To assess the 2-year effectiveness of intravitreal ranibizumab combined with a dietary supplement rich in docosahexaenoic acid (DHA) plus antioxidants in 62 patients with diabetic macular edema.In a randomized single-blind controlled study, 33 subjects (42 eyes) received intravitreal ranibizumab alone and 29 (34 eyes) combined with DHA (1,050 mg/day). Monthly ranibizumab (0.5 mg) was given for the first 4 months followed by on as-needed treatment.At 24 months, the difference between groups in the decrease of central subfield macular thickness was significant in favor of the DHA supplementation group (95% confidence interval of the difference 7.20-97.656; P = 0.024), although improvement in best-corrected visual acuity measured in the Early Treatment Diabetic Retinopathy Study letters did not reach statistical significance (95% confidence interval 5.4-11.2, P0.66). At 24 months, gains of5 and10 letters were significantly higher in the DHA supplementation group as compared with controls when the worse and better seeing eyes were considered but other differences at 12 months and 24 months were not found.Intravitreal ranibizumab combined with DHA supplementation reduced central subfield macular thickness after 2 years of follow-up as compared with ranibizumab alone in patients with diabetic macular edema. This anatomical improvement was accompanied by a trend for an amelioration of vision.

207. Incidental maternal death due to lightning injury: a case report

Resumen Los relámpagos se presentan con una frecuencia mayor a 50 veces por segundo a nivel mundial; sin embargo, las descargas directas suceden rara vez y son responsables del 5% de los eventos fatales. El mecanismo de lesión inicial es la parálisis de los centros vitales, lo que ocasiona apnea, fibrilación auricular o paro cardíaco. En mujeres embarazadas, el accidente por rayo puede producir complicaciones graves, dependiendo del punto de entrada en el cuerpo y su efecto indirecto sobre el feto, produciendo trastornos de la conducción en el sistema cardíaco y daño a nivel del sistema nervioso central. En la literatura mundial, solo catorce casos de embarazo y traumatismo por relámpago han sido reportados, sobreviviendo en todos la madre, con un único reporte de muerte fetal. Se presenta un caso de accidente por relámpago en una mujer embarazada en la semana 23 de gestación, cuyo tipo de lesión fue por contacto directo en el abdomen, y quien ingresó con compromiso neurológico, taquicardia materna y bradicardia fetal. El deceso materno ocurrió por infarto agudo extenso del miocardio y la formación de un émbolo intracavitario en el ventrículo izquierdo, con la consecuente muerte del binomio. Se describe su frecuencia, el mecanismo de lesión, las manifestaciones clínicas y el manejo actual. MÉD.UIS.2019;32(1):21-6 Abstract Lightning occurs with a frequency rate of more than 50 times per second worldwide. Nevertheless, direct lightning strikes are not common, and they are responsible for about 5% of all lightning injury fatalities. The initial mechanism of injury is the paralysis of the vital centers, which causes apnea, atrial fibrillation and cardiac arrest. In pregnant women, lightning can produce severe complications which depend on the point of entrance and its indirect effect on the fetus, affecting the cardiac conduction system and the central nervous system. We report a case of a lightning accident by direct strike on a 23-week pregnant woman, who's type of injury was by direct contact on the abdomen. She arrives to the emergency room with neurologic symptoms, maternal tachycardia and fetal bradycardia. The mother died due to myocardial infarction and the presence of an intracavitary embolus in the left ventricle, with the consequent maternal-fetal death. The incidence and injury mechanisms, as well as the clinical features and clinical management are described. MÉD.UIS.2019;32(1):21-6.

Multiple sclerosis is a chronic, demyelinating, and degenerative disease of the central nervous system with an immune-based pathologic origin. The present pilot study aimed to assess whether the change in the route of treatment administration is associated with a variation in adherence and whether there is a change in quality of life, treatment satisfaction, and fatigue.Patients with relapsing-remitting multiple sclerosis who were18 years of age and who used to receive immunomodulatory parenteral treatment and were ready to change administration route were eligible for the study. Data were collected at baseline and 3 months later. Adherence, quality of life, treatment satisfaction, and fatigue were measured via the following questionnaires: Morisky-Green questionnaire on patient-reported medication adherence, Multiple Sclerosis Quality of Life Instrument, Treatment Satisfaction Questionnaire for Medication, and Modified Fatigue Impact Scale.The study sample included 30 patients (mean age, 43.2 years; age range, 24-71 years; 60% female and 40% male). There was a significant improvement in adherence (p = 0.048). Mean (SD) physical and mental health quality-of-life summary scores varied from 52.50 (24.15) and 54.13 (21.24) to 67.55 (20.92) and 62.30 (21.75) (p 0.001 and p = 0.001, d = -0.426 and d = -0.643, respectively). In the Treatment Satisfaction Questionnaire for Medication, an improvement of the score was observed in effectiveness of the medication (p = 0.0041, d = -0.563), adverse effects of the medication (p 0.001, d = -0.976), convenience of the medication (p 0.001, d = -1.235), and global satisfaction (p = 0.006, d = -0.725). Patients had a higher mean (SD) score (45.13 [26.7]) on the Modified Fatigue Impact Scale while receiving injectable treatment compared with that obtained with oral treatment (34.86 [23.16]; p = 0.009, d = 0.41).When the route of administration changed from injectable to oral, there was an increase in adherence, quality of life, and degree of patient satisfaction with their treatment and a decrease in the degree of fatigue.

The efficacy of pomegranate (Punica granatum) extract (PE) for improving performance and post-exercise recovery in an active population was equivocal in previous studies. In this study, a randomised, double-blinded, placebo-controlled, balanced, cross-over trial with two arms was conducted. Eligibility criteria for participants were as follows: male, amateur cyclist, with a training routine of 2 to 4 sessions per week (at least one hour per session). The cyclists (n = 26) were divided into treatment (PE) and placebo (PLA) groups for a period of 15 days. After physical tests, the groups were exchanged after a 14-day washout period. Exercise tests consisted of endurance bouts (square-wave endurance exercise test followed by an incremental exercise test to exhaustion) and an eccentric exercise drill. The objective was to assess the efficacy of PE in performance outcomes and post-exercise muscular recovery and force restoration after a prolonged submaximal effort. Twenty-six participants were included for statistical analysis. There was a statistically significant difference in total time to exhaustion (TTE)(17.66&ndash, 170.94 s, p <, 0.02) and the time to reach ventilatory threshold 2 (VT2)(26.98&ndash, 82.55 s, p <, 0.001), with greater values for the PE compared to the PLA group. No significant results were obtained for force restoration in the isokinetic unilateral low limb test. PE, after a prolonged submaximal effort, may be effective in improving performance outcomes at maximal effort and might help to restore force in the damaged muscles.

BackgroundVertical jumps can be used to assess neuromuscular status in sports performance. This is particularly important in Cerebral Palsy Football (CP Football) because players are exposed to high injury risk, but it may be complicated because the gold standard for assessing jump performance is scarce in field evaluation. Thus, field techniques, such as mobile apps, have been proposed as an alternative method for solving this problem. ObjectiveThis study aims to evaluate the reliability of the measures of the MyJump2 app to assess vertical jump performance in professional CP Football. MethodsWe assessed 40 male CP Football athletes (age 28.1 [SD 1.4] years, weight 72.5 [SD 6.2] kg, and height 176 [SD 4.2] cm) through the countermovement jump (CMJ) and squat jump (SJ) using a contact mat. At the same time, we assessed the athletes using the MyJump2 app. ResultsThere were no significant differences between the instruments in SJ height (P=.12) and flight time (P=.15). Additionally, there were no significant differences between the instruments for CMJ in jump height (P=.16) and flight time (P=.13). The aim of the study was to analyze how cardiovascular risk factors can be modified using nutritionally improved cooked ham enriched with a pool of antioxidants to influence relevant metabolic targets. Sixty-five untreated subjects (49.2% males, 50.8% females, mean age 40.92 ± 9.03 years) with total cholesterol level ≥180 mg/dL or LDL cholesterol ≥130 mg/dL participated in a 8-weeks randomized, double-blind controlled trial. Participant in the intervention group (51.5% males, 48.5% females, mean age 41.6 ± 9.8 years and mean BMI 25.1 ± 3.6 kg/m2) consumed cooked ham enriched with antioxidants (100 g/d) and controls (49.9% males, 53.1% females, mean age 40.2 ± 8.3 years and mean BMI 26.3 ± 3.2 kg/m2) received placebo. At 8 weeks, oxidized LDL decreased significantly between experimental and placebo groups (p < 0.036). Experimental group differences were also significant (p < 0.05). Similar findings in malondialdehyde, total cholesterol, high-sensitivity C-reactive protein, and interleukin 6 were observed in the intervention group. Significant between-group differences in these variables were also found, except for total cholesterol and interleukin 6. The effects on inflammation and oxidation support the direct action of these antioxidants on the etiopathogenic factors of atheromatous plaque. We also observed an improvement in the lipid profiles among the subjects.

- Abstract
Pelvic floor muscle training is commonly used for urine loss. However, research studies have not determined which training load is the most effective for women with stress urinary incontinence (SUI). Moreover, none of the previous reviews or studies have described the total effectiveness of pelvic floor muscle training (PFMT) with an objective test such as the pad test. The objectives were to analyze the effectiveness of pelvic floor muscle training in women with SUI and to determine which training load produces the greatest adaptations for decreasing urine loss. The search was conducted in three databases (PubMed, Web of Science and Cochrane), for randomized controlled trials (RCTs) that evaluated the effects of PFMT. Studies were included if they met the following criteria: participants were women; were older than 18; had SUI; were treated with PFMT; and the assessments of the effects were measured with a pad test. Finally, 10 articles (293 women) analyzed the pad test in women with SUI who performed PFMT. The meta-analysis showed that PFMT, independent of the protocol used in the study, resulted in decreased urine loss in women suffering from SUI. However, for large effects, the program should last 6–12 weeks, with >3 sessions/week and a length of session <45 min.

In order to determine the productive response of two Mexican potato genotypes in two substrates in the hydroponic cultivation system, three experiments were established with horticultural grade perlite and with the mixture of perlite with peat 1:1 v/v (aggregates), by what experiments were established in the autumn 2015, spring 2016 and summer 2016 cycles. With perlite, the Citlali variety and clone 99-39, produced on average 15.92 and 34.25 tubers per plant, respectively; in aggregates they produced 11.92 and 15.42 tubers per plant, respectively. Genotype 99-39 exceeded Citlali by up to 10.9 tubers per plant on average of the three crop cycles and produced 2.2 times more tubers in autumn than in spring, as well as 2.9 more tubers in autumn than in summer. Citlali produced 3.6 times more tubers in autumn than in summer and five times more in autumn than in spring. The most efficient substrate was horticultural grade perlite, clone 99-39 was more productive than theCitlali variety, the production of tubers in both genotypes was closely affected by the season of the year and the results will allow decisions on the management of the production of prebasic seed II in both genotypes, Con el objetivo de determinar la respuesta productiva de dos genotipos de papa mexicanos en dos sustratos en sistema hidropónicos de cultivo, se establecieron tres experimentos con perlita grado hortícola y con la mezcla de perlita con turba 1:1 v/v (agregados), por lo que se establecieron experimentos en los ciclos otoño 2015, primavera 2016 y verano 2016. Con perlita, la variedad Citlali y el clon 99-39, produjeron en promedio 15.92 y 34.25 tubérculos por planta, respectivamente; en agregados produjeron 11.92 y 15.42 tubérculos por planta, respectivamente. El genotipo 99-39 superó a Citlali hasta en 10.9 tubérculos por planta en promedio de los tres ciclos de cultivo y produjo 2.2 veces más tubérculos en otoño que en primavera, así como 2.9 más tubérculos en otoño que en verano. - Abstract
Lightning occurs with a frequency rate of more than 50 times per second worldwide. Nevertheless, direct lightning strikes are not common, and they are responsible for about 5% of all lightning injury fatalities. The initial mechanism of injury is the paralysis of the vital centers, which causes apnea, atrial fibrillation and cardiac arrest. In pregnant women, lightning can produce severe complications which depend on the point of entrance and its indirect effect on the fetus, affecting the cardiac conduction system and the central nervous system. We report a case of a lightning accident by direct strike on a 23-week pregnant woman, who´s type of injury was by direct contact on the abdomen. She arrives to the emergency room with neurologic symptoms, maternal tachycardia and fetal bradycardia. The mother died due to myocardial infarction and the presence of an intracavitary embolus in the left ventricle, with the consequent maternal-fetal death. The incidence and injury mechanisms, as well as the clinical features and clinical management are described. MÉD.UIS.2019;32(1):21-6, Los relámpagos se presentan con una frecuencia mayor a 50 veces por segundo a nivel mundial; sin embargo, las descargas directas suceden rara vez y son responsables del 5% de los eventos fatales. El mecanismo de lesión inicial es la parálisis de los centros vitales, lo que ocasiona apnea, fibrilación auricular o paro cardíaco. En mujeres embarazadas, el accidente por rayo puede producir complicaciones graves, dependiendo del punto de entrada en el cuerpo y su efecto indirecto sobre el feto, produciendo trastornos de la conducción en el sistema cardíaco y daño a nivel del sistema nervioso central. En la literatura mundial, solo catorce casos de embarazo y traumatismo por relámpago han sido reportados, sobreviviendo en todos la madre, con un único reporte de muerte fetal. Se presenta un caso de accidente por relámpago en una mujer embarazada en la semana 23 de gestación, cuyo tipo de lesión fue por contacto directo en el abdomen, y quien ingresó con compromiso neurológico, taquicardia materna y bradicardia fetal. El deceso materno ocurrió por infarto agudo extenso del miocardio y la formación de un émbolo intracavitario en el ventrículo izquierdo, con la consecuente muerte del binomio. This randomized double-blind and controlled single-center clinical trial was designed to evaluate the effect of a 6-week intake of a probiotic product (1 capsule/day) vs. a placebo on an oxidative stress model of physical exercise (high intensity and duration) in male cyclists (probiotic group, n = 22, placebo, n = 21). This probiotic included three lyophilized strains (Bifidobacterium longum CECT 7347, Lactobacillus casei CECT 9104, and Lactobacillus rhamnosus CECT 8361). Study variables were urinary isoprostane, serum malondialdehyde (MDA), serum oxidized low-density lipoprotein (Ox-LDL), urinary 8-hydroxy-2’-deoxiguanosine (8-OHdG), serum protein carbonyl, serum glutathione peroxidase (GPx), and serum superoxide dismutase (SOD). At 6 weeks, as compared with baseline, significant differences in 8-OHdG (Δ mean difference −10.9 (95% CI −14.5 to −7.3), p <, 0.001), MDA (Δ mean difference −207.6 (95% CI −349.1 to −66.1, 0.05), and Ox-LDL (Δ mean difference −122.5 (95% CI −240 to −4.5), 0.05) were found in the probiotic group only. Serum GPx did not increase in the probiotic group, whereas the mean difference was significant in the placebo group (477.8 (95% CI 112.5 to 843.2), 0.05). These findings suggest an antioxidant effect of this probiotic on underlying interacting oxidative stress mechanisms and their modulation in healthy subjects. Resumen Objetivo Analizar el contenido de los diferentes estudios publicados relacionados con el ejercicio fisico y sus efectos sobre la incontinencia urinaria, asi como determinar la efectividad de los programas de entrenamiento de suelo pelvico. Metodo Se realizo busqueda en las bases de datos de Pubmed, CINAHL, Biblioteca Cochrane Plus y The Cochrane Library, WOS, SPORTDiscus y una busqueda manual en el metabuscador Google Scholar de acuerdo a los descriptores de busqueda de documentos publicados en los ultimos 10 anos en espanol o en ingles, con resumen o a texto completo, tanto del tratamiento de la incontinencia urinaria en la mujer deportista como en la mujer en general. Resultados Se seleccionan 3 articulos a texto completo sobre el tratamiento de la incontinencia urinaria en la mujer deportista, y 6 articulos a texto completo y un resumen sobre el tratamiento de la incontinencia urinaria en la mujer en general. Los 9 estudios incluidos en la revision obtuvieron resultados positivos, es decir, en todos ellos hubo una mejora de la enfermedad. Conclusiones El ejercicio fisico, concretamente los programas de entrenamiento de la musculatura del suelo pelvico, tienen efectos positivos sobre la incontinencia urinaria. Este tipo de entrenamiento se muestra como un programa efectivo para el tratamiento de la incontinencia urinaria, especialmente la incontinencia urinaria de esfuerzo.

Objective Among Basque handball players, the repeated impact of a ball on the palms of their hands hundreds of thousands of times throughout their sporting careers produces Raynaud syndrome. Treating this patient group is complex. Our objective was to assess the efficacy of digital periarterial sympathectomy in this patient group. Methods The study included all of the federated amateur and professional Basque handball patients who presented with Raynaud syndrome assessed in the vascular surgery service between January 2005 and December 2012. The postoperative assessment included a physical examination, basal photoplethysmography and photoplethysmography after heat hyperemia, and arteriography or magnetic resonance angiography. Results All 182 digital periarterial sympathectomies in the 114 fingers of 60 patients were in Porter functional class III or IV. All patients were discharged within the first 48 hours. Follow-up results, with a mean of 2 years ± 5 months, were 100%. All patients presented immediate pain remission, recovery of comfort, normal nail growth, rapid healing of all ulcers, distal anhidrosis, and return to active sport participation. The results remain steady in 58 patients (93.5%). Mean time until return to sports activity was 9.95 ± 1.61 weeks. Conclusions Digital periarterial sympathectomy is a simple, relatively nonaggressive technique without adverse side effects and with excellent medium-term results. In patients with Raynaud syndrome refractory to medical treatment and with threat to the viability of one or several fingers, digital periarterial sympathectomy can be the first treatment option, especially in cases of arteritis associated with very severe spasms.

- Abstract
BACKGROUND Vertical jumps can be used to assess neuromuscular status in sports performance. This is particularly important in Cerebral Palsy Football (CP Football) because players are exposed to high injury risk, but it may be complicated because the gold standard for assessing jump performance is scarce in field evaluation. Thus, field techniques, such as mobile apps, have been proposed as an alternative method for solving this problem. OBJECTIVE This study aims to evaluate the reliability of the measures of the MyJump2 app to assess vertical jump performance in professional CP Football. METHODS We assessed 40 male CP Football athletes (age 28.1 [SD 1.4] years, weight 72.5 [SD 6.2] kg, and height 176 [SD 4.2] cm) through the countermovement jump (CMJ) and squat jump (SJ) using a contact mat. At the same time, we assessed the athletes using the MyJump2 app. RESULTS There were no significant differences between the instruments in SJ height (P=.12) and flight time (P=.15). Additionally, there were no significant differences between the instruments for CMJ in jump height (P=.16) and flight time (P=.13). In addition, it was observed that there were significant and strong intraclass correlations in all SJ variables varying from 0.86 to 0.89 (both P CONCLUSIONS We conclude that the MyJump2 app presents high validity and reliability for measuring jump height and flight time of the SJ and CMJ in CP Football athletes.

Aunque se han publicado guías de práctica clínica y puestas al día sobre el tema, el manejo práctico de esta complicación dista de ser óptimo. Objetivo: conocer el manejo real, las necesidades y las limitaciones de la anemia en la EII mediante una encuesta a especialistas de digestivo. Material y métodos: encuesta telemática autoadministrada, realizada entre abril- mayo 2017, y dirigida a socios de la SEPD con actividad clínica. La encuesta incluye cuatro apartados: demografía de los participantes, monitorización, tratamiento y limitaciones/necesidades. Resultados: ciento veintidós encuestas evaluables procedentes de todas las comunidades autónomas. La anemia ferropénica se considera una manifestación frecuente de la EII y que se monitoriza en todos los pacientes mediante determinación de la hemoglobina y la ferritina. En caso de anemia, los encuestados consideran necesario descartar la existencia de actividad de la EII, aunque solo el 14,8% indica el hierro intravenoso si la EII está activa. La dosis necesaria de hierro intravenoso es mayoritariamente calculada según las necesidades del paciente, pero solo el 33,1% utiliza dosis por infusión de 1 g o más elevadas. Conclusiones: la encuesta "Gestiona Hierro EII" sobre el manejo de la anemia en la EII demuestra una alta calidad asistencial, pero con aspectos a mejorar como la indicación de hierro intravenoso en los pacientes con actividad, el uso de hierro intravenoso de altas dosis o la aplicación de algoritmos en la práctica asistencial. ABSTRACT Introduction: iron deficiency anemia is a common and very relevant manifestation of inflammatory bowel disease (IBD). Although clinical practice guidelines have been published and updated on this subject, the management in the daily practice of this complication is far from optimal. Objective: to determine the actual management, needs and limitations of anemia in IBD by means of a survey of gastroenterology specialists. Material and methods: a self-administered telematic survey was carried out between April and May 2017 and was sent to SEPD members. The survey included four sections: participant demographics, monitoring, treatment and limitations/needs. Results: a total of 122 evaluable surveys were received from all Spanish autonomous communities. Iron deficiency anemia is considered as a frequent manifestation of IBD and is monitored in all patients via the measurement of hemoglobin and ferritin. In the case of anemia, the survey respondents found it necessary to rule out the presence of IBD activity. However, only 14.8% prescribed intravenous iron when IBD was active. The required dose of intravenous iron is mainly calculated according to patient needs but only 33.1% of clinicians infused doses of 1 g or more. Conclusions: the "Gestiona Hierro EII" survey on the management of anemia in IBD demonstrated a high quality of care, even though some aspects need to be improved. These included the prescription of intravenous iron for patients with disease activity, the use of high-dose intravenous iron and the implementation of algorithms into clinical practice.

Background: Corticosteroid infiltration (CI) is commonly used for treatment of plantar fasciosis. In recent years, however, interest has grown in the use of intratissue percutaneous electrolysis (EPI) for the treatment of tendinopathies. The aim of our study was to compare the effectiveness of the above techniques in the treatment of plantar fasciosis. Methods: The results achieved over a period of 1 year following the use of these techniques to treat plantar fasciosis were examined. There were 64 patients; 32 of whom were treated with ultrasound-guided EPI and 32 with ultrasound-guided CI. A clinical examination was performed and ultrasound taken before treatment and at 3, 6, and 12 months. Clinical assessments were made using a visual analog scale (VAS) to record pain and the Foot and Ankle Disability Index (FADI) to evaluate function. Ultrasound was used to determine the thickness of the plantar fascia. Results: Both the ultrasound-guided EPI and CI techniques were associated with significant clinical and echographic improvements at 12 months post-treatment ( P < .001). Conclusion: Both techniques were effective in the treatment of PF, providing excellent VAS pain and FADI results at 12 months. However, CI required fewer patient visits and appeared to provide somewhat better VAS and FADI results. Level of Evidence: Level III, retrospective comparative study.

The aims of this study were 1) to describe the effects of a 54 km and 111 km ultra-endurance mountain race on the biomarkers of muscle damage and inflammation, 2) to compare the effects between the two races regarding the biomarkers of muscle damage and inflammation. Sixteen ultra-endurance amateur runners volunteered to participate in this study. Ten runners completed a 54 km race (Group 1; age: 27.0 ± 5.7; height: 179.5 ± 5.8 cm; and body mass: 77.3 ± 10.7 kg) and six completed a 111 km race (Group 2; age: 30.5 ± 8.0; height: 179.4 ± 5.5 cm; and body mass: 76.2 ± 9.4 kg). Blood samples were taken at five different points during the investigation, 24 h before the race, immediately post-race, and again at 24, 48, and 72 h after the race. There were increases in leukocyte (Group 1: p ≤ .001, ES = 2.8; Group 2: p = .001, ES = 3.5) and platelet concentrations (Group 1: p ≤ .001, ES = 2.3; Group 2: p = .02, ES = 1.7) post-races. Significant inter-race differences were also observed in leukocyte at 72 h (Group 1: 5.5 ± 0.9, Group 2: 4.2 ± 0.9, p = .012, ES = 1.5). Erythrocytes, hematocrit and hemoglobin concentration decreased after 54 km and 111 km races at 24, 48 and 72 h (p ≤.001, ES = 2.0–3.18). Conclusiones: la encuesta "Gestiona Hierro EII" sobre el manejo de la anemia en la EII demuestra una alta calidad asistencial, pero con aspectos a mejorar como la indicación de hierro intravenoso en los pacientes con actividad, el uso de hierro intravenoso de altas dosis o la aplicación de algoritmos en la práctica asistencial.

PURPOSE: Increased awareness of the importance of dietary fibre has led to increased interest in "functional" fibre components like digestion-resistant maltodextrin (RMD). This randomized, placebo-controlled, double-blind study assessed the effects of RMD in the colonic transit time (CTT) and defecation characteristics (frequency, stool volume and consistency). METHODS: Sixty-six healthy adult volunteers (32 men) who did not have a daily defecation habit had a 7-day run-in period before the 21-day intervention period with RMD or placebo. CTT and segmental CTT (SCTT) were assessed by a single abdominal X-ray film taken at the end of both periods after radiopaque marker ingestion. Defecation characteristics and intestinal functions were also assessed, which were self-reported by patients. Intragroup comparisons were evaluated by Student's paired t test, Bonferroni test and Chi-square test, while time comparisons by analysis of variance (ANOVA) and time-by-treatment interaction by repeated-measures ANOVA. RESULTS: Fifty-seven subjects were assessed for CTT (placebo, n = 28; RMD, n = 29). In the RMD group, the total CTT, left SCTT and rectosigmoidal SCTT decreased significantly compared to baseline (p < 0.01 each; -13.3, -4.7, -8.7 h, respectively). Significant differences between groups were observed in total CTT and left SCTT. Significant time-by-treatment interaction was observed in the RMD group for stool volume (p = 0.014), increasing 56 % compared to baseline (p < 0.01), while remained unchanged in the placebo group. Stool consistency was improved only in the RMD group (p < 0.01). No adverse effects related to study products were observed. CONCLUSIONS: The results show that RMD improved CTT, stool volume, stool consistency and some intestinal functions in a healthy population. BACKGROUND: Corticosteroid infiltration (CI) is commonly used for treatment of plantar fasciosis. In recent years, however, interest has grown in the use of intratissue percutaneous electrolysis (EPI) for the treatment of tendinopathies. The aim of our study was to compare the effectiveness of the above techniques in the treatment of plantar fasciosis. METHODS: The results achieved over a period of 1 year following the use of these techniques to treat plantar fasciosis were examined. There were 64 patients; 32 of whom were treated with ultrasound-guided EPI and 32 with ultrasound-guided CI. A clinical examination was performed and ultrasound taken before treatment and at 3, 6, and 12 months. Clinical assessments were made using a visual analog scale (VAS) to record pain and the Foot and Ankle Disability Index (FADI) to evaluate function. Ultrasound was used to determine the thickness of the plantar fascia. RESULTS: Both the ultrasound-guided EPI and CI techniques were associated with significant clinical and echographic improvements at 12 months post-treatment ( P < .001). CONCLUSION: Both techniques were effective in the treatment of PF, providing excellent VAS pain and FADI results at 12 months. Este estudio aleatorizado (2:1), controlado con placebo y doble ciego, evaluó los efectos de la quinua sobre el índice de masa corporal (IMC), hemoglobina glicada (HbA1c), glucemia basal (GB) y el grado de saciedad y plenitud en pacientes prediabéticos. Material y método: se aleatorizaron (2:1) 30 pacientes en dos ramas a estudio: el producto Kuska Active (quinua procesada) y placebo (maltodextrina). El periodo de ingesta fue de 28 días. Antes de iniciar el tratamiento y a los 28 días se determinó el IMC, HbA1c y la GB. La sensación de saciedad y plenitud se evaluó mediante escala visual analógica (EVA). Para demostrar la eficacia de la quinua procesada Kuska Active sobre las variables a estudio se realizó ANOVA para medidas repetidas con dos factores a estudio (factor intrasujeto: tiempo y factor intersujeto: producto consumido). Resultados: finalizaron el estudio un total de 29 pacientes (placebo, n = 10; quinua, n = 19). Se detectó en el grupo quinua procesada una disminución significativa en los valores de IMC (p < 0,05) y HbA1c (p < 0,001) y un aumento en la sensación de saciedad y plenitud (p < 0,001). No se encontraron diferencias significativas en los niveles de GB desde el momento basal hasta el periodo postingesta. Conclusiones: los resultados muestran que la quinua procesada disminuye el IMC y los niveles de HbA1c, mantiene los niveles de GB y aumenta la sensación de saciedad y plenitud en pacientes prediabéticos. Material and method: Thirty patients were randomized (2:1) in two study arms: Kuska Active product (processed quinoa) and placebo (maltodextrin), with an intake period of 28 days. BMI, HbA1c and FPG were determined before starting treatment and at 28-day intake. Satiety and fullness sensation were just assessed by visual analog scale (VAS) at the day 28. ANOVA was performed for repeated measures with two factors to study (within-subject factor: time; intersubject factor: product consumed) to demonstrate the effectiveness of processed quinoa on the study variables. Results: Twenty-nine patients (placebo, n = 10; quinoa, n = 19) completed the study, and the quinoa group shows a significant decrease in BMI (p < 0.05) and HbA1c values (p < 0.001), and an increase in the satiation and fullness (complete) degree (p < 0.001). No significant differences were found in FPG levels from baseline to post-intake period. Conclusions: The results show that processed quinoa intake during 28 days decreases BMI and HbA1c levels, maintains FPG levels, and increases the satiation and fullness (complete) degree in prediabetic patients.

242. Análisis Comparativo del Desarrollo del Sistema Olivocerebeloso en Mamíferos y Aves

- Abstract
CONCLUSIONES: 1.- Los análisis de la reacción histoquímica de la actividad citocromo oxidasa demuestran que: i) la actividad respiratoria más intensa fue siempre detectada en botones de terminales axónicas y ii) la existencia de una compartimentalización que en general concuerda con los patrones de aferencias GABAérgicas a la oliva inferior de la rata. 2.- Ultraesctructuralmente, la oliva inferior del pollo es muy similar a la de rata. Destaca la existencia de glomérulos olivares y la ausencia de una correlación neta entre el contenido de las vesículas sinápticas y el tamaño de las especializaciones pre- y postsináptica. Nuestro estudio no ha conseguido detectar uniones electrotónicas entre las neuronas olivares del pollo. Aún sin descartar un error de observación por la existencia de un íntimo número de uniones gap que pudieron ser inadvertidas y a falta de un estudio inmunocitoquímico definitivo, estos resultados podrían de alguna manera poner en tela de juicio el papel preponderante que estas uniones juegan e la coordinación motora subcortical. 3.- Se confirma la existencia de dos rutas de migración para las neuronas olivares del pollo, al igual que ocurre en los mamíferos. La ruta de migración submarginal ontogenéticamente anterior a la marginal, posee un volumen y empaquetamiento celular mayor y aporta células a la lamela dorsal en toda la extensión rostrocaudal de la oliva inferior. La ruta marginal más tardía, posee un empaquetamiento celular menor y ocupa la mitad rostral del dominio olivar, en relación con la lamela ventral. 4.- La organización citoarquitectónica de la oliva inferior es mucho más compleja en los mamíferos que en las aves. Estas diferencias conllevan modificaciones en cuanto al volumen y topografía der las rutas de migración. No obstante, la comparación espacio-temporal entre ellas, ratifica la homología existente entre la lamela ventral y la parte caudal de la oliva accesoria medial. Sugiriendo que aquellas neuronas olivares que comparten un sitio de proyección común en el cerebelo, puedan emigrar por una misma ruta en aves y mamíferos. 5.- La axonogénesis de las jóvenes neuronas olivares de las aves y mamíferos es un fenómeno muy temprano, coincidente con la migración neuronal, no sólo en el tiempo sino también en el espacio, usando los axones olivares tanto la ruta submarginal como la marginal. El cruce de la mayoría de los axones por el centro de decisión representado por la comisura interolivar se realiza 2 días después de la generación neuronal. La morfología de los conos de crecimiento antes, durante y después de este cruce son una evidente expresión de las relaciones que mantienen con los diversos territorios que atraviesan. 6.- Como en su trayecto a lo largo del bulbo raquídeo los jóvenes axones bulbares que penetran inicialmente en el cerebelo presentan importante cambios morfológicos en cuanto a sus conos de crecimiento. También en este evento el intervalo temporal entre la fecha de generación de las neuronas olivares y la de entrada de los axones en el cerebelo es similar para el pollo y la rata, con una duración de 4 días. 7.- Las células de Purkinje del pollo muestran las mismas fases de desarrollo que las descritas en la rata. Como en los mamíferos, las células de Purkinje muestran una heterocronicidad en su desarrollo, no sólo entre las localizadas en distintos folios, sino en también en aquellas ubicadas en regiones corticales concretas de un mismo folio. A diferencia de los descrito hasta el momento, el grado de maduración de las células de Purkinje está invertido en el pollo, apareciendo siempre las de ápex del folio como más inmaduras. 8.- Las fibras trepadoras poseen también una marcada heterocronicidad en su desarrollo, coincidente temporo-espacialmente con el desarrollo de las células de Purkinje. 9.- La dendritogénesis de las neuronas olivares pasa por cuatro fases definidas: i) fase de joven neurona bipolar, ii) fase de neurona estrellada con dendritas primarias gruesas y dendritas secundarias largas y sinuosas y iii) fase de regresión filopódica y dendrítica y iv) fase de neurona de morfología madura. Entre las dos primeras existe una etapa de transición caracterizada por la abundancia de filopodios. Al igual que en la rata, las dos primeras fases coinciden con un escaso desarrollo sináptico en el neuropilo, índice de la existencia de cierta autonomía de las neuronas olivares, respecto a sus futuras aferencias, en este momento del desarrollo. La transición representada por las dos últimas fases coincide con el momento de mayor sinaptogénesis. 10.- AL igual que ocurre en la rata, las jóvenes neuronas olivares del pollo muestran una evidente heterogeneidad morfológica, que evidencia la asincronía en la maduración dendrítica de las neuronas olivares. En días tempranos, esta heterocronicidad muestra un evidente gradiente mediolateral que por los posteriores movimientos celulares dentro del núcleo quedará eliminado, observándose en cualquier área olivar células en diversos grados de maduración. 11.- La morfología que predomina en el soma de las neuronas adultas del pollo es fundamentalmente estrellada en contraposición con la de la rata que es redondeada. El campo dendrítico de las neuronas olivares muestra dos tipos de ramificación, una radiada y otra bipenachada, que se relacionan con el espacio disponible por el árbol dendrítico en el interior del núcleo. 12.- La sinaptogéneis de la oliva inferior del pollo, como en la rata, coincide con los momentos de máxima dendritogénesis de las neuronas olivares. La temprana aparición de sinapsis químicas y la ausencia de uniones gap nos hacen confirmar que, al menos en la oliva inferior de las aves y los mamíferos, dichas uniones no son una reminiscencia filogenética. 13.- Los recuentos de las poblaciones neuronales a lo largo del desarrollo de la oliva inferior, muestran una evolución similar en el pollo y la rata. Existe una disminución en su número durante el periodo de sinaptogeéneis y una recuperación similar en magnitud inmediatamente después. La evolución relativa de cada uno de los subnúcleos es similar en la rata y en el pollo. 14.- Al contrario de lo que ocurre en la rata, las variaciones de volumen durante el desarrollo de la oliva inferior en el pollo, evolucionan de una manera exponencial y constante sin apreciarse cambio en función de la maduración global de la oliva inferior. La evolución relativa de la volumetría de cada una de las lamelas es similar. 15.- A pesar de la muerte neuronal descrita durante el desarrollo de la oliva inferior, la razón glía/neurona, que permanece sensiblemente constante durante estos mismos días, abre al menos, dos apasionantes líneas de discusión y trabajo. De un lado la cuantificación real de los posibles fenómenos de muerte neuronal natural hasta ahora descritos en este núcleo. Y de otro, el papel que el número de neuronas pueda tener en el control de la proliferación glial en condiciones normales. 16.- La degeneración de las neuronas de la oliva inferior posthemicerebelectomía postnatal temprana proporciona una colección de eventos que se pueden utilizar como control de los ocurridos en la oliva inferior contralateral a las pedunculotomías. La degeneración de las neuronas olivares se encuentra estrechamente relacionada con la fecha de nacimiento de las mismas. 17.- Después de la pedunculotomía postnatal temprana disminuye la muerte neuronal en la oliva inferior ipsilateral a la lesión en la rata, sugiriendo la posibilidad de que algunas de las células destinadas a morir se recluten por un incremento de la diana disponible e inerven el hemicerebelo deprivado de aferencias olivares. 18.- La presencia de un mayor número de neuronas en la oliva ipsilateral a la lesión incluso un día después de la pedunculotomía, parece indicar que la muerte neuronal en la oliva inferior de la rata tiene lugar antes de lo que se ha descrito en la literatura y que en un primer momento el desarrollo, la pérdida efectiva de neuronas olivares podría permitir el autocontrol de aquellas neuronas que, después de su migración desde el lábio rómbico, han suspendido su búsqueda para una correcta localización dentro de la oliva inferior y por tanto pueden disturbar el patrón preliminar de la organización topográficas del núcleo antes del comienzo de la sinaptogénesis entre células de Purkinje y fibras trepadoras. 19.- Las neuronas olivares jóvenes responden degenerando antes a la lesión que las neuronas de los animales mayores. Ratificando en este núcleo, las observaciones realizadas en otras áreas del sistema nervioso, en las que existe una relación inversa entre el ritmo de degeneración neuronal postlesión y la edad en la que se produce la axotomía. La distinta reacción de las neuronas olivares a la axotomía durante y después del período de multinervación, parece sugerir que este período (coincidente con el proceso de muerte neuronal natural), pueda ser un momento crítico en el fin de las interdependencias entre las neuronas olivares y su diana. 20.- El comportamiento de la glía, evidenciado directamente por los recuentos de figuras mitóticas, es diferentes dependiendo de cuando se produce la lesión. La respuesta de la glía es más eficiente y rápida en los animales adultos que en los animales recién nacidos donde la respuesta es más lenta y sostenida. 21.- Durante el desarrollo de la proyección olivocerebelosa algunas colaterales axónicas que penetran por el pedúnculo cerebeloso contralateral cruzan la línea media cerebelosa, distribuyéndose dentro del hemicerebelo ipsilateral. Como en otras áreas del sistema nervioso central, esta inervación transitoria ipsilateral se eliminará después de la formación del patrón definitivo de conexiones sinápticas cruzadas. 22.- Se ha demostrado la existencia de una pequeña proyección ipsilateral tanto en el pollo como en la rata adultos. La coincidencia entre los patrones neurogenéticos y topográficos descritos para la rata y la ubicación de estas neuronas ipsilaterales en ambos animales, indica que se trata de un pequeño contingente residual que se mantiene en la filogenia y cuyo papel se desconoce.

243. Effect of a training programme on blood culture contamination rate in critical care

Blood culture contamination can occur from extraction to processing; its rate should not exceed 3%.To evaluate the impact of a training programme on the rate of contaminated blood cultures after the implementation of sample extraction recommendations based on the best evidence.Prospective before-after study in a polyvalent intensive care unit with 18 beds. Two phases were established (January-June 2012, October 2012-October 2015) with a training period between them. Main recommendations: sterile technique, surgical mask, double skin disinfection (70° alcohol and 2% alcoholic chlorhexidine), 70° alcohol disinfection of culture flasks and injection of samples without changing needles. Including all blood cultures of patients with extraction request.demographic, severity, pathology, reason for admission, stay and results of blood cultures (negative, positive and contaminated). Basic descriptive statistics: mean (standard deviation), median (interquartile range) and percentage (95% confidence interval). Calculated contamination rates per 100 blood cultures extracted. Bivariate analysis between periods.Four hundred and eight patients were included. Eight hundred and forty-one blood cultures were taken, 33 of which were contaminated. In the demographic variables, severity, diagnosis and stay of patients with contaminated samples, no differences were observed from those with uncontaminated samples. Pre-training vs post-training contamination rates: 14 vs 5.6 per 100 blood cultures extracted (P=.00003).An evidence-based training programme reduced the contamination of samples. It is necessary to continue working on the planning of activities and care to improve the detection of pollutants and prevent contamination of samples.

To report the immediate and prolonged (one week) effects of elastic bandage (EB) on balance control in subjects with chronic ankle instability.Twenty-eight individuals successfully completed the study protocol, of whom 14 were randomly assigned to the EB group (7 men, 7 women) and 14 were assigned to the non-standardised tape (NST) group (9 men, 5 women). To objectively measure postural sway we used computerised dynamic posturography (CDP) with sensory organisation test (SOT) and unilateral stance (US) test. We analysed the following SOT parameters: the composite SOT score, the composite SOT strategy and the SOT condition 2 and its strategy. In addition, we studied the centre of gravity (COG) sway velocity with open eyes and close eyes during the US test.Repeated measures ANOVA showed a significant effect for time in composite SOT score (F= 34.98; p= 0.01), composite SOT strategy (F= 12.082; p= 0.02), and COG sway with open eyes (F= 3.382; p= 0.039) in EB group and NST group. Therefore, there were improvements in balance control after bandage applications (defined as better scores in SOT parameters and decreased COG sway in US test). However, no differences between groups were observed in the most relevant parameters.This study did not observe differences between EB and NST during the follow-up in the majority of measurements. Several outcome measures for SOT and US tests improved in both groups immediately after bandage applications and after one week of use. EB of the ankle joint has no advantage as compared to the non-standardised tape. Implications for rehabilitation Elastic bandage (EB) of the ankle joint has no advantage as compared to the non-standardised tape. The effects of the bandages could be due to a greater subjective sense of security. It is important to be prudent with the use of bandage, since a greater sense of safety could also bring with it a greater risk of injury. The application of the bandage on subjects with chronic ankle instability (CAI) should be prolonged and used alongside other physiotherapy treatments.

[EN] The present work consists of two distinct parts. The objective is to analyze if the two alternatives proposed improve on the current situation of the A-1515, between the link with the A-23 and the municipality of Rubielos de Mora (Teruel), therefore, each one of the aspects considered in this work are analyzed both in the current road as in the two alternative proposals. The first of the parties focuses on the analysis of the traffic and road safety, while the second part is on the environmental impact, that is to say, how are going to impact the proposed alternatives to the environment, to finally make an overall assessment of the two alternatives. Traffic analysis is done, taking as a starting point a data capacity manual made in the road of study and making use of related gauging stations is estimated the current daily average intensity and with a prognosis of the traffic is predicted traffic flow for subsequent years. Once the current IMD and years to come, analyzes the level of service and the capacity of each of the years considered. In relation to road safety is performed, always in line with the geometric design, a study of the speeds of operation with the objective of ensuring that the users traveling in safe conditions. L'objectiu és analitzar si les dues alternatives proposades milloren la situació actual de la carretera A-1515, compresa entre l'enllaç amb l'A-23 i el municipi de Rubielos de Mora (Teruell), per tant, cada un dels aspectes considerats a al llarg d'aquest treball s'analitzaran tant a la carretera actual com en les dues alternatives proposades. La primera de les parts se centra en l'anàlisi del trànsit i la seguretat viària, mentre que la segona part tracta sobre l'impacte mediambiental, és a dir, com van a impactar les alternatives proposades al medi ambient, per finalment realitzar una valoració global de les dues alternatives. Referent al trànsit es realitza una anàlisi prenent com a dada de partida un aforament manual realitzat a la carretera d'estudi i fent ús d'estacions afins d'aforament s'estima la intensitat mitjana diària actual i mitjançant una prognosi del trànsit es prediu el flux de trànsit per anys posteriors. Un cop obtinguda la IMD actual i d'anys venidors, s'analitza el nivell de servei i la capacitat de cada un dels anys considerats. En relació a la seguretat viària es realitza, sempre d'acord amb el disseny geomètric, un estudi de les velocitats d'operació amb l'objectiu de garantir que els usuaris que circulin en condicions segures. El objetivo es analizar si las dos alternativas propuestas mejoran la situación actual de la carretera A-1515, comprendida entre el enlace con la A-23 y el municipio de Rubielos de Mora (Teruel), por tanto, cada uno de los aspectos considerados a lo largo de este trabajo se analizarán tanto en la carretera actual como en las dos alternativas propuestas. La primera de las partes se centra en el análisis del tráfico y la seguridad vial, mientras que la segunda parte trata sobre el impacto medioambiental, es decir, cómo van a impactar las alternativas propuestas al medioambiente, para finalmente realizar una valoración global de las dos alternativas. Referente al tráfico se realiza un análisis tomando como dato de partida un aforo manual realizado en la carretera de estudio y haciendo uso de estaciones afines de aforo se estima la intensidad media diaria actual y mediante una prognosis del tráfico se predice el flujo de tráfico para años posteriores. Una vez obtenida la IMD actual y de años venideros, se analiza el nivel de servicio y la capacidad de cada uno de los años considerados. En relación a la seguridad vial se realiza, siempre en consonancia con el diseño geométrico, un estudio de las velocidades de operación con el objetivo de garantizar que los usuarios que circulen en condiciones seguras. Para ello se analiza la consistencia con el objetivo de determinar si el trazado se corresponde con las expectativas de los usuarios y por último se realiza una estimación de accidentes. En la segunda parte de este trabajo se ha analizado como va a afectar cada una de las alternativas al medio ambiente. Pelvic floor muscle training is commonly used for urine loss. However, research studies have not determined which training load is the most effective for women with stress urinary incontinence (SUI). Moreover, none of the previous reviews or studies have described the total effectiveness of pelvic floor muscle training (PFMT) with an objective test such as the pad test. The objectives were to analyze the effectiveness of pelvic floor muscle training in women with SUI and to determine which training load produces the greatest adaptations for decreasing urine loss. The search was conducted in three databases (PubMed, Web of Science and Cochrane), for randomized controlled trials (RCTs) that evaluated the effects of PFMT. Studies were included if they met the following criteria: participants were women; were older than 18; had SUI; were treated with PFMT; and the assessments of the effects were measured with a pad test. Finally, 10 articles (293 women) analyzed the pad test in women with SUI who performed PFMT. The meta-analysis showed that PFMT, independent of the protocol used in the study, resulted in decreased urine loss in women suffering from SUI.
