Abstract

Background: Compared to very low gestational age (<32 weeks, VLGA) cohorts, very low birth weight (<1500 g; VLBW) cohorts are more prone to selection bias toward small-for-gestational age (SGA) infants, which may impact upon the validity of data for benchmarking purposes. Method(s): Data from all VLGA or VLBW infants admitted in the 3 Networks between 2008 and 2011 were used. Two-thirds of each network cohort was randomly selected to develop prediction models for mortality and composite adverse outcome (CAO: mortality or cerebral injuries, chronic lung disease, severe retinopathy or necrotizing enterocolitis) and the remaining for internal validation. Areas under the ROC curves (AUC) of the models were compared. Result(s): VLBW cohort (24,335 infants) had twice more SGA infants (20.4% vs. 9.3%) than the VLGA cohort (29,180 infants) and had a higher rate of CAO (36.5% vs. 32.6%). The two models had equal prediction power for mortality and CAO (AUC 0.83), and similarly for all other cross-cohort validations (AUC 0.81-0.85). Neither model performed well for the extremes of birth weight for gestation (<1500 g and >=32 weeks, AUC 0.50-0.65; >=1500 g and <32 weeks, AUC 0.60-0.62). Conclusion(s): There was no difference in prediction power for adverse outcome between cohorting VLGA or VLBW despite substantial bias in SGA population. Abstract

International audience; The present work focuses on the interaction between an electric discharge generated by a commercial pencil coil and surrounding air flow in terms of aerodynamics and heat release using advanced optical diagnostics. This investigation is firstly performed with pin-to-pin electrodes in air at rest under atmospheric pressure condition. The velocity of the gas surrounding the spark and the temperature field are measured respectively by Particle Image Velocimetry and Spontaneous Raman Scattering techniques. The time evolution of the current and voltage at the electrodes are monitored for an evaluation of the maximum deposited energy. In a second step, the topology evolution of the discharge in a laminar air flow is investigated for several flow velocities. High-speed intensified imaging technique is used for visualizing the spark interactions with the flow and its changes in growth, shape, and duration. The impacts of the electric discharge on the gas flow are investigated by means of high-resolution phase-locked PIV. The whole results build a database which intends to bring a better understanding of plasma-aerodynamics interactions but also to provide data for ignition models validations.

A new approach, which employs the atomic recombination in the dynamic secondary ion mass spectrometry (D-SIMS) for the purpose of macromolecular colocalization is presented. The macromolecules are shown to be colocalized only on being close to each other.

Abstract

Background and aims: The association between adiposity measures and dyslipidemia has seldom been assessed in a multipopulational setting. Methods and results: 27 populations from Europe, Australia, New Zealand and Canada (WHO MONICA project) using health surveys conducted between 1990 and 1997 in adults aged 35-64 years (n = 40,480). Dyslipidemia was defined as the total/HDL cholesterol ratio >6 (men) and >5 (women).Overall prevalence of dyslipidemia was 25% in men and 23% in women. Logistic regression showed that dyslipidemia was strongly associated with body mass index (BMI) in men and with waist circumference (WC) in women, after adjusting for region, age and smoking. Among normal-weight men and women (BMI<25 kg/m2), an increase in the odds for being dyslipidemic was observed between lowest and highest WC quartiles (OR = 3.6, p < 0.001). Among obese men (BMI ≥ 30), the corresponding increase was smaller (OR = 1.2, p = 0.036). A similar weakening was observed among women. Classification tree analysis was performed to assign subjects into classes of risk for dyslipidemia. BMI thresholds (25.4 and 29.2 kg/m2) in men and WC thresholds (81.7 and 92.6 cm) in women came out at first stages. High WC (>84.8 cm) in normal-weight men, menopause in women and regular smoking further defined subgroups at increased risk. Conclusion: standard categories of BMI and WC, or their combinations, do not lead to optimal risk stratification for dyslipidemia in middle-age adults. Sex-specific adaptations are necessary, in particular by taking into account abdominal obesity in normal-weight men, post-menopausal age in women and regular smoking in both sexes

Abstract

Variable angle sample spinning NMR of quadrupolar nuclei in the solid state has been theoretically investigated in order to determine the optimal angles to achieve high-resolved and quantitative spectra. The earlier literature contributions have been included in a more general model. Complete analytical expressions were derived and are reported for all possible cases. It is concluded that magic angle spinning (at 54.7°) is to be used whenever the dipolar interactions and/or the chemical shift anisotropy dominate the spectra. For the other cases, θ=43.5° (which would allow an easy discrimination of the quadrupolar frequency and the asymmetry parameter), and θ=60°–70° (where the linewidth is half of that obtained for MAS conditions) are proposed as preferred spinning angles. Abstract

The crystal structure of the title organic-inorganic hybrid material, [(C6H9N2)2CdCl4]0.305H2O, contains two crystallographically independent but chemically equivalent cadmium complexes with essentially the same geometry (rms deviation of all atoms: 0.152 Å). The metal centers have a 6-coordinated octahedral geometry and both Cd atoms exhibit exact crystallographic inversion symmetry. Crystal packing is stabilized by intermolecular N-H...Cl hydrogen bonds that connect individual zwitter-ionic complexes into a three dimensional lattice, which is further stabilized through p-p stacking interactions between aromatic rings of neighboring complexes, with centroid to centroid distances of 3.4406(14) and 3.7022(13) Å and interplanar separations of 3.299(1) and 3.302(1) Å. Interstitial space is partially filled with water molecules which are connected to the network through O-H...Cl and C-H...O hydrogen bonds. The 13C and 15N CP-MAS NMR spectra are in agreement with the X-ray structure. Four resonance peaks for the C3 carbon atom are observed due to the different environments of the aromatic rings caused by the presence of 0.305 water molecule per unit cell. Abstract

Background: 123I-MIBG myocardial scintigraphy and clonidine growth hormone test (CGH test) may help to distinguish multiple system atrophy (MSA) from Parkinson’s disease (PD). Their relevance in the first-stage parkinsonism of uncertain etiology is unknown.Methods: Patients experiencing parkinsonism of ambiguous etiology were clinically classified into the PD group or the MSA group as initial clinical diagnosis (ICD). Then, CGH test and myocardial scintigraphy were performed. Clinical assessment was repeated throughout the disease course until the final clinical diagnosis (FCD) could be established according to the criteria of PD and MSA, respectively.Results: Twenty-five patients with uncertain diagnosis were included (15 MSA and 10 PD as ICD). At the end of a 6-year follow-up, FCD was MSA in 11/25 patients and PD in 14/25. The CGH test and the scintigraphy showed a sensitivity of 82%, and a specificity of 71 and 93%, respectively, for the diagnosis of MSA. The combination of a normal scintigraphy (i.e., with myocardial MIBG uptake) with genitourinary dysfunction was the most relevant test to diagnose MSA, whereas an abnormal scintigraphy with a levodopa response of > 30% or an abnormal scintigraphy with the absence of OH was the most relevant combinations to diagnose PD. All these combinations had an accuracy superior than 90% and a specificity of 100%.Conclusion: Combinations of myocardial scintigraphy with genitourinary dysfunction, levodopa response of > 30%, or orthostatic hypotension could be of interest for the distinction between PD and MSA when the clinical diagnosis remains ambiguous at the first stage of the disease. Abstract

Drilling is a very old and very complex process. As the tool is embedded in the part, neither the material removal phenomena, nor the evacuation of chips are well understood. As a consequence a bad choice of drilling tool or poor drilling conditions may affect the surface integrity of the machined part. The aim of this article is to bring a contribution to the understanding of lubrication modes on cutting phenomena and surface integrity when drilling a 316L austenitic stainless steel, a 15-5PH martensitic stainless steel and an Inconel 718 nickel alloy. Residual stress states in the hole have been analysed after drilling with three lubrication modes (internal high pressure coolant, external low pressure coolant, dry). Results have been analysed with regard to the thermal and mechanical actions of the drill on parts, thanks to temperature sensors along the hole, and thrust force+torque measurements. First this work has highlighted that residual stress state varies along a hole. Second, it has been shown that lubrication is a critical parameter on surface integrity. Whereas efficient cooling conditions enable to obtain compressive residual stresses and a limited affected layer, a lack of lubrication generates tensile residual stresses, a larger deviation in the results and an increase in the thickness of the affected layer. Abstract

The production of parts by additive manufacturing technologies is a hot topic for industry. The Selective Laser Melting process is among the most widely used nowadays. Unfortunately the geometrical accuracy, as well as the surface roughness are very poor. Functional surfaces therefore require further machining operations in order to fulfill the specifications of the designer. However the influence of finishing operations on the surface integrity of such parts needs to be further investigated. This paper aims at analyzing the influence of milling or polishing operations on the rotary bending fatigue resistance of titanium samples generated by SLM. It is highlighted that finishing operations are able to improve significantly the fatigue resistance compared to the initial state after SLM + heat treatment. The fatigue results are also discussed in light of the surface integrity (roughness parameters and residual stresses). It is found that the surface topography skewness is an interesting parameter which correlates with the fatigue performance. None of these finishing techniques are able to lead to a fatigue resistance comparable to the one obtained with a wrought titanium part. [ABSTRACT FROM AUTHOR]

Abstract

Aim: This study evaluated the clinical and haemodynamic effects of norepinephrine infusion in preterm infants.Methods: The effects of norepinephrine therapy for refractory hypotension were evaluated in preterm infants between April 2009 and April 2011 at the neonatal intensive care unit of Sainte-Justine Hospital, Montreal, Quebec. Changes in haemodynamics and clinical parameters were analysed eight hours before and eight hours after the start of norepinephrine infusion, and eight hours after its cessation.Results: During the study, 30 preterm infants at a mean gestational age of 26.5 ± 2.6 weeks (median: 25.7, 23.4-34) and birthweight of 903 ± 437 g (median 827, 450-2550) received norepinephrine infusion for neonatal septic shock. After eight hours of treatment, mean blood pressure, urine output and FiO2 significantly improved. Eight hours after cessation of norepinephrine infusion, the number of patients treated with other inotropes decreased significantly, 24 patients (80%) had normal mean blood pressure and 27 patients (90%) had normal urine output.Conclusion: Norepinephrine therapy could be considered to improve blood pressure and urine output during neonatal septic shock in preterm infants. This paper presents a fully integrated direct sampling receiver for DOCSIS 3.0, consisting of a time-interleaved ADC, a digital multi-channel selection filter, and a PLL. The receiver can simultaneously receive 4 streams from arbitrary RF frequencies between 48 and 1002MHz and output these in a 13.5MS/s digital IQ format or at a low-IF through integrated DACs. It consumes 980mW from a split 1.2/1.3/1.6V supply when receiving 4 channels and occupies 16.8mm2 in 65nm CMOS.
