Possible biological side effects of exposure to X-rays are stochastic effects such as carcinogenesis and genetic alterations. In recent years, a number of new studies have been published about the special cancer risk that children may suffer from diagnostic X-rays. Children and adolescents who constitute many of the probands in forensic age-estimation proceedings are considerably more sensitive to the carcinogenic risks of ionizing radiation than adults. Established doses for X-ray examinations in forensic age estimations vary from less than 0.1 microSv (left hand X-ray) up to more than 800 microSv (computed tomography). Computed tomography in children, as a relatively high-dose procedure, is of particular interest because the doses involved are near to the lower limit of the doses observed and analyzed in A-bombing survivor studies. From these studies, direct epidemiological data exist concerning the lifetime cancer risk. Since there is no medical indication for forensic age examinations, it should be stressed that only safe methods are generally acceptable. This paper reviews current knowledge on cancer risks associated with diagnostic radiation and aims to help forensic experts, dentists, and pediatricians evaluate the risk from radiation when using X-rays in age-estimation procedures.

Amongst numerous other morphological, chemical, physical, and histological dating methods, the assessment of the chemiluminescence of weathered organic traces on bone finds by means of the luminol reaction is common practice. Opinion differs on the diagnostic value of this procedure for differentiating between historical and recent skeletonized human remains. Design Chemiluminescence (CL) was tested in randomized, blind experiments on pulverized bone samples with known postmortem intervals from four different periods (1st–3rd century, 1878–1902, 1965–1972, 1995–2003). In addition to determining the sensitivity and specifitiy of the CL-analysis, likelihood ratios and predictive values were calculated. The degree of interobserver and intraobserver agreement was determined using Kappa statistics. Results Based on our sample, we found false positive results in approximately 7.5% of all examined historical cases and false negative results in approximately 15% of the recent samples. Altogether, a correct classification of 88.75% (overall classification) was found (sensitivity = 0.85, specificity = 0.925; likelihood ratio = 11.3; odds ratio = 69.8). Taking a-priori probabilities into consideration (prevalence for recent = 26%), a positive predictive value (PPV) of only 79.9% could be calculated, while the negative predictive value (NPV) lay at 94.6%. The results from subsequent Kappa statistics further illustrated that the quality of the results was, on the whole, observer independent ( κ = 0.93; 95% CI = 0.84–1.0). Intraobserver bias calculation yielded a value of κ = 0.975 (95% CI = 0.92–1.0). Conclusion The analysis of CL on dry bones in determining the postmortem interval is not valid as an only method. In other respects, the luminol test is reproducible, repeatable and cost-effective and could help in assessing the postmortem interval (PMI). The results of this study document the potential danger of forensically relevant cases being erroneously sorted out if further examinations do not take place. We would like to emphasize that still only a combination of different methods including the analysis of environmental conditions and the interpretation of the anthropological profile can allow an acceptable estimate of the PMI from skeletonized remains, even if the methodological spectrum is being expanded by different absolute dating methods using radionuclide techniques.

Playing children discovered several teeth close to the wall surrounding a village church. A subsequent police search yielded further teeth, three neurocranial fragments, a metacarpal bone and the fragment of a long tubular bone. The circumstances along with the results of the investigation and the morphological findings suggested historical bones from a former graveyard. However, a large fragment of bone shaft from a long bone could not definitely be classified as being of human origin. The tibia of a deer was amongst the possibilities considered. Comparative tests run at the Institute of Veterinary Anatomy of the University of Giessen in addition to histological examinations, however, ultimately established the human specificity. Interindividual variability and decomoositional changes were determined as causes for the conspicuous macroscopic and microscopic findings for the tibia fragment. Pathological changes could not be demonstrated.

So far, a reliable determination of the postmortem interval (PMI) of human bones or skeletal remains is possible neither by morphological examinations nor by extensive technical investigations. Concerning forensic osteological practice, in most cases, the question of the length of the PMI can be restricted to the exclusion of a forensically relevant lay time (in the present context corresponding to a PMI of >50 years). We reviewed 21 original publications from the literature and surveyed the results on the basis of our own experience with macroscopical findings of soil-embedded bones with known lay times. A total of 1259 bones and the respective findings were evaluated. Eleven macromorphological findings are presented that are of relevance when present and, in addition, five findings that are relevant through their absence for the determination of a PMI of more than 50 years. In none of the reviewed publications were these criteria described in soil-embedded bones with a lay time of less than 50 years. These elaborated findings are a basis for the macromorphological exclusion of a forensically relevant lay time of soil-embedded skeletal remains in a given case.

When one thinks of the so-called barrel or wide-angle distortion, grotesquely warped faces may come to mind. For less extreme cases with primarily inconspicuous facial proportions, the question, however, still arises whether there may be a resulting impact on the identification of faces. In the first experiment, 3 test persons were photographed at a fixed camera-to-object distance of 2 m. In the second experiment, 18 test persons were each photographed at a distance of 0.5 m and 2.0 m. For both experiments photographs were taken from a fixed angle of view in alignment with the Frankfurt Plane. An isolated effect of the focal length on facial proportions could not be demonstrated. On the other hand, changes in the camera-to-object distance clearly influenced facial proportions and shape. A standardized camera-to-object distance for passport photos, as well as reconstruction of the camera-to-object distance from crime scene photos and the use of this same distance in taking photographs for comparison of suspects are called for. A proposal to refer to wide-angle distortion as the nearness effect is put forward.

The use of computed tomography (CT) scanners is rapidly becoming established in forensic medicine. Current multislice CT (MSCT) scanners attain a resolution of 0.42 mm. An isolated skull with gunshot injuries was examined with a high-resolution eXplore Locus Ultra flat-panel CT (eLU-CT) and MSCT. Structures and minute fissures in the bone interior, which were neither visible macroscopically nor with the MSCT data, could be imaged with the eLU-CT data. In addition, a tiny interior impact defect from a retained missile could be detected by eLU-CT, which clearly aided the reconstruction of the gunshots in this case.

In an age of global combat against terrorism, the recognition and identification of people on document images is of increasing significance. Experiments and calculations have shown that the camera-to-subject distance – not the focal length of the lens – can have a significant effect on facial proportions. Modern passport pictures should be able to function as a reference image for automatic and manual picture comparisons. This requires a defined subject distance. It is completely unclear which subject distance, in the taking of passport photographs, is ideal for the recognition of the actual person. We show here that the camera-to-subject distance that is perceived as ideal is dependent on the face being photographed, even if the distance of 2 m was most frequently preferred. So far the problem of the ideal camera-to-subject distance for faces has only been approached through technical calculations. We have, for the first time, answered this question experimentally with a double-blind experiment. Even if there is apparently no ideal camera-to-subject distance valid for every face, 2 m can be proposed as ideal for the taking of passport pictures. The first step would actually be the determination of a camera-to-subject distance for the taking of passport pictures within the standards. From an anthropological point of view it would be interesting to find out which facial features allow the preference of a shorter camera-to-subject distance and which allow the preference of a longer camera-to-subject distance.

In the course of an exhumation performed 5.5 years after death, several bone fragments were uncovered during the excavation of the clay-rich soil. Amongst others, there was a large piece of the frontal neurocranium. In addition, a so-called coffin stain was discernible. The exhumed coffin was intact. The forensic autopsy revealed a complete corpse with distinct adipocere formation. Consultation with the cemetery administration allowed the conclusion that the additional bone fragments were from the first use of the grave approximately 100 years ago. The heavily soil-encrusted skull fragment bore clear signs of a half sharp force, that could immediately be classified as postmortem. The pattern of injury pointed to an excavator as the cause. However, the question arose whether the postmortem trauma occurred 5.5 years ago during the excavation of the grave or during the current exhumation. First the skull fragment was dried. However, it was then impossible to remove the clay-rich soil without damaging the bone. The fragment was therefore carefully washed and dried again. The cut and fracture areas then showed distinctly lighter surfaces than the rest of the bone, which pointed to the exhumation as the time of origin. For comparison, fresh injuries were inflicted with a hatchet. These distinctly showed even lighter surfaces, so that the time of origin could be assumed to have been during the excavation of the grave 5.5 years ago.

When two human corpses are found in temporal and locational context the assumption of a non-natural cause of death is close at hand. Suicide, extended suicide, homicide or accident (particularly with carbon monoxide) come to mind. When, however, a natural cause of death emerges for both after autopsy and further examinations, this provokes the question whether this incidence is coincidental. In our two presented cases, a married couple each was found dead together. In both cases, the wife needed nursing and was being cared for by a husband who died of cardiac disease shortly before the wife died. In one case, the wife died of acute uncompensated chronic congestive heart failure. In the other case, the cause of death of the wife was hypovolemic shock due to acute gastrointestinal bleeding from recurrent gastric mucosal erosion. It is to be discussed for both cases whether, given the pre-existing illness, the acute stress of the situation in view to the husbands' death may at least have favored the occurrence of death and may, thus, have led to the criminologically conspicuous situation of finding.

The article describes critical investigations concerning the threshold value between endogenous and exogenous concentrations of gamma-hydroxybutyric acid (GHB/"liquid ecstasy") in human blood. The values of GHB in the blood samples of 50 blood donors and 50 postmortem cases were measured with a validated gas-chromatographic/mass-spectrometric procedure according to the guidelines of the GTFCh (Society of Toxicological and Forensic Chemistry). GHB-concentrations were found to range between 0.11 and 1.56 mg/L (mean value 0.54 mg/L/standard deviation 0.37 mg/L/coefficient of variation 68.4 %) in the donors' blood, and between 2.2 and 116 mg/L (mean value 32.4 mg/L/standard deviation 25.6 mg/L/coefficient of variation 79 %) in the postmortem samples, respectively.

Immunoassays are used worldwide for the rapid screening of drugs. Despite the fact that they are highly valuable tools for the testing of legal and illicit drugs, there is a non-negligable risk of false-positive and false-negative findings and many pitfalls must be taken into account when using these tests in an uncritical manner and without valid confirmation procedures. In order to check the correlation between cloned-enzyme donor immunoassay (CEDIA) readings and exact determined gas chromatography / mass spectrometry (GC/MS) values for benzoylecgonine, a total of 472 serum samples was measured with an immunoassay (CEDIA) as well as GC/MS. As a result, it was shown that in the lower area of concentration, up to approx. 300 ng equ. benzoylecgonine/ml, there is a semiquantitative useful correlation. With higher concentrations, the values shift more and more apart and many runaways are visible. Generally the use of tests for materials other than those declared is excluded from the manufacturer's warranty and is at the user's own risk, but the authors found that many immunochemical tests with a certification only for the use in urine are well suited to the screening of serum samples, and even low readings could be confirmed by GC/MS. With blood or serum as sample, it could be regularly observed that generally very small readings could not only be confirmed with mass spectrometry but could in most cases also be quantified. This was the key factor in determining the rate of confirmation in 495 cases. In total, an incredibly high reliability for the CEDIA cocaine test (via benzoylecgonine) was shown up to the area of detection (detection limit), far below the cut-off. These results confirm the extraordinary sensitivity and specificity of this test also for blood or serum as sample material to be tested.

The definition of the term "corpse" is relevant for medicine in general and most particularly so for forensic medicine, as it has direct consequences on practical work: legally, postmortems and autopsies may only be performed on corpses. With this in mind, the legal term "corpse" was examined on the basis of German State Law regulations and provisions. Approximately half the burial acts include a definition of "corpse", but even here there are certain variations. The definitions for "corpse" can be divided into four categories: 1) according to the lack of vital signs or sure signs of death 2) according to the degree of decomposition and/or cohesion of body parts 3) on the basis of severed body parts, and 4) terminological differentiation of stillbirths and neonatal fatalities from miscarriages. Laws vary greatly in their definition of the term "corpse". Therefore, the decision whether or not human remains or parts thereof qualify as a "corpse" should be left to medical discretion.

Nowadays immunoassays are used for the rapid screening of drugs world-wide. Despite the fact that they are a highly valuable tool for the test of legal and illicit drugs, there is a non-negligible risk of false positive and false negative findings, and many pitfalls must be taken into account when using these tests in an uncritical manner and without valid confirmation procedures. In accordance with the committee for "problems of threshold values for medical and addictive drugs" set up by German scientific societies (in particular GTFCh, DGRM and DGVM), immunoassays are recommended as a useful tool for screening. But confirmation analysis with conclusive methods (GC/MS or LC/MS) is indispensable for valid substance identification, discrimination between active and inactive metabolites, detection of escort substances and accurate determination of concentrations in body fluids.

The Council Regulation (EC) no. 1493/1999 of 17 May 1999 on the common organisation of the market in wine (Abl. L 179 dated 14/7/1999) and the GMO Wine 2000 (Annex VII A) stipulates that the labels of wine bottles have to indicate, among others, information on the sales designation of the product, the nominal volume and the alcoholic strength. The latter must not differ by more than 0.5% vol. from the alcoholic strength as established by analysis. Only when quality wines are stored in bottles for more than three years, the accepted tolerance limits are +/- 0.8% vol. The presented investigation results show that deviations have to be taken into account which may be highly relevant for forensic practice.

During a parish fair the police was called to a conflict between two rival right-wing youth groups in the early morning hours. Shortly afterwards a burning car was discovered near the marquee with a charred male body on the driver's seat. It was assumed that there was a connection between the two events. At autopsy no injuries were found that were not attributable to postmortem consumption by the fire. The BAC value was 1.76 per thousand, the CO-Hb concentration 78%. The body could be identified by comparing pre- and postmortem X-rays of the molars and premolars of the right upper jaw. Reconstruction of the course of events and classification as an accident was possible only after calling in technical experts: The victim had gone back to his car to take a nap and kept the engine running because of the cold temperature. The car had been fitted improperly with a sport exhaust pipe, which caused both the carbon monoxide poisoning and the fire.

A reliable determination of the post mortem interval (PMI) in human bones has neither been found by morphological nor by intensive analyses of other kinds. In most cases of legal medicine practice the question of PMI is restricted to a forensically relevant resting period. In a survey based upon literature, all osteological signs of decomposition, which have not been described for a soil embedded resting period of under 50 years, were collected. 21 research reports could be found, describing macroscopic findings on soil-embedded human bones with known PMI. All in all, 1295 individuals were examined in these investigations. A censorious review of the summarized results found in literature reveals that the following findings have never been seen in a PMI of 50 years or less. Outer appearance: macroscopically no adipocere, deep incisions of the outer compact layer, extensive defects of the bone surface, intense brownish-black lawn of microorganisms, fraying of the outer lamellar system, lifting of the cortical area, torsion of the tissue, attached brushit, bone manually breakable. On freshly sawn cross sections: lack of adipocere, brushit in the marrow area, reduced or lack of UV-fluorescence.

The article describes problems related to immunochemical screening-tests with special regard to benzodiazepines, which are commonly used for therapeutic purposes. Focal points are adulteration, manipulation and handling of samples, the selection of cut-off-values, specificity and cross-reactivity, influence of biotransformation and frequently observed pitfalls in the interpretation of the analytical results.

The bodies of two men in their early forties were found lying in a peculiar position in an apartment. Police and medical investigations suggested that both had, at the most, died 2 weeks earlier. One of the men was kneeling, holding on to the back of a chair-with his hand, and was bent over the second man lying on the floor. Both men were known as former addicts, but were assumed to be clean for a long period of time. The autopsy revealed that the two men died from an acute heroine intoxication in combination with alcohol. The man who lay on the floor additionally had chronic heart disease. It could be excluded that any other persons were involved in this case and that other causes of death had to be considered. The peculiar scene could be explained by the obvious order of death and as a consequence of mummification.

The authors report on a case of a dicygotic female twin pair, one of them containing a T/C heteroplasmy in two (heart and brain) of five analysed organs at position nt204 of the HV2 region. Results and especially the evidence of an unequal distribution of the heteroplasmy are discussed.

This case report describes a suicidal gunshot to the head using a solid hollow-point bullet (Quick Defense). There was an irregular skin defect measuring 16 x 6 cm on the right side of the head and a bone defect of 9 x 6 cm while a 1.2 x 0.7 cm skin defect was located at the left temple. This atypical wound morphology caused confusion at the scene but during autopsy, a muzzle imprint and an abrasion ring were found in the frontal aspect of the large skin defect in the right temporoparietal region, which therefore had to be the entrance wound. In addition, extensive backspatter consisting of brain tissue, bone fragments and blood had been expelled from the huge entrance wound and had travelled up to a distance of 4.6 m. The extraordinary entrance wound cannot be fully explained by special anatomical or ballistic features such as a muzzle velocity of 420 m/s or the solid hollow-point design of the bullet.

Bone finds make great demands on the examiner. The first priority is to establish the remains as human, because then further analyses have to be performed for identification. Especially for bone fragments it may be extremely difficult to determine whether they are of human or animal origin. In the reported case a bone fragment similar to a human rib was found in a medieval town together with more than 300 non-human bones. As it turned out, a medieval cesspit had been opened during excavation works. Even an mtDNA analysis could not answer the question whether the rib was of human origin. A direct comparison with numerous ribs of humans and mammals showed the great interindividual variability, but in the end it was possible to rule out a human origin and to identify the fragment as the third left rib of a pig.

The article discusses the question if and under what conditions trace evidence may be stored for DNA analysis. The authors report on several cases in which methodological improvements or even the introduction of DNA technology itself helped to identify the perpetrator or to rule out a suspect, who may already have been wrongly convicted, many years after the offence. Under medical and scientific aspects these cases seem to speak in favour of unlimited storage. German law does not contain explicit regulations on the storage of trace evidence of unknown origin, although it seems admissible under existing law. In view of a minor need of protection as against a high interest in the preservation of trace evidence, storage for an unlimited period of time seems necessary.

Depending on local conditions it may happen from time to time that no officer involved in the previous investigation is present during the forensic autopsy. In this case, the post mortem examiner often has to rely solely on written documentation concerning the identity of the deceased. The authors report a case where the body had already been mixed up with another body by the undertaker prior to the inspection by the police. In spite of clear distinguishing characteristics in the written documentation, this led to the wrong body being autopsied. For the post mortem examiner, the only safe way to identify a body seems to be the presence of persons who had known the deceased or of police officers as acknowledging witnesses. If these conditions are not given, other means of safe information on the identity are called for, for example a "police body card" similar to an evidence card.

Forensic anthropology can make important contributions to the identification of disaster victims within the framework of Identification Commission of the German Federal Office of Criminal Investigation (IDKO) operations. The application of anthropological methods is particularly necessary in cases of advanced putrefaction, skeletonization or carbonization. The basic professional qualification required is a degree in physical anthropology or advanced training as a medico-legal specialist. Additionally, adequate practical experience and participation in continuing and advanced training courses are necessary.
