218. Development and Validation of a Clinical Scoring System for Predicting Risk of HCC in Asymptomatic Individuals Seropositive for Anti-HCV Antibodies.

Abstract

Background: The development of a risk assessment tool for long-term hepatocellular carcinoma risk would be helpful in identifying high-risk patients and providing information of clinical consultation. Methods: The model derivation and validation cohorts consisted of 975 and 572 anti-HCV seropositives, respectively. The model included age, alanine aminotransferase (ALT), the ratio of aspirate aminotransferase to ALT, serum HCV RNA levels and cirrhosis status and HCV genotype. Two risk prediction models were developed: one was for all-anti-HCV seropositives, and the other was for anti-HCV seropositives with detectable HCV RNA. The Cox's proportional hazards models were utilized to estimate regression coefficients of HCC risk predictors to derive risk scores. The cumulative HCC risks in the validation cohort were estimated by Kaplan-Meier methods. The area under receiver operating curve (AUROC) was used to evaluate the performance of the risk models. Results: All predictors were significantly associated with HCC. The summary risk scores of two models derived from the derivation cohort had predictability of HCC risk in the validation cohort. The summary risk score of the two risk prediction models clearly divided the validation cohort into three groups (p<0.001). The AUROC for predicting 5-year HCC risk in the validation cohort was satisfactory for the two models, with 0.73 and 0.70, respectively. Conclusion: Scoring systems for predicting HCC risk of HCV-infected patients had good validity and discrimination capability, which may triage patients for alternative management strategies. Abstract

Summary We examined United States Renal Data System (USRDS) data for adult kidney transplant recipients in 1995-2003 ( n = 87 575) to investigate associations of 12-month renal function with long-term clinical outcomes. Estimated glomerular filtration rate (eGFR) was computed by the Modification of Diet in Renal Disease (MDRD) equation. Associations of eGFR at the first transplant anniversary with graft and patient-survival in years 1-9 post-transplant were evaluated by multivariate nonlinear regression with spline forms, adjusted for recipient, donor, and transplant factors. Regardless of donor type, the likelihood of graft failure and death increased significantly with lower eGFR. The impact of poor eGFR was more pronounced for graft failure than death. Relative effects were similar across donor types, but were strongest among living-donor recipients. For example, compared with reference eGFR of 80 ml/min/1.73 m2, 1-year eGFR of 20 ml/min/1.73 m2 was associated with adjusted hazards ratios for subsequent death-censored graft failure of 9.2 in living, 8.9 in standard criteria deceased, and 5.9 in expanded criteria deceased-donor recipients. First-year renal function after kidney transplantation has strong, nonlinear associations with subsequent allograft and patient survival regardless of donor type. Post-transplant eGFR may be a useful end-point for discriminating benefits of care strategies that differentially affect renal function. [ABSTRACT FROM AUTHOR]

Abstract

Abstract: Previous studies have demonstrated an association between certain second-generation antipsychotics (SGAs) and diabetes mellitus. The study assessed the impact of SGA dose on hemoglobin A1C (HbA1c >6.0) levels in a real-world setting. Patients aged ≥18years during 2002–2006 in Ingenix LabRx claims database were included. The database collects medical and prescription claims and a subset of laboratory results for an employed, commercially insured population distributed throughout the United States. Patients with previously diagnosed diabetes, identified by the ICD-9-CM code of 250.x or use of antidiabetic agents, were excluded. The main exposure measure was the cumulative dose over a 30day period before the HbA1c test, calculated as [sum of (number of pills per day×strength)]/100. A logistic regression was used to examine the relation with HbA1c >6.0 by tertile of the cumulative dose and average daily dose, adjusted for the covariates. The study included 391 patients on olanzapine, 467 on quetiapine, and 262 on risperidone. Patients treated with aripiprazole or ziprasidone (n=212) were included as a secondary reference because of their minimal metabolic risk. Compared to lower (Tertiles 1 and 2) cumulative doses of risperidone, patients with a high cumulative dose of risperidone (Tertile 3) had a significantly higher odds ratio (OR) for HbA1c >6.0 (adjusted OR=2.45; 95% confidence interval=1.13–5.32; P =0.023). A similar increase in OR was seen in patients with high cumulative dose of olanzapine (2.41; 1.19–4.89; P =0.015). Analyses of average daily dose revealed that quetiapine≥400mg/day and risperidone≥2mg/day had an OR of 2.29 (1.04–5.06; P =0.041) and 2.28 (1.08–4.83; P =0.032), respectively, compared to aripiprazole/ziprasidone. Both olanzapine groups (≥10 and <10mg/day) were associated with a significantly increased OR. All results remained similar after further adjustment for the predicated probability of having an HbA1c test and additional medication covariates. In this claims data study, use of olanzapine was associated with elevated HbA1c and risperidone and quetiapine appeared to have dose-related association with elevated HbA1c. One of the limitations of a claims data analysis is the lack of information on potential confounders such as ethnicity and weight. [Copyright &y& Elsevier]
