210. Sociodemographic and Pregnancy Events Associated with Women Who Declined Vaginal Birth after Caesarean Section.

Abstract

Objective: Certain sociodemographic characteristics and pregnancy events were shown to pre-dispose women to decline vaginal birth after cesarean section (CS) in the Western population. The study aimed to identify the sociodemographic factors and pregnancy events associated with Chinese women who declined to attempt vaginal birth after CS. Study design: This was a retrospective study of Hong Kong Chinese women who delivered in a public obstetric unit between 2002 and 2006. It was the unit’s policy that women should attempt vaginal birth after one previous lower segment CS. During the study period, 1317 healthy Hong Kong Chinese women carrying a singleton pregnancy had had one previous lower segment CS. Of these women, 787 had had no antenatal complications during their index pregnancy. Univariate and multivariate analyses were performed to identify the sociodemographic factors and pregnancy events associated with women who declined vaginal birth after CS. Results: Thirty-nine percent (39.26%; 95%CI 35.85, 42.67) of women had elective CS because they had declined a vaginal birth. A history of a previous successful vaginal birth was the strongest determinant negatively associated with declining a vaginal birth (OR 0.15; 95%CI 0.06, 0.37). Determinants that were positively associated with declining a vaginal birth were higher family social class (OR 1.61; 95%CI 1.02, 2.55); family income of more than US$ 3850 per month (OR 1.56; 95%CI 1.03, 2.34), and women who attended private antenatal care before their first visit to the public unit (OR 1.47; 95%CI 1.02, 2.12). Conclusions: Women who experienced a previous successful vaginal birth were positively associated with attempting vaginal birth after CS. Women who declined vaginal birth after CS exhibited certain social characteristics and treatment-seeking behavior. Future studies into the social context of this group of women may help us understand the underlying reasons why they decline vaginal birth. 2008 Elsevier Irelan

Abstract

Pseudoamniotic band syndrome (PABS) is a rare iatrogenic complication that arises after invasive procedures in monochorionic twins. We report 3 cases of PABS, 2 after fetoscopic laser photocoagulation and 1 after bipolar cord coagulation. Two cases were detected antenatally by ultrasound; out of the two, one underwent successful fetoscopic release of amniotic band, which is the first report in twin pregnancy to our knowledge. In our centre, the incidence of PABS was found to be 2%. There were 25 cases of PABS reported previously, of which 12 cases with clinical details were reviewed together with our 3 cases. The fetal limbs were involved in all 15 cases, leading to constriction or amputation. The umbilical cord was involved in 2 cases, resulting in fetal death in one and pregnancy termination in the other. Antenatal detection of PABS is rare (27%; 4/15) as this requires a high index of suspicion. Serial postoperative targeted ultrasound surveillance of the fetal limbs and umbilical cord is necessary, particularly when features of septostomy or chorioamniotic membrane separation are found. Colour Doppler examination for the perfusion of the affected limb should be performed when PABS is detected. Fetoscopic release of amniotic band could salvage the fetal limb from amputation when impaired blood flow is detected. [ABSTRACT FROM AUTHOR]

224. Quantification of force applied during external cephalic version.

Abstract

External cephalic version (ECV) involves turning a fetus in utero by manipulation through the maternal abdomen and the uterine wall., Many clinicians and patients, however, still decline ECV in favour of Caesarean section. This could be due to the lack of experience of ECV, and fear of complications or pain during the version., Summary. The force applied during ECV can be measured and analysed using a customized pair of gloves incorporating piezo-resistive pressure sensors and suitable analytical software. The degree of force required for a successful version is highly variable. Failure of version is not usually due to insufficient force. Uterine tone is the most important factor affecting the degree of force applied during a version attempt. The degree of force applied is associated with the changes in fetal cerebral blood flow after ECV, and the amount of pain perceived by the patients. (Abstract shortened by UMI.), The lack of information in this area is primarily due to the lack of a suitable device that would allow measurements of the force applied without interfering with the ECV. A suitable device would therefore have to be sufficiently robust so that it could be worn on the hands, durable so that it could be used repeatedly, incorporate multiple individual sensors, each of which is capable of making dynamic and mutually independent measurements during the version procedure., There is no report in the literature on quantification of the force applied during ECV. It is also unknown whether the degree of force applied is related to the version outcome. In particular, it is unclear whether a failed attempt is related to insufficient force, or whether an increase in force may help to achieve version after a failure. Furthermore, it is also not known if any patients' factors may influence how much force is applied through the operator's hands. Although the chance of successful version could be predicted by some clinical factors, whether these factors may also affect the degree of applied force is not known., This thesis reports on the design and development of a suitable measuring device fulfilling the requirements described above.
