Chronic granulomatous disease (CGD) is a rare congenital disorder characterized by repeated bacterial and fungal infections. Aside from a high incidence of liver abscess, little is known about hepatic involvement in CGD. The aim of this study was to describe the spectrum of liver abnormalities seen in CGD. The charts of 194 patients with CGD followed at the NIH were reviewed, with a focus on liver abnormalities. Liver enzyme elevations occurred on at least one occasion in 73% of patients during a mean of 8.9 years of follow-up. ALT elevations were generally transient. Although transient alkaline phosphatase (ALP) elevations were also common, persistent ALP elevations lasting up to 17.6 years were seen in 25% of patients. Liver abscess occurred in 35% of patients. Drug-induced hepatotoxicity was documented in 15% of patients but likely occurred more frequently. Hepatomegaly was found in 34% and splenomegaly in 56% of patients. Liver histology showed granulomata in 75% and lobular hepatitis in 90% of specimens. Venopathy of the portal vein was common (80%) and associated with splenomegaly. Venopathy of the central vein was also common (63%) and was associated with the number of abscess episodes. Nodular regenerative hyperplasia (NRH) was seen in 9 patients, including 6 of 12 autopsy specimens. CONCLUSION: Liver enzyme abnormalities occur frequently in patients with CGD. In addition to liver abscesses and granulomata, drug hepatotoxicity is likely underappreciated. Vascular lesions such as venopathy and--to a lesser extent--NRH are common. The cause and clinical consequences of venopathy await prospective evaluation.

Nonetheless, this model requires further validation to fully realize its translational potential.Methods: Woodchucks infected at birth with WHV that had developed HCC (n=12) were studied. Computed tomography, ultrasound, and magnetic resonance imaging were performed under anesthesia. LI-RADS scoring and correlative histologic analysis of sectioned tissues were performed. For immune characterization of tumors, CD3 (T cells), CD4 (T helpers), NCAM (Natural killers), FOXP3 (T-regulatory), PDL-1 (inhibitory checkpoint protein), and the human hepatocellular carcinoma (HCC) biomarker alpha-fetoprotein (AFP) immunohistochemical stains were performed.Results: Forty tumors were identified on imaging of which 29 were confirmed to be HCC with 26 categorized as LR-4 or 5. The remainder of the tumors had benign histology including basophilic foci, adenoma, and lipidosis as well as pre-malignant dysplastic foci. LR-4 and LR-5 lesions showed high sensitivity (90%) and specificity (100%) for malignant and pre-malignant tumors. Natural killers count was found to be 2– 5 times lower in tumors relative to normal parenchyma while other immune cells were located in the periphery of tumors. Tumors expressed AFP and did not express PD-L1.Conclusion: Woodchucks chronically infected with WHV developed diverse hepatic tumor types with diagnostic imaging, pathology, and immune patterns comparable to that in humans. Several diazotrophic strains of Klebsiella oxytoca and K. terrigena that colonize the plant-host interior were able to produce the plant cell wall depolymerising enzyme pectate lyase (Pel). The activity of the K. oxytoca enzyme was weaker than that of phytopathogenic bacteria, and it was located mainly inside the cells. A small fraction of the cells (10⁻⁶ to 10⁻⁵) in populations grown in nonselective media was able to grow in a selective medium with polygalactorunate (PC) as sole carbon source. After passage through selective medium cells were converted to the Pet+ -phenotype, and total Pet-activity in population of K. oxytoca increased. The increased level of Pelactivity of K. oxytoca and K. terrigena correlated with a 10-fold higher rate of internal colonization of wheat roots. Cultures of K. oxytoca VN13 grown in selective medium with PG also showed increased stimulation of wheat growth. Seedlings inoculated with such cultures exhibited better development resulting in higher biomass. Невелика кількість клітин популяції (10⁻⁶ –10⁻⁵ ) спроможна рости на селективному середовищі з полігалактуронатом натрію (ПГ), який використовували як джерело вуглецю. Після пасажу через селективне середовище всі клітини набували Реl+ -фенотипу, і загальна ПЛ-активність К. oxytoca зростала. Підвищена ПЛ-активність бактерій K. oxytoca та K. terrigena корелювала з посиленням у 10 разів внутрішньої колонізації коренів пшениці. Культура бактерій, яка виростала в селективному середовищі з ПГ, краще стимулювала розвиток пшениці, що проявлялося у збільшенні її біомаси, ніж культура, зрощена без селекції У нескольких изученных штаммов Klebsiella oxytoca и K. terrigena, способных колонизировать растения изнутри, обнаружена активность пектат лиазы (ПЛ) –фермента, который деполимеризует клеточную стенку. Активность ПЛ была ниже, чем у фитопатогенных бактерий, и сосредоточена внутри клетки. - Abstract
Composite tissue allotransplantation (CTA) is a recently introduced option for limb replacement and reconstruction of tissue defects. As with other allografts, CTA can undergo immune-mediated rejection; therefore standardized criteria are required for characterizing and reporting severity and types of rejection. This article documents the conclusions of a symposium on CTA rejection held at the Ninth Banff Conference on Allograft Pathology in La-Coruna, Spain, on 26 June 2007, and proposes a working classification, the Banff CTA-07, for the categorization of CTA rejection. This classification was derived from a consensus discussion session attended by the first authors of three published classification systems, pathologists and researchers from international centers where clinical CTA has been performed. It was open to all attendees to the Banff conference. To the extent possible, the format followed the established National Institutes of Health (NIH) guidelines on Consensus Development Programs. By consensus, the defining features to diagnose acute skin rejection include inflammatory cell infiltration with involvement of epidermis and/or adnexal structures, epithelial apoptosis, dyskeratosis and necrosis. Five grades of severity of rejection are defined. Cell-based therapy after myocardial infarction (MI) is a promising therapeutic option but the relevant cell subsets and dosage requirements are poorly defined. We hypothesized that cell therapy for myocardial infarction is improved by ex vivo expansion and high-dose transplantation of defined hematopoietic progenitor cells (HPCs). Since beta-catenin promotes self-renewal of stem cells we evaluated the therapeutic efficacy of beta-catenin-mediated ex vivo expansion of mouse HPCs in a mouse model of myocardial ischemia/reperfusion followed by intraarterial cell delivery. The impact of cell dose was determined by comparing a low-dose (LD, 5 x 10(5) cells) vs. a high-dose (HD, 1 x 10(7) cells) cell transplantation regimen of beta-catenin-HPCs. The impact of beta-catenin modification of HPCs was determined by comparing control-transduced HPCs (GFP-HPCs) vs. transgenic beta-catenin-HPCs. HD beta-catenin-HPCs significantly improved LV function and end-systolic and end-diastolic dimensions as compared to saline and LD beta-catenin-HPCs. Furthermore, while treatment with HD GFP-HPC resulted in a modest cardiac improvement the application of beta-catenin-HPCs was superior, resulting in a significant improvement in EF, FS and LVESD over saline and control GFP-HPC treatment. Although myocardial engraftment of HPCs was only transient, as determined by cell quantification after dye labeling, beta-catenin-HPC treatment significantly decreased infarct size, reduced cardiomyocyte apoptosis and increased capillary angiogenesis in vitro and in vivo. Ex vivo expanded HPCs improve cardiac function and remodeling post MI in a cell number- and beta-catenin-dependent manner.

227. Is professional prescription of a commercially derived dietary supplement in colectomysed patients necessary?

- Abstract
Introduction Colorectal cancers are the second most common cause of mortality in the USA, Western Europe and also Hungary. After the removal of a tumor, cancer predisposition still remains, therefore secondary prevention is needed. Several reports are known about the effects of nutritional supplements in the improvement of patient life quality. Therefore the question arises whether natural food supplement products with several bioactive agents can modify immune reactivity or influence redox homeostasis. Methods In the present study we report on the negative output of the commercially derived dietary supplement “Vegetable and fruit color compound concentrates OÉTI 45/É sample” in colectomysed patients. Quantitative analysis of anthocyanins and flavonoids in this dietary supplement, was carried out using HPLC-DAD-ESI–MS/MS. Caucasian volunteers ( N = 26) were treated with the dietary supplement for 3 months 5–10 years after their colectomy. The healthy control volunteers ( N = 10) of both genders received the supplement the same way. The dose was 2 × 3 g/day. Ultrasonography, routine laboratory tests (30 parameters), tumor marker (AFP, CEA, CA19-9, PSA) determinations and redox parameters (SOD, GSHPx, HbA1c, reducing power, chemiluminescence intensity) were carried out for each person. Results Diet-related bioactive compounds were intensively examined in vitro, although in vivo and in vitro effects are different and the mode of actions in vivo are not yet known in detail. Tentative identification of dietary supplements based on mass spectrometry data for published constituents of plants, twenty anthocyanins and flavonoid glycosides were identified. Several routine laboratory parameters were not altered after treatment compared to start time data in colectomysed patients and control volunteers. The treatment did not change the tumor marker levels significantly, but in each case the values were elevated in examined groups. Redox parameters of colectomysed patients showed strengthened free radical reactions, namely rebound effect against antioxidant excess, although tendency was observed in controls as well. Anti-inflammatory effects of the natural product was not clearly observed. Conclusion Today, there are countless products on the market, thus experts in healthcare, doctors, pharmacists and dieticians should have adequate knowledge of dietary supplements, and how can they become important tools for a healthy lifestyle. - Abstract
Sex-determining cascades are supposed to have evolved in a retrograde manner from bottom to top. Wilkins' 1995 hypothesis finds support from our comparative studies in Drosophila melanogaster and Musca domestica, two dipteran species that separated some 120 million years ago. The sex-determining cascades in these flies differ at the level of the primary sex-determining signal and their targets, Sxl in Drosophila and F in Musca. Here we present evidence that they converge at the level of the terminal regulator, doublesex ( dsx), which conveys the selected sexual fate to the differentiation genes. The dsx homologue in Musca, Md-dsx, encodes male-specific (MdDSX(M)) and female-specific (MdDSX(F)) protein variants which correspond in structure to those in Drosophila. Sex-specific regulation of Md-dsx is controlled by the switch gene F via a splicing mechanism that is similar but in some relevant aspects different from that in Drosophila. MdDSX(F) expression can activate the vitellogenin genes in Drosophila and Musca males, and MdDSX(M) expression in Drosophila females can cause male-like pigmentation of posterior tergites, suggesting that these Musca dsx variants are conserved not only in structure but also in function. Furthermore, downregulation of Md-dsx activity in Musca by injecting dsRNA into embryos leads to intersexual differentiation of the gonads. These results strongly support a role of Md-dsx as the final regulatory gene in the sex-determining hierarchy of the housefly.
