Abstract

Background Metabolomics, the comprehensive profile of small-molecule metabolites found in biological specimens, has the potential to provide insights into the pathogenesis of disease states and lead to the identification of new biomarkers. Abstract

Background: Hypertrophic cardiomyopathy (HCM) is a genetic paradigm of cardiac hypertrophy. Cardiac hypertrophy is a major determinant of risk of sudden death and morbidity in HCM. Treatment with statins reverses hypertrophy in animal models of HCM. Thus, statins may afford therapeutic benefits in HCM. Methods: We performed a feasibility study with atorvastatin to gather the pre-requisite data for designing randomized efficacy studies. Results: We screened 32 patients with HCM in 18 months. Twenty-one patients met the study criteria and consented to participate. The demographics and echocardiographic phenotype of those who did and those who did not participate were not significantly different. We treated the participants with escalating doses of atorvastatin (20, 40 and 80 mg day-1) for 2 years. We performed ECG and echocardiography and measured plasma lipids, liver enzymes, creatine kinase and B-type natriuretic peptide levels before and after 3, 6, 12 and 24 months of therapy. Fifteen, 12 and 11 patients completed 6, 12 and 24 months of therapy respectively. Six patients discontinued atorvastatin because of perceived lack of benefit. We stopped atorvastatin in 4 patients because of modest elevations in liver enzymes, creatine kinase or back pain. The characteristics of those who did or did not complete the study were not significantly different. The mean plasma low-density lipoprotein-cholesterol level was reduced by 55%. However, echocardiographic indices of cardiac hypertrophy and function remained unchanged. Conclusions: The findings illustrated the challenges that will be encountered in designing efficacy studies to test the potential beneficial effects of atorvastatin in human HCM. © 2010 The Authors. Abstract

Epothilones are a novel class of microtubule-targeting anticancer agents that have a similar mechanism of action to the taxanes (i.e. polymerization of tubulin dimers into microtubules and stabilization of preformed microtubules) but show antiproliferative activity in taxane-resistant tumor cells. In order to study the epothilone B (EpoB)-induced neurotoxicity in clinical practice we have investigated the EpoB toxic effect in vitro and we have characterized in vivo his general and neurological side effects in the Wistar and Fischer rats. The in vitro experiment performed in Dorsal Root Ganglia explants showed a dose-dependent effect of EpoB exposure on neurite elongation; concentrations equal to 50nM or higher induced a significant reduction in neurite elongation after 24h and this effect was even more severe after 48h. A dose-dependent neurotoxic effect was confirmed in both in vivo studies. In these models EpoB peripheral neurotoxicity was investigated with neurophysiological, behavioral (Hot plate test) and pathological (morphologic and morphometric analysis, Intraepidermal Nerve Fibers density) methods. Moreover, in order to evidence the EpoB effect at the molecular level we focused on tubulin and myelin proteins. All the results consistently demonstrated that EpoB was more toxic in Fischer than in Wistar rats at the general and neurological levels in the dose range 0,25-1,5 mg/kg. Our results describing the dose-dependent neurological effects of EpoB and demonstrating that tubulin hyper-polimerization occurs at neurotoxic doses can be a solid basis for future studies

209. EVALUATION OF THE CONTINUOUS BUPRENORPHINE DELIVERY ANALGESIC EFFECT IN AN EXPERIMENTAL RAT MODEL OF PAINFUL DIABETIC NEUROPATHY

Abstract

Diabetic neuropathy (DN) can induce loss of nociception, as well as mechanical hyperalgesia and tactile allodynia. Recent pharmacological and clinical studies have shown that buprenorphine, a low molecular weight, lipophilic, opioid analgesic, acts not only on nociceptive and visceral pain, but also on neuropathic pain. To assess the analgesic effect of buprenorphine we used a well-established experimental rat model of painful DN in which two buprenorphine doses were administered by implantable Alzet® osmotic pumps for 3 weeks. Diabetic rats developed severe hyperglycaemia, a marked weight gain reduction and a decrease in soleum and extensor digitorum longus muscles weight vs. control rats. Buprenorphine treatment did not modify all these diabetes-induced alterations. Na+,K+-ATPase activity was significantly reduced in diabetic animals and buprenorphine administration increased this activity. After 6 weeks of diabetes, nerve conduction velocity (NCV) and behavioural responses to noxious mechanical and thermal stimuli were assessed. Diabetic rats showed an impairment of NCV, mechanical allodynia and thermal hypoalgesia. Both doses of buprenorphine significantly reverted the DN-induced allodynia up to day 7 of treatment. Buprenorphine did not alter either thermal perception or NCV. In conclusion, buprenorphine induced analgesia in short term period, suggesting a possible use in the management of patients with DN-associated neuropathic pain. Abstract

Il glaucoma secondario colpisce il cane con frequenza maggiore di quanto apparentemente appaia. Con questo lavoro si cerca di individuare eventuali correlazioni che possono esistere fra patologie primitive e razza, sesso,età, ecc. con il glaucoma secondario. Su di un gruppo di 475 pazienti affetti da patologie oculari, sono stai individuati 111 con glaucoma secondario. Per l’elaborazione statistica è stato impiegato il software SPSS for Windows versione 13.0 inc. Chicago Illinois 2004., utilizzando il test ² di Pearson. Per la quale sono stati considerati due gruppi di animali: un primo costituito da soggetti affetti da glaucoma secondario e un secondo costituito dai restanti soggetti affetti da altra patologia oculare. Il test ² è stato impiegato per valutare la possibile correlazione di questi gruppi con le variabili indagate nella casistica. È stato possibile utilizzare il test del 2 su due variabili non parametriche. Fra i dati significativi trovati citiamo: l’incidenza del glaucoma secondario presenta una differenza significativa nel corso degli anni, con picco di maggiore incidenza nella fascia di età compresa fra i 5 ed i 10 anni; nella ricerca di una possibile correlazione fra patologia primaria e glaucoma secondario, esiste un buon grado di probabilità che in caso di lussazione del cristallino si verifichi il glaucoma. Infine i risultati ottenuti sono stati confrontati con quelli della Letteratura a disposizione.

Abstract

AIM: Diabetic peripheral neuropathy (DPN) is a complication of inadequately treated diabetes mellitus leading to sensory loss and neuropathic pain. DPN of different severity is present during in the majority of diabetic patients. Although most patients with DPN do not have pain, a clinically-relevant proportion of them complain of chronic painful symptoms that affect their quality of life, disrupt sleep, and can lead to depression. Recent advances in understanding DPN-associated pain are likely to provide significant pathophysiological insights and offer better therapies. Opioids can be effective in the management of severe neuropathic pain. Buprenorphine is a low molecular weight, lipophilic, opioid analgesic, recently available as a transdermal matrix patch formulation (Buprenorphine TDS). Recent pharmacological and clinical studies shown that buprenorphine acts not only on nociceptive and visceral pain, but also on neuropathic pain (1). Aim of this study was to evaluate through a multimodal approach the analgesic effect of buprenorphine in an experimental rat model of painful DPN. MATHERIALS&METHODS: Diabetes was induced in Sprague Dawley rats by a single intraperitoneal injection of steptozotocin (STZ, Sigma-Aldrich) at the dose of 60mg/kg. After 1 month, buprenorphine at doses of 7.2 ug/24h and 14.4 ug/24h for 7 days was gradually administered by implantable osmotic pumps (ALZET) in diabetic rats. General toxicity was monitored by body weight measure while neuropathic conditions were evaluated by tail nerve conduction velocity (SNCV). Abstract

1-Deoxysphingolipids (1-deoxySLs) are atypical neurotoxic sphingolipids that are formed by the serinepalmitoyltransferase (SPT). Pathologically elevated 1-deoxySL concentrations cause hereditary sensory and autonomic neuropathy type 1 (HSAN1), an axonal neuropathy associated with several missense mutations in SPT. Oral L-serine supplementation suppressed the formation of 1-deoxySLs in patients with HSAN1 and preserved nerve function in an HSAN1 mouse model. Because 1-deoxySLs also are elevated in patients with type 2 diabetes mellitus, L-serine supplementation could also be a therapeutic option for diabetic neuropathy (DN). This was tested in diabetic STZ rats in a preventive and therapeutic treatment scheme. Diabetic rats showed significantly increased plasma 1-deoxySL concentrations, and L-serine supplementation lowered 1-deoxySL concentrations in both treatment schemes (P < 0.0001). L-serine had no significant effect on hyperglycemia, body weight, or food intake. Mechanical sensitivity was significantly improved in the preventive (P < 0.01) and therapeutic schemes (P < 0.001). Nerve conduction velocity (NCV) significantly improved in only the preventive group (P < 0.05). Overall NCV showed a highly significant (P = 5.2E-12) inverse correlation with plasma 1-deoxySL concentrations. In summary, our data support the hypothesis that 1-deoxySLs are involved in the pathology of DN and that an oral L-serine supplementation could be a novel therapeutic option for treating DN.

Abstract

Background Scores for cardio-embolic and bleeding risk in patients with atrial fibrillation are described in the literature. However, it is not clear how they co-classify elderly patients with multimorbidity, nor whether and how they affect the physician's decision on thromboprophylaxis. Methods Four scores for cardio-embolic and bleeding risks were retrospectively calculated for ≥ 65 year old patients with atrial fibrillation enrolled in the REPOSI registry. The co-classification of patients according to risk categories based on different score combinations was described and the relationship between risk categories tested. The association between the antithrombotic therapy received and the scores was investigated by logistic regressions and CART analyses. Results At admission, among 543 patients the median scores (range) were: CHADS22 (0-6), CHA2DS2-VASc 4 (1-9), HEMORR2HAGES 3 (0-7), HAS-BLED 2 (1-6). Most of the patients were at high cardio-embolic/high-intermediate bleeding risk (70.5% combining CHADS2and HEMORR2HAGES, 98.3% combining CHA2DS2-VASc and HAS-BLED). 50-60% of patients were classified in a cardio-embolic risk category higher than the bleeding risk category. In univariate and multivariable analyses, a higher bleeding score was negatively associated with warfarin prescription, and positively associated with aspirin prescription. The cardio-embolic scores were associated with the therapeutic choice only after adjusting for bleeding score or age. Conclusion REPOSI patients represented a population at high cardio-embolic and bleeding risks, but most of them were classified by the scores as having a higher cardio-embolic than bleeding risk. Yet, prescription and type of antithrombotic therapy appeared to be primarily dictated by the bleeding risk. © 2013 European Federation of Internal Medicine.

Abstract

We discuss the case of a rare and often unrecognized neurologic syndrome, called Acquired Hepatocerebral Degeneration (AHD), observed in patients with advanced liver disease and portosystemic shunts. The clinical manifestations can be very heterogeneous and in our case included a combination of cerebellar and extrapyramidal signs, arisen in a period of few days. Brain Magnetic Resonance Imaging (MRI) showed, in T1-weighted images, diffuse bilateral hyper intensities in basal ganglia and biemispheric brain and cerebellar cortices, resembling paramagnetic deposits. No other neurological impairments, like stroke, infection or neoplasia, were found. It was excluded an episode of acute hepatic encephalopathy. We also ruled out Wilsonian degeneration, iron overload and autoimmune encephalitis and we lastly found high manganese levels as the possible cause of the brain paramagnetic deposits. Even though either serum Mn determination or its accumulation in the brain are not specific for AHD, however the chronic and progressively worsening of the neurological manifestations advocated a degenerative condition, possibly AHD. We finally opted for the early restoration of liver function by OLT, and we observed complete clinical symptoms' resolution and partial MRI reversal after a follow up of 6 months. Abstract

Aims Accumulation of carbon dioxide ( CO2) in cucumber fermentations is known to cause hollow cavities inside whole fruits or bloaters, conducive to economic losses for the pickling industry. This study focused on evaluating the use of a malic acid decarboxylase ( MDC)-deficient starter culture to minimize CO2 production and the resulting bloater index in sodium chloride-free cucumber fermentations brined with CaCl2. Methods and Results Attempts to isolate autochthonous MDC-deficient starter cultures from commercial fermentations, using the MD medium for screening, were unsuccessful. The utilization of allochthonous MDC-deficient starter cultures resulted in incomplete utilization of sugars and delayed fermentations. Acidified fermentations were considered, to suppress the indigenous microbiota and favour proliferation of the allochthonous MDC-deficient Lactobacillus plantarum starter cultures. Inoculation of acidified fermentations with L. plantarum alone or in combination with Lactobacillus brevis minimally improved the conversion of sugars. However, inoculation of the pure allochthonous MDC-deficient starter culture to 107 CFU per ml in acidified fermentations resulted in a reduced bloater index as compared to wild fermentations and those inoculated with the mixed starter culture. Conclusions Although use of an allochthonous MDC-deficient starter culture reduces bloater index in acidified cucumber fermentations brined with CaCl2, an incomplete conversion of sugars is observed. Significance and Impact of the Study Economical losses due to the incidence of bloaters in commercial cucumber fermentations brined with CaCl2 may be reduced utilizing a starter culture to high cell density. Context Pancreatic arteriovenous malformations are a rare entity that can be incidentally discovered during MDCT examinations. Case report We describe a rare case of asymptomatic arteriovenous malformation presenting at MDCT as a hypervascular mass in the pancreatic head.
