Studies evaluating combined penetrating keratoplasty and pars plana vitrectomy for infectious aetiologies are limited. To evaluate the outcomes of combined penetrating keratoplasty and pars plana vitrectomy in patients with endophthalmitis and poor corneal clarity.Retrospective interventional case series conducted at a tertiary care eye hospital in North India.Review of records of 43 eyes of 43 patients. Mean age of patients was 53.39 ± 12.94 years (26 males). Patients with age14 years with minimum follow-up of 6 months were included.Combined penetrating keratoplasty and pars plana vitrectomy was performed in all eyes.We analysed the preoperative, intraoperative and microbiological characteristics of patients undergoing combined surgeries. Anatomic and functional success and failure were pre-defined.Aetiology for corneal opacification was corneal ulcer in 30(69.7%) eyes, corneal graft infection in eight(18.6%) eyes, bullous keratopathy in four (9.3%) eyes and corneal scar in one eye. Postoperative visual acuity improved in 20(46.5%) eyes, did not change in 14 (32.5%) eyes and deteriorated in nine eyes (20.9%). Anatomical failure (uncontrolled infection leading to phthisis bulbi or evisceration) was seen in 15 (34.8%) eyes. Microbiological analysis revealed bacterial growth in 26, fungal in 14 and no growth in three eyes. Fungal infection had a poorer outcome (P = 0.03). Six out of 11 monoocular patients regained ambulatory vision.Combined penetrating keratoplasty and pars plana vitrectomy is a complex but globe salvaging procedure for poor prognosis eyes which otherwise may need evisceration. Fungal infection carries a poorer prognosis.

Purpose: To report our experience of using Endobronchial ultrasound-guided transbronchial lymph node aspiration (EBUS-TBNA) in assessing the etiology in uveitis from two tertiary eye-care centers in India. Materials & Methods: Retrospective interventional case series. Results: A total of 99 patients of uveitis who underwent EBUS-TBNA were included in the study. Mycobacterium tuberculosis (MTb) was detected on culture in 5 cases, 3 cases by polymerase chain reaction and caseating granulomas in 25 cases. None of these patients had any symptoms of systemic tuberculosis (TB). Biopsy was inconclusive in eight cases. Granulomatous inflammation was seen in 91 cases. In 51 cases, it was difficult to differentiate between sarcoidosis and TB on biopsy. None of the patients had any significant complications. There was a fair agreement between the biopsy diagnosis and the final ocular diagnosis. Conclusions: In high TB endemic countries, procedures like EBUS-TBNA are useful in obtaining tissue diagnosis of TB when ocular diagnosis may be inconclusive due to paucity of ocular tissue.

Aim The aim of the study was to test the antibacterial activity of manuka honey and compare its efficacy with another commercially available honey (Dabur honey) on the cariogenic bacteria on Streptococcus mutans and Lactobacillus. Materials and methods An in vitro study was carried out on 40 agar specimens; the samples were divided into two groups of 20 samples consisting of S. mutans and Lactobacil-lus respectively. The 20 samples in each group were further subdivided into four groups of five each, which were tested with 25% manuka honey, 100% manuka honey, 25% Dabur honey, and 100% Dabur honey for both Lactobacillus and S. mutans groups. The antibacterial activity was tested using the agar well diffusion method against S. mutans and Lacto-bacillus. Antibacterial activity was assessed by measuring the diameter of inhibition of zones surrounding the wells. The results obtained were statistically analyzed (one-way analysis of variance test, p-value). Results The results showed that 25% of manuka honey has statistically significant (p ≤ 0.001) antibacterial effect than 25% of Dabur honey on both Streptococcus and Lactobacillus species, and manuka honey with 100% concentration showed a statistically significant (p ≤ 0.001) antibacterial effect than 100% Dabur honey on the same species of bacteria. 100% of both the honeys showed statistically significant (p ≤ 0.001) antibacterial effect than 25% concentrations of the same on S. mutans and Lactobacillus. Conclusion Manuka honey had more antibacterial activity than Dabur honey on S. mutans and Lactobacillus bacteria in the in vitro study. This effect was dependent on the concentration of honey used. Every real-time application need to store large amount of data and processes this data is required for their successful operation [1],[2]. Evolution of real time system makes its applications more complex which require obtaining extent of data that is why it is important for us to manage data in an organized and systematic manner, so in past few years there came concept of “merging” database and real time technology which jointly called Real-Time database System (RTDBMS). All features of real time database system is same as traditional database system like-Data independence, Concurrency control, Transactions, Database schedules etc. however, a RTDBMS has the beyond responsibility of ensuring a certain level of confidence in meeting the system's timing prerequisites. Performance goal of RTDBMS is quite different from conventional database, primary and most important performance goals of RTDBMS's are — “correctness criteria” and “pre-assumption of application” whereas conventional database basically focuses on — “average response time” [3],[4]. Evaluation of RTDBMS can be done by calculating how often transactions are missing there deadlines, the cost encountered when transactions miss there deadlines, the average “lateness” and “tardiness” is also Calculated of transactions when deadline is being missed, data temporal consistency and data external consistency should also be known. Also conventional database differ from real time database in many aspects [9]: • They have variety of goals to perform the task. • Number of criteria to perform the task correctly. • Assumption of application. As real time system is basically emphasis on time constraints which are generally given by the designers of application.

Human papillomavirus (HPV) associated cervical cancer is the leading cause of deaths in India. However, cytological/HPV screening may result in early detection of cervical cancer, resulting in early treatment and reduced mortality. Although reports related to general population is available, data on HPV prevalence among women attending AFMS health care facilities is scarce. Cervical samples were collected for cytological staining by Pap test and molecular detection by PCR, genotyping by HPV specific primers and sequencing. Apart from finding of atypical cells of undetermined significance (ASCUS) in one subject, no evidence of malignancy was observed. A high prevalence of HPV was found in this study group, which was intermediate between previous reports from general population and cervical cancer patients. All the subjects had infection of high risk HPV type16. HPV prevalence was found similar between different age groups. Although, none of the study subjects had malignant changes, but due to high prevalence of high risk HPV infection and other associated risk factors, these subjects might be at an elevated risk of developing cervical cancer. Regular follow-up of these patients who were detected HPV positive are required to screen for cervical malignancy.

Type II alveolar epithelial cell (AEC) apoptosis is a prominent feature of fibrotic lung diseases and animal models of pulmonary fibrosis. While there is growing recognition of the importance of AEC injury and apoptosis as a causal factor in fibrosis, the underlying mechanisms that link these processes remain unknown. We have previously shown that targeting the type II alveolar epithelium for injury by repetitively administering diphtheria toxin to transgenic mice expressing the diphtheria toxin receptor off of the surfactant protein C promoter (SPC-DTR) develop lung fibrosis, confirming that AEC injury is sufficient to cause fibrosis. In the present study, we find that SPC-DTR mice develop increased activation of caspase 3/7 after initiation of diphtheria toxin treatment consistent with apoptosis within AECs. We also find evidence of efferocytosis, the uptake of apoptotic cells, by alveolar macrophages in this model. To determine the importance of efferocytosis in lung fibrosis, we treated cultured alveolar macrophages with apoptotic type II AECs and found that the uptake induced pro-fibrotic gene expression. We also found that the repetitive intrapulmonary administration of apoptotic type II AEC or MLE-12 cells induces lung fibrosis. Finally, mice lacking a key efferocytosis receptor, CD36, developed attenuated fibrosis in response to apoptotic MLE-12 cells. Collectively, these studies support a novel mechanism linking AEC apoptosis with macrophage pro-fibrotic activation via efferocytosis and reveal previously unrecognized therapeutic targets.

Interleukin 17A (IL-17A) and complement (C') activation have each been implicated in the pathogenesis of idiopathic pulmonary fibrosis (IPF). We have reported that IL-17A induces epithelial injury via TGF-β in murine bronchiolitis obliterans; that TGF-β and the C' cascade present signaling interactions in mediating epithelial injury; and that the blockade of C' receptors mitigates lung fibrosis. In the present study, we investigated the role of IL-17A in regulating C' in lung fibrosis. Microarray analyses of mRNA isolated from primary normal human small airway epithelial cells indicated that IL-17A (100 ng/ml; 24 h; n = 5 donor lungs) induces C' components (C' factor B, C3, and GPCR kinase isoform 5), cytokines (IL8, -6, and -1B), and cytokine ligands (CXCL1, -2, -3, -5, -6, and -16). IL-17A induces protein and mRNA regulation of C' components and the synthesis of active C' 3a (C3a) in normal primary human alveolar type II epithelial cells (AECs). Wild-type mice subjected to IL-17A neutralization and IL-17A knockout (il17a-/- ) mice were protected against bleomycin (BLEO)-induced fibrosis and collagen deposition. Further, BLEO-injured il17a-/- mice had diminished levels of circulating Krebs Von Den Lungen 6 (alveolar epithelial injury marker), local caspase-3/7, and local endoplasmic reticular stress-related genes. BLEO-induced local C' activation [C3a, C5a, and terminal C' complex (C5b-9)] was attenuated in il17a-/- mice, and IL-17A neutralization prevented the loss of epithelial C' inhibitors (C' receptor-1 related isoform Y and decay accelerating factor), and an increase in local TUNEL levels. RNAi-mediated gene silencing of il17a in fibrotic mice arrested the progression of lung fibrosis, attenuated cellular apoptosis (caspase-3/7) and lung deposition of collagen and C' (C5b-9). Compared to normals, plasma from IPF patients showed significantly higher hemolytic activity. Our findings demonstrate that limiting complement activation by neutralizing IL-17A is a potential mechanism in ameliorating lung fibrosis.-Cipolla, E., Fisher, A. J., Gu, H., Mickler, E. Tuberculosis (TB) is an important cause of infectious uveitis causing significant morbidity and mortality especially in endemic countries. Mycobacterium tuberculosis (MTb) may infect any tissue or organ in the body, lungs being the most commonly affected. Ocular TB occurs due to hematogenous dissemination of MTb or due to a hypersensitivity reaction to the bacteria. It may lead to a very severe inflammation and destruction of ocular tissues leading to an irreversible loss of vision. Infection of the eye by MTb most commonly involves the uveal tissue causing anterior, intermediate, and posterior or panuveitis. Awareness of the varied clinical manifestations is important to suspect the ocular TB which may often be difficult to confirm by diagnostic tests available. A timely diagnosis and management may help in preventing severe loss of vision and the eye.

Progressive fibrosis involves accumulation of activated collagen-producing mesenchymal cells. Fibrocytes are hematopoietic-derived cells with mesenchymal features that potentially have a unique and critical function during fibrosis. Fibrocytes have been proposed as an important direct contributor of type I collagen deposition during fibrosis based largely on fate-mapping studies. To determine the functional contribution of hematopoietic cell-derived type I collagen to fibrogenesis, we use a double-transgenic system to specifically delete the type I collagen gene across a broad population of hematopoietic cells. These mice develop a robust fibrotic response similar to littermate genotype control mice injured with bleomycin indicating that fibrocytes are not a necessary source of type I collagen. Using collagen–promoter GFP mice, we find that fibrocytes express type I collagen. However, fibrocytes with confirmed deletion of the type I collagen gene have readily detectable intracellular type I collagen indicating that uptake of collagen from neighboring cells account for much of the fibrocyte collagen. Collectively, these results clarify several seemingly conflicting reports regarding the direct contribution of fibrocytes to collagen deposition.

Tubercular uveitis is a common manifestation of tuberculosis (TB) in TB-endemic countries. Due to lack of gold standard diagnostic tests for confirming intraocular TB, it is often either underdiagnosed or rarely over diagnosed. We report a case of a 28-year-old woman with diminution of vision in the left eye. She was started on antitubercular treatment (ATT) and steroids but she did not comply with the treatment. She developed progressive painful loss of vision consequently and was treated with vitrectomy and silicone oil tamponade. After a complete course of ATT, she recovered, salvaging of eye and restoration of some vision was possible. Mycobacterium tuberculosis is rarely demonstrated in the ocular fluid samples of the suspected cases of TB due to the paucibacillary nature of the disease. A vitreous sample can be obtained from these patients. Surgical intervention in the form of vitrectomy and silicone oil tamponade can help in avoiding enucleation/evisceration.

Aim: To assess the constraints among patients while opting dental implant as a treatment option among the randomly selected sample of dental patients attending the outpatient department (OPD) of Vaidik Dental College and Research Centre, Daman, India. Materials and Methods: Information on demographic characteristics, knowledge about implant as an option for missing tooth replacement, source of information, and knowledge about other options of tooth replacement were obtained from patients visiting various dental OPDs of hospital and private dental clinics using nationwide self-explanatory survey. Results: Among the 1000 responses retrieved, 27% of the respondents felt moderately well informed about the dental implant treatment. Only 9% of the respondents had dental implant treatment before and 17% felt well informed about the different alternatives of replacing missing teeth. Dentists were the main source of information regarding dental implant treatment modality followed by friends and electronic media. A total of 55.6% of the respondents felt implant to be as good as own teeth in function, whereas high cost was the major limiting factor for implant treatment. Conclusion: A total of 56% of Daman population were aware of dental implant as an alternative for replacing missing teeth. Necessary efforts and measures should be made to raise the awareness of dental implant treatment in the country.

Background: Drying the root canals in pulp therapy is often ignored, but is essential for a successful clinical outcome. The conventional method used for drying root canals is by the use of paper points, but recently, various other methods have also been employed for this purpose. Aim: The purpose of this study was to volumetrically analyze root canal fillings in deciduous teeth, after using different canal-drying methods namely 95% ethanol, CANAL CLEAN (Cerkamed Medical Company), and Endo-Aspirator (Cerkamed Medical Company), and compare with the conventional paper point drying method. Methodology: Access cavities were prepared on eighty extracted primary canines, and irrigation was done with 1% sodium hypochlorite and normal saline while enlarging the canals. The specimens were then scanned using a cone-beam computed tomography (CBCT), and preobturation volume (X) of each tooth was measured. The teeth were then randomly divided into two groups – Group 1 – Zinc oxide eugenol (ZOE) group and Group 2 – Metapex group. Each group was further divided randomly into four subgroups based on the drying agent used – Subgroup A – Control group, Subgroup B – 95% ethanol group, Subgroup C – CANAL CLEAN group, and Subgroup D – Endo-Aspirator group. After obturation, a second CBCT was taken to measure the postobturation volume (Y). The percentage of obturated volume was calculated by the following formula: (postobturation volume/preobturation volume) ×100 ([Y/X] ×100). Statistical Analysis: The obtained data were tabulated and subjected to statistical analysis using one-way ANOVA test and unpaired t-test. Results: A significantly high postobturation volume was seen after using 95% ethanol followed by CANAL CLEAN, Endo-Aspirator, and paper points. On comparing the obturation volumes within Metapex and ZOE groups, Metapex group had significantly high obturation volumes irrespective of the drying method used. Conclusion: 95% ethanol is the best intracanal drying agent as it provides optimum pupal obturation.

Purpose: We conducted a prospective, randomized study to evaluate the efficacy of dexmedetomidine as an additive to peribulbar block for vitreoretinal surgery in terms of onset time of block, hemodynamic stability profile, patient comfort, and surgeon satisfaction. Methods: One hundred patients of American Society of Anesthesiologists grade 1 and 2 scheduled for vitreoretinal surgery were randomly assigned into two groups: control group (n = 50) received lignocaine bupivacaine block, and Dex group (n = 50) received lignocaine bupivacaine plus 20 μg dexmedetomidine peribulbar block. Information regarding time for onset of block, hemodynamic data, visual analog scale for pain, sedation levels, total duration of surgery, and surgeon satisfaction levels were collected. Results: All the demographic characteristics including age, gender, American Society of Anesthesiologists grade, onset of anesthesia, and duration of surgery were comparable in both groups. At the baseline, there was no statistically significant difference in heart rate, mean arterial pressure, diastolic blood pressure, and respiratory rate between the two groups, with a difference noted in systolic blood pressure at the baseline. There was significant difference noted in the systolic blood pressure and mean arterial pressure at different time intervals with a decreasing trend as time progressed. The mean sedation score was significantly higher in the Dex group than that in the control group. The surgeon satisfaction was higher in the Dex group than that in the control group. Conclusion: Dexmedetomidine is a useful and safe drug in combination with lignocaine bupivacaine in peribulbar for vitreoretinal surgery as it maintains hemodynamic stability and provides sedation, which enables full cooperation and potentially better operating conditions.

A multiprocessor which shares the memory among processors and uses multiples translation lookaside buffers (TLBs) can face various problems. One such problem is the problem of inconsistency which may occur when the page table entry (PTE) is updated because of the multiple copies of same page table entry in various TLBs. Commonly, the inconsistency problem exists in virtually tagged caches, which keep page table entry information, like reference bit, dirty bit and protection bit, in every cache line. This paper describes the problem of translation lookaside buffer inconsistency in multiprocessors with shared memory. First of all problem statement is given. Then paper presents the argument that inconsistency problem exist even when in cache address translation removes a separate translation lookaside buffer. Later, the TLB inconsistency classification and causes of inconsistency because of operating system operations are discussed.

The study was conducted with the aim of investigating the effects of perception of the hospital workplace factors on the job satisfaction and psychological well-being of a sample of paramedical health care employees (N =200) from a medical college (teaching) hospital and public (non-teaching) government-run hospitals. Statistical analysis of the data revealed that despite significant differences in perceived work-place factors, the reported levels of psychological well-being and job satisfaction of the participants were nearly similar in the two types of public hospitals. Stepwise regression analysis of the data revealed that the organization’s structure-related factors, co-ordination and work autonomy were significantly predictive of job satisfaction and psychological well-being of health care employees, while the process-related workplace factors, participative decision making and intra-professional relations, emerged as significant predictors of psychological well-being and job satisfaction in both types of hospitals. Results have significant implications for public hospitals which are currently facing competition from the state-of-the-art health care technology being introduced by private hospitals, and are therefore under pressure for retention of skilled health care employees.

Pulp exposures secondary to caries are the most common in primary teeth due to the relatively large size of pulp chamber. Pulp in primary teeth is capable of healing, following control of infection and inflammation. Pulpotec and LSTR are the simple noninstrumentation endodontic treatment procedures which are capable of compensating for the inconvenience caused by conventional pulpectomy, and thereby preserve the vitality of the pulp. This study was conducted to assess clinical efficacy of pulpotec and LSTR and compare with that of conventional ZOE pulpectomy at 1, 3, 6 and 12 months postoperatively. Around 34 children in the age group of 4 to 9 years with deep carious lesions affecting the pulps of 60 primary mandibular molars were randomly divided into three groups with 20 teeth in each group. Results Clinical evaluation was done at 1 month's interval. Both clinical and radiographic evaluations were done at 3, 6 and 12 months. Data obtained was analyzed statistically using Fisher's exact test. The results concluded that pulpotomy and pulpotec could be a good alternative for conventional ZOE pulpectomy. Long-term radiographic evaluations should be undertaken to further strengthen the efficacy of lesion sterilization and tissue repair (LSTR) as NIET.

The present study examined the effects of certain hospital workplace factors on job involvement among healthcare employees at the paramedical levels and quality of patient care in public hospitals in North India. The sample consisted of paramedical healthcare employees (N = 200), from a medical college affiliated teaching hospital and public hospitals (non-teaching) run by the railway services. Data were analyzed statistically to examine the hypothesized differences between the two types of hospitals. Results showed that employee perceptions of the workplace factors, namely, participation in decision making, innovative practices, perceived organizational support and role efficacy were significantly different in the teaching and non-teaching hospitals. Stepwise regression analysis of the data revealed that the factor ‘innovative practices’ was a positive predictor of job involvement in the non-teaching hospitals but a negative predictor of in job involvement in the teaching hospital. Job involvement significantly mediated the relationship between the perception of workplace factors and quality of patient care only in the non-teaching hospitals. The present study has important implications for initiating changes in the hospital system, particularly in public hospitals, where no systematic study has yet been conducted to determine the effects of workplace factors on job involvement of healthcare professionals and the quality of patient care in India.

BACKGROUND Cyclin dependent kinase inhibitor 2A/B (CDKN2A/B) genes are implicated in many malignancies including acute lymphoblastic leukemia (ALL). These tumor suppressor genes, with a key regulatory role in cell cycle are located on chromosome 9p21.3. Previous studies involving CDKN2A/B gene deletions have shown mixed associations with survival outcome in childhood ALL. PROCEDURE Hundred and four newly diagnosed children with ALL (1-14 years) were enrolled in this study. Genomic DNA from pretreatment bone marrow/peripheral blood samples of these children was investigated for copy number alterations in CDKN2A/B genes using multiplex ligation dependent probe amplification assay. Immunophenotype subtyping and cytogenetic and molecular analysis of ALL was performed at start of induction chemotherapy in all children. Children were monitored for response to prednisolone (Day 8), complete morphological remission, and minimal residual disease at the end of induction. The minimum postinduction follow-up period was 6 months. RESULTS CDKN2A/B deletions were seen in 19.8% (18/91) of B lineage acute lymphoblastic leukemia (B-ALL) and 38.5% (5/13) of T lineage acute lymphoblastic leukemia (T-ALL). Monoallelic CDKN2A/B deletions were found in 61.1% of total deletions in B-ALL while all the children with T-ALL harbored biallelic deletions. The prevalence of CDKN2A/B gene deletions was found to be significantly higher in older children (P = 0.002), in those with higher leukocyte count (P = 0.037), and in National Cancer Institute high risk group patients (P = 0.001) in the B-ALL subgroup. Hazard ratio was significantly high for children with CDKN2A/B deletions in total cohort (P = 0.004). Children with CDKN2A/B deletion had significantly lesser event free survival (P = 0.03). CONCLUSIONS CDKN2A/B deletions were significantly more prevalent in T-ALL subgroup and were found to have higher hazard ratio and lesser event free survival in total cohort in our study.

Background: Preeruptive caries are an abnormal, well-circumscribed, radiolucent area, occurring within the coronal dentinal tissue close to the dentinoenamel junction of unerupted teeth. OPGs are commonly taken in children for assessing the dental age and eruption sequence. It can be used as a good diagnostic source for detection of PEIR defects. Aim: The aim of this study is to determine the prevalence of Preeruptive intracoronal dentin radiolucencies in unerupted permanent teeth from orthopantomogram. Settings and Design: A total of 1000 standard orthopantomographs of children below 12 years of age were collected and examined. Methodology: The OPGs were examined for PIER defects in the unerupted teeth. The prevalence of PEIR defects was assessed with respect to age, sex, tooth, and the arch involved. Statistical Analysis: The data obtained were tabulated and analyzed. The prevalence was calculated in percentage. Results: The participant prevalence was 13.6%. The teeth prevalence was 1.20%. The majority (38.9%) of defects were seen in the in the mandibular first premolar. Almost half of the lesions were located on the mesial side (52.3%), with a size less than one-third of dentine thickness (53%). Conclusion: PIER defects constitutes an important part of anomalies associated with unerupted teeth and thus needs a proper understanding of its prevalence, etiology, manifestations, and complications. These defects are usually overlooked by clinicians while interpreting radiographs; however, it is of utmost importance to promptly diagnose these defects thus preventing further complications.

The present publication provides a comprehensive wood anatomical survey of woods of Indian tree species of the family Anacardiaceae. Thirtyfive species belonging to 19 genera are described as per the feature list of IAWA. Intrusive fibre cavities and perforated ray cells have been reported in Holigarna arnottiana and Pistacia terebinthus respectively. Two species, Choerospondias axillaris and Rhus hookeri, lacked helical thickening despite being ring-porous. Most tribes of the Anacardiaceae appear to be heterogeneous wood anatomically, except Semecarpeae which are homogeneous. The tribes Mangiferae and Semecarpeae are quite similar and may be placed together. Interesting findings were made regarding Indian species of Rhus, which can be divided into two groups. It is suggested to recognise Rhus Group II as a separate section. The ecological trends suggest that anatomical differentiation exists between tropical and temperate species as well as deciduous and evergreen species.
