High-quality single crystals of WTe2 were grown using a Te flux followed by a cleaning step involving self-vapor transport. The method is reproducible and yields consistently higher-quality single crystals than are typically obtained via halide-assisted vapor transport methods. Magnetoresistance (MR) values at 9 tesla and 2 kelvin as high as 1.75 million %, nearly an order of magnitude higher than previously reported for this material, were obtained on crystals with residual resistivity ratio (RRR) of approximately 1250. The MR follows a near B2 law and, assuming a semiclassical model, the average carrier mobility for the highest-quality crystal was found to be at 2 K. A correlation of RRR, MR ratio and average carrier mobility is found with the cooling rate during the flux growth. [ABSTRACT FROM AUTHOR]

The article presents a study which fabricated Josephson Junctions of Kagome metal, K1-xV3Sb5 and induced superconductivity over long junction lengths. Topics discussed include proximity effect-induced superconductivity, magnetic field sweep direction dependence of mangetoresistance, and anisotropic interference pattern for in-plane and out-of-plane magnetic field.

We investigate the orthorhombic phase (Td) of the layered transition-metal dichalcogenide MoTe2 as a Weyl semimetal candidate. MoTe2 exhibits four pairs of Weyl points lying slightly above (~6meV) the Fermi energy in the bulk band structure. Different from its cousin WTe2, which was recently predicted to be a type-II Weyl semimetal, the spacing between each pair of Weyl points is found to be as large as 4% of the reciprocal lattice in MoTe2 (six times larger than that of WTe2). When projected onto the surface, the Weyl points are connected by Fermi arcs, which can be easily accessed by angle-resolved photoemission spectroscopy due to the large Weyl point separation. In addition, we show that the correlation effect or strain can drive MoTe2 from a type-II to a type-I Weyl semimetal. [ABSTRACT FROM AUTHOR]

Three-dimensional Dirac semimetals (DSMs) are materials that have massless Dirac electrons and exhibit exotic physical properties. It has been suggested that structurally distorting a DSM can create a topological insulator but this has not yet been experimentally verified. Furthermore, Majorana fermions have been theoretically proposed to exist in materials that exhibit both superconductivity and topological surface states. Here we show that the cubic Laves phase Au2Pb has a bulk Dirac cone that is predicted to gap on cooling through a structural phase transition at 100 K. The low temperature phase can be assigned a Z2 = - 1 topological index, and this phase becomes superconducting below 1.2 K. These characteristics make Au2Pb a unique platform for studying the transition between bulk Dirac electrons and topological surface states as well as studying the interaction of superconductivity with topological surface states, combining many different properties of emergent materials-superconductivity, bulk Dirac electrons, and a topologically nontrivial Z2 invariant. We report an observation of a pressure-induced breakdown of the three-dimensional Dirac semimetal (3D-DSM) state in Cd3As2, evidenced by a series of in situ high-pressure synchrotron x-ray diffraction and single-crystal transport measurements. We find that Cd3As2 undergoes a structural phase transition from a metallic tetragonal phase in space group I41/acd to a semiconducting monoclinic phase in space group P21/c at critical pressure 2.57 GPa; above this pressure, an activation energy gap appears, accompanied by distinct switches in Hall resistivity slope and electron mobility. These changes of crystal symmetry and corresponding transport properties manifest the breakdown of the 3DDSM state in pressurized Cd3As2. [ABSTRACT FROM AUTHOR]

We present an ARPES study of the surface states of Ru2Sn3, a new type of a strong 3D topological insulator (TI). In contrast to currently known 3D TIs, which display two-dimensional Dirac cones with linear isotropic dispersions crossing through one point in the surface Brillouin Zone (SBZ), the surface states on Ru2Sn3 are highly anisotropic, displaying an almost flat dispersion along certain high-symmetry directions. This results in quasi-one dimensional (1D) Dirac electronic states throughout the SBZ that we argue are inherited from features in the bulk electronic structure of Ru2Sn3 where the bulk conduction bands are highly anisotropic. Unlike previous experimentally characterized TIs, the topological surface states of Ru2Sn3 are the result of a d-p band inversion rather than an s-p band inversion. We present the structure and magnetic properties of Na3Ni2BiO6, which is an ordered variant of the α-NaFeO2 structure type. This layered compound has a 2:1 ordering of (Ni2+/Bi5+)O6 octahedra within the a-b plane and sodium in octahedra between the layers. The structure is presented in the space group C2/m, determined through a combination of single crystal X-ray, powder neutron, and powder X-ray diffraction. Temperature dependent magnetic susceptibility measurements show Na3Ni2BiO6 to display long-range antiferromagnetic ordering below 11 K, despite the dominance of ferromagnetic interactions above TN as indicated by a positive Weiss constant. Heat capacity measurements and low-temperature neutron diffraction support the magnetic ordering and are consistent with a TN of 10.4 K. A magnetic phase can be refined with (010) antiferromagnetic ordering along the b-axis in the honeycomb layer and moments aligned parallel to c. The compounds Na3Mg2BiO6 and Na3Zn2BiO6, synthesized as nonmagnetic analogues of Na3Ni2BiO6, are briefly described. [ABSTRACT FROM AUTHOR]

Abstract: The structure and elementary physical properties of a new intermetallic compound, , are reported. has a tetragonal structure (space group I4/mmm) with lattice constants of a=3.7634(11)Å and c=13.499(4)Å. The structure, which consists of double layers of Hf stacked with edge-sharing CuGe4 squares, is not typical for intermetallic compounds and appears to be a new structure type. Antiferromagnetically doped topological insulators (ATI) are among the candidates to host dynamical axion fields and axion polaritons, weakly interacting quasiparticles that are analogous to the dark axion, a long sought after candidate dark matter particle. Here we demonstrate that using the axion quasiparticle antiferromagnetic resonance in ATIs in conjunction with low-noise methods of detecting THz photons presents a viable route to detect axion dark matter with a mass of 0.7 to 3.5 meV, a range currently inaccessible to other dark matter detection experiments and proposals. The benefits of this method at high frequency are the tunability of the resonance with applied magnetic field, and the use of ATI samples with volumes much larger than 1 mm³. [ABSTRACT FROM AUTHOR]

A comprehensive analysis of in vitro pumps used in cardiovascular research is provided in this review, with a focus on the characteristics of generated flows and principles of flow generations. The cardiovascular system, vital for nutrient circulation and waste removal, generates complex hemodynamics critical for endothelial cell function. Cardiovascular diseases (CVDs) could be caused by the disturbances in these flows, including aneurysms, atherosclerosis, and heart defects. In vitro systems simulate hemodynamic conditions on cultured cells in the laboratory to study and evaluate these diseases to advance therapies. Pumps used in these systems can be classified into contact and non-contact types. Contact pumps, such as piston and gear pumps, can generate higher flow rates, but they have a higher risk of contamination due to the direct interaction of pump with the fluid. Non-contact pumps, such as peristaltic and lab-on-disk centrifugal pumps, minimize contamination risks, but they are limited to lower flow rates. Advanced pumps including piezoelectric and I-Cor diagonal pumps are focused on improving the accuracy of flow replication and long-term stability. The operational principles, advantages, and some disadvantages of these pump categories are evaluated in this review, while providing insights for optimizing in vitro cardiovascular models and advancing therapeutic strategies against CVDs. The outcomes of the review elaborate the importance of selecting an appropriate pump system, to accurately replicate cardiovascular flow patterns.

Acute respiratory distress syndrome (ARDS) presents a formidable challenge in critical care, often resulting in high mortality rates, particularly in severe cases or those compounded by preexisting conditions. Despite substantial advancements in critical care, the heterogeneous nature of ARDS necessitates nuanced clinical approaches. ARDS is generally diagnosed through clinical evaluation, radiographic imaging, and laboratory tests, as well as acute onset, bilateral lung infiltrates on imaging, and a partial pressure of oxygen in arterial blood (PaO2)/fraction of inspiratory oxygen concentration (FiO2) ratio of less than 300 mmHg. Management involves measurements to improve oxygenation and provide mechanical ventilation to assist breathing. The typical manifestation of ARDS is diffuse lung involvement, which affects multiple lobes symmetrically. Here, we report an unusual case of ARDS in a 53-year-old female who was brought into the hospital in an unresponsive state, exhibiting hypoxic and hypotension requiring intubation. Subsequent imaging revealed a distinctive pattern: the preservation of the right middle lobe, diverging from the conventional diffuse pulmonary affliction. This case underscores the need for clinical vigilance and adaptability, as such atypical presentations can confound diagnosis and management, posing unique clinical challenges. This case highlights the importance of recognizing ARDS' diverse presentations. Moreover, understanding the mechanisms behind the lobar sparing could provide greater insight into the disease heterogeneity and guide tailored therapeutic approaches. There is an ongoing interest in kagome materials because they offer tunable platforms at the intersection of magnetism and electron correlation. Herein, we examine single crystals of new kagome materials, Ln x Co 3 (Ge 1- y Sn y ) 3 (Ln = Y, Gd; y = 0.11, 0.133), which were produced using the Sn flux-growth method. Unlike many of the related chemical analogues with the LnM 6 X 6 formula (M = transition metal and X = Ge, Sn), the Y and Gd analogues crystallize in a hybrid YCo 6 Ge 6 /CoSn structure, with Sn substitution. While the Y analogue displays temperature-independent paramagnetism, magnetic measurements of the Gd analogue reveal a magnetic moment of 8.48 μ B , indicating a contribution from both Gd and Co. Through anisotropic magnetic measurements, the direction of Co-magnetism can be inferred to be in plane with the kagome net, as the Co contribution is only along H// a . Crystal growth and structure determination of Y x Co 3 (Ge,Sn) 3 and Gd x Co 3 (Ge,Sn) 3 , two new hybrid kagome materials of the CoSn and YCo 6 Ge 6 structure types. Magnetic properties, heat capacity, and resistivity on single crystals are reported.

In order to achieve the benefits of targeted drug delivery, this study intended to encapsulate doxorubicin in a linear polyamidoamine and its PEGylated co-polymer. The drug was loaded by using the emulsion solvent evaporation method. By adjusting the doxorubicin to polymer ratios to 1:10, 1:20 and 1:30, three formulations of each polymer/copolymer were prepared. The drug release profile was investigated using phosphate buffered saline. In vitro cytotoxicity investigation was executed on liver cancer cell line (Hep G2 cell lines) by 3-[4,5-dimethylthiazol-2-yl]-2,5 diphenyl tetrazolium bromide assay. The outcome demonstrated that doxorubicin had been successfully loaded on polyamidoamine and its PEGylated co-polymer with a drug loading efficiency of about 90%. Nanocarrier sizes were between 245±1.10 nm -579±1.00 nm and the zeta potential range was +22.4±0.5 mV-+37.9±0.3 mV. In-vitro drug release investigations revealed a characteristic pH-dependent drug release. The cytotoxicity testing of optimal formulation revealed that the doxorubicin was successfully released from the formulations and exerted therapeutic effect. According to our research, doxorubicin could be loaded onto linear polyamidoamines for potent antitumor effects on the target liver cancer cell lines (Hep G2).

A 75-year-old male, with a past medical history of chronic kidney disease stage 3 (CKD3) and a recent diagnosis of bilateral hydronephrosis and Foley catheter placement, presented to the emergency department for fever. Blood cultures grew Aerococcus urinae. Transthoracic echo (TTE) demonstrated thickened aortic valve leaflets with perforation, multiple echo densities, and severe aortic regurgitation. The patient developed decompensated congestive heart failure and cardiogenic shock. En route to surgery for emergent aortic valve replacement, the patient lost pulse and was resuscitated. The patient was subsequently transferred to the ICU where the family decided to initiate comfort care measures. Capsicum has been an important part of diet since centuries and has been widely used traditionally for ailments. The objective of the present analysis was to determine the diuretic and antidiarrheal potentials of four variations of Capsicum annum L. fruits available in local market of Karachi, Pakistan. It belongs to family Solanaceae and extracts were prepared in 95% ethanol and was given in doses of 200 and 400mg/kg and the activity was correlated with commonly used diuretics and anti-diarrheal further reinforcing its conventional use. Fruits extracts were evaluated for anti-diarrheal properties in castor-oil induced diarrhea to confirm its activity. The diuretic activity of the 95% ethanolic extracts of Capsicum annum L. was assessed based on diuresis of 24 hr. collected through metabolic cage. Urinary excretion of water and electrolytes (Na+ and K+) is significantly increased by the standard diuretic drug furosemide, which is comparable with the extract. The anti-diarrheal effects of 95% ethanolic extracts of Capsicum annum L. were examined by castor oil induced diarrhea and all varieties of Capsicum annum L. significantly inhibited the number of defecations.

The superconducting analogue to the semiconducting diode, the Josephson diode, has long been sought with multiple avenues to realization being proposed by theorists 1-3 . Showing magnetic-field-free, single-directional superconductivity with Josephson coupling, it would serve as the building block for next-generation superconducting circuit technology. Here we realized the Josephson diode by fabricating an inversion symmetry breaking van der Waals heterostructure of NbSe 2 /Nb 3 Br 8 /NbSe 2 . We demonstrate that even without a magnetic field, the junction can be superconducting with a positive current while being resistive with a negative current. The ΔI c behaviour (the difference between positive and negative critical currents) with magnetic field is symmetric and Josephson coupling is proved through the Fraunhofer pattern. Also, stable half-wave rectification of a square-wave excitation was achieved with a very low switching current density, high rectification ratio and high robustness. This non-reciprocal behaviour strongly violates the known Josephson relations and opens the door to discover new mechanisms and physical phenomena through integration of quantum materials with Josephson junctions, and provides new avenues for superconducting quantum devices., (© 2022. Current-induced control of magnetization in ferromagnets using spin-orbit torque (SOT) has drawn attention as a new mechanism for fast and energy efficient magnetic memory devices. Energy-efficient spintronic devices require a spin-current source with a large SOT efficiency (ξ) and electrical conductivity (σ), and an efficient spin injection across a transparent interface. Herein, single crystals of the van der Waals (vdW) topological semimetal WTe 2  and vdW ferromagnet Fe 3 GeTe 2 are used to satisfy the requirements in their all-vdW-heterostructure with an atomically sharp interface. The results exhibit values of ξ ≈ 4.6 and σ ≈ 2.25 × 10 5  Ω -1 m -1 for WTe 2 . Moreover, the significantly reduced switching current density of 3.90 × 10 6 A cm -2 at 150 K is obtained, which is an order of magnitude smaller than those of conventional heavy-metal/ferromagnet thin films. In ferromagnetic semiconductors, the coupling of magnetic ordering with semiconductor character accelerates the quantum computing. The structural stability, Curie temperature (T c ), spin polarization, half magnetic ferromagnetism and transport properties of ZnX 2 Se 4 (X = Ti, V, Cr) chalcogenides for spintronic and thermoelectric applications are studied here by density functional theory (DFT). The highest value of T c is perceived for ZnCr 2 Se 4 . The band structures in both spin channels confirmed half metallic ferromagnetic behavior, which is approved by integer magnetic moments (2, 3, 4) μ B of Ti, V and Cr based spinels. The HM behavior is further measured by computing crystal field energy ΔE crystal , exchange energies Δ x ( d ), Δ x ( pd ) and exchange constants (N o α and N o β). The thermoelectric properties are addressed in terms of electrical conductivity, thermal conductivity, Seebeck coefficient and power factor in within a temperature range 0-400 K. The positive Seebeck coefficient shows p-type character and the PF is highest for ZnTi2Se4 (1.2 × 10 11 W/mK 2 ) among studied compounds.

Takayasu's arteritis is a rare chronic granulomatous vasculitis that predominantly affects the aorta and its branches. It is estimated to affect 2.6/million/annum, predominantly women in the second or third decade of their lives. This case report describes the case of a 21-year-old female, who initially only had low-grade fever and acrodynia, which over a few months, progressed to arm and leg claudication, weight loss, nausea, headache, and dizziness. A year later, the patient experienced impalpable radial pulses bilaterally, and her CT angiogram revealed multi-level arterial stenosis. A diagnosis of Takayasu's arteritis was made and the patient was started on steroids and methotrexate. A consult was made with vascular surgery but no intervention was deemed necessary and the patient responded well to the medical treatment given. Background and Aims: Mutations in ATPase phospholipid transporting 8B1 (ATP8B1) can lead to familial intrahepatic cholestasis type 1 (FIC1) deficiency, or progressive familial intrahepatic cholestasis type 1. The rarity of FIC1 deficiency has largely prevented a detailed analysis of its natural history, effects of predicted protein truncating mutations (PPTMs), and possible associations of serum bile acid (sBA) concentrations and surgical biliary diversion (SBD) with long-term outcome. We aimed to provide insights by using the largest genetically defined cohort of patients with FIC1 deficiency to date., Approach and Results: This multicenter, combined retrospective and prospective study included 130 patients with compound heterozygous or homozygous predicted pathogenic ATP8B1 variants. Patients were categorized according to the number of PPTMs (i.e., splice site, frameshift due to deletion or insertion, nonsense, duplication), FIC1-A (n = 67; no PPTMs), FIC1-B (n = 29; one PPTM), or FIC1-C (n = 34; two PPTMs). Survival analysis showed an overall native liver survival (NLS) of 44% at age 18 years. NLS was comparable among FIC1-A, FIC1-B, and FIC1-C (% NLS at age 10 years: 67%, 41%, and 59%, respectively; P = 0.12), despite FIC1-C undergoing SBD less often (% SBD at age 10 years: 65%, 57%, and 45%, respectively; P = 0.03). sBAs at presentation were negatively associated with NLS (NLS at age 10 years, sBAs < 194 µmol/L: 49% vs. sBAs ≥ 194 µmol/L: 15%; P = 0.03). SBD decreased sBAs (230 [125-282] to 74 [11-177] μmol/L; P = 0.005). SBD (HR 0.55, 95% CI 0.28-1.03, P = 0.06) and post-SBD sBA concentrations < 65 μmol/L (P = 0.05) tended to be associated with improved NLS., Conclusions: Less than half of patients with FIC1 deficiency reach adulthood with native liver. The number of PPTMs did not associate with the natural history or prognosis of FIC1 deficiency. sBA concentrations at initial presentation and after SBD provide limited prognostic information on long-term NLS., (© The Authors. The aim of study was to evaluate the analgesic and anti-inflammatory activity of four different colored (green, yellow, orange and red) sweet bell peppers (Capsicum annuum L.) available in the local market of Karachi Pakistan. Their 95% ethanol extracts at 200 and 400 mg/kg were prepared and compared with commonly used analgesic (aspirin) and anti-inflammatory agents supporting its traditional use. The analgesic effects of 95% ethanol extracts of Capsicum annum L. were investigated by acetic acid induced writhing, tail immersion and hot plate test. The anti-inflammatory activities were observed using carrageenan-induced edema of hind paw in rats. Animals were divided into 10 groups (n=7): (1) Control (2) CAG 200 (3) CAG 400 (4) CAR 200 (5) CAR 400 (6) CAO 200 (7) CAO 400 (8) CAY 200 (9) CAY 400 and (10) Standard. All the extracts were given orally. Acute toxicity was also determined by increasing the dose till 3000 mg/kg, which showed no evidence of mortality. All extracts of Capsicum significantly increased the hot plate pain threshold, moreover remarkably reduced the carrageenan-induced rat paw edema. Results obtained were compared with corresponding control group revealed that the fresh fruits extract of all four kinds of bell pepper (200 mg/kg and 400mg/kg) possess anti-inflammatory and pain suppressing activities possibly mediated via PG synthesis inhibition.

This article focuses on the output feedback control of single-link flexible-joint robot manipulators (SFJRMs) with matched disturbances and parametric uncertainties. Formally, four sensing elements are required to design the controller for single-link manipulators. We have designed a robust control technique for the semiglobal stabilization problem of the angular position of the link in the SFJRM system, with the availability of only a position sensing device. The sliding mode control (SMC) based output feedback controller is devised for SFJRM dynamics. The nonlinear model of SFJRM is considered to estimate the unknown states utilizing the high-gain observer (HGO). It is shown that the output under SMC using HGO-based estimated states coincides with that using original states when the gains of HGO are sufficiently high. Finally, the results are presented showing that the designed control technique works well when the SFJRM model is uncertain and matched perturbations are expected.

Background: Dietary fiber has played a consistent role in weight management, with efficacy potentially attributed to increased viscous fiber consumption., Purpose: To summarize the effects of viscous fiber on body weight and other anthropometric parameters, along with a calorie-deficient diet, through a systematic review and meta-analysis., Methods: MEDLINE, EMBASE, and the Cochrane library were searched through July 24, 2019 for randomized controlled trials that assessed the effect of viscous fiber supplementation as part of a restricted calorie diet for ≥ 4 weeks relative to comparator diets. Data were pooled using the generic inverse-variance method with random-effects models and expressed as mean differences with 95% confidence intervals. Inter-study heterogeneity was assessed using Cochran's Q and quantified with I 2 . The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach was used to evaluate the overall certainty of evidence., Results: Findings from 15 studies (n = 1347) showed viscous fiber supplementation significantly decreased body weight (- 0.81 kg [- 1.20, - 0.41]; p < 0.0001), BMI (- 0.25 kg/m 2 [- 0.46, - 0.05]; p = 0.01), and body fat (- 1.39% [- 2.61, - 0.17]; p = 0.03), compared to control. No effect on waist circumference was found. The certainty of evidence was graded as "moderate" for body weight, BMI, and body fat based on downgrades for imprecision. Waist circumference was graded "low" for downgrades of inconsistency and imprecision., Conclusion: Viscous fiber within a calorie-restricted diet significantly improved body weight and other markers of adiposity in overweight adults and those with additional risk factors for cardiovascular disease. Td-WTe 2 (non-centrosymmetric and orthorhombic), a type-II Weyl semimetal, is expected to have higher-order topological phases with topologically protected, helical one-dimensional hinge states when its Weyl points are annihilated. However, the detection of these hinge states is difficult due to the semimetallic behaviour of the bulk. In this study, we have spatially resolved the hinge states by analysing the magnetic field interference of the supercurrent in Nb-WTe 2 -Nb proximity Josephson junctions. The Josephson current along the a axis of the WTe 2 crystal, but not along the b axis, showed a sharp enhancement at the edges of the junction, and the amount of enhanced Josephson current was comparable to the upper limits of a single one-dimensional helical channel. Our experimental observations suggest a higher-order topological phase in WTe 2 and its corresponding anisotropic topological hinge states, in agreement with theoretical calculations. Our work paves the way for the study of hinge states in topological transition-metal dichalcogenides and analogous phases.

Understanding the electronic transport properties of layered, van der Waals transition metal halides (TMHs) and chalcogenides is a highly active research topic today. Of particular interest is the evolution of those properties with changing thickness as the 2D limit is approached. Here, we present the electrical conductivity of exfoliated single crystals of the TMH, cluster magnet, Nb 3 Cl 8 , over a wide range of thicknesses both with and without hexagonal boron nitride (hBN) encapsulation. The conductivity is found to increase by more than three orders of magnitude when the thickness is decreased from 280 µm to 5 nm, at 300 K. At low temperatures and below ∼50 nm, the conductance becomes thickness independent, implying surface conduction is dominating. Temperature dependent conductivity measurements indicate Nb 3 Cl 8 is an insulator, however, the effective activation energy decreases from a bulk value of 310 meV to 140 meV by 5 nm. X-ray photoelectron spectroscopy (XPS) shows mild surface oxidation in devices without hBN capping, however, no significant difference in transport is observed when compared to the capped devices, implying the thickness dependent transport behavior is intrinsic to the material. A conduction mechanism comprised of a higher conductivity surface channel in parallel with a lower conductivity interlayer channel is discussed.

Background & Aims: Mutations in ABCB11 can cause deficiency of the bile salt export pump (BSEP), leading to cholestasis and end-stage liver disease. Owing to the rarity of the disease, the associations between genotype and natural history, or outcomes following surgical biliary diversion (SBD), remain elusive. We aimed to determine these associations by assembling the largest genetically defined cohort of patients with severe BSEP deficiency to date., Methods: This multicentre, retrospective cohort study included 264 patients with homozygous or compound heterozygous pathological ABCB11 mutations. Patients were categorized according to genotypic severity (BSEP1, BSEP2, BSEP3). The predicted residual BSEP transport function decreased with each category., Results: Genotype severity was strongly associated with native liver survival (NLS, BSEP1 median 20.4 years; BSEP2, 7.0 years; BSEP3, 3.5 years; p <0.001). At 15 years of age, the proportion of patients with hepatocellular carcinoma was 4% in BSEP1, 7% in BSEP2 and 34% in BSEP3 (p = 0.001). SBD was associated with significantly increased NLS (hazard ratio 0.50; 95% CI 0.27-0.94: p = 0.03) in BSEP1 and BSEP2. A serum bile acid concentration below 102 μmol/L or a decrease of at least 75%, each shortly after SBD, reliably predicted NLS of ≥15 years following SBD (each p <0.001)., Conclusions: The genotype of severe BSEP deficiency strongly predicts long-term NLS, the risk of developing hepatocellular carcinoma, and the chance that SBD will increase NLS. Serum bile acid parameters shortly after SBD can predict long-term NLS., Lay Summary: This study presents data from the largest genetically defined cohort of patients with severe bile salt export pump deficiency to date. The genotype of patients with severe bile salt export pump deficiency is associated with clinical outcomes and the success of therapeutic interventions. Background: The role of dietary fiber in obesity management remains debatable. Evidence suggests that intake of viscous fiber may have the potential to facilitate weight loss., Objective: We aimed to summarize and quantify the effects of viscous fiber on body weight, BMI, waist circumference, and body fat, independent of calorie restriction, through a systematic review and meta-analysis of randomized controlled trials., Methods: Trials ≥4 wk in duration that assessed the effect of viscous fiber supplemented to an ad libitum diet along with comparator diets were included. MEDLINE, EMBASE, and the Cochrane library were searched through 24 July, 2019. Two independent reviewers extracted relevant data. Data were pooled using the generic inverse variance method and random-effects models and expressed as mean differences with 95% CIs. Interstudy heterogeneity was assessed (Cochran Q statistic) and quantified (I2 statistic). The overall certainty of evidence was explored using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) approach., Results: Findings from 62 trials (n = 3877) showed that viscous fiber reduced mean body weight (-0.33 kg; 95% CI: -0.51, -0.14 kg; P = 0.004), BMI (in kg/m2) (-0.28; 95% CI: -0.42, -0.14; P = 0.0001), and waist circumference (-0.63 cm; 95% CI: -1.11, -0.16 cm; P = 0.008), with no change in body fat (-0.78%; 95% CI: -1.56%, 0.00%; P = 0.05) when consumed with an ad libitum diet. Greater reductions in body weight were observed in overweight individuals and those with diabetes and metabolic syndrome. The certainty of evidence was graded moderate for body weight, high for waist circumference and body fat, and low for BMI., Conclusions: Dietary viscous fiber modestly yet significantly improved body weight and other parameters of adiposity independently of calorie restriction.
