Introduction Over the past two decades, the incidence of acute rheumatic fever (ARF) and chronic rheumatic heart disease (RHD) have dramatically declined in wealthier regions of the world as a result of preventative programmes, improved living standards and access to cardiac surgery. Nevertheless, ARF and RHD are still public health problems in less-developed regions of the world such as Oceania, south Asia and sub-Saharan Africa. Aim We report on clinical, therapeutic and prognostic aspects as well as the difficulties encountered during this first series of surgery for rheumatic valve disease in Mali. Methods This was a prospective, descriptive study conducted at the Andre Festoc Cardiac Surgery Centre from September 2018 to August 2019. Results The frequency of patients having been operated on for rheumatic valve disease was 44.73% (68 patients). The mean age of the patients was 18 ± 10 years with extremes of five and 60 years. The gender ratio was 0.7. The delay to treatment was between one and three years for 39.7% of the patients. The main diagnoses found were: mitral regurgitation in 50% of patients, mitral stenosis in 16.2% and aortic regurgitation in 10.3%. Pulmonary artery systolic pressure was 35-50 mmHg in 19.1% of patients and more than 50 mmHg in 25%. The median cardiopulmonary bypass time was 132 minutes (60-276) and median extubation time was three hours (0-96). The main complications were cardiac, renal, neurological, respiratory, gastrointestinal and infectious. In the immediate postoperative period, we recorded three deaths, which is a mortality rate of 4.4%. Conclusions Humanitarian efforts have led non-governmental organisations (NGOs) to launch surgical programmes in low-and middle-income countries in an attempt to fill the gap in these fragile healthcare systems. Cardiac surgery requires much expertise from the medical staff, as well as many material and financial resources. Empowerment of the local team is a challenge that is being realised since taking these essential steps of companionship with the NGO la Chaine de l'Espoir.

Summary Background WHO recently approved a partially effective vaccine that reduces clinical malaria in children, but increased vaccine activity is required to pursue malaria elimination. A phase 1 clinical trial was done in Mali, west Africa, to assess the safety, immunogenicity, and protective efficacy of a three-dose regimen of Plasmodium falciparum sporozoite (PfSPZ) Vaccine (a metabolically active, non-replicating, whole malaria sporozoite vaccine) against homologous controlled human malaria infection (CHMI) and natural P falciparum infection. Methods We recruited healthy non-pregnant adults aged 18–50 years in Doneguebougou, Mali, and surrounding villages (Banambani, Toubana, Torodo, Sirababougou, Zorokoro) for an open-label, dose-escalation pilot study and, thereafter, a randomised, double-blind, placebo-controlled main trial. Pilot study participants were enrolled on an as-available basis to one group of CHMI infectivity controls and three staggered vaccine groups receiving: one dose of 4·5 × 105, one dose of 9 × 105, or three doses of 1·8 × 106 PfSPZ via direct venous inoculation at approximately 8 week intervals, followed by homologous CHMI 5 weeks later with infectious PfSPZ by direct venous inoculation (PfSPZ Challenge). Main cohort participants were stratified by village and randomly assigned (1:1) to receive three doses of 1·8 × 106 PfSPZ or normal saline at 1, 13, and 19 week intervals using permuted block design by the study statistician. The primary outcome was safety and tolerability of at least one vaccine dose; the secondary outcome was vaccine efficacy against homologous PfSPZ CHMI (pilot study) or against naturally transmitted P falciparum infection (main study) measured by thick blood smear. Combined artesunate and amodiaquine was administered to eliminate pre-existing parasitaemia. Outcomes were analysed by modified intention to treat (mITT; including all participants who received at least one dose of investigational product; safety and vaccine efficacy) and per protocol (vaccine efficacy). This trial is registered with ClinicalTrials.gov , number NCT02627456 . Findings Between Dec 20, 2015, and April 30, 2016, we enrolled 56 participants into the pilot study (five received the 4·5 × 105 dose, five received 9 × 105, 30 received 1·8 × 106, 15 were CHMI controls, and one withdrew before vaccination) and 120 participants into the main study cohort with 60 participants assigned PfSPZ Vaccine and 60 placebo in the main study. Adverse events and laboratory abnormalities post-vaccination in all dosing groups were few, mainly mild, and did not differ significantly between vaccine groups (all p>0·05). Unexpected severe transaminitis occured in four participants: one participant in pilot phase that received 1·8 × 106 PfSPZ Vaccine, one participant in main phase that received 1·8 × 106 PfSPZ Vaccine, and two participants in the main phase placebo group. During PfSPZ CHMI, approximately 5 weeks after the third dose of 1·8 × 106 PfSPZ, none of 29 vaccinees and one of 15 controls became positive on thick blood smear; subsequent post-hoc PCR analysis for submicroscopic blood stage infections detected P falciparum parasites in none of the 29 vaccine recipients and eight of 15 controls during CHMI. In the main trial, 32 (58%) of 55 vaccine recipients and 42 (78%) of 54 controls became positive on thick blood smear during 24-week surveillance after vaccination. Vaccine efficacy (1–hazard ratio) was 0·51 per protocol (95% CI 0·20–0·70; log-rank p=0·0042) and 0·39 by mITT (0·04–0·62; p=0·033); vaccine efficacy (1–risk ratio) was 0·24 per-protocol (0·02–0·41; p=0·031) and 0·22 mITT (0·01–0·39; p=0·041). Interpretation A three-dose regimen of PfSPZ Vaccine was safe, well tolerated, and conferred 51% vaccine efficacy against intense natural P falciparum transmission, similar to 52% vaccine efficacy reported for a five-dose regimen in a previous trial. Context and objective. Little is known about the risk and consequences of teenage pregnancies in Sub-Saharan African countries. The aim of the present study was to determine the perinatal outcome of infants born to adolescent mothers. Methods. A cross-sectional study involving newborns from mothers aged 18-year-old or under at the time of childbirth was conducted between September 1, 2018 and April 3, 2019 at Ziguinchor hospital. Children born at home and newborns whose obstetric-neonatal record was incomplete were excluded. Parameters of interest were epidemiological, sociodemographic, obstetric and perinatal. Results. There were 170 births for a population of 166 adolescent girls, including 162 single fetal pregnancies and 4 twin pregnancies. Stillbirths were 7 and live births 163. The average age of adolescent girls was 17 years [13 - 18], 56 % of them had not reached the 4 antenatal consultations. Arterial hypertension and eclampsia were the most common obstetric pathologies (27.1 %). The sex ratio H/F of the newborns was 1.2. The mean gestational age was 35.2 weeks of amenorrhea [32 – 43.5] and the mean birth weight was 2788 grams [1300 - 4400]. Eighty-nine newborns were hospitalized, a rate of 52.4 %. The main neonatal pathologies were perinatal asphyxia (n = 41); low birth weight (n = 41); neonatal infection (n = 36). The mean hospital stay was 5.5 days [3 - 16]. Conclusion. Newborns of adolescent mothers in Ziguinchor appear to be at increased risks of prematurity, hypotrophy, perinatal asphyxia and infection while their mothers, of hypertension. Contexte et objectif. Le risque et les conséquences de la grossesse des adolescentes restent peu documentés dans les pays d’Afrique subsaharienne. L’objectif de l’étude était de déterminer le devenir périnatal des nouveau-nés de mères adolescentes. Méthodes. Il s’agissait d’une étude transversale incluant les nouveau-nés des mères adolescentes (âgés de ≤ 18 ans), entre les 1er septembre 2018 et 03 avril 2019, à l’hôpital de Ziguinchor. Etaient exclus, les enfants nés à domicile et les nouveau-nés dont le dossier obstétrico-néonatal était incomplet. Les paramètres d’intérêt englobaient les données épidémiologiques, sociodémographiques, obstétricales et périnatales. Résultats. Au total, il y a eu 170 naissances pour une population de 166 adolescentes dont 162 grossesses mono fœtales et 4 grossesses gémellaires. Les mort-nés étaient au nombre de 7 et les naissances vivantes au nombre de 163. L’âge moyen des adolescentes était de 17 ans [13 – 18], 56% d’entre eux n’avaient pas réalisé les 4 consultations prénatales requises. L’hypertension artérielle, l’éclampsie étaient les pathologies obstétricales les plus représentées (27,1%). Le sex-ratio des nouveau-nés était de 1,2. L’âge gestationnel moyen était de 35,2 semaines d’aménorrhée [32 - 43,5] et le poids de naissance moyen était de 2788 grammes [1300 - 4400]. Quatre-vingt-neuf nouveau-nés étaient hospitalisés, soit un taux de 52,4%. Les principales pathologies néonatales étaient l’asphyxie périnatale (n = 41) ; le petit poids de naissance (n=41) ; l’infection néonatale (n=36). La durée moyenne d’hospitalisation était de 5,5 jours [3 - 16]. Conclusion. Les nouveau-nés de mère adolescente à Ziguinchor présentent des risques de prématurité, d’hypotrophie, d’asphyxie périnatale et d’infection, tandis que leur mère, l’hypertension artérielle.

Introduction: In resource-limited countries, epilepsy prevalence is underestimated and little is known about its risk factors. Objectives: This study aimed to determine the prevalence and risk factors for epilepsy in six health districts (HDs) in Mali. Methods: A community-based cross-sectional and nested case-control study was conducted in 180 villages with the highest number of suspicious epilepsy cases (SECs) in the six study HDs. The SECs were observed as part of a Phase 1 screening conducted by community health workers. For the nested case-control study, one case was matched with at least one control based on residence and age. A case of epilepsy was a person diagnosed with convulsive epilepsy after clinical assessment by a neurologist. A control was a person diagnosed as normal after neurological assessment by a neurologist. Data were collected on sociodemographic characteristics, familial and medical history of epilepsy, consanguinity, place of delivery, preterm birth, length/type of delivery, and history of meningitis and cerebral malaria. A univariate and multivariate binomial logistic regression model was used to analyse factors associated with epilepsy. Results: A total of 1,506 cases of epilepsy and 2,199 controls were enrolled in six HDs. The mean prevalence of epilepsy was 2‰, with the highest in Kenieba (3‰), a previously meso-endemic-onchocerciasis HD, and the lowest in Kadiolo (1.5‰), a hypo-endemic-onchocerciasis HD. Age (adjusted odds ratio [aOR] = 1.02 [95% confidence intervals [CI] 1.02–1.03]), history of cerebral malaria (aOR = 11.41 [95% CI 8.86–14.85]), history of meningitis (aOR = 1.95 [95% CI 1.16–3.29]), living in the HD of Tominian (aOR = 1.69 [95% CI 1.29–2.22]), delayed delivery (aOR = 3.21 [95% CI 2.07–5.07]), and dystocia (aOR = 3.37 [95% CI 2.03–5.73]) were all significantly associated with epilepsy. Conclusion: The prevalence of epilepsy (3‰) in a previously meso-endemic-onchocerciasis HD was much lower than the prevalence (13.35‰) documented in onchocerciasis endemic areas in 2,000. This decrease epilepsy prevalence in the previously meso-endemic region was induced by onchocerciasis, and the reduction was due to an effective community direct treatment with ivermectin programme. Cerebral malaria and obstetrical complications were the main risk factors for epilepsy and interventions improving malaria prevention/treatment and optimizing prenatal and obstetrical care need to be implemented to reduce incidence.

Introduction The pandemic of a novel coronavirus disease 2019 (COVID-19) caused by a severe acute respiratory coronavirus 2 (SARS-CoV-2) infection has been problematic worldwide. A new SARS-CoV-2 diagnostic test (SmartAmp) was licensed in Japan in July 2021. This method, which enables us to diagnose COVID-19 as well as a gene mutation on the virus, is promising to reduce medical costs and staff labor. Patients and methods To analyze the diagnostic accuracy of the SmartAmp assay for diagnosing COVID-19, we performed this retrospective study at our institute during April and May 2021. We compared the results of the SmartAmp assay and real-time reverse transcription-polymerase chain reaction (rRT-PCR) using a saliva sample from individuals suspected as having COVID-19. Results Out of 70 samples tested, the SmartAmp assay had 50 (71%) positive and 20 (29%) negative results. Using rRT-PCR as a reference, the diagnostic accuracy displayed a sensitivity of 84%, a specificity of 95%, a positive predictive value of 97.7%, and a negative predictive value of 70.4%. On the other hand, false-negative cases were found in 7 (10%), and there was no significant difference of Ct-value between true positive and false negative cases (Mean Ct-value 25.2 vs. 27.5 cycles, p = 0.226 by Mann-Whitney U test). Conclusion The SmartAmp assay is a valuable method to diagnose COVID-19 rapidly. However, the negative predictive value is not high enough to diagnose the disease, so that negative results should be considered for rRT-PCR testing if patients are suspected of having COVID-19.

Facile techniques are required for the debonding of polymeric adhesives from metals for material recycling. A promising solution is a pulsed discharge debonding method. In this approach, a pulsed discharge with a high voltage is applied to the system over a short time. Rapid expansion of gasified materials and shock-wave emissions are produced from the discharge passage, which results in debonding. A drawback of the conventional pulsed discharge method is that it must be conducted in water, otherwise, the pulsed discharge creeps along the material surface without destroying the adhesion. In this study, filler was added to the structure adhesive for the effective pulsed discharge debonding of metal adherend. Epoxy resins with fillers of carbon black, copper and barium titanate particles were fabricated, and their properties and adhesive strength before and after application of the pulsed discharge were investigated. We succeeded in introducing the pulsed discharge inside the adhesive by adding 3.4 vol% of carbon black to the epoxy adhesive; the rapid expansion of gasified adhesion that was caused by the discharge, resulted in debonding. The threshold volume fraction of carbon black for effective pulsed discharge inside the adhesive was discussed based on the percolation model with experimental data.
