Abstract

Primary aldosteronism (PA) is a common curable cause of hypertension. Adrenal venous sampling (AVS) is recommended for subtype diagnosis but is a difficult procedure. Recently, an increased prevalence of PA was reported, creating a greater demand for treatment of the condition in clinical facilities. The aim of the present study was to identify the historical changes over time and the differences between facilities in the success rate and subtype diagnosis of PA. The database of the PA registry developed by the Japan PA Study (JPAS) was used. A total of 2599 patients with PA who underwent AVS were evaluated. The overall success rate of AVS was 88%. The bilateral subtype was the dominant subtype, comprising 69% of cases. During the period 2004−2011 to 2011−2017, there were significant changes in the total number of AVS procedures (from 562 to 1732), ratio of ACTH administration with AVS (75 to 97%), success rate (79 to 90%), and proportion with bilateral subtype diagnosis (53 to 72%). There were also significant inter-facility differences in the number of AVS procedures (6 to 322), success rate (59 to 97%), and proportion with the bilateral subtype (44 to 86%). The principal enrolled department was Endocrinology (86%), and the ratio of unilateral PA was significantly higher in this department than in others (32% vs. 25%). In conclusion, the number of AVS procedures performed, the success rate, and the proportion with the bilateral subtype increased over time after normalizing the centre difference. Significant differences were observed between the centres.

Abstract

Context In patients with primary aldosteronism (PA), it remains unclear whether aldosterone-producing adenomas are likely to develop in the left or right adrenal gland. Objective To investigate left-right differences of PA laterality diagnoses via CT imaging and adrenal vein sampling (AVS). Design Retrospective, observational study. Patients From the Japan Primary Aldosteronism Study, 1493 patients with PA were enrolled who underwent CT and ACTH-stimulated AVS. Measurements Left or right adrenal nodular lesion distribution and laterality observed on CT scans and from AVS were noted. Results Both on CT scans and AVS, unilateral results were observed more frequently on the left side than on the right side (25.1% vs 15.4% and 17.3% vs 13.5%, respectively; P < 0.01for both diagnostic techniques). There was no significant difference in the concordance rate for CT and AVS between patients with left and right unilateral nodular lesions observed on CT scans (44.1% and 50.9%, respectively; P = 0.15). In patients with nodules <20 mm, the concordance rate was significantly greater on the right side than the left side (45.8% vs 56.4%; P = 0.03). In patients with bilateral results of AVS, unilateral nodular lesions were detected more frequently on the left side than the right side (17.8% vs 9.4%; P < 0.01). Conclusion These results suggest aldosterone-producing adenomas and nonfunctioning tumors are more likely to develop on the left side in patients with PA and that misdiagnosis of CT-based lateralization may occur more frequently on the left side. Abstract

We present the VIMOS Spectroscopic Survey of a Supercluster in the COSMOS field (VIS3COS) at z ∼ 0.84. We use VIMOS high-resolution spectra (GG475 filter) to spectroscopically select 490 galaxies in and around the superstructure and an additional 481 galaxies in the line of sight. We present the redshift distribution, the catalogue to be made public, and the first results on the properties of individual galaxies and stacked spectra (3500 Å < λ <  4200 Å rest-frame). We probe a wide range of densities and environments (from low-density field to clusters and rich groups). We find a decrease in the median star formation rate from low- to high-density environments in all bins of stellar mass and a sharp rise of the quenched fraction (from ∼10% to ∼40 − 60%) of intermediate-stellar-mass galaxies (10 <  log10 (M⋆/M⊙) <  10.75) from filaments to clusters. The quenched fraction for massive galaxies shows little dependence on environment, being constant at ∼30 − 40%. We find a break in the strength of the [O II] emission, with nearly constant line equivalent widths at lower densities (∼ − 11 Å) and then a drop to ∼ − 2.5 Å towards higher densities. The break in the [OII] line strength happens at similar densities (log10(1 + δ) ∼ 0.0 − 0.5) as the observed rise in the quenched fraction. Our results may provide further clues regarding the different environmental processes affecting galaxies with different stellar masses and highlight the advantages of a single dataset in the COSMOS field probing a wide range of stellar masses and environments. Abstract

術後急性肺塞栓症および深部静脈血栓症の診断, 治療, 予防について臨床的に検討した.術後急性肺塞栓症の患者6名と深部静脈血栓症の患者3例を対象にした.血管造影後に発症した1例を除き, いずれの症例にも骨盤内手術や長時間手術, 特殊な体位あるいは肥満傾向などの何らかの血栓塞栓症の危険因子を認めた.周術期急性肺塞栓症の初期症状では突然の呼吸困難, 冷汗, 失神が多かった.また, 重篤かつ急速な経過をとるもが多く, 死亡率は67%であった.深部静脈血血栓症の3例は悪性腫瘍に対する骨盤内手術後に発症した症例であり, 診断の方法としては超音波診断が非常に有用であった.全3例は肺塞栓症の発症を認めずに治癒し, 深部静脈血血栓症の予防が重要であると考えられた, Acute pulmonary thromboembolism (APTE) and deep venous thrombosis (DVT) are categorized into venous thromboembolism. APTE is one of the most critical post-operative complications because of its high mortality. Meanwhile, DVT is responsible for more than 80-95% of APTE cases. APTE occurred in 6 patients and DVT in 3 patients in the post-operative period during the past eleven years at our hospital. The median age of these patients, 5 men and 4 women, was 62 years (45-77). In many cases, the course of APTE showed a rapid progression. Four of the six patients with APTE died, and two of these four patients died within a few hours following the onset. Once it occurs, it is often lethal. Therefore, the prevention of thromboembolism is most important. In this study, we examined these 6 APTE cases and 3 DVT cases and discussed low dose heparin and intermittent sequential pneumatic compression (ISPC), prophylaxis against APTE in the post-operative period. We have been using ISPC for the high-risk group of thromboembolism in the post-operative period since 1999. In addition to using ISPC, we have been administering low dose heparin since November 2001. These 3 cases of DVT occurred regardless of adoption of ISPC. However, there were no cases of APTE after adoption of ISPC, and no cases of DVT or APTE after adoption of ISPC and administration of low dose heparin.

Abstract

The intergalactic medium (IGM) is the dominant reservoir of baryons, delineates the large scale structure of the universe at low to moderate overdensities, and provides gas from which galaxies form and evolve. Simulations of a Cold Dark Matter (CDM) dominated universe predict that the IGM is distributed in a cosmic web of filaments, and that galaxies should form along and at the intersections of these filaments (Bond, Kofman, & Pogosyan 1994; Miralda-Escude et al. 1996). While observations of QSO absorption lines and the large-scale distribution of galaxies have confirmed the CDM paradigm, the cosmic web of IGM has never been confirmed by direct imaging. Here we report our observation of the Lyα blob-2 (LAB2) in SSA22, with the Cosmic Web Imager. This is an integral field spectrograph optimized for low surface brightness, extended emission. With 22 hours of total on- and off-source exposure, CWI has revealed that LAB2 has extended Lyα emission which is organized into azimuthal zones consistent with filaments. We perform numerous tests with simulations and the data to secure the robustness of this result, which relies on data with modest signal-to-noise ratio. We have developed a smoothing algorithm that permits visualization of data cube slices along image or spectral-image planes. With both raw and smoothed data cubes we demonstrate that the filaments are kinematically associated with LAB2 and display double-peaked profiles characteristic of optically thick Lyα emission. The flux is 10-20 times brighter than expected for the average emission from the IGM but is consistent with boosted fluorescence from a buried QSO or gravitation cooling radiation. Using simple emission models we infer a baryon mass in the filaments of at least 1−4 × 10^(11)M_☉, and the dark halo mass is at least 2 × 10^(12)M_☉. The spatial-kinematic morphology is more consistent with inflow from the cosmic web than outflow from LAB2, although an outflow feature maybe present at one azimuth. LAB2 and the surrounding gas have significant and coaligned angular momentum, strengthening the case for their association.

Abstract

Summary: Context: The current Endocrine Society Guideline suggests that patients aged <35 years with marked primary aldosteronism (PA) and unilateral adrenal lesions on adrenal computed tomography (CT) scan may not need adrenal vein sampling (AVS) before proceeding to unilateral adrenalectomy. This suggestion is, however, based on the data from only one report in the literature. Objective: We sought to determine the accuracy of CT findings in young PA patients who had unilateral adrenal disease on CT with hypokalaemia and elevation of aldosterone. Design and patients: We retrospectively studied 358 PA patients (n = 30, aged <35 years; n = 39, aged 35‐40 years; n = 289, aged ≥40 years) with hypokalaemia and elevation of aldosterone and unilateral disease on CT who had successful AVS. Main outcome measure: Accuracy of CT findings is determined by AVS findings and/or surgical outcomes in patients aged <35 years. Results: Concordance of the diagnosis between CT and AVS was 90% (27/30) in patients aged <35 years, 79% (31/39) in patients aged 35‐40 years and 69% (198/289) in those aged ≥40 years (trend for P < .01). Surgical benefit was confirmed in three patients aged <35 years and in three patients aged 35‐40 years with the available surgical data who had discordance between CT and AVS findings. Collectively, the diagnostic accuracy of CT findings was 100% (30/30) if aged <35 years and 87% (34/39) if aged 35‐40 years. Conclusion: Primary aldosteronism patients aged <35 years with hypokalaemia and elevation of aldosterone and unilateral disease on adrenal CT could be spared AVS. [ABSTRACT FROM AUTHOR]

222. Constraining Dust and Molecular Gas Properties in Lyα Blobs at z ~ 3

Abstract

In order to constrain the bolometric luminosities, dust properties, and molecular gas content of giant Lyα nebulae, the so-called Lyα blobs, we have carried out a study of dust continuum and CO line emission in two well-studied representatives of this population at z ~ 3: an Lyα blob discovered by its strong Spitzer Multiband Infrared Photometer 24 μm detection (LABd05) and the Steidel blob 1 (SSA22-LAB01). We find that the spectral energy distribution of LABd05 is well described by an active-galactic-nucleus-starburst composite template with L_(FIR) = (4.0 ± 0.5) × 10^(12) L_☉, comparable to high-z submillimeter galaxies and ultraluminous infrared galaxies. New Large APEX Bolometer Camera 870 μm measurements rule out the reported Submillimeter Common-User Bolometer Array detection of the SSA22-LAB01 (S_(850 μm) = 16.8 mJy) at the >4σ level. Consistent with this, ultradeep Plateau de Bure Interferometer observations with ~2'' spatial resolution also fail to detect any 1.2 mm continuum source down to ≈0.45 mJy beam^(–1) (3σ). Combined with the existing (sub)millimeter observations in the literature, we conclude that the FIR luminosity of SSA22-LAB01 remains uncertain. No CO line is detected in either case down to integrated flux limits of S_νΔV ≾ 0.25-1.0 Jy km s^(–1), indicating a modest molecular gas reservoir, M(H_2) < (1-3) × 10^(10) M_☉. The non-detections exclude, with high significance (12σ), the previous tentative detection of a CO J = 4-3 line in the SSA22-LAB01. The increased sensitivity afforded by the Atacama Large Millimeter/submillimeter Array will be critical in studying molecular gas and dust in these interesting systems.

223. Characterization of the distribution of the Lly��emitters in the 53W002 field at z = 2.4

Abstract

We present the results of our wide-field narrow band imaging of the field around the radio galaxy 53W002 at z = 2.390 with Subaru/Suprime-Cam. A custom made filter NB413 centered at 4140 �� with the width of 83 �� is used to observe the 31' x 24' area around the radio galaxy. We detected 204 Ly��emitters (LAEs) at z = 2.4 with a rest frame equivalent width larger than 25 �� to the depth of 26 AB mag (in NB413). The entire LAE population in the 53W002 field has an average number density and distributions of equivalent width and size that are similar to those of other fields at z ~ 2. We identify a significant high density region (53W002F-HDR) that spreads over ~ 5' x 4' near 53W002 where the LAE number density is nearly four times as large as the average of the entire field. Using the probability distribution function of density fluctuation, we evaluate the rareness probability of the 53W002F-HDR to be 0.9^{+2.4}_{-0.62}%, which corresponds to a moderately rich structure. No notable environmental dependency at the comoving scale of 10 Mpc is found for the distributions of the Ly��equivalent width and luminosity in the field. We also detected 4 Ly��blobs (LABs), one of which is newly discovered. They are all found to be located in the rims of high density regions. Abstract

Adrenal venous sampling is considered to be the most reliable diagnostic procedure to lateralize aldosterone excess in primary aldosteronism (PA). However, normative criteria have not been established partially because of a lack of data in non-PA hypertensive patients. The aim of the study was to investigate aldosterone concentration and its gradient in the adrenal vein of non-PA hypertensive patients. We retrospectively studied the results of cosyntropin-stimulated adrenal venous sampling in 40 hypertensive patients who showed positive screening testing but negative results in 2 confirmatory tests/captopril challenge test and saline infusion test. Plasma aldosterone concentration, aldosterone/cortisol ratio, its higher/lower ratio (lateralization index) in the adrenal vein with cosyntropin stimulation were measured. Median plasma aldosterone concentration in the adrenal vein was 25 819 pg/mL (range, 5154-69 920) in the higher side and 12 953 (range, 1866-36 190) pg/mL in the lower side (P<0.001). There was a significant gradient in aldosterone/cortisol ratio between the higher and the lower sides (27.2 [5.4-66.0] versus 17.3 [4.0-59.0] pg/mL per μg/dL;P<0.001) with lateralization index ranging from 1.01 to 3.87. The aldosterone lateralization gradient was between 1 to 2 in 32 patients and 2 to 4 in 8 patients. None of the patients showed lateralization index ≥4. The present study demonstrated that plasma aldosterone concentration in the adrenal veins showed significant variation and lateralization gradient even in non-PA hypertensive patients. Adrenal venous sampling aldosterone lateralization gradients between 2 and 4 should be interpreted with caution in patients with PA because these gradients can be found even in patients with negative confirmatory testing for PA.
