Background Advanced heart failure (AdvHF) is associated with high morbidity and mortality. Patients with this clinical condition are potential candidates for heart transplantation or mechanical circulatory support. Initially, however, they are usually supported with inotropic drugs. Recent studies have suggested that levosimendan, independently of hemodynamic improvements, may lead to outcome benefits. Objectives To present clinical experiences concerning the indications, effectiveness, tolerance, and safety of levosimendan in the real-life therapy of patients with decompensated AdvHF in 3 cardiac centers in Poland. Material and methods This is a prospective, observational, three-center study. Forty-nine patients with AdvHF admitted with decompensation were included (88% men, mean age 58 years, 65% ischemic etiology, left ventricular ejection fraction (LVEF) in median 20%) and followed up for an early (3 months) and prolonged period (1 year) after infusion of levosimendan. Patients were analyzed in relation to death. Results Levosimendan therapy was associated with reduced HF symptoms and signs, New York Heart Association (NYHA) class and level of B-type natriuretic peptide (BNP) at discharge. Five patients died during hospitalization, a further 10 during the three-month follow-up and 3 died during the next nine-month follow-up. During the three-month follow-up, 22 patients were re-hospitalized due to HF and in the next nine-month follow-up 8 were re-hospitalized. A multivariate analysis indicated the QRS duration at discharge (hazard ratio (HR) = 1.02; 95% confidence interval (95% CI) = 1.003-1.03; p = 0.018), high-sensitivity C-reactive protein (hsCRP) (HR = 1.01; 95% CI = 1.004-1.02; p = 0.002), and simultaneous dobutamine infusion (HR = 6.54; 95% CI = 1.4-30.5; p = 0.017) were independent risk factors for death in the one-year follow-up. There were no side effects leading to the interruption of the levosimendan infusion. Conclusions The use of levosimendan was safe and associated with clinical improvement and reduction in BNP level in AdvHF patients hospitalized due to HF decompensation, although the mortality and re-hospitalization rate during the one-year follow-up remains high.

Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure

Heart failure with preserved ejection fraction (HFpEF) is an increasingly common condition, particularly in the context of the aging of the population. HFpEF is associated with high morbidity, mortality, and rate of heart failure rehospitalization as well as poor quality of life. Previous studies on HFpEF failed to reach a positive outcome. There is currently no approved treatment for HFpEF. The overall PARAGON‑HF trial population showed a 13% reduction in the primary endpoint (cardiovascular death and total heart failure hospitalizations) with sacubitril / valsartan treatment as compared with valsartan, which was of borderline statistical significance. Analyses of the secondary endpoints, including the clinical status, quality of life, and kidney function, imply that sacubitril / valsartan offers benefits compared with valsartan alone. The results of the PARAGON‑HF trial revealed that patients with HFpEF and particular clinical profiles (lower strata of ejection fraction below 57% and female sex), for whom no evidence‑based therapy is available, may benefit from treatment with sacubitril / valsartan. This review article summarizes opinions on the PARAGON‑HF results as well as a mechanistic discussion.

This work presents second-order nonlinear optical investigations of Ba(NO 3 ) 2 /Al 2 O 3 crystalline nanocomposite. Studied sample was obtained by crystal growth in nanoporous Al 2 O 3 membrane. Nonlinear optical studies were characterized by means of second harmonic generation via Maker fringe method and by SHG surface imaging. Additionally, surface of studied crystalline nanocomposite was characterized by scanning electron microscopy (SEM).

Accelerated heart rate is a risk factor of general and cardiovascular mortality in different populations. The classical treatment to achieve heart rate reduction by beta-blockers and calcium channel antagonists from the group of nondihydropyridines (verapamil and diltiazem), is often limited by contraindications or adverse reactions. Ivabradine is a unique, modern drug which mechanism of action is blocking the If current in the sinoatrial node. This translates into a reduction in heart rate — a parameter associated with poor prognosis in patients with heart failure. The following document contains the opinion of an expert group summarizing current knowledge on ivabradine and its use in population of patients with heart failure.

210. Gambling in Prisons – A Nationwide Polish Study of Sentenced Men

Despite the abandonment of the criterion of committing illegal acts in the diagnosis of pathological gambling in fifth edition of Diagnostic and Statistical Manual of Mental Disorders (DSM-V), research confirms the significant link between crime, gambling, and gambling addiction. In Poland, this connection is observed by psychologists working in the prison service, who simultaneously report the need for more structured interactions that would solve gambling problems among prisoners. The lack of any data on the involvement of persons committing crimes in gambling in Poland formed the basis for the implementation of a survey of gambling behaviour and gambling problems among male offenders in Polish correctional institutions. A total of 1,219 sentenced men took part in the study. The research tool included 75 questions, including queries from the South Oaks Gambling Screen (SOGS). Based on SOGS, the prevalence rate of severe problem gambling was 29.4% over the lifetimes of the prisoners. As many as 13.1% of respondents admitted to having gambled in prison. This activity usually involved cards, bets or dice. More than 74% of incarcerated men who gambled in prison met the criteria for pathological gambling. Prisoners who gambled more in prison than at liberty made up 27.7%. As many as 69.3% of respondents declared that while in prison, they had met fellow convicts experiencing problems because of gambling. The study shows that criminals continue gambling after detention, especially those who are problem gamblers, an overall finding which implies the need to implement preventive and therapeutic interventions in correctional institutions.RésuméMalgré l’abandon du critère d’actes illégaux dans le diagnostic du jeu pathologique, dans la cinquième édition du Manuel diagnostique et statistique des troubles mentaux (DSM-V), la recherche confirme le lien important entre criminalité, jeu et dépendance. En Pologne, ce lien est observé par des psychologues travaillant dans le service pénitentiaire, signalant en même temps le besoin d’interactions plus structurées qui résoudraient les problèmes de jeu chez les détenus. L’absence de données sur la participation de personnes commettant des délits aux jeux d’argent en Pologne a servi de base à la mise en œuvre d’une enquête sur les comportements et les problèmes de jeu chez les délinquants de sexe masculin dans les établissements correctionnels polonais. Au total, 1 219 prisonniers ont participé à l’étude. L’outil de recherche comprenait 75 questions, dont des questions tirées du South Oaks Gambling Screen (SOGS). Selon le SOGS, le taux de prévalence du jeu problématique sévère était de 29,4 % au cours de la vie des détenus. Jusqu’à 13,1 % des répondants ont reconnu avoir joué en prison. Les jeux comprenaient généralement des cartes, des paris ou des dés. Plus de 74 % des hommes incarcérés qui ont joué en prison répondaient aux critères de jeu pathologique. Les détenus qui jouaient plus en prison qu’en liberté représentaient 27,7 %. Jusqu’à 69,3 % des répondants ont déclaré qu’en prison, ils ont rencontré des personnes éprouvant des problèmes de jeu. L’étude montre que les criminels continuent de jouer après la détention, en particulier ceux qui sont des joueurs compulsifs, ce qui laisse entrevoir la nécessité de mettre en œuvre des interventions préventives et thérapeutiques dans les établissements correctionnels.

- Abstract
Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores >2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score >2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores >2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores >2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial. In the drug therapy of a 65-year-old male with chronic heart failure with reduced ejection fraction, New York Heart Association class III, with numerous co-morbidities including severe functional mitral regurgitation (FMR), in accordance with the 2016 European Society of Cardiology guidelines, angiotensin receptor blocker was replaced with sacubitril/ /valsartan. During further ambulatory observation, significantly reduced FMR was noted. This case report presents the current state of knowledge regarding the broad benefits of using sacubitril/valsartan in everyday clinical practice.

QUALIFY registry gives important data about outpatients with heart failure with reduced ejection fraction (HFrEF) and their pharmacotherapy and prognosis. The results of QUALIFY point the need of therapy improvement in HFrEF in ambulatory care. We still do not implement the optimal recommended therapy in HFrEF.

We present the case of a 24-years old asymptomatic pregnant woman in 18hbd with hypertrophic cardiomyopathy (HCM). An echocardiogram revealed the hypertrophy of all walls of left ventricle (LV), except for the posterolateral wall, from 21mm to 31mm and septal hypertrophy up to 36mm. During the first 48-h-ECG monitoring 5 episodes of slowVT consisted of 3 ExV up to 108/min were recorded. The 5-year HCM SCD (sudden cardiac death) risk score revealed the low risk of 2,25% - implantable cardioverter-defibrylator (ICD) not indicated. After a C-section delivery in 37hbd the control echocardiography revealed the enlargement of LV wall hypertrophy up to 38mm. In 48h-ECG monitoring 2 episodes of asymptomatic nsVT consisted of 4 and 7 ExV up to 162/min were registered. The 5-year HCM SCD risk came up to intermediate level: 5,91% (ICD may be considered, class IIb B of recommendations). Based on the clinical and echocardiographic findings with dynamic progress in the LV hypertrophy, exacerbation of ventricular arrhythmias and increase of NT-proBNP, the ICD was implanted. As presented by Maron & Maron at ESC Congress in London 2015, an MRI scanning with the late gadolinium enhancement (LGE) estimation may be helpful in making the decision on the ICD implantation, especially within the group of the intermediate 5-year risk of SCD (4-6%) with massive LV hypertrophy. Authors suggest the extensive LGE (≥15%) as a primary SCD risk factor and also as a potential risk factor when conventional evaluation of the ICD implantation indications is ambiguous.

Background There is limited evidence whether being on fludrocortisone prevents vasovagal syncope. Objectives The authors sought to determine whether treatment with fludrocortisone reduces the proportion of patients with recurrent vasovagal syncope by at least 40%, representing a pre-specified minimal clinically important relative risk reduction. Methods The multicenter POST 2 (Prevention of Syncope Trial 2) was a randomized, placebo-controlled, double-blind trial that assessed the effects of fludrocortisone in vasovagal syncope over a 1-year treatment period. All patients had >2 syncopal spells and a Calgary Syncope Symptom Score >−3. Patients received either fludrocortisone or matching placebo at highest tolerated doses from 0.05 mg to 0.2 mg daily. The main outcome measure was the first recurrence of syncope. Results The authors randomized 210 patients (71% female, median age 30 years) with a median 15 syncopal spells over a median of 9 years equally to fludrocortisone or placebo. Of these, 96 patients had ≥1 syncope recurrences, and only 14 patients were lost to follow-up before syncope recurrence. There was a marginally nonsignificant reduction in syncope in the fludrocortisone group (hazard ratio [HR]: 0.69: 95% confidence interval [CI]: 0.46 to 1.03; p = 0.069). In a multivariable model, fludrocortisone significantly reduced the likelihood of syncope (HR: 0.63; 95% CI: 0.42 to 0.94; p = 0.024). When the analysis was restricted to outcomes after 2 weeks of dose stabilization, there was a significant benefit due to fludrocortisone (HR: 0.51; 95% CI: 0.28 to 0.89; p = 0.019). Conclusions The study did not meet its primary objective of demonstrating that fludrocortisone reduced the likelihood of vasovagal syncope by the specified risk reduction of 40%. The study demonstrated a significant effect after dose stabilization, and there were significant findings in post hoc multivariable and on-treatment analyses. AIMS: To assess tolerability and optimal time point for initiation of sacubitril/valsartan in patients stabilised after acute heart failure (AHF). METHODS AND RESULTS: TRANSITION was a randomised, multicentre, open-label study comparing two treatment initiation modalities of sacubitril/valsartan. Patients aged ≥ 18 years, hospitalised for AHF were stratified according to pre-admission use of renin-angiotensin-aldosterone system inhibitors and randomised (n = 1002) after stabilisation to initiate sacubitril/valsartan either ≥ 12-h pre-discharge or between Days 1-14 post-discharge. Starting dose (as per label) was 24/26 mg or 49/51 mg bid with up- or down-titration based on tolerability. The primary endpoint was the proportion of patients attaining 97/103 mg bid target dose after 10 weeks. Median time of first dose of sacubitril/valsartan from the day of discharge was Day -1 and Day +1 in the pre-discharge group and the post-discharge group, respectively. Comparable proportions of patients in the pre- and post-discharge initiation groups met the primary endpoint [45.4% vs. 50.7%; risk ratio (RR) 0.90; 95% confidence interval (CI) 0.79-1.02]. The proportion of patients who achieved and maintained for ≥ 2 weeks leading to Week 10, either 49/51 or 97/103 mg bid was 62.1% vs. 68.5% (RR 0.91; 95% CI 0.83-0.99); or any dose was 86.0% vs. 89.6% (RR 0.96; 95% CI 0.92-1.01). Discontinuation due to adverse events occurred in 7.3% vs. 4.9% of patients (RR 1.49; 95% CI 0.90-2.46). CONCLUSIONS: Initiation of sacubitril/valsartan in a wide range of heart failure with reduced ejection fraction patients stabilised after an AHF event, either in hospital or shortly after discharge, is feasible with about half of the patients achieving target dose within 10 weeks.
