weeks. Urinary bladders were fixed with Bouin’s or glutaraldehyde fixative and processed by critical point drying (CPD) or by hexamethyldisilazane (HMDS) for comparison of the cytotoxic and proliferative changes induced by diuron in the urothelium. Histological and cell proliferation evaluations were also performed. After seven days, no differences in the incidence of histological urothelial lesions or labeling indices were detected among the groups; however, the incidence of urothelial lesions after 15 weeks was significantly increased in the animals fed diuron or NaS. After seven days or 15 weeks, the severity of urothelial alterations was significantly higher in animals fed with diuron. Both fixative and drying methods allowed for the identification of prenecrotic swollen superficial cells in the urothelium after seven days exposure to 2500 ppm diuron. Our results confirmed that the presence of prenecrotic swollen cells in the urothelium is an early key event due to diuron cytotoxicity and is not an artifact related to the processing methods used

Accordingly, USEPA categorized Diuron as a “known/likely” human carcinogen. The accepted non-genotoxic mode of action (MOA) of Diuron encompasses urothelial necrosis induced by direct cytotoxicity, followed by regenerative cell proliferation and sustained urothelial hyperplasia that may favor neoplasia development. Scanning electron microscopy (SEM), light microscopy and labeling index are essentials tools for identification and classification of cytotoxic and proliferative changes in the bladder. The present study evaluated the dose-response of Diuron regarding urothelial lesions. Sixty male Wistar rats were fed Diuron for 20 weeks mixed in the diet at 0, 60, 125, 500, 1,250, or 2,500 ppm. Simple hyperplasia was significantly increased in the Diuron 1,250 and 2,500 ppm groups, and the cell proliferation at 2,500 ppm group. By SEM, the incidences and severity of lesions were significantly greater in the 500 and 1,250 ppm. Although numerically increased, the incidence of lesions in the 2,500 ppm group did not differ significantly from the control. The present study documented a doseresponse influence of Diuron on the rat urothelium, with a no observed effect level (NOEL) of 125 ppm.

Desta forma, é necessário acompanhar rigorosa e periodicamente os pacientes acometidos por tal neoplasia, bem como empregar técnicas sensíveis para a detecção precoce da doença, tanto em pacientes submetidos à ressecção do tumor de bexiga, quanto em pacientes considerados como grupo de risco para o desenvolvimento do CCT. O presente estudo tem por objetivo utilizar o teste do micronúcleo como ferramenta para a avaliação de danos cromossômicos em células uroteliais obtidas por lavado vesical de pacientes com história de CCT. A freqüência de células uroteliais micronucleadas foi avaliada em 77 pacientes (não tabagistas, tabagistas atuais e ex-tabagistas) sem ou com história de CCT, mas com diagnóstico atual negativo para neoplasia. Foi detectado aumento significativo (P=0,003) de células micronucleadas somente nos pacientes não fumantes e com história de CCT, quando comparados aos indivíduos do grupo controle (não fumantes e sem história de CCT). Não foram detectados efeitos do tabagismo na freqüência de células micronucleadas, e nem associação desse hábito com o grau do tumor. Concluindo, indivíduos não tabagistas com história de neoplasia urotelial apresentaram freqüência aumentada de micronúcleos em células esfoliadas da bexiga, mesmo após a ressecção do tumor. Portanto, o epitélio citologicamente normal da bexiga de indivíduos com história de CCT, pode apresentar células geneticamente instáveis, que poderiam conferir um risco aumentado para desenvolvimento neoplásico. The main feature of transitional cell carcinoma (TCC) is the high recurrences rates of superficial carcinomas. Therefore, patients must be monitored regularly by periodic cystoscopies. The employment of sensible techniques are important for detecting bladder cancer and early disease in patients undergoing tumor resection and individuals with high risk for tumor development. To evaluate whether cytogenetic disorders can be evolved in the tumor development and recurrences, the frequency of micronucleated cells (MNC) was established in non-neoplastic exfoliated bladder cells from patients with history of TCC. Seventy-seven patients with and without history of bladder cancer, either smokers or non-smokers, with current diagnosis negative for neoplasia were included. The results showed a significant increase (P < 0.01) of MNC in patients with history of TCC and non-smokers when compared to counterpart group (without history of TCC and non-smokers). However, the same association was not observed in patients with TCC and smokers and in patients without history of TTC and smokers. Furthermore, was not observed correlation between smoking habits and tumor grade. These results suggesting that non smokers with history of urothelial tumor had an increase of MNC even after tumor resection. Thus, the macroscopically normal looking urothelium of patients of history of TCC, still could be harbored genetically instable cells that can be related to high risk for neoplastic development.

Objective: to understand how religiosity can influence the health of individuals diagnosed with mental disorders, based on comprehensive care., Methods: this is an integrative literature review, with the inclusion of articles in Portuguese, English and Spanish, between 2010 and 2018., Results: the critical analysis and qualitative synthesis of the 24 selected studies were categorized into two subtopics: The influence of religiosity in promoting comprehensive mental health care; Mental health versus religiosity: influencing conditions for effective access to comprehensive care., Final Considerations: a positive influence of religiosity was identified in the lives of individuals diagnosed with mental disorders; however, evidence shows that health teams do not feel comfortable and prepared to work with religiosity as an expression of spirituality. This being one of the dimensional aspects of health, it can be inferred, on the results, the existence of this gap in the comprehensive care approach.

- Abstract
The diagnosis of chronic pulmonary aspergillosis (CPA) depends on the radiologic image and the identification of specific antibodies. The present study aimed to evaluate accuracy parameters of enzyme-linked immunosorbent assay (ELISA) and of the determination of serum galactomannan level in the diagnosis of patients with CPA, comparing these results with the double agar gel immunodiffusion (DID) test. In addition, the prevalence of cross-reactivity and the serological progression after treatment were evaluated by comparing DID and ELISA. Six study groups were formed: G1: 22 patients with CPA, 17 of whom had Aspergillus fungus ball, one chronic cavitary pulmonary aspergillosis (CCPA) and four chronic fibrosing pulmonary aspergillosis (CFPA); G2: 28 patients with pulmonary tuberculosis (TB); G3: 23 patients with histoplasmosis (HST); G4: 50 patients with paracoccidioidomycosis (PCM); G5: 20 patients with cryptococcosis (CRC); and G6: 200 healthy controls. Serum antibodies were measured by DID and ELISA, with two antigen preparations--Aspergillus fumigatus (DID1, ELISA1) and a pool of A. fumigatus, A. flavus and A. niger antigens (DID2, ELISA2). The Platélia Aspergillus Enzyme Immunoassay (EIA) kit was used to measure galactomannan. The cut-off points of ELISA were determined for each antigen preparation and for the 95% and 99% confidence intervals. Despite the low sensitivity, DID was the technique of choice due to its specificity, positive and negative predictive values and positive likelihood ratio-especially with the antigen pool and due to the low frequency of cross-reactivity. ELISA1 and a 0.090 cut-off showed high sensitivity, specificity and negative predictive value, but a high frequency of cross-reactivity with CRC. The best degree of agreement was observed between ELISA1 and ELISA2. The detection of serum galactomannan showed high sensitivity, comparable to ELISA2. The immunodiffusion test showed an excellent relationship with the progression after treatment, which made it the reaction of choice for patient follow-up.

Diuron [3-(3,4-dichlorophenyl)-1,1-dimethylurea] is a herbicide that induced urothelial tumors in the urinary bladder of Wistar rats fed 2500ppm during a long-term study. The currently suggested non-genotoxic mode of action (MOA) of diuron encompasses in succession urothelial necrosis induced by direct cytotoxicity, regenerative cell proliferation and sustained urothelial hyperplasia that increases the likelihood of neoplasia development. This study evaluated the dose-response profile of urothelial histological and ultrastructural lesions induced by diuron. Sixty male Wistar rats were fed ad libitum diuron mixed in the diet at 0, 60, 125, 500, 1250, or 2500ppm for 20 weeks. The incidences of urothelial simple hyperplasia and the cell proliferation index were significantly increased in the diuron-fed 1250 and 2500ppm groups. By scanning electron microscopy, the incidences and severity of lesions were significantly increased in the 500 and 1250ppm groups. The incidences of urothelial hyperplasia in the kidney pelvis were significantly increased in the 500, 1250 and 2500ppm groups. - Abstract
Background: The present study intends to investigate the effects of anti-IL2 receptors (anti-IL2R) vs. lymphocyte-depleting agents in the early steroid withdrawal (ESW) scheme., Methods: This is a retrospective cohort of 167 consecutive adult renal transplant recipients. Immunosuppression was based on tacrolimus and mycophenolate mofetil. Antibody induction therapy was carried out with lymphocyte-depleting agent (thymoglobulin) or anti-IL2R (Basiliximab or Daclizumab). ESW protocol was performed by administering intravenous methlyprednisolone as follows: 500 mg on day 0, 250 mg on day 1, 125 mg on day 2, 60 mg on day 3, and then stopped., Results: Among the 167 studied patients, 79 (47.3%) received anti-IL2R and 88 (52.7%) received thymoglobulin induction. Significantly fewer episodes of acute rejection were seen at one year in patients treated with thymoglobulin as compared to anti-IL2R (25.6% vs. 11.4%, p=0.01). At five years, a significant difference in graft survival was observed in anti-IL2R-treated patients compared with thymoglobulin (83.5% vs. 95.5%, p=0.01). Multivariate analysis disclosed that female sex, antibody induction therapy using thymoglobulin and a trough tacrolimus level higher than 10 were protective factors against acute rejection, while there was a trend to increased risk of acute rejection at first year post-transplantation in patients presenting delayed graft function (DGF). Antibody induction was independently associated with patient and graft survival at five years (OR 0.213, 95% CI 0.046-0.991, p=0.04)., Conclusion: ESW scheme seems to be safe and its use is beneficial since there are fewer adverse effects. Thymoglobulin induction therapy is associated with fewer rejection episodes. Diuron (3-(3,4-dichlorophenyl)-1,1-dimethylurea) is a substituted urea herbicide that induces rat urinary bladder urothelial tumors at high dietary levels (2500 ppm). The specific mode of action and molecular alterations triggered by diuron, however, have not been clarified. The present study evaluated the dose-dependent effects of mucosal alterations and transcriptional changes in the urinary bladder of rats exposed to diuron. Six-week-old male Wistar rats were treated with 0, 60, 125, 1250, and 2500 ppm of diuron in the diet for 20 weeks. Histologic examination showed urothelial hyperplasia present in rats treated with either 1250 or 2500 ppm of diuron but not 60 or 125 ppm. Comprehensive gene expression analyses of urothelial cell RNA were conducted using Affymetrix microarrays. The numbers of differentially expressed transcripts between each treatment group and control increased with diuron dose. Based on similar histology and gene expression responses, the treatment groups were regrouped into a high-dose (1250 and 2500 ppm) and low-dose group (60 and 125 ppm). These data suggest that persistent exposure to high dietary concentrations of diuron induces oxidative stress, increases cellular metabolism, and enhances cell death that is associated with sustained urothelial hyperplasia.

- Abstract
An improved method for the Agrobacterium infiltration of epicotyl segments of 'Pineapple' sweet orange [Citrus sinensis (L.) Osbeck] and 'Swingle' citrumelo [Citrus paradisi Macf. X Poncirus trifoliata (L.) Raf.] was developed in order to increase transformation frequency. Sonication-assisted Agrobacterium-mediated transformation (SAAT), vacuum infiltration, and a combination of the two procedures were compared with conventional Agrobacterium-mediated inoculation method ('dipping' method). It was observed that the morphogenic potential of the epicotyl segments decreased as the duration of SAAT and vacuum treatments increased. Transient GUS expression was not affected by the different SAAT treatments, but it was significantly enhanced by the vacuum infiltration treatments. The highest transformation efficiencies were obtained when the explants were subjected to a combination of SAAT for 2 s followed by 10 min of vacuum infiltration. PCR and Southern blot analysis of the uidA gene were used to confirm the integration of the transgenes. The transformation frequencies achieved in this study (8.4% for 'Pineapple' sweet orange and 11.2% for 'Swingle' citrumelo) are the highest ones reported for both cultivars.

- Abstract
Objective: The objective was to analyze the effect of raloxifene on the vaginal epithelium of postmenopausal women., Study Design: In this non-randomized clinical trial, 80 women (mean age = 60.6 years) were prospectively studied. Forty patients received 60 mg/day of raloxifene (RG), and 40 women constituted a non-treated control group (CG), paired by age and time since menopause. The treated group consisted of patients with osteoporosis of the lumbar spine. Those with a diagnosis of infection in the lower genital tract and using hormone therapy (HT) up to 6 months prior to the study were excluded. Vaginal smears were collected at baseline and after 6 months of intervention. The vaginal maturation value (VMV) was determined, and counts of superficial, intermediate and parabasal cells were performed. Smears were analyzed by only one cytopathologist who was blinded to patient data. The t-test, Wilcoxon test, and Chi-Squared test were used in the statistical analysis., Results: The study groups were homogeneous regarding age, time since menopause, parity, HT use, smoking, and body mass index. No statistically significant differences were observed in VMV median values (RG, 39.7 and 35.7; CG, 50.0 and 50.0, respectively) or in the percentage of superficial, intermediate and parabasal cells between the groups at baseline and after 6 months (p>0.05). There was no significant correlation between VMV and age, time since menopause, previous HT use, or body mass index, in either of the groups., Conclusion: Treatment with raloxifene for 6 months has no effect on the maturation of the vaginal epithelium in postmenopausal women with osteoporosis.

- Abstract
Objective: To investigate the efficacy of soy isoflavone on climacteric symptoms in postmenopausal women., Design: In this double-blind, randomized, placebo-controlled study, a total of 80 women (mean age = 55.1 years), who reported 5 or more hot flush episodes per day, were randomized to receive either 250 mg of standardized soy extract (Glycine max AT) a total of 100mg/day of isoflavone (n = 40) or placebo (n = 40). Exclusion criteria included: contra-indication for hormone therapy (HT), chronic gastrointestinal diseases, and users of HT within the preceding 6-months. For 10-months, climacteric symptoms were evaluated using a score card and the menopausal Kupperman index. Compliance and safety were also assessed. At baseline and the end of the study, lipid and hormonal profiles, as well as vaginal, mammographic and ultrasonographic parameters were measured. The t-test, Wilcoxon test and ANOVA were used in the statistical analysis., Results: At baseline, the mean number of hot flushes was 9.6 +/- 3.9 per day in the isoflavone group and 10.1+/-4.9 in the placebo group (p>0.05). After 10 months, there was a significant reduction in frequency of hot flushes among isoflavone users when compared to those on placebo (3.1 +/- 2.3 and 5.9 +/- 4.3, respectively) (p<0.001). Kupperman index mean values showed a significant reduction in both groups. However, soy isoflavone was significantly superior to placebo, in reducing hot flush severity (69.9% and 33.7%, respectively) (p<0.001). Endometrial thickness, mammography, vaginal cytology, lipids and hormonal profile did not change in both groups. No serious adverse event related to isoflavone treatment was reported., Conclusions: The soy isoflavone extract exerted favorable effects on vasomotor symptoms and good compliance, providing a safe and effective alternative therapeutic for postmenopausal women.

- Abstract
The modifying potential of ginger on the development of preneoplasia and tumors in the male Wistar rat urinary bladder was investigated in a 36-week-long initiation-promotion assay for chemical carcinogenesis. Groups G1 to G3 were given 0.05% N-butyl-N-(4-hydroxybutyl)-nitrosamine (BBN) in drinking water for 5 weeks and a 3% uracil meal for the subsequent 3 weeks. Groups G4 and G5 were treated with 3% uracil only for the same period. After these steps, groups G2, and G3 and G4 were fed for 26 weeks a ginger extract mixed at 0.5 and 1.0% in a basal diet, respectively. Thirty six weeks after the beginning of the experiment all rats were killed. The multiplicity of urothelial lesions (hyperplasia and neoplasia) was significantly lower (P = 0.013) in group G3 than in groups G1 and G2. The results suggest that 1.0% ginger meal exerts a protective effect on the post-initiation stage of rat chemically-induced urothelial carcinogenesis.

- Abstract
In order to determine if patients with a history of previous urothelial cell carcinoma (UCC) but with current normal urinary cytology have DNA damage in urothelial cells, the single-cell gel electrophoresis (comet) assay was conducted with cells obtained by urinary bladder washings from 44 patients (28 with a history of previous UCC). Increased DNA damage was observed in cytologically "normal" urothelial cells of patients with a history of UCC when compared with referents with no similar history and after correcting the data for smoking status and age (P < 0.018). Increased DNA damage also correlated with the highest tumor grade, irrespective of time or course of the disease after clinical intervention (Kendall tau correlation, 0.37, P = 0.016). Moreover, aneuploidy, as assessed by DNA content ratio (DCR; 75th/25th percentile of total DNA fluorescence of 50 comets/patient) was unaltered by smoking status, but increased with UCC grade: 1.39 +/- 0.12 (median +/- 95% confidence interval; referents); 1.43 +/- 0.11 (Grade I UCC; P = 0.264, against referents); 1.49 +/- 0.16 (Grade II UCC; P = 0.057); 1.57 +/- 0.16 (Grade III UCC; P = 0.003). Micronucleated urothelial cells (MNC) were also scored on Giemsa-stained routine cytological smears and were found not to correlate with DNA damage or DCR. MNC frequencies were higher for patients with a history of UCC and/or smoking than referents with neither history, but there was no statistical difference between groups. Taken together, these results suggest that the normal-appearing urothelium of patients resected for UCC still harbor genetically unstable cells., (Copyright 2002 Wiley-Liss, Inc.)
