Objective: To investigate the diagnostic value of whole-body magnetic resonance imaging (MRI) including diffusion-weighted imaging with background signal suppression (DWIBS) for preoperative assessment of non-small-cell lung cancer (NSCLC) in comparison to (18)F-fluorodeoxyglucose (18)FDG) positron emission tomography/computed tomography (PET/CT).Methods: Thirty-three patients with suspected NSCLC were enrolled. Patients were examined before surgery with PET/CT and whole-body MRI including T1-weighted turbo spin echo (TSE), T2-weighted short tau inversion recovery (STIR) and DWIBS sequences (b = 0/800). Histological or cytological specimens were taken as standard of reference.Results: Whole-body MRI with DWIBS as well as PET/CT provided diagnostic image quality in all cases. Sensitivity for primary tumour detection: MRI 93%, PET/CT 98%. T-staging accuracy: MRI 63%, PET/CT 56%. N-staging accuracy: MRI 66%, PET/CT 71%. UICC staging accuracy: MRI 66%, PET/CT 74%. Sensitivity for metastatic involvement of individual lymph node groups: MRI 44%, PET/CT 47%. Specificity for individual non-metastatic lymph node groups: MRI 93%, PET/CT 96%. Assessment accuracy for individual lymph node groups: MRI 85%, PET/CT 88%. Observer agreement rate for UICC staging: MRI 74%, PET/CT 90%.Conclusion: Whole-body MRI with DWIBS provides comparable results to PET/CT in staging of NSCLC, but shows no superiority. Most relevant challenges for both techniques are T-staging accuracy and sensitivity for metastatic lymph node involvement.Key Points: Numerous radiological methods are available for the crucial staging of lung cancer. Objective: This study analyses the diagnostic potential of Diffusion-Weighted Imaging with Background Suppression (DWIBS) in the detection of focal bone marrow lesions from multiple myeloma. The signal and contrast properties of DWIBS are evaluated in correlation with the serum concentration of M-component (MC) and compared with established T1- and T2-weighted sequences.Methods: Data from 103 consecutive studies in 81 patients are analysed retrospectively. Signal intensities and apparent Diffusion Coefficients (ADC) of 79 focal lesions in the lumbar spine or pelvis of 38 patients are determined and contrast-to-noise-ratio (CNR) is calculated. Data from patients with low (<20 g/L) and high (>20 g/dL) MC are evaluated separately.Results: Signal intensities of focal myeloma lesions on T2w-STIR vary significantly depending on the MC, which leads to a loss in CNR in patients with high MC. No signal variation is observed for T1w-TSE and DWIBS. The CNR values provided by DWIBS in patients with high MC are slightly higher than those of T2w-STIR. ADC values in patients with low MC are significantly higher than in patients with high MC.Conclusion: Whole-body DWIBS has the potential to improve the conspicuity of focal myeloma lesions and provides additional biological information by ADC quantification. We evaluated automated nucleic acid (NA) extraction from a variety of different biological specimens using the QIAsymphony SP instrument. QIAsymphony DNA kits were used for DNA purification from human blood and from diverse human and animal tissue specimens. RNA was isolated from human blood stabilized in PAXgene Blood RNA tubes with the QIAsymphony PAXgene Blood RNA kit, and from human colon and bladder carcinoma biopsies using the QIAsymphony RNA kit. Photometric measurement, gel electrophoresis, and LabChip analysis on an Agilent 2100 Bioanalyzer (Agilent, Palo Alto, California) showed that the purified NAs were highly pure and intact, and that excellent yields were obtained. The DNA purified from blood and tissues performed well in single nucleotide polymorphism (SNP) array analysis, shown by call rates for the Affymetrix Genome-Wide Human 6.0 SNP arrays of >99%. No significant differences were observed when array results of DNA purified either with magnetic particle technology or silica membrane technology were compared. The quality of the DNA allowed accurate allelic discrimination by TaqMan SNP PCR. Gene expression analyses of purified RNA either by 'Human Endogenous Control Panel' TaqMan low-density array or on Affymetrix HG U133 plus 2.0 GeneChips revealed high concordance between manually purified samples and those extracted on the QIAsymphony SP. Background and Objective: Under physiological conditions, cerebral oxygen delivery is kept constant by adaptation of the regional cerebral blood flow (CBF) in relation to the oxygen content. So far, decreases of the regional CBF induced by a higher arterial oxygen content have been produced under hyperbaric or hyperviscous conditions. We tested whether local CBF is also reduced by a high haemoglobin (Hb) concentration at a normal haematocrit (Hct).Methods: Compared with controls (n=8), Hb content was increased to 19 g dl(-1) in conscious rats by isovolaemic replacement of the plasma fraction with an artificially high Hb solution (Hb-based oxygen carriers; HH group, n=8). In another group (n=8), Hct was decreased by isovolaemic exchange with an Hb-based oxygen carrier resulting in a normal Hb content (NH group). Mean and regional CBF was measured by iodo-[(14)C]-antipyrine autoradiography. Oxygen delivery was calculated from arterial oxygen content and CBF.Results: Compared with the controls (Hb 15.3 g dl(-1), Hct 0.44), mean CBF was lower in the HH (Hb 20.3 g dl(-1), Hct 0.44) group by 23% (P < or = 0.05), but remained unchanged in the NH group (Hb 15.0 g dl(-1), Hct 0.29). On a local level, hyperoxygenation reduced CBF in 22 out of 39 brain regions. In the NH group mean CBF was unchanged, whereas local CBF was higher in 10 areas. In both groups, overall cerebral oxygen delivery was unchanged compared with the control group. Locally though, high arterial Hb content decreased oxygen delivery in one-third of the brain structures.Conclusion: Whereas the overall cerebral oxygen delivery in the brain is maintained during hyperoxygenation and haemodilution, local oxygen delivery is decreased by high arterial Hb content in some brain regions. Using a high resolution meteorology-chemistry transport model, simulations were performed to estimate the sensitivity of the model results to nesting. The model results are compared with airplane measurements made during the TRACT field measuring campaign in September, 1992. For the meteorological part of the model the performance is enhanced using one-way nesting in a larger scale model, if the quality of the large scale driving data is sufficient. The sensitivity of the NOx concentration results with respect to nesting of chemical quantities is rather low due to the poor quality of the forcing data. A correct description of the emission rates and the meteorological conditions may be more important. For ozone, the best results can be achieved with either no nesting or a meteorological and chemical nested model simulation, which is again a result of the poor quality of the forcing data. Elite athletes are high-performance outliers within their specific sports. Even though science seeks to understand the nature of expertise and elite performance, much knowledge remains compartmentalized within subdisciplines. Despite this multidimensionality being acknowledged, an interdisciplinary approach to understanding elite athletes is still rare. This paper synthesizes insights across scientific domains in order to describe the population and individual characteristics of elite athletes. We analyzed diagnostic data from approximately 300 German squad athletes across eight different sports (e.g., gymnastics, volleyball, ice hockey, 3 × 3 basketball etc., agefemale = 18.95 ± 4.84 years, agemale = 19.32 ± 4.19 years) with expertise values ranging from 2 (low expertise) to 16 (high expertise). Data covered muscular strength, lower-body dynamics, muscle-power genetics, blood micronutrients, basic cognitive function, mental health, social support, and training conditions. Results of logistic regressions identified basic cognitive function (B = 0.89) and well-balanced blood micronutrients (B = 1.22) as critical factors distinguishing elite athletes. Additionally, multiple linear regressions suggested that lower-body dynamics (ß = 0.72) is related to increasing expertise values. We examined interactions between determinants of elite performance, and found that social support is positively associated with mental health and training conditions, whereas muscular strength correlates with lower-body dynamics. Focusing on top elite athletes in contrast to semi-elite athletes, we found higher within-group similarities in basic cognitive function and blood micronutrients. To examine at which stage in the multistep process of head and neck tumorigenesis numerical chromosomal alterations can be detected by fluorescence in situ hybridization (FISH), biopsies and cell smear preparations of clinically healthy oral tissue, premalignant lesions (leukoplakias), and tumors were analyzed by FISH using chromosome-specific centromeric probes. Aberrations found in tumor biopsies and in tumor cell smears consisted of trisomy of chromosomes 1, 7, 10, and 17 and monosomy of chromosomes 1, 7, 9, 10, and 17. In five of eight dysplastic oral leukoplakia biopsies, aberrations were seen consisting of trisomy of chromosome 1, 7, and 17, and monosomy of chromosome 9. No aberrations were found in biopsies of hyperplastic lesions (n = 8), or in oral cell smears of persons at risk. Because numerical chromosomal aberrations seem to be highly specific for malignant cells, FISH may help to identify leukoplakias that have a high risk of malignant conversion. [ABSTRACT FROM AUTHOR]

With the emergence of eosinophilic esophagitis (EoE) as a common cause of food impaction (FI) and a presumed increase in incidence of EoE in the population, the effect on the incidence of FI has not been well described. The aim of this study is to describe the incidence of FI and endoscopic findings in these patients and the association with EoE. A population-based retrospective chart review of the Rochester Epidemiology Project database was performed to identify all patients within Olmsted County that presented with FI from 1976 to 2012. A review of all endoscopic findings, biopsy results, and demographic data was performed. 497 patients were identified with FI from 1976 to 2012. The overall incidence of FI has changed from 1976 to 2012 (Fig. 1) (P  < 0.001). The peak incidence of 17.12 per 100,000 people occurred in the time period 1995 to 2000. Both the incidence of comorbid gastroesophageal reflux disease (GERD) and proton pump inhibitor (PPI) use increased over the time period of the study (P  < 0.001 for both). Of these patients, 188 (46.7%) had no abnormalities on their endoscopy. The most common endoscopic finding was stricture in 71 (17.6%) patients followed closely by Schatzki's ring in 68 (16.9%) patients. 139 patients had biopsies performed within 2 years of FI and 50 (36.0%) of those were diagnosed with EoE. We present for the first time the changing incidence of FI over the last 35 years in a population-based setting.
