211. Cognitive-behavioral treatment of depressed inpatients: six- and twelve-month follow-up

Abstract

Forty-five hospitalized patients who suffered from severe depression were randomly separated into three groups. All groups were initially treated as inpatients and upon release, continued treatment as outpatients. Follow-up data was gathered at six months and again at twelve months. One group was treated by standard methods for depression, which included therapy in a hospital setting and the use of antidepressant drugs, which were monitored. The second group was treated using cognitive-behavioral therapy in addition to the standard methods. A goal of cognitive-behavioral therapy is to improve patients understanding and awareness of their behavior. The third group received social skills therapy, also in combination with standard treatment. Social skills therapy emphasizes the development or improvement of appropriate social behavior. The groups that received cognitive therapy and social skills therapy were combined for reporting purposes and measured against the group that received standard treatment alone. The conclusions were limited by the small size of the sample population, but it was concluded that this study was significant because the sampling was composed of inpatients who were severely depressed and their progress was tracked over a long period of time. This represents the first controlled observation with this particular scope. Results of this study in light of other studies suggest a distinct long-term benefit of cognitive-behavioral and social skills therapies in combination with standard treatment, as opposed to standard treatment (drugs) alone. The researchers felt that the correlation between the results of this long-term study and similar studies reinforces their conclusions. Overall, the data also suggests that the patients in this study required a greater amount of treatment when compared with outpatients treated for depression described in previous studies.

224. Demographic and clinical characteristics associated with comorbid cannabis use disorders (CUDs) in hospitalized patients with bipolar I disorder.

Abstract

Background: Published data suggest that cannabis use is associated with several negative consequences for individuals with bipolar disorder (BD), including new manic episode onset, psychosis, and functional disability. Yet much less is known about cannabis use disorders (CUDs) in this population, especially in more acutely symptomatic groups. Methods: To evaluate correlates of CUD comorbidity in BD, a retrospective chart review was conducted for 230 adult patients with bipolar I disorder (BDI) who were admitted to a university-affiliated private psychiatric hospital. Using a computer algorithm, a hospital administrator extracted relevant demographic and clinical data from the electronic medical record for analysis. Results: Thirty-six (16%) had a comorbid CUD. CUD comorbidity was significantly associated with younger age, manic/mixed episode polarity, presence of psychotic features, and comorbid nicotine dependence, alcohol use disorder (AUD), and other substance use disorders, but was associated with decreased likelihood of anxiety disorder comorbidity. With the exception of manic/mixed polarity and AUD comorbidity, results from multivariate analyses controlling for the presence of other SUDs were consistent with univariate findings. Conclusion: Patients with BD and comorbid CUDs appear to be a complex population with need for enhanced clinical monitoring. Given increasing public acceptance of cannabis use, and the limited availability of evidenced-based interventions targeted toward CUDs in BD, psychoeducation and other treatment development efforts appear to be warranted. Abstract

Nonsuicidal self-injury (NSSI) is a prevalent and dangerous behavior among young adults, but no treatments specific to NSSI have been developed for patients without borderline personality disorder. The purpose of this study was to develop and investigate a novel intervention for NSSI among young adults. The intervention is a 9-session behavioral treatment designed to decrease the frequency of NSSI behaviors and urges. Using an open pilot design, feasibility and acceptability were investigated in a small sample ( n = 12) over a 3-month follow-up period. A preliminary investigation of change in NSSI was also conducted. Feasibility and acceptability of the intervention were supported. Medium to large effect sizes were found for decreases in NSSI behaviors and urges over the follow up period. Results of this open pilot trial support the further evaluation of this intervention. 230. Executive function and suicidality: A systematic qualitative review.

Abstract

Deficits in executive function (EF) have been proposed as a possible explanation for the “cognitive rigidity” often observed in suicidal individuals. This article provides a systematic review of the existing literature testing the relations between EF and suicidality, across various diagnostic and demographic populations, using the influential multidimensional model of EF proposed by Miyake and colleagues (2000) as an organizing framework. Forty-three journal articles on this topic published before January of 2014 were reviewed. Collectively, results from these studies provide tentative support for an association between EF deficits and suicidality. However, there is some evidence that this association is moderated by other factors (e.g., suicide attempt lethality). Importantly, this relationship may vary across diagnostic groups. Specifically, more studies that used depressive disorder samples reported some positive findings (75%), followed by mixed diagnostic samples (54%). In contrast, fewer positive findings have emerged from studies with bipolar or psychotic disorder samples (29% and 33% respectively), and some even found that suicidality is associated with better EF in individuals with psychotic disorders. Firm conclusions about relationships between specific dimensions of EF and/or aspects of suicidality are difficult to draw this time.
