212. Sentinel lymphadenectomy compared to extended lymphadenectomy in men with prostate cancer undergoing prostatectomy

Abstract

To evaluate the reliability of sentinel lymphadenectomy compared to extended lymphadenectomy in men undergoing radical prostatectomy (RP).A consecutive cohort of men with intermediate- to high-risk prostate cancer underwent RP with sentinel LA with intraoperative frozen section. In addition, extended LA was carried out in all cases. The endpoint was lymph node-positivity.In total, 54 men with a mean age of 65.3 (50.9-75.6) years were analyzed. The mean preoperative prostate-specific antigen was 10.6 (2.8-66.5) ng/ml, mean number of disease-positive cores was 5.8 (1-13), digital rectal examination was positive in 29 men (53.7%). In 12 men (22.2%), a positive lymph node was found (pN1). sLA was positive in 11 cases. One patient had a positive lymph node in eLA not found with sLA. The positive predictive value of frozen section was 50%; the respective figure for sLA compared to eLA was 91.6%.In this cohort, revealing a high prevalence of disease-positive lymph nodes, sLA was a reliable technique with a low rate of false negativity.

Abstract

Die Studie untersuchte die postoperativen Ergebnisse nach einer monomaxillären Operation im Unterkiefer an insgesamt 41 Patienten mit einer Klasse II und Klasse III Anomalie im Zeitraum von 18 Monaten. Mit Hilfe der kephalometrischen Analyse, der Profilanalyse und der Tensoranalyse erfolgte die Beurteilung anhand von FRS-Bildern, bei welcher Dsygnathieform die Ergebnisse durch die monomaxilläre Operationstechnik postoperativ stabil blieben. Die Patienten der Klasse II zeigten in allen 3 Analysen postoperativ geringere signifikante Verschlechterungen im Vergleich mit den Patienten der Untersuchungsgruppe Klasse III.

Abstract

An operant preferential looking (OPL) test with random dot stereo targets was used to gather developmental data on stereoacuity thresholds in 180 children between 18 and 65 months of age. Results indicated a steady improvement in stereoacuity with age from 250 sec arc in the youngest children tested to 60 sec arc in the oldest group of children. The greatest improvement in stereoacuity occurs at 30 months of age where mean values improve from 225 to 125 sec arc. This large change in stereo threshold appears to be attributable to the significantly higher variability in responses in the children under 30 months of age vs. the lower variability in responses in children over 30 months of age. The overall steady improvements in stereoacuity appear to be a result of the developmental changes in the variability of responses rather than actual neurophysiological changes within the visual system.

Abstract

This dissertation investigates speech perception in three different groups of native adult speakers of Dutch; an aphasic and two age-varying control groups. By means of two different experiments it is examined if the availability of visual articulatory information is beneficial to the auditory speech perception of aphasic speakers. In the context of audiovisual speech perception, the McGurk effect in Dutch is also investigated. The first experiment consists of two auditory discrimination tasks. The differing minimal pairs contrast with respect to either one of the following phonetic features: manner of articulation, place of articulation, or voicing. The results show that auditory discrimination was clearly impaired in the aphasic group, especially for place of articulation. The second experiment is based on these results and on earlier research with non-brain-damaged speakers that showed that in noisy listening conditions, place of articulation is more difficult to perceive than other features. However, perception improved considerably when visual information was available (e.g. Sumby & Pollack, 1954; Miller & Nicely, 1955). The second experiment, an identification task, examines auditory and audiovisual speech perception and the McGurk effect. Results indicate that the availability of visual articulatory information enhances auditory speech perception of the aphasic speakers. In addition, there is evidence for a moderate McGurk effect in Dutch. Not only the size of the McGurk effect was subject to group-specific differences, but also the alternative response pattern. Finally, in all three groups the perception of the McGurk effect was susceptible to the lexical status of the stimulus material.

Abstract

A new tool to measure changes in random dot stereoacuity development from 6 months to 5 years of age was developed and tested. Either a forced choice or operant preferential looking (PL) paradigm with a happy face target was successfully used to test 136 children. Results indicate that stereoacuity measurements continue to develop through the first years of life. Although children below 24 months of age can be expected to have stereo thresholds in the range of 300 sec arc, there is a transition at approximately 24 months of age after which stereoacuity approaches adult levels. The sensitivity of this test in detecting binocular vision problems was 80%. These results provide guidelines for the assessment of stereoacuity in young children and also provide a new tool for the early detection of binocular vision anomalies.

Abstract

There was considerable anticipation in the mid-1960s with the marketing and uptake of the oral contraceptive pill and the IUD that many other new forms of contraception would be available to the general public before the turn of the century. Much of that enthusiasm about contraceptive advances has since waned. Only three new contraceptive methods received US Food and Drug Administration approval during the 1990s, none as soon as advocates had expected, while the only other important changes were the gradual adjustment of pill formulations to expose users to a smaller total dosage of hormones and the introduction of new hormones already widely used in Europe. The lack of decisive advances is not the result of a paucity of candidate methods. In 1993, for example, an estimated 100 experimental contraceptive methods were being studied around the world. Many of those methods had, however, been in the works for years, while others were only slight modifications of products already on the market. Regulatory and legal issues, public opinion and politics, and financial issues have been identified as key obstacles to why the contraceptive reality has failed to match 1960s expectations. The author explains who sponsors contraceptive research and describes the ongoing development of spermicides and barrier methods, injectables, vaginal rings, implants, IUDs, methods for men, vaccines, and menses induction.
