Most common human traits and diseases have a polygenic pattern of inheritance: DNA sequence variants at many genetic loci influence the phenotype. Genome-wide association (GWA) studies have identified more than 600 variants associated with human traits, but these typically explain small fractions of phenotypic variation, raising questions about the use of further studies. Here, using 183,727 individuals, we show that hundreds of genetic variants, in at least 180 loci, influence adult height, a highly heritable and classic polygenic trait. The large number of loci reveals patterns with important implications for genetic studies of common human diseases and traits. First, the 180 loci are not random, but instead are enriched for genes that are connected in biological pathways (P = 0.016) and that underlie skeletal growth defects (P < 0.001). Second, the likely causal gene is often located near the most strongly associated variant: in 13 of 21 loci containing a known skeletal growth gene, that gene was closest to the associated variant. Third, at least 19 loci have multiple independently associated variants, suggesting that allelic heterogeneity is a frequent feature of polygenic traits, that comprehensive explorations of already-discovered loci should discover additional variants and that an appreciable fraction of associated loci may have been identified. Fourth, associated variants are enriched for likely functional effects on genes, being over-represented among variants that alter amino-acid structure of proteins and expression levels of nearby genes. Our data explain approximately 10% of the phenotypic variation in height, and we estimate that unidentified common variants of similar effect sizes would increase this figure to approximately 16% of phenotypic variation (approximately 20% of heritable variation). To evaluate the contribution of common genetic variation to normal physiologic variation in serum concentrations of these cations, we conducted genome-wide association studies of serum magnesium, potassium, and sodium concentrations using ~2.5 million genotyped and imputed common single nucleotide polymorphisms (SNPs) in 15,366 participants of European descent from the international CHARGE Consortium. Study-specific results were combined using fixed-effects inverse-variance weighted meta-analysis. SNPs demonstrating genome-wide significant (p<5×10-8) or suggestive associations (p<4×10-7) were evaluated for replication in an additional 8,463 subjects of European descent. The association of common variants at six genomic regions (in or near MUC1, ATP2B1, DCDC5, TRPM6, SHROOM3, and MDS1) with serum magnesium levels was genome-wide significant when meta-analyzed with the replication dataset. All initially significant SNPs from the CHARGE Consortium showed nominal association with clinically defined hypomagnesemia, two showed association with kidney function, two with bone mineral density, and one of these also associated with fasting glucose levels. Common variants in CNNM2, a magnesium transporter studied only in model systems to date, as well as in CNNM3 and CNNM4, were also associated with magnesium concentrations in this study. We observed no associations with serum sodium or potassium levels exceeding p<4×10-7. Follow-up studies of newly implicated genomic loci may provide additional insights into the regulation and homeostasis of human serum magnesium levels.

Aim: The biofilm formation plays a crucial role in the pathogenesis of many acute and subacute microbial infections as well as of chronic wounds. For the stimulation of wound healing tissue tolerable plasma (TTP) is an alternative to antiseptic or chemotherapeutic therapies. The mechanical influence of argon plasma on the penetration of bacteria into the tissue related to the antimicrobial effect of the plasma is unknown. Hence the effects of argon plasma versus argon gas were examined for bacterial reduction as well as for the penetration depth of P. aeruginosa in a 3-D human epidermis model, the so-called combi-model. Method: 3-D NHEK cell cultures were inoculated with P. aeruginosa SG81 and treated after 20 min incubation with TTP. The distribution and penetration depth of the bacteria in the cell culture were compared by confocal laser scan microscope (CLSM) between control and TTP effect. Result: The penetration of bacteria into the epidermis tissue and a significant reduction of the bacteria could be proven by means of TTP. Therefore justified chances result for treatment of wounds with TTP., Problemstellung: Die Biofilmbildung spielt eine entscheidende Rolle in der Pathogenese vieler akuter und subakuter mikrobieller Infektionen sowie bei chronischen Wunden. Zur Förderung der Wundheilung eignet sich gewebeverträgliches Plasma (tissue tolerable plasma - TTP) als Alternative zu antiseptischen oder chemotherapeutischen Therapien. Unbekannt ist dabei der mechanische Einfluss von Argon-Plasma auf das Eindringen der Bakterien in das Gewebe in Verbindung mit der anmikrobiellen Wirkung des Plasmas. Dazu wurden die Effekte von Argon-Plasma versus reinem Argongas auf die bakterielle Reduktion und die Eindringtiefe im Kombimodell von P. aeruginosa auf 3-D-Epidermismodellen untersucht. Methode: Es wurden 3-D-NHEK-Zellkulturen mit P. aeruginosa SG81 beimpft und nach 20 min Inkubation mit TTP behandelt. Die Verteilung und Eindringtiefe der Bakterien in der Zellkultur wurde mittels konfokalem Laserscanmikroskop (CLSM) zwischen Kontrolle (Argongas) und TTP-Einwirkung verglichen. Die Ermittlung der antimikrobiellen Wirkung von TPP erfolgte anhand der Bestimmung der Koloniebildenden Einheiten (KbE). Ergebnis: Es konnten die Penetration der Bakterien in das Epidermisgewebe und eine signifikante Erregerreduktion mittels TTP nachgewiesen werden. Damit ergeben sich begründete Chancen für eine Wundbehandlung mit TTP.

Hintergrund: Zahlreiche Studien haben den therapeutischen Nutzen von systemischem Antibiotika in der Behandlung schwerer Parodontitis gezeigt. Bisher ist allerdings noch nicht geklärt, welches antibiotische Behandlungkonzept das Geeignetste ist. Daher sollen in einer randomisierten, klinischen Blindstudie zwei verschiedene, systemische Antibiotika adjuvant zur konventionellen, instrumentellen Behandlung von Patienten mit rapid progressiver Parodontitis (RPP) miteinander verglichen werden. Material und Methode: 33 Patienten mit klinisch und radiologisch gesicherter RPP-Diagnose wurden auf 2 Gruppen verteilt: (1) AM-Gruppe (n=17): 500 mg Amoxicillin und 250 mg Metronidazol (3*/ Tag - 10 Tage), (2) D-Gruppe (=16) 200mg Doxycyclin am 1.Tag und 100mg Doxycyclin 13 Tage. Zu Beginn erhielten alle Patienten 3* eine professionelle Zahnreinigung, und anschlie§end Scaling und Wurzelglättung unter Lokalanästhesie in 2 Sitzungen. 3 Monate später wurde ein Recall und die Antibiose durchgeführt. Im Abstand von jeweils 3 Monaten erfolgten 2 weitere Recallsitzungen. Nach erfolgreicher Mundhygieneinstruktion und zu allen Recallsitzungen wurden alle klinischen Parameter wie Taschentiefe, relatives Attachmentlevel und das Bluten bzw. Pus nach Sondieren mit der Florida probe eruiert. Die Bestimmung 8 verschiedener Parodontalpathogene wurde mit dem DNS-Sondentest von Meridol durchgeführt. Die mikrobiologischen Proben wurden mit sterilen Papierspitzen an den vier tiefsten Taschen vor der Antibiose und zu den anschlie§enden Recalls entnommen und mit der Gensondentechnik im Labor der Wybert GmbH elmex Forschung, Lörrach, analysiert.
